Major depression (MD) is a highly heterogeneous diagnostic category. Diverse symptoms such as sad mood, anhedonia, and fatigue are routinely added to an unweighted sum-score, and cutoffs are used to distinguish between depressed participants and healthy controls. Researchers then investigate outcome variables like MD risk factors, biomarkers, and treatment response in such samples. These practices presuppose that (1) depression is a discrete condition, and that (2) symptoms are interchangeable indicators of this latent disorder. Here I review these two assumptions, elucidate (...) their historical roots, show how deeply engrained they are in psychological and psychiatric research, and document that they contrast with evidence. Depression is not a consistent syndrome with clearly demarcated boundaries, and depression symptoms are not interchangeable indicators of an underlying disorder. Current research practices lump individuals with very different problems into one category, which has contributed to the remarkably slow progress in key research domains such as the development of efficacious antidepressants or the identification of biomarkers for depression. The recently proposed network framework offers an alternative to the problematic assumptions. MD is not understood as a distinct condition, but as heterogeneous symptom cluster that substantially overlaps with other syndromes such as anxiety disorders. MD is not framed as an underlying disease with a number of equivalent indicators, but as a network of symptoms that have direct causal influence on each other: insomnia can cause fatigue which then triggers concentration and psychomotor problems. This approach offers new opportunities for constructing an empirically based classification system and has broad implications for future research. (shrink)

Objective: To examine current research payment practices and to inform development of clearer guidelines for researchers and ethics committees.Design: Exploratory email based questionnaire study of current research participant reimbursement practices. A diverse sample of organisations and individuals were targeted.Setting: Australia.Participants: Contacts in 84 key research organisations and select electronic listservers across Australia. A total of 100 completed questionnaires were received with representations from a variety of research areas .Main measurements: Open-ended and fixed alternative questions about type of research agency; type (...) of research; type of population under study; whether payment is standard; amounts and mechanisms of payment; factors taken into account when deciding on payment practices; and whether payment policies exist.Results: Reimbursement practice is highly variable. Where it occurs it is largely monetary and is for time and out-of-pocket expenses. Ethics committees were reported to be often involved in decision making around reimbursement.Conclusions: Research subject payment practices vary in Australia. Researchers who do provide payments to research participants generally do so without written policy and procedures. Ethics committees have an important role in developing guidelines in this area. Specific guidelines are needed considering existing local policies and procedures; payment models and their application in diverse settings; case study examples of types and levels of reimbursement; applied definitions of incentive and inducement; and the rationale for diverse payment practices in different settings. With the passage by virtually every state legislature of healthcare proxy laws, the medical profession increasingly can expect to rely on the participation of surrogates in making decisions on behalf of incompetent patients. Several concerns about the legitimacy of proxy decision making have been discussed in the ethical and general medical literature: the lack of concordance between the views of patients and their surrogates have been documented on multiple occasions, and cases of abuse by proxies or potential conflict of interest (...) have been reported. Another dilemma that deserves discussion arises when proxies demand withdrawal of treatment that physicians and nurses regard as essential to the wellbeing of the patient. Last year, the Joint Commission on Accreditation of Healthcare Organizations for the first time included provisions in its Hospital Accreditation Manual requiring institutions to have mechanisms in place to consider ethical issues arising in the care of patients and to educate care givers and patients on bioethical issues. This new requirement is notably vague. There is no indication of what type of mechanisms would be appropriate or how those involved in considering ethical issues should conduct themselves. This vagueness is by (...) no means accidental, but it reflects a recognition that clinical bioethics is still in its formative stages and that there are currently almost as many different mechanisms for dealing with bioethical issues in the healthcare setting as there are institutions that have such mechanisms. The Joint Commission's action, however, also implies that the time for experimentation will soon be over. (shrink)

In lieu of an abstract, here is a brief excerpt of the content:Reply to Russell's letter of 16 May 1960 by Albert Shalom EDITORIAL NOTE To illustrate a list ofrecent acquisitions in Russell (Summer 1981), we printed in facsimile Russell's letter of 16 May 1960 to Professor Albert Shalom concerning the interpretation of Wittgenstein's Tractatus LogicoPhilosophicus. The correspondence between Russell and Shalom began when Shalom wrote on I May 1960 asking whether Russell had the time and inclination to (...) read a translation of a lecture which Shalom had given the previous December in Paris. The paper was entitled "The Metaphysical Thinking Underlying Wittgenstein's Tractatus". Shalom had received "different and even contradictory appraisals" of his paper, and he very much wanted to have Russell's opinion of it. Close to the age ofninety, Russell was entering a hectic period in his life in which he worked assiduouslyfor the Campaign for Nuclear Disarmament. He had published three books in 1959 and had produced prefaces and introductions for other people. As usual, he was besieged with mail including numerous requests to read unsolicited manuscripts sent to him quite regularly. Yet he had managed to answer correspondence in a very personal way. In November 1959, he had been involved in a controversy regarding the refusal ofthe editor of Mind to review Ernest Gellner's Words and Things, a book which attacked the then prevalent philosophy of linguistic analysis and for which Russell had written the introduction. In the early months of1960, he took time out from his usual writing on the nuclear threat to reply to several articles devoted to his theory of descriptions and his ethical theory in the journal Philosophy. Russell's reply (4 May 1960) to Shalom's first letter states: "Thank you for your letter ofMay I. I shall be interested to see your paper on Wittgenstein's Tractatus, but, as I am very busy, it may be some time before I can read it carefully." On 8 May Shalom wrote back in appreciation enclosing his paper and an offprint of an article, "Qu'est-ce qu'un concept?", Revue internationale dephilosophie, 50 (1959): I-IS. The main objection raised against his paper, Shalom admitted, was that Wittgenstein was not concerned with problems of epistemology at the time of his writing the Tractatus. Nonetheless, Shalom felt that the "picture theory" of reality implies an epistemology, and he was unable to see a fallacy in his paper: "Perhaps the fact that I'm not a logician has something to do with the matter. Ifyou could enlighten me on this point I should be most grateful to you." 45 28th April 1982. 46 Russell winter 1982-83 Russell's letter of 16 May followed. On 25 May Shalom challenged Russell's interpretation. Far /rom underestimating the notion of structure, Shalom claimed that he had emphasized the notion as the key to understanding the Tractatus. "In fact, my main point is that Wittgenstein's analyses of various concepts are unintelligible unless understood as arguments in terms of the dominating idea ofstructure or form. And that is what I understand as a metaphysical argument." Shalom went on to say that his interest in any philosopher was to try to discover the chief concepts employed in the philosopher's analysis and to examine the manner in which the analysis was performed. For this reason, the later Wittgenstein was not devoid ofinterest. In a postscript, he asked Russell whether he would be in London in July ofthat year, and ifso, whether a meeting was possible. Russell replied on 18June as follows: "Thank you for your letter ofMay 25. I am sorry that at present my time and thought are so occupied with matters very remote from those with whichyour letter deals that I cannot offeryou a reasoned reply. Unfortunately, I do not expect to be in London in July so that I am afraid a meeting will hardly be possible." On 14 Shalom requested the return of his paper, and Russell complied on 19 July. Shalom is now Professor ofPhilosophy at McMaster. We invited him to comment on his exchange with Russell, particularly Russell's letter of16 May 1960. Blending social analysis and philosophy, Albert Borgmann maintains that technology creates a controlling pattern in our lives.

Despite recent increasing support for the brain disease model of alcohol and drug addiction, the extent to which the model may clinically impact addiction treatment and client behaviour remains unclear. This qualitative study explored the views of community-based clinicians in Australia and examined: whether Australian community-based clinicians support the BDM of addiction; their attitudes on the impact the model may have on clinical treatment; and their views on how framing addiction as a brain disease may impact addicted clients’ behaviour. Six (...) Australian community-based clinicians participated in semi-structured in-depth interviews that were analysed using thematic analysis. Whilst the BDM was not fully supported by this purposive sample of Australian community-based clinicians, there was acceptance that addiction neuroscience formed a key part of a wider addiction framework. Participants believed the BDM ignored key social, psychological and environmental factors important for successful treatment. The BDM was seen as potentially irrelevant for certain client types, however the model was believed to integrate with particular therapies. Participants believed that for clients viewing their addiction in terms of a brain disease, there were potential positive and negative impacts on client behaviour. Implications for addiction treatment practice and public health policy are discussed. (shrink)

The history of biochemistry and molecular biology is replete with examples of erroneous theories that persisted for considerable lengths of time before they were rejected. This paper examines patterns of dissolution of three such erroneous hypotheses: The idea that nucleic acids are tetrads of the four nucleobases (‘the tetranucleotide hypothesis’); the notion that proteins are collinear with their encoding genes in all branches of life; and the hypothesis that proteins are synthesized by reverse action of proteolytic enzymes. Analysis of these (...) cases indicates that amassed contradictory empirical findings did not prompt critical experimental testing of the prevailing theories nor did they elicit alternative hypotheses. Rather, the incorrect models collapsed when experiments that were not purposely designed to test their validity exposed new facts. (shrink)

The identification of molecules controlling embryonic patterning and their functional analysis has revolutionized the fields of Developmental and Cell Biology. The use of new sequence information and modern bioinformatics tools has enriched the list of proteins that could potentially play a role in regulating cell behavior and function during early development. The recent application of efficient methods for gene knockout in zebrafish has accelerated the functional analysis of many proteins, some of which have been overlooked due to their small size. (...) Two recent publications report on the identification of one such protein and its role in zebrafish embryogenesis. The protein, currently designated Apela, was shown to act as a secreted protein whose absence adversely affected various early developmental processes. Additional signaling proteins that have been identified in one of the studies are likely to open the way to unraveling hitherto unknown developmental pathways and have the potential to provide a more comprehensive understanding of known developmental processes. This paper attempts to signal the potential relevance of A Test of the News, written by Lippmann and Merz, for Critical Discourse Analysis. It seems that the study is overlooked by CDA’s experts as a pioneering work in press analysis. In order to demonstrate links between CDA and the research, in the first part, the work of Lippmann and Merz is situated within a wider picture of the theoretical and historical background as well as common views on politics and the (...) role of the press. Then, the reasons for the choice of the Times as a medium of research and the Russian Revolution as a topic are stated. The authors’ methodological assumptions are discussed in brief. The second part begins with an attempt to define the concept of CDA. The next section presents a theoretical framework of CDA in order to indicate that Lippmann’s and Merz’s analysis demonstrates a similar theoretical approach. Additionally, the tools used by authors are compared with those used in CDA. The parallel between the central role of the media discourse in CDA and the concern of the authors of the role of the press is also discussed. Since the active role of the reader and the hearer in constructing the meaning of the text corresponds with the views of the authors on the discrepancy between the real world and its representation, the next section is devoted to this issue. Finally, readers’ abilities, discussed by Lippmann and Merz, are viewed in relation to the significance of reader’s experiences in constructing the meaning of a text. (shrink)

Cognitive functions like perception, memory, language, or consciousness are based on highly parallel and distributed information processing by the brain. One of the major unresolved questions is how information can be integrated and how coherent representational states can be established in the distributed neuronal systems subserving these functions. It has been suggested that this so-called ''binding problem'' may be solved in the temporal domain. The hypothesis is that synchronization of neuronal discharges can serve for the integration of distributed neurons into (...) cell assemblies and that this process may underlie the selection of perceptually and behaviorally relevant information. As we intend to show here, this temporal binding hypothesis has implications for the search of the neural correlate of consciousness. We review experimental results, mainly obtained in the visual system, which support the notion of temporal binding. In particular, we discuss recent experiments on the neural mechanisms of binocular rivalry which suggest that appropriate synchronization among cortical neurons may be one of the necessary conditions for the buildup of perceptual states and awareness of sensory stimuli.
