The process of deciding which books academics submit should be published favors authors who are associated with the most prestigious universities and other research institutions. Some feel this bias could be minimized if the review of academic books were carried out as anonymously as is the review of articles for journal publication. Not likely to happen soon, however, because both academic and publishing industries promote the hierarchy of perceived excellence that permeates the process of publishing academic books. To (...) find this process to be in any way unethical one needs to move beyond individual-oriented approaches and adopt a social perspective that fosters fundamental fairness, equal opportunity, and equal respect. Henry Richardson has recently published the first book ever devoted to ancillary care obligations, which roughly concern what medical researchers are morally required to provide to participants beyond what safety requires. In it Richardson notes that he has presented the ‘only fully elaborated view out there’ on this topic, which he calls the ‘partial-entrustment model’. In this article, I provide a new theory of ancillary care obligations, one that is grounded on ideals of communion salient in the African (...) philosophical tradition and is intended to rival and surpass Richardson’s model, which is a function of Western considerations of autonomy. Originally published thirty years ago, Critique of the Legal Order remains highly relevant for the twenty-first century. Here Richard Quinney provides a critical look at the legal order in capitalist society. Using a traditional Marxist perspective, he argues that the legal order is not intended to reduce crime and suffering, but to maintain class differences and a social order that mainly benefits the ruling class. Quinney challenges modern criminologists to examine their own positions. As "ancillary agents of power," criminologists (...) provide information that governing elites use to manipulate and control those who threaten the system. Quinney's original and thorough analysis of "crime control bureaucracies" and the class basis of such bureaucracies anticipates subsequent research and theorizing about the "crime control industry," a system that aims at social control of marginalized populations, rather than elimination of the social conditions that give rise to crime. He forcefully argues that technology applied to a "war against crime," together with academic scholarship, is used to help maintain social order to benefit a ruling class. Quinney also suggests alternatives. Anticipating the work of Noam Chomsky, he suggests we must first overcome a powerful media that provides a "general framework" that serves as the "boundary of expression." Chomsky calls this the manufacture of consent by providing necessary illusions. Quinney calls for a critical philosophy that enables us to transcend the current order and seek an egalitarian socialist order based upon true democratic principles. In 2016, Discourse Studies published a special issue on the ‘epistemics of epistemics’ comprising six papers, all of which took issue with a strand of my research on how knowledge claims are asserted, implemented and contested through facets of turn design and sequence organization. Apparently coordinated through some years of discussion, the critique is nonetheless somewhat confused and confusing. In this article, I take up some of more prominent elements of the critique: my work is ‘cognitivist’ substituting causal psychological analysis (...) for the classic conversation analytic focus on the normative accountability of social action, my work devalues and indeed flouts basic tenets of CA methodology such as the ‘next-turn proof procedure’, my analysis of epistemic stance introduces unwarranted themes of conflict and hostility into CA thinking, various concepts that I have introduced involve the invocation of ‘hidden orders’ of social conduct that is inimical to the traditions of our field and that my work rests on an unwarranted ‘informationism’ – the discredited idea that much of human interaction is driven by a need to traffic in information. In this rebuttal, I refute all of these commentaries and correct many other ancillary mistakes of representation and reasoning that inhabit these papers. (shrink)

Understanding the nature of science has long been a desired outcome of science education, despite ongoing disagreements about the content, structure, and focus of NOS expectations. Addressing the concern that teachers likely focus only on student learning expectations appearing in standards documents, this study examines the current state of NOS in science education standards documents from nine diverse countries to determine the overt NOS learning expectations that appeared, NOS statements provided near those learning expectations, but not identified as learning outcomes, (...) and NOS statements found in ancillary text. Findings indicate that NOS ideas rarely occur as expectations for student learning and are far more commonly found in ancillary material. Moreover, consensus was not apparent in the overt learning outcomes for students. Given the well-documented poor state of NOS instruction and the consistent lack of NOS appearing in published curriculum materials, the NOS standards appearing in nearly all documents analyzed are unlikely to provide sufficient conceptual or pedagogical support for NOS to be accurately interpreted or translated into meaningful experiences for students. The World Brain Death Project clarified many aspects of the diagnosis of brain death/death by neurologic criteria. Clearer descriptions than previously published were presented concerning the etiology, prerequisites, minimum clinical criteria, apnea testing targets, and indications for ancillary testing. Nevertheless, there remained many epistemic and metaphysical assertions that were either false, ad hoc, or confused. Epistemically, the project was not successful in explaining away remaining brain functions, complex reflexes as “spinal,” the risk and lack of utility of the apnea (...) test, the ignored and often present confounders of central endocrine dysfunction and high-cervical-spinal-cord injury, the limitations of ancillary tests, or the cases of reversibility of some findings of brain death/death by neurologic criteria. Metaphysically, the World Brain Death Project variously suggested different concepts of death that were not supported with argument. Concepts offered included simply restating the criterion of brain death/death by neurologic criteria; personhood, without recognizing it is a higher-brain concept; and emergent functions of the organism as a whole, without specifying what these might be, if not biologic anti-entropic integration that actually remains after brain death/death by neurologic criteria. The World Brain Death Project only offered confused metaphysical discussion, and gave no reason why the state they described as brain death/death by neurologic criteria should be considered death itself. The main epistemic and metaphysical problems with brain death/death by neurologic criteria remain untouched by the World Brain Death Project. (shrink)

New York: Oxford University Press, 1998.An edition of Hume's philosophic writings on rigorous, modern bibliographic principles has long been a scholarly desideratum. Readers in the many fields in which Hume's thought and style have made a profound impression have (...) basically relied on the originals or derivatives of efforts of the Victorian era. One is Hume's Philosophical Works (4 v., London: Longmans, Green & Co., 1874-75), edited by the Oxford dons, T. H. Green and T. H. Grose. Well-known for his idealist philosophy and theory of liberalism, Green contributed introductions to the first and second volumes criticizing Hume from an anti-empirical standpoint. Grose offered a "History of [Hume's] Editions" as an introduction to the third volume, and claimed to have revised Hume's text throughout. This edition has the virtue of comprehensiveness, but is deficient in its offering of variant readings. Later there came from Oxford University Press editions of the Treatise of Human Nature (1888), deemed generally sound, and the two Enquiries (1894), marred by departures from Hume's text, prepared by a prominent civil servant, Sir Lawrence Amherst Selby-Bigge, whose chief contribution was to provide useful analytical indices. An attempt at achieving higher editorial standards was made by P. H. Nidditch, Professor of Philosophy in the University of Sheffield, in a "second" edition of Selby-Bigge's Treatise (1976) and a "third" edition of his text of the Enquiries (1975). Limits were imposed by Oxford University Press, apparently unwilling to make more then small changes in the existing pages. Nidditch announced (Enquiries, 1975, at p.348, n.2) that he had a "fresh" edition in preparation, but he did not live to complete this project.In the event, the challenge to produce a critical edition of Hume's philosophical, political, and literary writings was canvassed at early meetings of the Hume Society formed in 1973. Thereafter, in 1982, a team of general editors was formed by T. L. Beauchamp, D. F. Norton, and M. A. Stewart, determined to organize and finish the task in hand, and Oxford University Press finally conceded it was time to replace Selby-Bigge. He wrote an "Advertisement" to this effect, and it was inserted as a preface to the authoritative and posthumous 1777 edition of his Essays and Treatises. Their phenomenological approach to the human world has (...) exerted such a powerful influence in recent years that one can hardly enter a discussion about the philosophical meaning of hermeneutics that is not already framed in the terms of their discourse. At the same time, there is less familiarity with the origins and sources of the hermeneutic method or with the thought of Wilhelm Dilthey. The editors of Dilthey’s Selected Works, Rudolf Makkreel and Frithjof Rodi, have set about trying to rectify this situation. They have sought to provide a richer context for the study of hermeneutics by bringing out a six-volume collection of writings from one of the major hermeneutical philosophers in the Western tradition. The publication of Volume Four in this series helps to accomplish this task by offering new translations of important Dilthey texts covering both the history and genesis of hermeneutics and a genealogical account of Dilthey’s own hermeneutical approach to history. Only one short essay in this volume, “The Rise of Hermeneutics” (1900), has been published before, and even this older translation has been revised and supplemented by material from the Nachlass. But what is the significance of this material and how does it help to achieve a fuller understanding of hermeneutics and its history?The selections in Volume Four cover the period from 1860 to 1903 with a heavy emphasis on the period from 1860 to 1868 (over two-thirds of the book focuses on Dilthey’s 1860 Preisschrift on “Schleiermacher’s Hermeneutical System in Relation to Earlier Protestant Hermeneutics,” notes from his 1867–68 lectures “On Understanding and Hermeneutics,” and three reviews from 1862 on historical literature from Buckle, Burckhardt, and F.C. Schlosser). The remaining essays from 1900–1903 provide a sense of Dilthey’s later reflections on hermeneutics and on the meaning of the eighteenth century for historiography and as a philosophical approach to history. The editors of this collection have chosen these texts carefully; the selections in Volume Four are organized around a specific theme. By culling significant early texts about hermeneutics and history and juxtaposing them with a number of later texts on the same topics, they hope to show the genesis of Dilthey’s own path of thought. What emerges from an attentive reading of these sources—especially when carried out within the context of the editors’ own principle of selection—is a highly persuasive view of Dilthey and his work. The front pages of newspapers have featured stories about grandmothers giving birth to their own grandchildren, couples "renting" wombs from surrogates, and researchers prepared to transplant fetal ovaries into women unable to produce viable eggs. With each new and bolder foray into reproductive realms, the question (...) of whether to set limits on the use of such technologies surfaces. An ancillary debate inevitably ensues about the proper forum for discussing and deciding such limits. The recent announcement of human embryo splitting or cloning research is no exception to this pattern. Articles in both the popular press and academic circles in the wake of the reported research contain lamentations about the absence of a national ethics body that could address the ethical issues raised by the cloning experiment. This research also breathed new life into congressional calls to create a government bioethics commission.1At times, it appears that the substantive questions raised by a procedure like cloning are subsumed under the debate about whether this ethics group or that should be convened to study the matter. But the forum in which options are discussed is as crucial to the development of sound ethical thought and public policy as are the conclusions themselves. Thus, we address the question of whether a private, as opposed to a public, bioethics body provides a distinctively useful forum for reaching an understanding of issues such as cloning that have significant ethical and public policy implications.A 1993 Office of Technology Assessment report, in which the authors suggest that a new federal bioethics commission might be useful, has helped to frame recent discussions about the value of government-sponsored bioethics bodies (Office of Technology Assessment 1993). To most people, the question of whether a national forum for bioethics is needed is synonymous with the question of whether a new federal body should be convened. Yet the existence of the National Advisory Board on Ethics in Reproduction (NABER) suggests that the scope of the question should be broadened. [End Page 283]NABER was born in 1992, in part as a response to an Institute of Medicine report on infertility. That document endorsed the formation of a nongovernment body to develop guidelines for assisted reproductive measures and fetal research in the event that a government body was not forthcoming (Institute of Medicine 1989). NABER, which was founded by the American College of Obstetricians and Gynecologists and the American Fertility Society, now functions wholly independently of those groups and is supported by private foundations including the Ford, Greenwall, and Josiah Macy, Jr., Foundations and the Walter and Elise Haas Fund. NABER has issued a report on ethical issues related to the use of fetal tissue (Cohen and Jonsen 1993), is about to publish a report and background papers on oocyte donation, and presents a report on human cloning in this issue of the Kennedy Institute of Ethics Journal. In addition, it has developed a protocol for NABER review of research proposals from federal agencies subject to federal guidelines on the use of human subjects in research.Thus, in function and form, NABER was envisioned as a substitute for a government body, and it now functions in a manner similar to several types of government bioethics groups. In lieu of an abstract, here is a brief excerpt of the content:Journal of the History of Ideas 65.3 (2004) 455-474 [Access article in PDF] "Conscience the Ground of Consciousness": The Moral Epistemology of Coleridge's Aids to Reflection Jeffrey Hipolito Everett Community College. It will hardly come as a shock to the readers of this journal that Kant has been the philosophical gatekeeper of all those who have come after him and that the scale of his achievement was recognized even (...) by his immediate peers and the generation which followed them.1 It may be rather more surprising that Samuel Taylor Coleridge belongs among the most interesting members of that second generation, comparable to Fichte, Schelling, Schopenhauer, and Hegel in the originality of his response—not because of the Biographia Literaria (1817), the book for which he is now most famous, but because of the stream of work that followed the publication of the revised Friend (1818). Though the Collected Coleridge project has made available a mass of previously unpublished material, which in turn has stimulated a great deal of interesting research, one must still agree with Elinor Shaffer's recent assessment that "we have no accurate or adequate conception of the late Coleridge from 1819 to his death in 1834."2 Shaffer's range of dates is not accidental, indicating that the revised [End Page 455] Friend (and, more specifically, the break with Schelling) marks the beginning of the "late" Coleridge. In the "Essays on Method" that were a part of the revised Friend (1818) Coleridge effectively freed himself of Schelling's Idealism, which is so notoriously crucial to philosophical chapters of the Biographia, by moving away from the latter's metaphysics of the Absolute towards a metaphysics grounded in an ongoing and in principle unending exploration of "inquiry" itself.3 Though it did not appear until 1825, Aids to Reflection was the first book published by Coleridge following the revised Friend. It was his most popular and influential work during the nineteenth century, but it has been read almost exclusively in strictly theological terms. The implicit contention that the "middle" Coleridge of the Biographia was philosophical and that the "late," post-Biographia Coleridge turned to theology as a refuge from the Biographia's failure was ratified by Thomas McFarland's classic argument concerning the late Coleridge's "trinitarian resolution" to the problems that the middle Coleridge could not solve.4 One pernicious effect of this view (which is more nuanced than I have presented it here) is that until fairly recently it was still possible to describe Aids to Reflection as simply "an effort to argue away the terrifying import of The Ancient Mariner, which would lead us, since it jars our moral feelings, to hatred of God."5I argue in this essay that although Aids to Reflection employs theological vocabulary it is more than—indeed, cannot rightly be considered—an exercise in dogmatic Christian apologetics. More specifically, I will show that the book includes a vigorous engagement with Kant's moral philosophy not primarily by undercutting the metaphysical basis of the critical project's architectonic, though this is an ancillary effect, but by putting pressure on the model of judgment that underlies Kantian ethics.The importance of Kant to Coleridge's intellectual development is not a secret. In what it will be convenient to call “the Scandinavian school”; of jurisprudence, Hagerstrom is clearly the master. But his leadership is of a somewhat special kind. For all that he wrote a large book on Roman law, Hägerström was trained as, and continued to be, a philosopher, not a jurisprudentialist or a sociologist. His essays on law and morals are ancillary to his main purpose: to destroy transcendental metaphysics. The epigraphhe chose to head his contribution to Die Philosophic (...) der Gegenwart in Selbstdarstellungen was uncompromising: “Praeterea censeo metaphysicam esse delendam.” If, in his published work, he to so considerable a degree concentrated his attention on ethics and jurisprudence, that is because he took them to be a particularly rich source of metaphysical mystery-mongering. Through a study of men's moral and legal ideas, Hägerström thought he could bring out the sources, the defects—and even, in a way, the strength—of metaphysical thinking. (shrink)

In terms of the details of Plato's life, the composition and order of the Dialogues and Epistles, and the political and scholastic climate of Plato's Athens and the broader Hellenic culture, this is a daringly imaginative book; critics may find it too imaginative. Ryle argues against the authenticity of all the Epistles, basing his conclusion on a bit of close detective work involving the date of Dionysius I's death and the date of Plato's invitation to Syracuse: Epistle VII is all (...) a bit of baseless Dionist propaganda. Ryle's conjecture that the majority of the Dialogues were designed for public dramatization and open competition, on the model of the Greek tragedies, at local and Olympic games is a distinct possibility. But will we be convinced by the many "straws" that support the hypothesis that the Apology-Crito-Phaedo sequence of Dialogues was written by Plato, not to justify Socrates, but to defend himself against a contemporary charge of defamation? that the Academy was founded in 371, not 387? that the Phaedrus was composed in 361-360, after the Laws? that the Republic was never published in Plato's lifetime? While Ryle vigorously supports all these contentions, inconsistencies or more plausible hypothesis remain. The book is unfortunately completely silent about any of the substantive issues in Plato's philosophy; but perhaps enough has been gained if Plato scholars are sent scurrying back to the ancillary Greek philosophies and histories, not to mention the stylometrists.—E. New York: Oxford University Press, 1998.An edition of Hume's philosophic writings on rigorous, modern bibliographic principles has long been a scholarly desideratum. Readers in the many fields in which Hume's thought and style have made a profound impression have (...) basically relied on the originals or derivatives of efforts of the Victorian era. One is Hume's Philosophical Works (4 v., London: Longmans, Green & Co., 1874-75), edited by the Oxford dons, T. H. Green and T. H. Grose. Well-known for his idealist philosophy and theory of liberalism, Green contributed introductions to the first and second volumes criticizing Hume from an anti-empirical standpoint. Grose offered a "History of [Hume's] Editions" as an introduction to the third volume, and claimed to have revised Hume's text throughout. This edition has the virtue of comprehensiveness, but is deficient in its offering of variant readings. Later there came from Oxford University Press editions of the Treatise of Human Nature (1888), deemed generally sound, and the two Enquiries (1894), marred by departures from Hume's text, prepared by a prominent civil servant, Sir Lawrence Amherst Selby-Bigge, whose chief contribution was to provide useful analytical indices. An attempt at achieving higher editorial standards was made by P. H. Nidditch, Professor of Philosophy in the University of Sheffield, in a "second" edition of Selby-Bigge's Treatise (1976) and a "third" edition of his text of the Enquiries (1975). Limits were imposed by Oxford University Press, apparently unwilling to make more then small changes in the existing pages. Nidditch announced (Enquiries, 1975, at p.348, n.2) that he had a "fresh" edition in preparation, but he did not live to complete this project.In the event, the challenge to produce a critical edition of Hume's philosophical, political, and literary writings was canvassed at early meetings of the Hume Society formed in 1973. Thereafter, in 1982, a team of general editors was formed by T. L. Beauchamp, D. F. Norton, and M. A. Stewart, determined to organize and finish the task in hand, and Oxford University Press finally conceded it was time to replace Selby-Bigge. With the appearance of Tom Beauchamp's "critical edition" of the second Enquiry (hereafter identified as EPM), published on 4 August 1999, but bearing the date 1998, the long wait for a reliable text, bibliographic apparatus, and ancillary scholarship, worthy of Hume, must be declared over.If we grant that it is appropriate to introduce the new Clarendon Edition of the Works of David Hume with the piece he deemed "best," it must also be asked why he made this judgment. Philosophers have generally held that his Treatise of Human Nature (1739-40) is the most profound and searching statement of his epistemological and moral thought, but its seeming lack of immediate success with readers made Hume dissatisfied with it. Such were his negative feelings, coupled with irritation at attacks on him focused on this "juvenile" work, that he partially repudiated the Treatise at the end of his life. He wrote an "Advertisement" to this effect, and it was inserted as a preface to the authoritative and posthumous 1777 edition of his Essays and Treatises. Ancillary care is care that research participants need that is not essential to make the research safe or scientifically valid and is not needed to remedy injuries that eventuate as a result of the research project itself. Ancillary care duties have recently been defended on the grounds of beneficence, entrustment, utility and consent. Justice has also been mentioned as a possible basis of ancillary care duties, but little attention has been paid to this approach. How international research might contribute to justice in global health has not been substantively addressed by bioethics. This article describes how the provision of ancillary care can link international clinical research to the reduction of global health disparities. It identifies the ancillary care obligations supported by a theory of global justice, showing that Jennifer Ruger’s health capability paradigm requires the delivery of ancillary care to trial participants for a limited subset of conditions that cause severe morbidity and (...) mortality. Empirical research on the Shoklo Malaria Research Unit’s (SMRU) vivax malaria treatment trial was then undertaken to demonstrate whether and how these obligations might be upheld in a resource-poor setting. Our findings show that fulfilment of the ancillary care obligations is feasible where there is commitment from chief investigators and funders and is strongly facilitated by SMRU’s dual role as a research unit and medical non-governmental organization. This paper argues that, in a community of rights, the prima facie responsibilities of researchers to attend to the ancillary-care needs of their participants would be determined by a four-stage test . This test, it is suggested, sets a standard for common law courts that are invited to recognize the ancillary-care responsibilities of researchers, whether as a matter of contract or tort law.

It is axiomatic that the first responsibility of researchers, whether they are working in the developed or the developing world, is to do no harm to those who participate in their studies or trials. However, on neither side of the Atlantic is there any such settled view with regard to the responsibility of researchers to attend to the ancillary-care needs of their participants – that is, a responsibility to advise or assist participants who have medical condition X in circumstances (...) where the research concerns medical condition Y, and the research did not contribute to the presence of condition X in participants, nor did the having of condition X contribute to the research. Consider, for example, the following hypothetical posed by Leah Belsky and Henry Richardson:Researchers testing a new treatment for tuberculosis in a developing country discover some patients have HIV infection. Researchers do not owe their subjects the same level of care that physicians owe patients, but they owe more than merely what the research protocol stipulates. In keeping with the dynamics of the relationship between researcher and subject, they have limited but substantive fiduciary obligations.

In this article, I provide a new account of the basis of medical researchers' ancillary care obligations. Ancillary care in medical research, or medical care that research participants need but that is not required for the validity or safety of a study or to redress research injuries, is a topic that has drawn increasing attention in research ethics over the last ten years. My view, the relationship-based approach, improves on the main existing theory, Richardson and Belsky's ‘partial-entrustment model’, (...) by avoiding its problematic restriction on the scope of health needs for which researchers could be obligated to provide ancillary care. Instead, it grounds ancillary care obligations in a wide range of morally relevant features of the researcher-participant relationship, including the level of engagement between researchers and participants, and weighs these factors against each other. I argue that the level of engagement, that is, the duration and intensity of interactions, between researchers and participants matters for ancillary care because of its connection to the meaningfulness of a relationship, and I suggest that other morally relevant features can be grounded in researchers' role obligations. The philosopher Henry Richardson's short book is a defense of a position on a neglected topic in medical research ethics. Clinical research ethics has been a longstanding area of study, dating back to the aftermath of the Nazi death-camp doctors and the Tuskegee syphilis study. Most ethical regulations and institutions have developed in response to those past abuses, including the stress on obtaining informed consent from the subject. Richardson points out that that these ethical regulations do not address one of (...) the key dilemmas faced by medical researchers -- whether or not they have obligations towards subjects who need care not directly related to the purpose of the study, termed 'ancillary care obligation'. Recent work on incidental fndings, concentrating on the difcult problems posed by the ambiguous results often generated by high-tech medicine, has proceeded largely independently from recent work on medical researchers' ancillary-care obligations, the obligations that researchers have to deal with diseases or conditions besides the one(s) under study. This paper contends that the two topics are morally linked, and specifcally that a sound understanding of ancillary-care obligations will center them on incidental fndings. The paper sets out and defends (...) an understanding of ancillary-care obligations, which is based on the idea that when participants signed up for a study they may — independently of their beliefs and expectations and of those of the researchers — be taken to have partially entrusted certain aspects of their health into the researchers' hands. This partial-entrustment model of ancillary-care obligations, in turn, has substantive implications for how to deal ethically with incidental fndings; for instance, it suggests that researchers have no moral obligation to hunt for incidental fndings. This paper explores the convergence of two recent and growing streams of bioethical work and concern. Each has originated independently, but each arises from the fact that the Common Rule that has shaped medical research ethics, as institutionalized in the United States and also abroad, is largely silent about what needs to be done in response to researchers’ positive obligations. One stream concerns what to do about the sometimes vast range of findings that may arise incidentally to performing research procedures. (...) We argue, primarily by appeal to phenomenological considerations related to the experiential aspects of agency, that belief fixation is broadly agentive; although it is rarely voluntary, nonetheless, it is phenomenologically agentive because of its significant phenomenological similarities to voluntary-agency experience. An important consequence is that epistemic rationality, as a central feature of belief fixation, is an agentive notion. This enables us to introduce and develop a distinction between core and ancillary epistemic virtues. Core epistemic virtues involve several inter-related kinds (...) of epistemic rationality in belief fixation. Other “habits of mind” pertinent to belief fixation constitute ancillary epistemic virtues. Health researchers working in low-resource settings routinely encounter serious unmet health needs for which research participants have, at best, limited treatment options through the local health system (Taylor, Merritt, and Mullany 2011). A recent case discussion features a study conducted in Bamako, Mali (Dickert and Wendler 2009). The study objective was to see whether children with severe malaria develop pulmonary hypertension in order to improve the general understanding of morbidity and mortality associated with malaria. The problem of fake news and the spread of misinformation has garnered a lot of attention in recent years. The incentives and norms that give rise to the problem, however, are not unique to journalism. Insofar as academics and journalists are working towards the same goal, i.e., publication, they are both under pressures that pervert. This chapter has two aims. First, to integrate conversations in philosophy of science, epistemology, and metaphilosophy to draw out the publishing incentives that promote analogous (...) problems of fake news and misinformation in academic research communities more broadly. Second, to argue for a (partial) solution. This chapter argues that research communities benefit when authors believe what they argue for in print and it warns against attempts to loosen our publication norms to permit publishing without belief. This comment on the case presented in ‘Cholera and Nothing More’ argues that the physicians at this public-health centre did not have an ordinary clinician's obligations to promote the health of the people who came to them for care, as they were instead set up to serve a laudable and urgent public-health goal, namely, controlling a cholera outbreak. It argues that, nonetheless, these physicians did have some limited moral duties to care for other diseases they encountered—some ancillary-care duties—arising from (...) their voluntarily entering into a kind of intimate relationship with the patients they took in, one in which those patients effectively waive certain rights to bodily and medical privacy. In this paper, we discuss challenges associated with implementing a policy for Ancillary Care (AC) for related and unrelated (serious) adverse events during an Ebola vaccine trial conducted in a remote area of the Democratic Republic of the Congo. Conducting clinical trials in resourceconstrained settings can raise context-related challenges that have implications for study participants’ health and wellbeing. During the Ebola vaccine study, three participants were injured in road traffic accidents, but there were unexpected difficulties when trying to apply (...) the AC policy. First, because of the nature of the adverse events, the insurer refused to cover the costs. Second, the AC policy did not address treatments by traditional medicine, even though traditional medicines are frequently used and highly trusted in the study community. This highlighted a contrast between the researchers’ well-intentioned AC approach and the participants’ legitimate preferences. The way in which researchers should address their responsibility to provide AC is not straightforward; it requires contextualization. Our experience highlights the importance of involving community representatives and the local ethics committee to ensure development of an AC policy that is culturally and ethically appropriate. Additionally, the insurance contract should clearly stipulate which adverse events are linked to the trial participation, and thus eligible for coverage, to avoid controversies when claims are made. Introduction Numerous guidelines and policies for ethical research practice have evolved over time, how this translates to global health practice in resource-constrained settings is unclear. The purpose of this paper is to describe how the concept of ancillary care has evolved over time and how it is included in the ethics guidelines and policy documents that guide the conduct of research in the global south with both an international focus and providing a specific example of Malawi, where the first (...) author lives and works, as a case study. Methods Discourse analysis was conducted on 34 international ethics guidelines and policy documents. Documents were purposively selected if they contained a set of key terms that reflect the concept of ancillary care. Following a process of inductive discourse analysis, five key interrelated text phrases relating to ancillary care were extracted from the documents. The evolution of these phrases over time was explored as they represented the development of the concept of ancillary care as a component of ethical health research guidance and practice. Results We found key interrelated phrases that represent discourses regarding the evolution of ancillary care including participant protection; provide care as appropriate; supererogation; patient needs prevail over science; and ancillary care as an obligation. Arguments for the provision of ancillary care were characterised by safeguarding the safety, health rights and well-being of study participants. However, despite the evolution of discourse around ethical obligations to provide ancillary care, this is rarely made explicit within guidance documents, leaving interpretive space for differential application in practice. Conclusion While there have been major changes to the ethics guidance that reflect significant evolution in the ethical conduct of research, the specific vocabulary or language used to explain the ethics of researchers' ancillary care obligations to the health needs of their research participants, lacks clarity and consistency. As a result, the concept of ancillary care continues to be under-represented in local ethical guidelines and regulations, with no clear directives for country-level research ethics committees to apply in regulating ancillary care responsibilities.
