Este artículo tiene por objetivo ampliar la reflexión crítica sobre el ocio y la interculturalidad en América Latina. La discusión fue basada en una investigación bibliográfica y tiene como telón de fondo el repensar la modernidad, la ideología de progreso y las dicotomías que limitan las concepciones de ser humano y de mundo. El texto propone, además, discutir el potencial del ocio para enriquecer las prácticas educativas a partir de distintos lenguajes, teniendo en vista el caminar en dirección de una (...) educación intercultural que sea también problematizadora, crítica y transformadora. En cuanto una dimensión de la cultura, el ocio puede ser un tiempo/espacio social de resignificación y empoderamiento, contribuir para agudizar sensibilidades y ayudar a la comprensión de los contextos latinoamericanos en la perspectiva de la interculturalidad, estimulando el pensamiento crítico sobre las sociedades para la transformación social en búsqueda de un mundo más humano y solidario. In January 2022 the new Dutch Civic Integration programme was launched together with promises of improvements it would bring in facilitating the ‘integration’ of newcomers to the Netherlands. This study presents a critical discourse analysis of texts intended for municipalities to take on their new coordinating role in this programme. The analysis aims to understand the discourse in the texts, which actors are mobilized by them, and the role these texts and these actors play in processes of governmental racialization. The (...) analysis demonstrates shifting complex assemblages are brought into cascades of governance in which all actors are disciplined to accept the problem of integration as a problem of cultural difference and distance, and then furthermore disciplined to adopt new practices deemed necessary to identify and even ‘objectively’ measure the inherent traits contributing to this problematic. Lastly, the analysis displays that all actors are disciplined to accept the solution of ‘spontaneous compliance’; a series of practices and knowledges, which move the civic integration programme beyond an aim of responsibilization, into a programme of internalization, wherein newcomers are expected to own and address their problematic ‘nature’, making ‘modern’ values their own. Many physiotherapists are reluctant to treat a client with bone metastases, as treatment indications are not clearly defined. The paradigmatic case presented here, and a review of the literature, allow us to highlight some ethical and clinical issues in physiotherapy care at the end of life, particularly for patients with bone metastases. The objective of this article is not to create an ethical decision making model but rather to serve as additional guidance to existing decision models in rehabilitation. This guidance (...) is based on principlism and is specific to the oncology clients in palliative care. The proposed principles, without being exhaustive, are beneficence, non-maleficence, respect for autonomy and justice. With a review of clinical indications and precautions, the article also helps to clarify both the theoretical and the evidence-based risks. These principles provide avenues for reflection to guide the management of these patients. The physiotherapist will then be able to conduct a critical analysis, in collaboration with the interdisciplinary team, whose goal is to respect the client’s right to appropriate care at the right time and that is based on clinical need. Lantos, and Sonia GowdaMultiple sclerosis is believed to be an autoimmune disease of the central nervous system. However, according to Italian physician Paolo Zamboni, it is related to cerebrospinal vascular insufficiency. Zamboni claims that MS can be treated by remedying this condition with venous angioplasty. This surgery is offered as treatment for MS in various countries—Poland, Bulgaria, and Costa Rica, for (...) example. But the Medical Board of Quebec, Canada, does not allow its members to perform the surgery and strongly advises patients against seeking it elsewhere. This opposition is based on the lack of evidence to support the procedure and the serious risks associated with it (including venous thrombosis, stroke, and pulmonary embolism). As a result, many Canadian MS patients have traveled to other countries to receive the surgery. Quebec physicians have been reluctant to provide surgical follow-up for patients who undergo the surgery in other countries due to the lack of information regarding the procedure. No alternative treatments are effective to partially or completely cure MS, and so the treatments available in Canada aim only to manage its symptoms.Jade is a forty-year-old Canadian woman with MS. She is on long-term disability leave, complains of growing fatigue and muscle stiffness, and has painful sensations in her limbs. She has been followed by an interdisciplinary team in a specialized hospital in Quebec for the past twenty years, since she was diagnosed. At her last appointment, Jade told her physical therapist that she recently underwent the experimental Zamboni surgery in Poland, despite her physician’s strong recommendation not to. Jade fears her physician’s reaction, so she asks the physical therapist to keep this information from the rest of her medical team.Jade is shy and tends to distrust health care professionals. It took many years for her physical therapist to gain her trust and to develop a positive therapeutic relationship with her. However, the physical therapist knows that serious complications can occur in patients who have had Zamboni’s surgery. Not telling Jade’s physician about the surgery could result in harm; Jade could have complications that are not recognized. Providing the information to the rest of the team would allow them to adjust her treatment to respond to the surgery and give her information and postoperative education that she will need. Health professionals are deeply committed to helping patients; above all, they want to avoid harming them. Therefore, a key component of this case is the possibility that respecting Jade’s desire to conceal information from the team may put her at direct risk if any postoperative complications are not detected, and at indirect risk if Jade’s trust in her medical team is undermined to the point where she turns away from them.The physical therapist must determine if the possible physical risks to Jade are imminent and serious enough to go against her autonomous choice. One risk that can occur following Zamboni’s surgery is inflammation of a vein caused by a blood clot. This is a life-threatening situation (although it may not be a likely complication for Jade). But the physical therapist can teach Jade to recognize the physical signs that can alert her to this problem, giving her ample time to seek treatment. This article was compiled from a bibliographic review and aims to: analyze what a concept is; present the two main approaches by which leisure and recreation are understood in Latin America, expanding thus the possibilities for new meanings to these concepts; and discuss the alternatives that these concepts -from a new perspective- can provide to the construction of participative, solidary and sustainable societies. In conclusion, it's emphasized that most of the concepts of leisure and recreation widespread in Latin America , (...) were formulated in other contexts, showing to be insufficient and inappropriate to the problems, contradictions and potentialities of this region. That is why they need new approaches and resignificances. Understanding leisure as a dimension of culture outlines the need to promote a critical/creative education by and for a non-hegemonic leisure, which rescues and appreciates cultural diversity as one of the key elements for social transformation. Many health care professionals (HCPs) are understandably reluctant to treat patients in environments infested with bedbugs, in part due to the risk of themselves becoming bedbug vectors to their own homes and workplaces. However, bedbugs are increasingly widespread in care settings, such as nursing homes, as well as in private homes visited by HCPs, leading to increased questions of how health care organizations and their staff ought to respond. This situation is associated with a range of ethical considerations including the (...) duty of care, stigmatization, vulnerability, confidentiality, risks for third parties, and professional autonomy. In this article, we analyze these issues using a case study approach. We consider how patients whose living environments are infested with bedbugs can receive care in the community setting in a manner that supports their well-being, is consistent with fairness in care provision, and takes into account risks for HCPs and third parties. Transcranial magnetic stimulation can be used to mask visual stimuli, disrupting visual task performance or preventing visual awareness. While TMS masking studies generally fix stimulation intensity, we hypothesized that varying the intensity of TMS pulses in a masking paradigm might inform several ongoing debates concerning TMS disruption of vision as measured subjectively versus objectively, and pre-stimulus versus post-stimulus TMS masking. We here show that both pre-stimulus TMS pulses and post-stimulus TMS pulses could strongly mask visual stimuli. We found no dissociations (...) between TMS effects on the subjective and objective measures of vision for any masking window or intensity, ruling out the option that TMS intensity levels determine whether dissociations between subjective and objective vision are obtained. For the post-stimulus time window particularly, we suggest that these data provide new constraints for models of vision and visual awareness. Finally, our data are in line with the idea that pre-stimulus masking operates differently from conventional post-stimulus masking. (shrink)

O estudo dos fatos e episódios científicos é importante no sentido de possibilitar a compreensão do desenvolvimento das Ciências em contextos tanto do ensino, em diversos níveis, quanto da pesquisa. No entanto, a leitura dessa construção histórica precisa considerar visões de Ciência que superem as perspectivas lineares e neutras sobre a construção do processo de produção científica. Sob tal perspectiva, propõe-se nesse artigo uma releitura do movimento de elaboração do modelo da dupla hélice do DNA a partir do aporte epistemológico (...) de Thomas Kuhn. Entendemos ser este um contexto importante que permeia a formação científica, tanto na educação básica quanto no ensino superior, no campo das Ciências Biológicas e que os conceitos desenvolvidos a partir da elaboração do modelo da dupla hélice são basilares para a compreensão de conhecimentos relacionados à genética, citologia, evolução, dentre outros. No processo de revisitar esse recorte histórico, identificamos os embates teóricos entre pesquisadores de diversos campos, a competição e o consenso como elementos que mobilizam a busca de respostas na concretização de um novo paradigma. Dessa forma, a dinamicidade da Ciência se mostra presente rejeitando a ideia do dado pronto e valorizando o processo no qual o conhecimento se constrói. Propomos que este seja um exercício de reflexão considerando-se que revisitar a história da Ciência a partir de diferentes perspectivas epistemológicas pode propiciar aos sujeitos novos olhares em um devir que descortine conceitos construídos de maneira dinâmica por homens e mulheres que, entre ires e vires, dão significado aos fenômenos que nos rodeiam. With the world's population aging, hospitals are facing pressure to adequately meet the needs of a growing number of frail older patients. For this population, comorbidities combined with a limited ability to face stressful situations contribute to frailty whereby a small injury or illness can lead to significant loss of function. In the context of long waiting time to access rehabilitation services, a large majority of settings use referral prioritization to help manage waiting lists. Prioritization practices vary greatly between settings and there is little consensus on how best to prioritize referrals. This paper describes the prioritization processes for physiotherapy services in Québec and its potential implications in terms of equity in access to services. This is a secondary analysis of a survey of outpatient physiotherapy departments (n=98; proportion of participation was (...) 99%) conducted in 2015 across publicly funded hospitals in Québec. In many settings, persons with acute orthopaedic conditions were prioritized while chronic conditions were given a lower priority. There were 72 different combinations of prioritization criteria used in outpatient physiotherapy departments. Variability was also observed in the type of personnel involved in the prioritization process, the number of priority levels used to rank the referrals and the source of information used to prioritize referrals. These results highlight potential issues regarding equity in access to physiotherapy services: the prioritization of persons with acute conditions to the detriment of those with chronic conditions, the lack of consensus on a fair prioritization process and the importance to adequately assess patients’ needs for treatment. Further research and interventions on prioritization criteria and processes are needed to ensure equitable access to physiotherapy services, especially in the public sector. (shrink)

In addition to discussing decisional autonomy, rehabilitation also focuses on autonomy as functional independence. In practice, responding to patients with diminished autonomy is an important component of rehabilitation care, but also gives rise to tensions and challenges. Our objective was to better understand the complex and distinctive ways that autonomy is understood and upheld in the context of (...) rehabilitation care by reviewing how autonomy is discussed in the rehabilitation literature. Methods: We conducted a scoping review addressing issues of autonomy in the context of mental and physical rehabilitation. Our process followed three sequential steps. We extracted and analyzed bibliometric information. We then examined how autonomy was defined and conceptualized. Finally, we examined how the articles discussed the roles of rehabilitation health professionals in responding to patient autonomy. Findings: The articles include 16 empirical reports, 17 case studies and 30 theoretical papers. The most common conceptual accounts of autonomy drew upon principlism, rights-based and legal analyses, and relational/social approaches. We identified four broad approaches for responding to patient autonomy: supporting, promoting, respecting and advocating. Conclusion: This review helps clarify some of the ambiguities and conceptual distinctions underlying discussions and practices related to autonomy in rehabilitation. Both before and after the publication of Isaac Newton’s particulate theory of light, numerous wave theories of light were advanced by both philosophers and scientists (e.g., René Descartes, Thomas Hobbes, Robert Hooke, Francesco Grimaldi, and Christiaan Huygens). What is peculiar about this list, as frequently found in the scholarly literature on light, is that it refers to individuals who do not extend much further back than the seventeenth century. A close examination of Aristotle’s account of color and light in comparison (...) to Christiann Huygens’ theory of color and light, however, reveals that Aristotle deserves to be recognized as one who antedates and foreshadows these modern wave theorists. Indeed, we shall argue that Aristotle offered the first wave theory of color and light. On Aristotle’s theory, light is the actualization of a transparent medium such as air or water that makes possible the dissemination of color through the medium in a manner analogous to the dissemination of sound through air or water.
