This paper explores ethical concerns arising in healthcare integration. We argue that integration is necessary imperative for meeting contemporary and future healthcare challenges, a far stronger evidence base for the conditions of its effectiveness is required. In particular, given the increasing emphasis at the policy level for the entire healthcare infrastructure to become better integrated, our analysis of the ethical challenges that follow from the logic of integration itself is timely and important and has hitherto received insufficient attention. We evaluated (...) an educational intervention which aims to improve child health outcomes by making transitions between primary to secondary care more efficient, ensuring children and parents are better supported throughout. The programme provided skills for trainee paediatricians and general practitioners in co-designing integrated clinical services. The key ethical challenges of integrated care that arose from a clinical perspective are: professional identity and autonomy in an integrated working environment; the concomitant extent of professional responsibility in such an environment; and the urgent need for more evidence to be produced on which strategies for integrating at scale can be based. From our analysis we suggest a tentative way forward, viewed from a normative position broadly situated at the intersection of deontology and care ethics. We adopt this position because the primary clinical ethical issues in the context of integrated care concern: how to ensure that all duties of care to individual patients are met in a newly orientated working environment where clinical responsibility may be ambiguous; and the need to orientate care around the patient by foregrounding their autonomous preferences and ensuring good patient clinician relationships in clinical decision-making. Philosophical issues such as reality and appearance, God and world, self and not-self, rebirth and immortality, free will and determination, mysticism, etc., have been examined by eastern and western philosophers as far back as the sages of Upanishads (700 BCE) in the East, and Plato (400 BCE) in the West. However, there was no significant communication among the philosophers of the East and West perhaps until the eighteenth century. Arthur Schopenhauer (1788-1860) was one of the first among the great western (...) philosophers to recognize the value of Indian philosophies. Despite the lack of communication there are striking similarities of concepts and issues addressed by Indian and western philosophies. Also interesting is how similar are the mystical versions of different religions of the East and West. The author, Dr. Arun Chatterjee, has attempted to bring out the similarities and differences in various chapters of this book dealing with different topics. Among Indian philosophies the focus is on Vedanta philosophy and the views of Sri Aurobindo (1872-1950). The views of Swami Vivekananda (1863-1902) also are presented. Other Indian philosophies such as Sankhya and Buddhism are discussed briefly. Among western religions Judaism, Christianity, and Islam are discussed. (shrink)

Under multistakeholderism, private philanthropic foundations have played an increasingly influential role in global development. As part of which, foundations have promoted what we call “privatization creep” (i.e., mainstreaming market-centric solutions to development). Sidelining redistributive approaches altogether, “privatization creep” favours profit-making over everything else, doing little to “save the world.”.

Scholars of business and management studies have recently turned their attention to inequality, a key issue for business ethics given the role of private firms in transmitting—and potentially challenging—inequalities. However, this research is yet to examine inequality from a subaltern perspective. In this paper, we discuss the alleviation of inequalities in organizational and institutional contexts by drawing on the ideas of Dr. B. R. Ambedkar, a jurist, political leader and economist, and one of the unsung social theorists of the twentieth (...) century. Specifically, we focus on Ambedkar’s critique of the Indian caste system, his outline of comprehensive reform, and prescription of representational politics to achieve equality. We contend that an Ambedkarite ethical manifesto of persuasion—focussed on state-led institutional reforms driven by the subaltern—can help management researchers reimagine issues of inequality and extend business ethics beyond organizational boundaries. Purpose Digital transformation leverages digital technologies to change current processes and introduce new processes in any organisation’s business model, customer/user experience and operational processes. Artificial intelligence plays a significant role in achieving DT. As DT is touching each sphere of humanity, AI led DT is raising many fundamental questions. These questions raise concerns for the systems deployed, how they should behave, what risks they carry, the monitoring and evaluation control we have in hand, etc. These issues call for the need (...) to integrate ethics in AI led DT. The purpose of this study is to develop an “AI led ethical digital transformation framework”. Design/methodology/approach Based on the literature survey, various existing business ethics decision-making models were synthesised. The authors mapped essential characteristics such as intensity and the individual, organisational and opportunity factors of ethics models with the proposed AI led ethical DT. The DT framework is evaluated using a thematic analysis of 23 expert interviews with relevant AI ethics personas from industry and society. The qualitative data of the interviews and opinion data has been analysed using MAXQDA software. Findings The authors have explored how AI can drive the ethical DT framework and have identified the core constituents of developing an AI led ethical DT framework. Backed by established ethical theories, the paper presents how DT pillars are related and sequenced to ethical factors. This research provides the potential to examine theoretically sequenced ethical factors with practical DT pillars. Originality/value The study establishes deduced and induced ethical value codes based on thematic analysis to develop guidelines for the pursuit of ethical DT. The authors identify four unique induced themes, namely, corporate social responsibility, perceived value, standard benchmarking and learning willingness. The comprehensive findings of this research, supported by a robust theoretical background, have substantial implications for academic research and corporate applicability. The proposed AI led ethical DT framework is unique and can be used for integrated social, technological and economic ethical research. (shrink)

This review essay provides a critical analysis of the book ‘The Great Tech Game,’ authored by Anirudh Suri. For the analysis, other literature published in a similar area is considered and pitched the arguments against the ones made in the book. During the year this book was released, there were numerous debates on accountability and trust in frontier digital technologies like AI. These debates have reached a systemic level where the entire global community is divided into two camps headed by (...) the US and China in drafting rules and regulations for the internet and allied technologies. A critical comment on a well-circulated book would add clarity to the debates on the impacts of frontier digital technologies. This review contributes to the critical voices of the emerging scholarship on the confluence of digital technology and global politics. The power of technology to transform religions, science, and political institutions has often been presented as nothing short of revolutionary. Does technology have a similarly transformative influence on societies’ morality? Scholars have not rigorously investigated the role of technology in moral revolutions, even though existing research on technomoral change suggests that this role may be considerable. In this paper, we explore what the role of technology in moral revolutions, understood as processes of radical group-level moral change, amounts to. We do (...) so by investigating four historical episodes of radical moral change in which technology plays a noteworthy role. Our case-studies illustrate the plurality of mechanisms involved in technomoral revolutions, but also suggest general patterns of technomoral change, such as technology’s capacity to stabilize and destabilize moral systems, and to make morally salient phenomena visible or invisible. We find several leads to expand and refine conceptual tools for analysing moral change, specifically by crystallizing the notions of ‘technomoral niche construction’ and ‘moral payoff mechanisms’. Coming to terms with the role of technology in radical moral change, we argue, enriches our understanding of moral revolutions, and alerts us to the depths of which technology can change our societies in wanted and unwanted ways. A structural theorem for Kleene algebras is proved, showing that an element of a Kleene algebra can be looked upon as an ordered pair of sets, and that negation with the Kleene property is describable by the set-theoretic complement. The propositional logic \ of Kleene algebras is shown to be sound and complete with respect to a 3-valued and a rough set semantics. It is also established that Kleene negation can be considered as a modal operator, due to a perp (...) semantics of \. Moreover, another representation of Kleene algebras is obtained in the class of complex algebras of compatibility frames. Moving forward rapidly in the clinical research phase, uterus transplantation may be a future treatment option for women with uterine factor infertility, which accounts for three per cent of all infertility in women. This new method of treatment would allow women, who currently rely on gestational surrogacy or adoption, to gestate and birth their own genetic offspring. Since uterus transplantation carries significant risk when compared with surrogacy and adoption as well as when compared with other organ transplants, it requires greater (...) justification because its goals are quality of life, not life-saving, in their scope. It is important to address questions regarding the physical, psychosocial and ethical risks and benefits of uterus transplantation for all three parties involved—the patient, the donor and the potential child—as well as discuss the regulatory implications as research on uterus transplantations moves forward. The environment impacts human health in profound ways, yet few theories define the form of the relationship between human physiology and the environment. It is conjectured that such complex systems cannot interact directly, but rather their interaction requires the formation of an intermediary “interface.” This position contrasts with current epidemiological constructs of causation, which implicitly assume that two complex systems transfer information directly while remaining separate entities. Further, it is contended that dynamic, process‐based interfaces incorporate components from all the interacting (...) systems but exhibit operational independence. This property has many consequences, the foremost being that characteristics of the interface cannot be fully resolved by only studying the systems involved in the interaction. The interface itself must be the subject of inquiry. Without refocusing the attention on biodynamic interfaces, how the environment impacts health cannot be discerned. BackgroundResuscitation and treatment of critically ill newborn infants is associated with relatively high mortality, morbidity and cost. Guidelines relating to resuscitation have traditionally focused on the best interests of infants. There are, however, limited resources available in the neonatal intensive care unit, meaning that difficult decisions sometimes need to be made. This study explores the intuitions of lay people regarding resource allocation decisions in the NICU.MethodsThe study design was a cross-sectional quantitative survey, consisting of 20 hypothetical rationing scenarios. There were (...) 119 respondents who entered the questionnaire, and 109 who completed it. The respondents were adult US and Indian participants of the online crowdsourcing platform Mechanical Turk. Respondents were asked to decide which of two infants to treat in a situation of scarce resources. Demographic characteristics, personality traits and political views were recorded. Respondents were also asked to respond to a widely cited thought experiment involving rationing.ResultsThe majority of respondents, in all except one scenario, chose the utilitarian option of directing treatment to the infant with the higher chance of survival, higher life expectancy, less severe disability, and less expensive treatment. As discrepancy between outcomes decreased, however, there was a statistically significant increase in egalitarian responses and decrease in utilitarian responses in scenarios involving chance of survival, life expectancy, and cost of treatment. In the classic ‘lifeboat’ scenario, all but two respondents were utilitarian.ConclusionsThis survey suggests that in situations of scarcity and equal clinical need, non-health professionals support rationing of life-saving treatment based on probability of survival, duration of survival, cost of treatment or quality of life. However, where the difference in prognosis or cost is very small, non-health professionals preferred to give infants an equal chance of receiving treatment. (shrink)

Central to Kuhn's notion of incommensurability are the ideas of meaning variance and lexicon, and the impossibility of translation of terms across different theories. Such a notion of incommensurability is based on a particular understanding of what a scientific language is. In this paper we first attempt to understand this notion of scientific language in the context of incommensurability. We consider the consequences of the essential multisemiotic character of scientific theories and show how this leads to even a single theory (...) being potentially 'internally incommensurable'. We then discuss Kuhn's lexicon-based approach to incommensurability and the problems associated with it. Finally we argue that this approach by Kuhn has interesting overlaps with the problem of meaning associated with multisemiosis, particularly the challenge of understanding the process of symbolization in scientific theories. Soon, the realization that, while economies had been growing, systemic problems of social inequality and environmental unsustainability were becoming intolerable, led to the Sustainable Development Goals, which all countries signed up to achieve. A new toolkit is required to attain these goals that go beyond the precepts of good business management and prevalent best practices in government as well as civil society organizations.

Through the continuous investigation of humans in organizations, we have learned much about motivation, attitudes, and performance. For example, Yukl and others have helped increase our understanding of influence tactics and the effect they have on the performance of subordinates, supervisors, and peers. Some tactics (and combinations of tactics) lead to resistance, some lead to compliance, and some lead to commitment. In this study, we raise the question of whether or not we incorporate our knowledge of these research findings into (...) the design, implementation, and interpretation of our own research studies that require the participation of human subjects. In a survey of 134 subjects from a previous social science study, we found that performance varied across the sample, consistent with the concepts of resistance, compliance, and commitment. In addition, the variance in performance could be explained, in part, by task interest and perceived task demands. Implications are discussed. (shrink)

This essay analyzes the representations of time and memory in Holocaust literature through a comparative study of Charlotte Delbo’s memoir Days and Memory and Ida Fink’s three stories “A Scrap of Time,” “A Second Scrap of Time,” and “Traces.” Although both the writers make use of time and memory to represent the Holocaust, their ways of representation vary significantly. Memory and time are used in Delbo to show the timelessness in complex layers of memory and to recreate a reality through (...) inventive narrative style. Whereas, in Fink, they are used to delineate the scraps of time in the ruins of memory and to create a tragic domestic reality through conventional narrativity. Moreover, this essay cautions against the danger of misrepresentation of memory as “amnesia,” often represented in the canonical postmodernist views of memory. (shrink)

This research attempts to construct a second order observation model in understanding the significance of Artificial intelligence (AI) in changing the global power structure. Because of the inevitable ubiquity of AI in the world societies’ near future, it impacts all the sections of society triggering socio-technical iterative developments. Its horizontal impact and states’ race to become leader in the AI world asks for a vivid understanding of its impact on the international system. To understand the latter, Triple Helix (TH) model (...) along with Shannon’s information entropy has been used to operationalize system’s theory. This model uses Shannon’s information theory to calculate the uncertainty generated from the interactions between the sub-systems considered. Data for the latter has been taken from Sanford Artificial Intelligence Laboratory 2019 report. It is found that European countries are the most effected in the AI era, with probability of losing their global influence and thus creating power void. Emerging powers such as India, Canada, South Africa and Brazil have better chances to fill the void and emerge as global influences. (shrink)

The development of new technologies that enable autonomous weapon systems poses a challenge to policymakers and technologists trying to balance military requirements with international obligations and ethical norms. Some have called for new international agreements to restrict or ban lethal autonomous weapon systems. Given the tactical and strategic value of the technologies and the proliferation of threats, the military continues to explore the development of new autonomous technologies to execute national security missions. The rapid global diffusion and dual-use nature of (...) autonomous systems necessitate a proactive approach and a shared understanding of the technical realities, threats, military relevance, and strategic implications of these technologies from these communities. Ultimately, developing AI-enabled defense systems that adhere to global norms and relevant treaty obligations, leverage emerging technologies, and provide operational advantages is possible. The development of a workable and realistic regulatory framework governing the use of lethal autonomous weapons and the artificial intelligence that underpins autonomy will be best supported through a coordinated effort of the regulatory community, technologists, and military to create requirements that reflect the global proliferation and rapidly evolving threat of autonomous weapon systems. This essay seeks to demonstrate that: (1) the lack of coherent dialogue between the technical and policy communities can create security, ethical, and legal dilemmas; and (2) bridging the military, technical, and policy communities can lead to technology with constraints that balance the needs of military, technical, and policy communities. It uses case studies to show why mechanisms are needed to enable early and continuous engagement across the technical, policymaking, and operational communities. The essay then uses twelve interviews with AI and autonomy experts, which provide insight into what the technical and policymaking communities consider fundamental to the progression of responsible autonomous development. It also recommends practical steps for connecting the relevant stakeholders. The goal is to provide the Department of Defense with concrete steps for building organizational structures or processes that create incentives for engagement across communities. (shrink)

Research teams have made considerable progress in treating absolute uterine factor infertility through uterus transplantation, though studies have differed on the choice of either deceased or living donors. While researchers continue to analyze the medical feasibility of both approaches, little attention has been paid to the ethics of using deceased versus living donors as well as the protections that must be in place for each. Both types of uterus donation also pose unique regulatory challenges, including how to allocate donated organs; (...) whether the donor / donor's family has any rights to the uterus and resulting child; how to manage contact between the donor / donor's family, recipient, and resulting child; and how to track outcomes moving forward. As AI technologies are rolled out into healthcare, academia, human resources, law, and a multitude of other domains, they become de-facto arbiters of truth. But truth is highly contested, with many different definitions and approaches. This article discusses the struggle for truth in AI systems and the general responses to date. It then investigates the production of truth in InstructGPT, a large language model, highlighting how data harvesting, model architectures, and social feedback mechanisms weave together disparate understandings of veracity. It (...) conceptualizes this performance as an operationalization of truth, where distinct, often-conflicting claims are smoothly synthesized and confidently presented into truth-statements. We argue that these same logics and inconsistencies play out in Instruct’s successor, ChatGPT, reiterating truth as a non-trivial problem. We suggest that enriching sociality and thickening “reality” are two promising vectors for enhancing the truth-evaluating capacities of future language models. We conclude, however, by stepping back to consider AI truth-telling as a social practice: what kind of “truth” do we as listeners desire? (shrink)

In this study Arun Bala examines the implications that Niels Bohr's principle of complementarity holds for fields beyond physics. Bohr, one of the founding figures of modern quantum physics, argued that the principle of complementarity he proposed for understanding atomic processes has parallels in psychology, biology, and social science, as well as in Buddhist and Taoist thought. But Bohr failed to offer any explanation for why complementarity might extend beyond physics, and his claims have been widely rejected by scientists (...) as empty speculation. Scientific scepticism has only been reinforced by the naïve enthusiasm of postmodern relativists and New Age intuitionists, who seize upon Bohr's ideas to justify anti-realist and mystical positions. Arun Bala offers a detailed defence of Bohr's claim that complementarity has far-reaching implications for the biological and social sciences, as well as for comparative philosophies of science, by explaining Bohr's parallels as responses to the omnipresence of grown properties in nature. Complaints about the state of medical care are increasing in today s India; whether it s unnecessary investigations, botched operations or expensive, sometimes even harmful, medication. But while the unease is widespread, few outside the profession understand the extent to which the medical system is being distorted. Dr Arun Gadre and Dr Abhay Shukla have gathered evidence from seventy-eight practising doctors, in both the private and public medical sectors, to expose the ways in which vulnerable patients are exploited by (...) a system that promotes unscrupulous medical practices. At a time when the medical sector is growing rapidly, especially in urban areas, with the proliferation of multi-specialty hospitals and the adoption of ever-more sophisticated technologies, rational and ethical medical care is becoming increasingly rare. Honest doctors feel under siege, professional bodies meant to regulate the medical sector fail to do so, and the influence of the powerful pharmaceutical industry becomes even more pervasive. Drawing on the frank and courageous statements of these seventy-eight doctors dismayed at the state of their profession, Dissenting Diagnosis lays bare the corruption afflicting the medical sector in India and sets out solutions for a healthier future. We argue for the need for a new language for business – one that is capable of changing the current business decision-making frame of wins and losses to a frame of community and social learning. This paper outlines a classroom exercise about Nelson Mandela’s leadership, involving movies, case studies and poetry, and shows how the more holistic approach helps shift student views of the triple bottom line. Since neuroscience literature has shown that poetry can help enhance learning, students carefully study (...) “Invictus,” a poem written by Victorian-era poet Henley and revered by Mandela. As students dig deeper into the case study through classroom discussion, they begin to see Mandela’s leadership through a new frame, and in doing so broaden their own views. This more holistic approach to studying leadership may allow decision makers to adjust both their goals and the focus of their analysis to go beyond monetary outcomes to include socially and environmentally sustainable outcomes.
