One of the main challenges in medicine is to guarantee an appropriate drug supply according to the real needs of patients. Closed-loop strategies have been widely used to develop automatic solutions based on feedback variables. However, when the variable of interest cannot be directly measured or there is a lack of knowledge behind the process, it turns into a difficult issue to solve. In this research, a novel algorithm to approach this problem is presented. The main objective of this study (...) is to provide a new general algorithm capable of determining the influence of a certain clinical variable in the decision making process for drug supply and then defining an automatic system able to guide the process considering this information. Thus, this new technique will provide a way to validate a given physiological signal as a feedback variable for drug titration. In addition, the result of the algorithm in terms of fuzzy rules and membership functions will define a fuzzy-based decision system for the drug delivery process. The method proposed is based on a Fuzzy Inference System whose structure is obtained through a decision tree algorithm. A four-step methodology is then developed: data collection, preprocessing, Fuzzy Inference System generation, and the validation of results. To test this methodology, the analgesia control scenario was analysed. Specifically, the viability of the Analgesia Nociception Index as a guiding variable for the analgesic process during surgical interventions was studied. This work focuses on the application of machine learning techniques to assist the clinicians in the administration of analgesic drug during general anaesthesia. Specifically, the main objective is to propose the basis of an intelligent system capable of making decisions to guide the opioid dose changes based on a new nociception monitor, the analgesia nociception index. Clinical data were obtained from 15 patients undergoing cholecystectomy surgery. By means of an off-line study, machine learning techniques were applied to analyse the possible (...) relationship between the analgesic dose changes performed by the physician due to the hemodynamic activity of the patients and the evolution of the ANI. After training different classifiers and testing the results under cross validation, a preliminary relationship between the evolution of ANI and the dosage of remifentanil was found. These results evidence the potential of the ANI as a promising index to guide the infusion of analgesia. (shrink)

Las expresiones de este socialismo no son homogéneas, convivieron con otras ideas rivales llegando en ocasiones a mezclarse con ellas; y fueron formuladas siempre como respuestas concretas ante los problemas que planteaba cada momento histórico. Desde el socialismo republicano de algunos seguidores de Robert Owen hasta los (...) desafíos que planteó la New Left, las teorías fiduciarias encontraron hueco para abrirse paso en los escritos de estos socialistas libertarian. The objective of this paper is to provide some historical and conceptual keys to understand the history of libertarian British socialism, and its relationship with the fiduciary conception of political power and economic power. The expressions of this libertarian socialism are not homogeneous, they coexisted with other rival conceptions, sometimes mixing with them. Those articulations were always formulated as concrete responses to the problems laid out by different historical moments. From the republican socialism of some Robert Owen’s followers to the challenges exposed by the New Left, fiduciary theory found room to break through in the writings of these libertarian socialists. This paper relates Donaldson and Dunfee’s Integrative Social Contracts Theory to the problem of gender discrimination. We make the assumption that multinational managers might seek some guidance from ISCT to resolve ethical issues of gender discrimination in countries indifferent or hostile to gender equaIity. The role of Donaldson and Dunfee’s “hypernorms” seems especially cruciaI, and we find that, under their writings thus far, no “hypernorms” exist to make unethical the most blatant acts of sex discrimination in a host country whose (...) local norms tolerate such discrimination. The genesis of “hypernorms” as “global moral minimums” is recounted, and specific application of ISCT to a familiar ethics case (“A Foreign Assignment”) is provided. (shrink)

Urban communities in 21st century America are facing severe economic challenges, ones that suggest a mandate to contemplate serious changes in the way America does business. The middle class is diminishing in many parts of the country, with consequences for the economy as a whole. When faced with the loss of its economic base, any business community must make some difficult decisions about its proper role and responsibilities. Decisions to support the community must be balanced alongside and against responsibilities to (...) owners, shareholders and relevant “stakeholders” in a relatively new context. Corporations in urban communities “hollowed out” by white flight or urban sprawl must decide what level of support they can and should provide. This paper examines corporate decisions within the emerging urban prosperity initiatives, using the framework of integrative social contract theory proposed by Donaldson and Dunfee. We suggest that urban prosperity initiatives present a mandate on corporations sufficiently strong as to qualify as an authentic norm. Further, we argue that strict adherence to a corporate bottom line approach or “corporate isolationism” is not congruent with contemporary community standards. (shrink)

The first contribution of this article is a politico-philosophical map that, drawing upon two common sets of arguments against modern natural rights, might help to explain the prevailing neo-republican position on natural rights. Under the label ‘abstraction argument’, we explore the view that natural rights are a metaphysical construct that usually ends in a violent application of speculative principles to society. Under ‘self-interest argument’, we discuss the notion that natural rights endorse an atomistic and selfish conception of the human being. (...) Second, we show how Cold War authors replicated these two arguments, conveying a biased, largely anti-republican and anti-democratic view of natural rights to the twentieth century. Third, drawing on these two arguments, we critically assess the narrow view of natural rights inherited by neo-republican scholars. (shrink)

El presente artículo analiza la conexión humana que acaece en el proceso cinematográfico. Se analiza la conexión desde su capacidad de influencia: emisores y receptores se influyen mutuamente. Se abandona así una concepción mecánica y lineal de la conexión para abordar una conexión más humana y polidireccional. En este sentido, se muestra que, desde una perspectiva productiva, perceptiva y cognitiva, el proceso cinematográfico implica la existencia, en mayor o menor grado, de interacción entre autores, contexto y público. Para comprender cómo (...) tiene lugar la conexión en el cine, se examina desde el psicoanálisis, la sociología y la filosofía del proceso de identificación. Se analizan en detalle seguidamente los distintos procesos humanos de conexión que suceden en el cine, prestando especial atención a la existente entre: emisores/autores y receptores/público, por medio del objeto cinematográfico, la película. Cette guerre interétatique est le résultat du cycle de déploiement des printemps et des soulèvements de la dernière décennie et a des répercussions synchroniques sur un clivage interne qui traverse d’autres pays. L’article se conclut par une vision alternative entre un conflit inter-impérialiste dans lequel l’Europe est menacée par le bloc atlantiste et une dichotomie autocraties/démocraties.

À partir d’auteurs comme Negri, Cocco, Viveiros de Castro et Fanon, cet article discute le devenir-indien comme un concept pour les forces expressives de luttes et de réinvention dans le Sud. À travers de formes concrètes d’intervention, on analyse un mode de résistance à la fois créatif et alternatif, qui prend le contre-pied de l’actuel agenda capitaliste et néo-développementiste.

U.S. multinational enterprises must now follow the policies of Title VII of the Civil Rights Act of 1964 in their overseas operations, at least with respect to U.S. expatriate employees. Doing so in a culture which discourages gender equality in the workplace raises difficult issues, both practically and ethically. Vigorously importing U.S. attitudes toward gender-equality into a social culture such as Japan or Saudi Arabia may seem ethnocentric, a version of ethical imperialism. Yet adapting to host country norms risks a (...) kind of moral relativism. This article supports the view that MNEs which promote workplace equality in a host country such as Japan, which is actively involved in the international economic and political community, is not ethical imperialism in any pejorative sense and is preferable to a moral relativism or social contract approach.We hold these truths to be self-evident: that all men are created equal, and endowed by their creator with certain rights — life, liberty, and the pursuit of happiness. The intersection of ethics, computing, and genetics plots a space not yet adequately mapped, despite its importance, indeed, its rapidly growing importance. Its subdomains are well-enough known: or the study of ethical issues in genetics and genomics, is part of core curricula everywhere. Ethics and computing is an established subfield. Computing and geneticshas in little more than a decade progressed from subsubspecialty to the sine qua non of contemporary biomedical research, and it bids fair to transform clinical practice. This book proposes a shift in the very concept of neoliberalism as an ambivalent product of subjectivity. It is not resolved in dichotomies between the included and excluded, interior and exterior, capitalist and noncapitalist. Neoliberalism operates in blurred lines, through flexible structures, and amid internal gradients and varying tensions.

Automatic control of physiological variables is one of the most active areas in biomedical engineering. This paper is centered in the prediction of the analgesic variables evolution in patients undergoing surgery. The proposal is based on the use of hybrid intelligent modelling methods. The study considers the Analgesia Nociception Index to assess the pain in the patient and remifentanil as intravenous analgesic. The model proposed is able to make a one-step-ahead prediction of the remifentanil dose corresponding to the current state (...) of the patient. The input information is the previous remifentanil dose, the ANI variable and the electromyogram signal. Modelling techniques used are Artificial Neural Networks and Support Vector machines for Regression combined with clustering methods. Both training and validation were done with a real dataset from different patients. Results obtained show the potential of this methodology to calculate the drug dose corresponding to a given analgesic state of the patient. (shrink)

Closed-loop administration of propofol for the control of hypnosis in anesthesia has evidenced an outperformance when comparing it with manual administration in terms of drug consumption and post-operative recovery of patients. Unlike other systems, the success of this strategy lies on the availability of a feedback variable capable of quantifying the current hypnotic state of the patient. However, the appearance of anomalies during the anesthetic process may result in inaccurate actions of the automatic controller. These anomalies may come from the (...) monitors, the syringe pumps, the actions of the surgeon or even from alterations in patients. This could produce adverse side effects that can affect the patient postoperative and reduce the safety of the patient in the operating room. Then, the use of anomaly detection techniques plays a significant role to avoid this undesirable situations. This work assesses different one-class intelligent techniques to detect anomalies in patients undergoing general anesthesia. Due to the difficulty of obtaining real data from anomaly situations, artificial outliers are generated to check the performance of each classifier. The final model presents successful performance. Brownlee (bio)Though seemingly alien discourses, romance and historiography are perennially linked. Far from offering an atemporal imaginary universe that bears no resemblance to historical specificity, romance is constructed as a response to it. Rather than simply projecting for the reader the naïve appeal of a prelapsarian escapism from the harsh realities of history, romance involves a continuous and sophisticated reinvention (...) of itself as a response to an ever-changing historico-political configuration. It is, in fact, a reciprocal relationship. We frequently see history appropriating romance paradigms explicitly, as, for example, when Bernal Díaz chooses to present Spain’s New World conquest and colonization as a continuation of the exploits of Amadís [see Gilman].Whatever form it takes, romance is committed to the celebration of a coherent system of socio-political values. This extra-textual frame of reference can take a variety of forms—from political propaganda that offers a self-aggrandizing depiction of the nobility or patron for whom the text is produced, to escapist fantasy—futuristic or archaizing. It is nostalgia for the lost world of chivalric romance which Cervantes embodies in the figure of Don Quijote, a foolish old man driven mad by his obsession with this perennial literary form. Cervantes is rightly credited with a brilliant and programmatic use of romance constructs to communicate the ongoing historical crisis of the Spanish empire. A byproduct of this romance scrutiny of history was his invention of the modern novel.Testimony to the long-standing interaction between romance and history in Spain, as elsewhere in Europe, is evident in the fact that its first romances were based on history, the so-called “romances of antiquity.” In those texts historical figures and events are evoked for the purpose of legitimating the present, vernacular poets as they celebrate the empowering myths projected by their society, as well as their own literary endeavors by invoking venerable Latin models. The well known translatio studii et imperii topos offers such allegory of empire.At the same time, the confrontation of romance and historical representation can also have a destabilizing effect, and this is programmatically explored in Pedro del Corral’s Crónica sarracina (c. 1430). His text ponders the possibilities of romance and historiography in an obsessive manner and he accomplishes his task by examining representation of the most iconic figures associated with the fall of Spain in 711, namely, La Cava and her father, Count Julián, King Rodrigo, and Pelayo, the messianic hope of the Reconquest effort which would last nearly 800 years.Of related and perhaps even broader importance, Corral’s text merits careful reading because its exploration of iconicity related to Spain’s fall offers meditations on the workings of ideology, which inevitably proposes “imaginary or formal solutions to unresolvable contradictions” [Jameson 79]. Ideology—its unmasking—is intriguingly posited by Corral in powerful ways.If we turn to Corral’s text, we find a work offering an extraordinary consideration of history and romance—their profound interaction—whose full title reads, La crónica [End Page 119] del rey don Rodrigo con la destrucción de España (The Chronicle of King Rodrigo with the Destruction of Spain). My interest in the Crónica here is primarily to consider the mechanisms by which it gestures boldly toward what Žižek has described as the project of the postmodern critique of ideology—though it is clearly a recurring cultural constant—namely to “designate the elements which point towards the system’s antagonistic character, and thus ‘estrange’ us to the self-evidence of its established identity” [Žižek 7]. A reading of the Crónica provides an extended paradigm of such estrangement.Iberia is the area of the Mediterranean which underwent the most protracted encounter between Christianity and Islam, and the events of the Moorish invasion of 711 constitute Spain’s unique foundational subject-matter—on a par with the Troy legend and with King Arthur [see Deyermond 355]. What we see is a Saint Bartholomew with another's integument in his hand. We next consider an aspect of the self-portrait which even La Cava left out (...) of account - its relative siting. This has to matter since the portrait lies in the path of Christ's imminent action. More than that, it lies on a diagonal that traverses the fresco like a heraldic bend chief to base - from left top to right bottom. The twofold competence thus assumed by the self-portrait - in its concrete location and in the range of its influence - is something to marvel at. A hangdog face flops to one side, helpless and limp. But the tilt of its axis projected upward across the field strikes the apex of the left-hand lunette, the uppermost point of the fresco. And if, departing once again from the skin's facial axis, we project its course netherward, we discover the line produced to aim straight at the fresco's lower right corner. Such results do not come by chance. To put it literally, letting metaphor fall where it may: it is the extension of the self's axis that strings the continuum of heaven and hell. · 1.
