Certain health care services and goods, although legal and often generally accepted in a society, are by some considered morally problematic. Debates on conscientious objection in health care try to resolve whether and when physicians, nurses and pharmacists should be allowed to refuse to provide medical services and goods because of their ethical or religious beliefs. These debates have most often focused on issues such as how to balance the interests of patients and health care professionals, and the compatibility of (...) conscientious objection with professional obligations, but it is also possible to think about conscientious objection in terms of how to respond to moral disagreement and the requirements of liberal neutrality. In this book G. A. Cohen examines the libertarian principle of self-ownership, which says that each person belongs to himself and therefore owes no service or product to anyone else. This principle is used to defend capitalist inequality, which is said to reflect each person's freedom to do as as he wishes with himself. The author argues that self-ownership cannot deliver the freedom it promises to secure, thereby undermining the idea that lovers of freedom should embrace capitalism and the inequality (...) that comes with it. He goes on to show that the standard Marxist condemnation of exploitation implies an endorsement of self-ownership, since, in the Marxist conception, the employer steals from the worker what should belong to her, because she produced it. Thereby a deeply inegalitarian notion has penetrated what is in aspiration an egalitarian theory. In this paper, I examine Hannah Arendt’s notion of “enlarged mentality.” I use a close textual exposition of enlarged mentality in Arendt’s writings in order to offer an interpretation of Denktagebuch Notebook XXII, in which Arendt initially sketched her political interpretation of the Critique of Judgment. I maintain that a close examination of enlarged mentality—particularly as it appears in Arendt’s notebooks—answers basic questions about Arendt’s appropriation of Kant’s third Critique that have eluded scholarly commentators. In this paper, I seek to (...) answer one such question: why did Arendt turn to Kant’s Critique of Judgment? I argue that in turning to Kant for a model of political judgment Arendt took herself to be correcting methodological inconsistencies that she believed she located in the Critique of Judgment. A grandfather clause is a provision whereby an old rule continues to apply to some existing situation while a new rule applies to all future cases. This paper focuses on the use of grandfather clauses in health technology appraisals (HTAs) issued by the National Institute for Health and Care Excellence (NICE) in the United Kingdom. NICE provides evidence‐based guidance on healthcare technologies and public health interventions that influence resource allocation decisions in the National Health Service (NHS) and the broader public (...) sector in England and Wales. In this context, a grandfather clause is included when NICE does not recommend treatment with a given technology. The grandfather clause provides an exemption from the general recommendation for patients who have already started treatment with the technology in question, before the publication of the NICE guidance. In this paper we first lay out the contexts in which grandfather clauses occur in NICE guidance, and then consider ethical arguments against and in support of grandfather clauses and the continuation of treatment. We argue that NICE’s current practice of automatic inclusion of a grandfather clause is ethically problematic and unfair. While the inclusion of a grandfather clause may be appropriate and justified in specific cases, we argue that inclusion of such a clause should be considered as part and parcel of the decision making process on a case by case basis, rather than adopted as the default. This book is intended as an exposition of a particular theory of time in the sense of an interrelated set of attempted solutions to philosophical problems about it. Generally speaking there are two views about time held by philosophers and some scientists interested in philosophical issues. The first called the A-theory (after McTaggart's expression A-determinations for the properties of being past, present or future) is often thought to be closer to our commonsense view of time or to the concept of (...) time presupposed by ordinary language. It includes at least the following theses, (a) Logic ought really to include tensed quantifiers for existence on one of its important usages means, present existence. More generally, we can't reduce all tensed locutions to tenseless ones. (b) The distinction between past, present and future is an objective one. If television news viewers are conceived as active audience members, their interpretations should be a crucial factor in the study of the ‘effects’ of television news. Here, viewers’ interpretations are understood as subjective constructions of a news item. In a previous contribution, we argued that interpretations can vary both within and between viewers in regard to the level of complexity. Complexity is the degree to which interpretations are a) differentiated, and b) integrated. In this contribution, we will operationalize the concept (...) of differentiation of television news interpretations by its viewers. Furthermore, we will present a procedure for measuring differentiation based on the thoughts viewers reported while they watched a television news program. Results of a small-scale study provided first indications that the procedure is able to discriminate between viewers with varying levels of differentiation in interpreting television news. (shrink)

The unfairness of health inequalities depends on the more fundamental question of the relationship between justice in health and distributive justice more generally. In this article, I discuss some constraints on how health should be incorporated in a theory of justice and their implications for when health inequalities can be considered to be unfair. I argue against adopting separate distributive principles for health, and in favour of conceiving justice in health as interrelated with, and contingent on, justice in the distribution (...) of other important social goods and resources, in particular income. Accordingly, health inequalities are unfair when they are the result of an unfair distribution of resources. (shrink)

Although interpretation is often considered a vital factor in the effects of news, its conceptualization and operationalization have been problematic. In this study, interpretation is defined in terms of the structural attribute of complexity. In a previous contribution, one aspect of interpretive complexity, differentiation, was operationalized and measured to test the usefulness of the concept in news research. This follow-up study introduces a method for measuring and analyzing a second aspect of interpretive complexity: Integration. Whereas differentiation represents the broadness of (...) interpretations, integration refers to the cohesiveness of interpretations. This contribution describes two dimensions of integration, called micro-integration and macro-integration, and attempts to test their utility by operationalizing and measuring them in a small-scale study. Results illustrate that the method yields data that are helpful in systematically exploring and comparing how viewers interpret television news by assessing differences in cohesiveness. The merits of the concept and method and their use for the study of news effects are evaluated. (shrink)

This article discusses Ronald Preston's understanding of William Temple and the relationships between the two thinkers. It shows how both develop a theology of Christian realism which places great emphasis on the autonomy of the social sciences and the importance of economic expertise. Questions are raised about the appropriateness of this method, as well as their understanding of the state as an order of creation: these can easily lead to the reduction of the sphere of political morality and its substitution (...) with a form of technical rationality. After a brief discussion of the cult of the expert and the manager in contemporary British politics, and the limitation of political action through the rhetoric of ‘there is no alternative', the article concludes by calling for a remoralisation of political life against an economic reductionism which threatens to remove ethics from politics altogether. A theory is presented to explain how accurate, single-joint movements are controlled. The theory applies to movements across different distances, with different inertial loads, toward targets of different widths over a wide range of experimentally manipulated velocities. The theory is based on three propositions. (1) Movements are planned according to “strategies” of which there are at least two: a speed-insensitive (SI) and a speed-sensitive (SS) one. (2) These strategies can be equated with sets of rules for performing diverse movement tasks. (...) The choice between SI and SS depends on whether movement speed and/or movement time (and hence appropriate muscle forces) must be constrained to meet task requirements. (3) The electromyogram can be interpreted as a low-pass filtered version of the controlling signal to the motoneuron pools. This controlling signal can be modelled as a rectangular excitation pulse in which modulation occurs in either pulse amplitude or pulse width. Movements to different distances and with loads are controlled by the SI strategy, which modulates pulse width. Movements in which speed must be explicitly regulated are controlled by the SS strategy, which modulates pulse amplitude. The distinction between the two movement strategies reconciles many apparent conflicts in the motor control literature. (shrink)
