There is general agreement that the principles espoused by the Global Economic Ethic Manifesto are to be commended. Despite this, the expectations of wide adoption of the Ethic Manifesto that were expressed at the 2009 launch have proved to be overly optimistic as Hemphill and Lillevik discovered in their study. They propose a number of recommendations to address some of the Ethic Manifesto’s limitations and increase adoption of it, particularly by organizations. However, it is not clear that, even if all (...) their recommendations were to be fully implemented, organizational adoption rates of the Ethics Manifesto will reach the same level as those of the UN Global Compact. For this to occur, the Ethics Manifesto must move beyond providing a moral compass to individuals in order to inform organizational systems and structures in a meaningful way. (shrink)

What is the relationship between electoral and economic performance? Previous literature posits that poor economic performance hurts the incumbent at the ballot box because overall economic performance serves as a competence signal, which voters can readily access at low costs. Building on an emerging economic voting literature exploring heterogeneity in the electorate, this article argues that social groups are affected differently by various dimensions of economic performance and that their sociotropic sanctioning of incumbents is contingent on the retrospective performance (...) of these dimensions. It theorizes how four social groups—low-skilled workers, pensioners, public sector employees, and high-income individuals—are differently affected by each of four economic dimensions: unemployment, inflation, stock market performance, and public spending; as a result, they penalize the incumbent to varying extents. Results from a multilevel logistic regression analysis from four modules of the Comparative Study of Electoral Systems containing around seventy electoral contexts are consistent with the argument. Attributions of what we shall call normative responsibilities play a central role in everyday moral thinking. It is commonly thought, for example, that parents are responsible for the wellbeing of their children, and that this has important normative consequences. Depending on context, it might mean that parents are morally required to bring their children to the doctor, feed them well, attend to their emotional needs, or to see to it that someone else does. Similarly, it is sometimes argued that countries (...) that emit most greenhouse gases are responsible for preventing catastrophic climate change. This responsibility might imply that these countries are morally required to take necessary steps individually and jointly to come to an agreement on and implement a workable plan, and to avoid steps that worsen the situation. More trivially, the grading of your student’s essays might be your responsibility, as might making sure there is wine at tomorrow’s picnic, and you might thus be required to see to it that essays are competently graded and suitable wine brought to the picnic. -/- Though attributions of normative responsibilities are legion, such responsibilities have received surprisingly little philosophical attention compared to its normative relatives, obligations and reasons, and compared to retrospective responsibility. This chapter hopes to improve on this situation by taking on two main tasks. The first, attempted in section 1, is to spell out the general structure of normative responsibility, in particular the relation between normative responsibilities and corresponding obligations and demands. We suggest that normative responsibilities are constituted by normative requirements that the responsible agents care appropriately about how well things go in certain regards, and that obligations generally can be seen as straightforward upshots of requirements to care. -/- The second task, taken on in section 2, is to provide an overview of prominent sources of normative responsibility and its distribution among agents. Why would the children’s wellbeing be the parents’ responsibility? Why not the neighbor’s, or the state’s, or everyone’s? Here we discuss a range of possible sources, including agents’ abilities, costs involved in taking on the responsibility in question, retrospective responsibility for the situation, promises or contracts, and certain social relationships. IntroductionConcussion is a growing public health concern. No uniformly established therapy exists; neurofeedback studies report treatment value. We use infralow frequency neuromodulation to remediate disabling neurological symptoms caused by traumatic brain injury and noted improved outcomes with a novel concussion protocol. Postconcussion symptoms and persistent postconcussion symptoms are designated timelines for protracted neurological complaints following TBI. We performed a retrospective study to explore effectiveness of ILF in PCS/PPCS and investigated the value of using this concussion protocol.MethodPatients with PCS/PPCS seen (...) for their first neurology office visit or received their first neurofeedback session between 1 August 2018 and 31 January 2021 were entered. Outcomes were compared following treatment as usual vs. TAU with ILF neurotherapy. The study cohort was limited to PPCS patients; the TAU+ILF group was restricted further to PPCS patients receiving at least 10 neurotherapy sessions. Within the TAU+ILF group, comparisons were made between those who trained at least 10 sessions using concussion protocol and those who trained for at least 10 sessions of ILF regardless of protocol.ResultsAmong our resultant PPCS cohort leading persistent neurological complaints were headache, memory impairment, and brain fog. PPCS patients in TAU+ILF+CP demonstrated greater net and percent improvement of symptoms compared to PPCS subjects in TAU. PPCS patients in TAU+ILF-CP trended toward significant symptom improvements compared to TAU, and TAU+ILF+CP trended toward greater efficacy than TAU+ILF-CP.ConclusionPPCS patients who received TAU+ILF+CP demonstrated significantly greater improvement as a group when compared to TAU. When used as an integrative modality to treatment as usual in managing patients with PPCS, ILF neuromodulation with use of concussion protocol provided significant symptom improvements. (shrink)

Many philosophers think the non-identity problem undermines the ability for future generations to have been wronged by past ones. This problem has prompted a number of responses, some of which purport to vindicate the relevant claims of wrongdoing. However, I argue that a closely related issue remains even for those convinced by these responses. It is commonly thought that wrongdoing makes certain retrospective attitudes, such as resentment, fitting toward the wrongdoer. In this paper, I shift a familiar problem of (...) future generations from wrongdoing to the fittingness of retrospective attitudes, and I show that a narrative sense of identity provides one means of addressing the puzzle of how these attitudes can be fi tting in non-identity cases. BackgroundCognitive impairments are prevalent in patients with unipolar and bipolar depressive disorder. Considering the fact assessing cognitive functions is increasingly feasible for clinicians and researchers, targeting these problems in treatment and using them at baseline as predictors of response to treatment can be very informative.MethodIn a naturalistic, retrospective study, data from 120 patients with UDD and BDD were analyzed. Patients received 20 sessions of bilateral rTMS and were assessed regarding their depressive symptoms, sustained attention, working memory, and executive functions, (...) using the Beck Depression Inventory and Neuropsychological Test Automated Battery Cambridge, at baseline and after the end of rTMS treatment course. Generalized estimating equations and logistic regression were used as the main statistical methods to test the hypotheses.ResultsFifty-three percentage of all patients responded to treatment. In particular, 53.1% of UDD patients and 46.9% of BDD patients responded to treatment. Bilateral rTMS improved all cognitive functions except for visual memory and resulted in more modulations in the working memory of UDD compared to BDD patients. More improvements in working memory were observed in responded patients and visual memory, age, and sex were determined as treatment response predictors. Working memory, visual memory, and age were identified as treatment response predictors in BDD and UDD patients, respectively.ConclusionBilateral rTMS improved cold cognition and depressive symptoms in UDD and BDD patients, possibly by altering cognitive control mechanisms, and processing negative emotional bias. As consumer lifestyles have changed over recent decades, people have increasingly turned to meals prepared away from home. A major consequence of this shift in eating patterns has been a concomitant rise in obesity rates worldwide. Research has consistently documented that consumers tend to make less healthy choices when purchasing prepared meals away from home. In part, this can be attributed to inadequate information at the time of purchase; both nutrition experts and lay consumers tend, for example, to underestimate calories (...) in food purchased away from home and thus portion sizes tend to be larger. Thus, by requiring restaurants and other purveyors of prepared foods to provide the calorie content and other nutrition information at the point of purchase, consumers should be able to make more informed decisions and public health should be improved. The often maligned Affordable Care Act (i.e., “Obamacare”) addressed this issue in a mandate calling for compulsory nutrition labeling of prepared foods at the point of sale. In response to this effort, conservatives in Congress introduced the “Common Sense Nutrition Disclosure Act” (CSNDA) which, if enacted, would have effectively eviscerated the menu labeling provisions mandated by the Affordable Care Act. The stated rationale behind this legislation: to protect small business and prevent job loss resulting from overly‐burdensome federal regulations. This article provides a historical look at the evolution of federally mandated menu labeling in the United States with particular emphasis on opposition to its passage. The position is taken such that this legislation is beneficial, warranted, and long overdue, and its opposition, as reflected in the CSNDA, points to the need for new metrics when evaluating public policy. Many philosophers endorse the idea that there can be no moral responsibility without a moral community and thus hold that such responsibility is essentially interpersonal. In this paper, various interpretations of this idea are distinguished, and it is argued that no interpretation of it captures a significant truth. The popular view that moral responsibility consists in answerability is discussed and dismissed. The even more popular view that such responsibility consists in susceptibility to the reactive attitudes is (...) also discussed, and it is argued that this view at best supports only an etiolated interpretation of the idea that moral responsibility is essentially interpersonal. This paper focuses on Hegel's views on the idea of retrospective and intersubjective determination of intention. The main point is to distinguish four perspectives to human action: 1) The agent's "moral" perspective and the understanding and description under which the agent acted; from this perspective we can thematize the operative intention-in-action and distinguish "action" from "deed". 2) The agent's retrospective awareness and appropriation of the action: was what I did really justified and did it express my true goals? (...) 3) The retrospective takes of others concerning the action, from within relevant social practices and the prevailing Sittlichkeit. 4) The perspective of Reason or Idea as emerging in the world history. In this paper I use this differentiation of perspectives to discuss Hegel's views on action. I defend the primacy of the individual herself in the first aspect mentioned, in what Hegel calls the "moral" perspective. I hope to show that while the Hegelian or expressivist theme of "retrospective determination of intention" as discussed by Charles Taylor and Robert Pippin is true and important concerning retrospective awareness, retrospective appropriation and retrospective justification of action, we should not take it to imply that the intention-in-action is determined intersubjectively or retrospectively. The intention is formed by the self-determining agent herself, before or at the time of the action, just like a common sense theory of individual action assumes. Hegel's distinction in Philosophy of Right between "action" and "deed" and his views concerning the role of intended results and foreseen consequences in discussing the responsibilities of the moral agent make clear that this is also Hegel's mature view. In Phenomenology, Hegel toys with the idea of retrospective determination of intention, but does not put it forward as his own view. (shrink)

Over the past four decades, I have been asked many questions regarding the substance and methodology of my essay “Person and Community in African Thought”. I cannot in the space of these pages retrieve or reframe the content and implications of these several questions and it would be fool-hardy to attempt an answer to all of them here. But that is no reason not to try to say a few things, by way of additional commentary, on the occasion of this (...) retrospective on the essay. This paper provides a retrospective and prospective overview of TU Delft’s approach to engineering ethics education. For over twenty years, the Ethics and Philosophy of Technology Section at TU Delft has been at the forefront of engineering ethics education, offering education to a wide range of engineering and design students. The approach developed at TU Delft is deeply informed by the research of the Section, which is centered around Responsible Research and Innovation, Design for Values, and Risk Ethics. These (...) theoretical approaches are premised on the notion that technologies are inherently value-laden, and as such contain the possibility of fostering or hindering moral values. Each of these approaches encourages students to take a proactive attitude with respect to their projects and profession, thinking creatively about – and taking responsibility for – how to both prevent harm and do good via the technologies they help develop. To explain how this is put into practice, this paper sketches a brief history of ethics teaching at TU Delft, outlines current activities, and presents future plans for Bachelor and Master’s level engineering ethics education at TU Delft. In this article, I critically reassess Iris Marion Young's late works, which centre on the distinction between liability and social connection responsibility. I concur with Young's diagnosis that structural injustices call for a new conception of responsibility, but I reject several core assumptions that underpin her distinction between two models and argue for a different way of conceptualising responsibility to address structural injustices. I show that Young's categorical separation of guilt and responsibility is not supported by (...) the writings of Hannah Arendt, which Young draws on, and that it is also untenable on independent systematic grounds. Furthermore, I argue that several of Young's other criteria fail to clearly demarcate two distinct phenomena. I therefore propose to transcend Young's distinction between two models in favour of a related, but conceptually different distinction between two forms of responsibility: interactional and structural. Embracing this terminology facilitates the conceptualisation of the general features of responsibility that are shared by both forms, including their retrospective and prospective time-direction and their applicability to individual, joint and group agency. The distinction between interactional and structural responsibility also yields a more compelling general account of the role of background structures, and of blame within ascriptions of political responsibility. Among the many forms of research misconduct, publishing fraudulent data is considered to be serious where the confidence and validity of the research is detrimentally undermined. In this study, the trend of 303 retracted publications from 44 authors (with more than three retracted publications each) was analysed. The results showed that only 6.60% of the retracted publications were single-authored and the discovery of fraudulent publications had reduced from 52.24 months (those published before the year 2000) to 33.23 months (those published (...) on the year 2000 and onwards). It appears that with the widely accessible public databases like PubMed, fraudulent publications can be detected more easily. The different approaches adopted by authors who had previous publications retracted are also discussed herein. (shrink)

Through critical examination of three main contemporary approaches to describing moral responsibility, this book illustrates why philosophers must take into account the relationship between retrospective moral responsibility and desert of praise or blame. The author advances the moral attitude account, whereby desert of praise and blame depends on the agent’s moral attitudes in response to moral reasons, and retrospective moral responsibility results from expressions of those attitudes in overt behavior.

The social and legal practices of blaming, praising, punishing and rewarding are inextricably linked with the process of ‘holding responsible’. Blame, praise, and the like exist as means of holding agents to account that is distinct from, but reliant upon, attributions of responsible agency. When claims of accountability are made without access to an underlying shared attribution of responsibility, the communicative role of accountability is undermined. Disagreement over blame and praise is reduced to disparity: able to hear only that (...) something is a bad or good thing, we are left unable to understand what the bad thing is, or why it is bad. Responsibility as we employ it offers the basis for our evaluations of agents. However, conceptions of responsibility that focus on agent capacities, namely control and rationality, fail to give responsibility a meaning capable of fulfilling this purpose. This study has been designed to investigate whether Corporate Social Responsibility (CSR) orientations have shifted in their priority in response to society's changing expectations. For this sample of U.S.-based multinational chemical subsidiaries, it appears that the top priority continues to be economic responsibilities, followed closely by legal responsibilities. A socially accountable corporation ... must be a thoughtful institution, able to rise above economic interest to anticipate the impact of its actions on all individuals and groups, from shareholders to employees (...) to customers, to fellow-breathers of the air and fellow-sharers of the land. Artificially intelligent systems (AISs) are being created by software developing companies (SDCs) to influence clinical decision‐making. Historically, clinicians have led healthcare decision‐making, and the introduction of AISs makes SDCs novel actors in the clinical decision‐making space. Although these AISs are intended to influence a clinician's decision‐making, SDCs have been clear that clinicians are in fact the final decision‐makers in clinical care, and that AISs can only inform their decisions. As such, the default position is that clinicians should hold responsibility (...) for the outcomes of the use of AISs. This is not the case when an AIS has influenced a clinician's judgement and their subsequent decision. In this paper, we argue that this is an imbalanced and unjust position, and that careful thought needs to go into how personal moral responsibility for the use of AISs in clinical decision‐making should be attributed. This paper employs and examines the difference between prospective and retrospective responsibility and considers foreseeability as key in determining how personal moral responsibility can be justly attributed. This leads us to the view that moral responsibility for the outcomes of using AISs in healthcare ought to be shared by the clinical users and SDCs. (shrink)

Intersexual infants and infants with other genital abnormalities often receive genital surgery for sex assignment or for normalizing purposes. The wisdom and beneficence of these practices have been questioned by intersexual individuals, support groups, some doctors, and the media. Because the practices have been developed without long-term studies to evaluate them, pediatric urologists and parents of such children must face decisions with very little guidance from empirical support. In the face of ignorance about what is really the best medical response (...) to intersexuality or genital abnormalities, some have argued for a moratorium on infant genital surgery until empirical studies are available. The urgent need for retrospective studies is now being recognized in medical journals. Because genital surgery may be appropriate and beneficent in some of these conditions, or in some degrees of these conditions, but not in others, retrospective studies must be devised to examine the degree of success of surgery for each of these conditions, or levels thereof. In this paper a social group’s (retrospective) responsibility for its actions and their consequences are investigated from a philosophical point of view. Building on Tuomela’s theory of group action, the paper argues that group responsibility can be analyzed in terms of what its members (jointly) think and do qua group members. When a group is held responsible for some action, its members, acting qua members of the group, can collectively be regarded as praiseworthy or blameworthy, in the (...) light of some normative standard, for what the group has done. The paper gives a necessary and sufficient conditions analysis of a group’s responsibility for its actions and their outcomes, and the conditions can be cashed out in terms of the group members’ joint (and other) actions. This article addresses prospective and retrospective responsibility issues connected with medical robotics. It will be suggested that extant conceptual and legal frameworks are sufficient to address and properly settle most retrospective responsibility problems arising in connection with injuries caused by robot behaviours (which will be exemplified here by reference to harms occurred in surgical interventions supported by the Da Vinci robot, reported in the scientific literature and in the press). In addition, it will be pointed out (...) that many prospective responsibility issues connected with medical robotics are nothing but well-known robotics engineering problems in disguise, which are routinely addressed by roboticists as part of their research and development activities: for this reason they do not raise particularly novel ethical issues. In contrast with this, it will be pointed out that novel and challenging prospective responsibility issues may emerge in connection with harmful events caused by normal robot behaviours. Purpose Although the term “responsibility” plays a central role in bioethics and public health, its meaning and implications are often unclear. This paper defends the importance of a more systematic conception of responsibility to improve moral philosophical as well as descriptive analysis. Methods We start with a formal analysis of the relational conception of responsibility and its meta-ethical presuppositions. In a brief historical overview, we compare global-collective, professional, personal, and social responsibility. The value of our analytical (...) matrix is illustrated by sorting out the plurality of responsibility models in three cases (organ transplantation, advance directives, and genetic testing). Results Responsibility is a relational term involving at least seven relata. The analysis of the relata allows distinguishing between individual versus collective agency, retrospective versus prospective direction, and liability versus power relations. Various bioethical ambiguities result from insufficient, implicit, or inappropriate ascriptions of responsibility. Conclusions A systematic conception of responsibility is an important tool for bioethical reflection. It allows an in-depth understanding and critique of moral claims on a meta-ethical level without presuming one particular normative approach. Considering the concept of responsibility can also help to complement the current bioethical focus on individual autonomy by including the perspectives of other actors, such as family members or social groups. Indonesia invaded the territory in December 1975, relying on US diplomatic support and arms, used illegally, but with secret authorisation from Washington; there were even new arms shipments sent under the cover of an official "embargo". There was no need to threaten bombing or even sanctions. It would have sufficed for the US and its allies to withdraw their active participation, and inform their close associates in the Indonesian military command that the atrocities must be terminated and the territory granted (...) the right of selfdetermination that has been upheld by the United Nations and the International Court of Justice. We cannot undo the past, but we should at least be willing to recognise what we have done, and face the moral responsibility of saving the remnants and providing ample reparations - a small gesture of compensation for terrible crimes. Because the propaganda model challenges basic premises and suggests that the media serve antidemocratic ends, it is commonly excluded from mainstream debates on media bias. Such debates typically include conservatives, who criticize the media for excessive liberalism and an adversarial stance toward government and business, and centrists and liberals, who deny the charge of adversarialism and contend that the media behave fairly and responsibly. The exclusion of the propaganda model perspective is noteworthy, for one reason, because that perspective is consistent (...) with long standing and widely held elite views that 'the masses are notoriously short-sighted' (Bailey 1948: 13) and are 'often poor judges of their own interests' (Lasswell 1933: 527), so that 'our statesmen must deceive them' (Bailey 1948: 13); and they 'can be managed only by a specialized class whose personal interests reach beyond the locality' (Walter Lippmann 1921: 310). In Lippmann's view, the 'manufacture of consent' by an elite class had already become 'a self-conscious art and a regular organ of popular government' by the 1920s (Lippman 1921: 248). (shrink)

Ensuring the trustworthiness of autonomous systems and artificial intelligence is an important interdisciplinary endeavour. In this position paper, we argue that this endeavour will benefit from technical advancements in capturing various forms of responsibility, and we present a comprehensive research agenda to achieve this. In particular, we argue that ensuring the reliability of autonomous system can take advantage of technical approaches for quantifying degrees of responsibility and for coordinating tasks based on that. Moreover, we deem that, in certifying (...) the legality of an AI system, formal and computationally implementable notions of _responsibility_, _blame_, _accountability_, and _liability_ are applicable for addressing potential responsibility gaps (i.e. situations in which a group is responsible, but individuals’ responsibility may be unclear). This is a call to enable AI systems themselves, as well as those involved in the design, monitoring, and governance of AI systems, to represent and reason about who can be seen as responsible in prospect (e.g. for completing a task in future) and who can be seen as responsible retrospectively (e.g. for a failure that has already occurred). To that end, in this work, we show that across all stages of the design, development, and deployment of trustworthy autonomous systems (TAS), responsibility reasoning should play a key role. This position paper is the first step towards establishing a road map and research agenda on how the notion of responsibility can provide novel solution concepts for ensuring the _reliability_ and _legality_ of TAS and, as a result, enables an effective embedding of AI technologies into society. BackgroundResponsive neurostimulation has been utilized as a treatment for intractable epilepsy. The RNS System delivers stimulation in response to detected abnormal activity, via leads covering the seizure foci, in response to detections of predefined epileptiform activity with the goal of decreasing seizure frequency and severity. While thalamic leads are often implanted in combination with cortical strip leads, implantation and stimulation with bilateral thalamic leads alone is less common, and the ability to detect electrographic seizures using RNS System thalamic leads is (...) uncertain.ObjectiveThe present study retrospectively evaluated fourteen patients with RNS System depth leads implanted in the thalamus, with or without concomitant implantation of cortical strip leads, to determine the ability to detect electrographic seizures in the thalamus. Detailed patient presentations and lead trajectories were reviewed alongside electroencephalographic analyses.ResultsAnterior nucleus thalamic leads, whether bilateral or unilateral and combined with a cortical strip lead, successfully detected and terminated epileptiform activity, as demonstrated by Cases 2 and 3. Similarly, bilateral centromedian thalamic leads or a combination of one centromedian thalamic alongside a cortical strip lead also demonstrated the ability to detect electrographic seizures as seen in Cases 6 and 9. Bilateral pulvinar leads likewise produced reliable seizure detection in Patient 14. Detections of electrographic seizures in thalamic nuclei did not appear to be affected by whether the patient was pediatric or adult at the time of RNS System implantation. Sole thalamic leads paralleled the combination of thalamic and cortical strip leads in terms of preventing the propagation of electrographic seizures.ConclusionThalamic nuclei present a promising target for detection and stimulation via the RNS System for seizures with multifocal or generalized onsets. These areas provide a modifiable, reversible therapeutic option for patients who are not candidates for surgical resection or ablation. (shrink)

As the rising costs of lifestyle-related diseases place increasing strain on public healthcare systems, the individual’s role in disease may be proposed as a healthcare rationing criterion. Literature thus far has largely focused on retrospective responsibility in healthcare. The concept of prospective responsibility, in the form of a lifestyle contract, warrants further investigation. The responsibilisation in healthcare debate also needs to take into account innovative developments in mobile health technology, such as wearable biometric devices and mobile apps, (...) which may change how we hold others accountable for their lifestyles. Little is known about public attitudes towards lifestyle contracts and the use of mobile health technology to hold people responsible in the context of healthcare. This paper has two components. Firstly, it details empirical findings from a survey of 81 members of the United Kingdom general public on public attitudes towards individual responsibility and rationing healthcare, prospective and retrospective responsibility, and the acceptability of lifestyle contracts in the context of mobile health technology. Secondly, we draw on the empirical findings and propose a model of prospective intention-based lifestyle contracts, which is both more aligned with public intuitions and less ethically objectionable than more traditional, retrospective models of responsibility in healthcare. Do past state actions, such as the American conquest of northern Mexico, the British colonization of South Asia, and the Spanish expulsion of the Sephardim and Moriscos, grant contemporary Mexicans, South Asians, and the descendants of the Sephardim and Moriscos a particular right to immigrate to the United States, the United Kingdom, and Spain respectively? In this paper I examine three theoretical models for addressing this question: retrospective responsibility for historic injustice; the principle of coercively constituted identities; and (...) the theory of remedial responsibility. I argue that remedial responsibility best justifies a particular right to immigrate through responsibility for the past for three reasons. First, it relieves us of the epistemological task of establishing causal responsibility. Second, it lessens the normative task of identifying a theory of unjust harm to establish moral responsibility. Finally, it facilitates the normative task of ranking the claims to immigrate of different individuals. (shrink)

In this paper I offer a retrospective rereading of my work on epistemic responsibility in order to see why this inquiry has found only an uneasy location within the discourse of Anglo-American epistemology. I trace the history of the work's production, circulation and reception, and examine the feminist implications of the discussions it has occasioned.

At one time, modern historians had come to be accustomed to using the paradigm of "Western challenge-Chinese response" to describe the development of modern China since the Opium War. However, in the past few decades, some scholars have begun to offer a very different opinion and argument. This is not only because Arnold J. Toynbee's "challenge and response" theory has continued to be repeatedly criticized and examined in a more unfavorable light, but also because people have come to believe that (...) the lessons of Chinese history over the past century and a half cannot simply be summarized as an outward response to the modern civilization, or the civilization of modernity, represented by the West, and that the conflict between the two—[i.e., between China and the West] must be resolved only through China's own modernization. Nonetheless, from a macrocultural perspective, the question of how a premodernistic Chinese culture may produce a creative response to the modernized culture of the West has remained and to a certain extent remains today a major topic for the study of the culture of modern and contemporary China. In a fundamental sense, the ebb and flow and the interweaving between the stubborn cultural identity of a cultural conservatism that was, and is, deeply rooted in a powerful spiritual-cultural tradition on the one hand and the antitraditional consciousness which is produced out of an urgent, even desperate concern for modernization on the other hand has formed the basic framework of the culture of modern China. To those defining euthanasia as a battle for the principle of self-determination, persons seeking physician assisted death (PAD) are soldiers in the fight for patient autonomy. The reasons they seek it, or the potential of other, non-life-threatening interventions is less important than this principle: individuals have the right not only to choose death (suicide), but to be assisted in dying. They should not be second guessed or denied on the basis of another's distaste for that decision. This paper offers a (...) general review of deaths attributed to Dr. Jack Kevorkian's PAD practice in an attempt to answer two questions: Why do persons seek physician assisted death, and, to what extent does induced death seem, in retrospect, a reasonable and perhaps necessary medical response to specific patient complaints? Electroacupuncture (EA) and electroconvulsive therapy (ECT) are often used in the management of schizophrenia. This study sought to determine whether additional EA and ECT could augment antipsychotic response and reduce related side effects. In this retrospective controlled study, 287 hospitalized schizophrenic patients who received antipsychotics (controls, n = 50) alone or combined with EA (n = 101), ECT (n = 55) or both (EA+ECT, n = 81) were identified. EA and ECT were conducted for 5 and 3 sessions per (...) week, respectively, with a maximum of 12 sessions for ECT during hospitalization. The Scale for the Assessment of Positive Symptoms (SAPS) and Scale for the Assessment of Negative Symptoms (SANS) were used to assess the severity of psychotic symptoms. Clinical response on SAPS and SANS, weight gain, and adverse events were compared. Survival analysis revealed that the ECT and EA+ECT groups had markedly greater clinical response rate than controls on SAPS [72.7% and 90.1% vs. 64.0%; relative risk (RR), 1.974 and 2.628, respectively, P ≤ 0.004] and on SANS (67.3% and 70.4% vs. 42.0%; RR, 1.951 and 2.009, respectively, P ≤ 0.015). A significantly greater response rate on SANS than controls was also observed in the EA group (64.4% versus 42.0%; RR = 1.938, P = 0.008). EA-containing regimens remarkably reduced weight gain and incidences of headache, insomnia, dry mouth, and electrocardiographic abnormalities. These results suggest that EA and ECT can serve as additional treatment for enhancing antipsychotic response and reduce the side effects in hospitalized patients with schizophrenia. (shrink)

As a young lecturer in philosophy and the eldest son of a prominent Jewish family, Alan Montefiore faced two very different understandings of his identity: the more traditional view that an identity such as his carried with it, as a matter of given fact, certain duties and obligations, and an opposing view, emphasized by his studies in philosophy, according to which there can be no rationally compelling move from statements of fact—whatever the alleged facts may be—to "judgments of value." According (...) to this second view, individuals must in the end take responsibility for determining their own values and obligations. In this book, Montefiore looks back on his attempts to understand the nature of this conflict and the misunderstandings it may engender. In the process, he illustrates through personal experience the practical implications of a characteristically philosophical issue. Montefiore finally settles on the following: while everyone has to accept that facts, including those of their own situation, are whatever they may be, both the "traditional" assumption that individuals must recognize certain values and obligations as rooted in those very facts, and the contrary view that individuals are ultimately responsible for determining their own values, are deeply embedded in differing conceptions of society and its relation to its members. Montefiore then examines the misunderstandings between those for whom identity constitutes in effect a conceptual bridge connecting the facts of who and what a person may be to the value commitments incumbent upon them, and those for whom the very idea of such a bridge can be nothing but a confusion. Using key examples from the notoriously vexed case of Jewish identity and from his own encounters with its conflicting meanings and implications, Montefiore depicts the practical significance of the differences between these worldviews, particularly for those who hove to negotiate them. I. A. Richards ushered the spirit of Cambridge realism into semantics and literary criticism. When he arrived as an undergraduate in 1911, Cambridge was in the midst of its finest philosophical flowering since the Puritanism and Platonism of the seventeenth century. The revolution of G. E. Moore and Bertrand Russell against Hegelian idealism had already occurred; the Age of Principia was under way. There was a reassertion of native empiricism and a new interest in philosophical psychology, and the whole discussion (...) was marked increasingly by a preoccupation with language. Richards, too, would break with the past, with the history of criticism in the previous two generations, gather psychological ideas to establish an empirical semantics and aesthetics, and center his attention on language. Although Romantic and late-Victorian values inform his theories, Richards set down an original criticism on first principles, not on tradition. Many of his books' titles show this rationalist strains: The Foundations of Aesthetics , The Meaning of Meaning , Principles of Literary Criticism , Basic Rules of Reason , and The Philosophy of Rhetoric . The originality and influence of Richards' criticism can be shown by the number of terms he put into circulation, terms which became the currency of debate for almost half a century: close reading, tone, pseudostatement, stock response, tension, equilibrium, tenor and vehicle of metaphor, emotive and referential language.John Paul Russo is a professor and chairman of the English department at the University of Miami. He is the editor of I. A. Richards' Complementarities: Uncollected Essays and the author of Alexander Pope: Tradition and Identity and an annotated bibliography of Richards' works. Using data obtained during a retrospective interview study of 30 women who had undergone genetic testing—BRCA1/2mutation searching—this paper describes how women, previously diagnosed with breast/ovarian cancer, perceive their role in generating genetic information about themselves and their families. It observes that when describing their motivations for undergoing DNA testing and their experiences of disclosing genetic information within the family these women provide care based ethical justifications for their actions. Finally, it argues that generating genetic information and disclosing this information (...) to kin raise different types of ethical issues. The implications of these findings for ethical debates about informed choice in the context of genetic testing are discussed. (shrink)

Introduction: Congenital myasthenic syndromes refer to a heterogenic group of neuromuscular transmission disorders. CMS-subtypes are diverse regarding exercise intolerance and muscular weakness, varying from mild symptoms to life-limiting forms with neonatal onset. Long-term follow-up studies on disease progression and treatment-response in pediatric patients are rare.Patients and Methods: We analyzed retrospective clinical and medication data in a cohort of 32 CMS-patients including the application of a standardized, not yet validated test to examine muscular strength and endurance in 21 patients at (...) the last follow-up. Findings obtained in our cohort were compared with long-term follow-up studies of CMS-cohorts from the literature by considering the underlying molecular mechanisms. Outcomes of CMS-ST were compared to results of normal clinical assessment.Results: Thirty-two pediatric patients with defects in eight different CMS-genes were followed by a median time of 12.8 years. Fifty-nine percentage of patients manifested with first symptoms as neonates, 35% as infants. While 53% of patients presented a reduced walking distance, 34% were wheelchair-bound. Even under adequate therapy with pyridostigmine and 3,4-diaminopyridine, CHAT-mutations led to the progression of muscular weakness partly in combination with persistent respiratory and bulbar symptoms. RAPSN, CHRND, and CHRNB1 patients with neonatal manifestation, early respiratory problems, and bulbar symptoms showed a good and maintained treatment response. CHAT and CHRNE patients required higher PS dosages, whereas RAPSN patients needed a lower mean dosage at the last follow-up. The benefits of short-term medication and long-term progression of symptoms were highly dependent on the specific genetic defect. CMS-ST was carried out in 17/21 patients, determined affected muscle groups including bulbar and ocular symptoms, some of which were not reported by the patients.Conclusions: Our findings and comparison with the literature- suggest a better treatment-response and less severe progression of symptoms present in patients suffering from mutations in CMS-genes directly associated with receptor deficiency, while patients with defects leading to synaptopathy and presynaptic defects tend to have worse outcomes. Assessment of affected muscular groups and clinical symptoms by CMS-ST may be a useful tool for optimal therapeutic management of the patients, especially for future clinical studies. (shrink)
