This chapter suggests that descriptive epidemiological studies like the Global Burden of Disease Study can usefully be divided into four tasks: describing individuals’ health states over time, assessing their health states under a range of counterfactual scenarios, summarizing the information collected, and then packaging it for presentation. The authors show that each of these tasks raises important and challenging ethical questions. They comment on some of the philosophical issues involved in measuring health states, attributing causes to health outcomes, choosing the (...) counterfactual against which to assess causes, aggregating and summarizing complex information across multiple domains, discounting, age-weighting, handling fetal deaths, measuring health inequalities, representing uncertainty, and assessing personal responsibility for health outcomes. Background:New graduate transition into the workforce is challenging and can involve managing ethical tensions. Ethical tensions cause new graduates to doubt their capabilities due to their lack of experience. To support new graduates, we need to know what these ethical tensions are.Objectives:To explore the ethical tensions perceived to occur in practice for new graduate health professionals.Research design:This qualitative systematic review involved a search of five databases (Medline, EMBASE, AMED, CINAHL and Scopus) which resulted in the retrieval of 3554 papers. After (...) the two-phased screening process, eight studies were identified that met the inclusion criteria and had rich data on the review question. Articles were read several times, critically appraised and analysed through thematic analysis.Ethical considerations:No ethical approval was required for the systematic review. The review was conducted following well-established reporting guidelines enabling transparency and rigour.Findings:Studies originated from Australia, United States, Iran and China. One study included speech pathologists and seven were with nurses. Four themes included the following: (1) enduring an unknown workplace culture that generates uncertainty without support for new graduates; (2) being vulnerable because of distress from bullying, exclusion and being a scapegoat; (3) constraining systems and institutional restrictions that cause dilemmas; and (4) experiencing disillusionment from lost ideals about ethical practice.Discussion:This review has brought to light the vulnerability of new graduates to negative workplace culture and collegial incivility. In addition, new graduates are subjected to ethical tensions created by institutional constraints which can create dilemmas and uncertainties through practice that does not align with what they anticipated.Conclusion:Understanding ethical tensions experienced by new graduates enables provision of informed support. There needs to be considerable cultural change for orientation and socialisation of new graduates to enable them to learn and manage ethical tensions. (shrink)

The Global Burden of Disease Study is one of the largest-scale research collaborations in global health, producing critical data for researchers, policy-makers, and health workers about more than 350 diseases, injuries, and risk factors. Such an undertaking is, of course, extremely complex from an empirical perspective. But it also raises complex ethical and philosophical questions. In this volume, a group of leading philosophers, economists, epidemiologists, and policy scholars identify and discuss these philosophical questions. Better appreciating the philosophical dimensions of a (...) study like the GBD can make possible a more sophisticated interpretation of its results, and it can improve epidemiological studies in the future, so that they are better suited to produce results that help us to improve global health. Ontological understanding of biological units is crucial to their use in experimental design, analysis, and interpretation. Conceptualizing fundamental units in biology as individuals or classes is important for subsequent development of discovery operations. While the criteria for diagnosing individuals are acknowledged, temporal boundedness is often misinterpreted and temporal minima are applied to units in question. This results in misdiagnosis or abandonment of ontological interpretation altogether. Biological units such as areas of endemism in biogeography and species in evolutionary biology fall victim (...) to such problems. Our goal here is to address the misconception that biological individuals such as species and areas of endemism have a temporal minimum. Areas of endemism can persist within small temporal boundaries in the context of metapopulation dynamics, island biogeography, and range expansion and contraction. Similarly, lineage reticulation illustrates examples of short-lived species. Here, examples of known entities are provided to illustrate their persistence on short time scales in attempt to rescue future interpretation of biological units from ontological misdiagnosis, elucidate the philosophical individuality of areas of endemism and species with short lifespans, and provide justification for the “snapshot in time” diagnostic approach. This paper proceeds in two parts. In the first part, I set out Fodor’s concerns about abduction in his recent books, The Mind Doesn’t Work That Way and In Critical Condition. In the second part, I attempt to meet these concerns by suggesting how - within the framework of the Massive Modularity Hypothesis - abduction functions, specifically in the context of means-end reasoning to connect Input Modules and Output Modules. My suggestion will be that natural selection is the Mother of (...) Abductive Inference and that it is evidenced in all sorts of sub-doxastic Means-End Reasoning contexts and for a variety of species. I will eventually discuss two representative cases to illustrate this suggestion: namely, dead reckoning and spatial reorientation. (shrink)

In this paper, I argue that there are two kinds of epistemic justification: one is objective and the other, subjective. Internalists are interested in the subjective variety of justification. Externalists are interested in the objective notion of justification. A paper by Stewart Cohen fails to distinguish these two varieties of epistemic justification and, as a result, criticizes externalists for failing to address the internalist, subjective notion of epistemic justification. But, since that notion is not the one that externallists care about, (...) I argue that his evil demon example fails to be a counterexample against externalists. (shrink)

The inadequate level of public health resources devoted to TB has also contributed to its spread. Still, even with these factors, it is questionable whether the escalating number of TB cases in this country would have occurred without (...) the reservoir of immunosuppressed persons, who are less resistant to the disease, created by the AIDS epidemic. The fear and urgency of this public health crisis, which has been emerging since the beginning of the last decade, are fueled by the rise of TB strains resistant to the first-line drugs and by the disease’s contagiousness. I argue for an evolutionary solution to Fodor's radical concept nativism puzzle.

I argue that knowledge is a natural kind found in the modules of a massively modular mind. As such, it is not a conceptual kind. The result is that knowledge must be studied empirically and not by appeal to a priori analysis.

In this defense of Reliabilism, I argue that there has been 'selection for' accurate indexical beliefs. I offer empirical evidence and examples to suggest that Steve Stich's defense of the opposite claim in The Fragmentation of Reason is misguided.

Nathan Glazer's The Limits of Social Policy presents an analysis of recent American social policy that Edmund Burke could applaud. A main cause of the failure of American social programs, Glazer argues, is that government ineluctably supplants and weakens the ?fine structure?; of society?the traditional institutions of family, religion, and community. Sometimes the government is merely clumsy and ignorant, but more often the ill effects are the result of ?the ruling doctrines of our age,?; equality and human rights, which cannot (...) bear to leave the convoluted and sometimes unlovely ways of custom and local mores to play themselves out unimpeded. But the very persuasiveness of Glazer's analysis of the past casts some doubt on his measured optimism about the future. Are moderate reforms going to be good enough? Perhaps things will work out for the middle class and affluent, the reviewer argues, but the underclass increasingly appears to be a casualty of moderate reforms. The reviewer offers some reasons for thinking about a return to Jeffersonian self?government, unpragmatic as that may seem in 1991. (shrink)

Canada is an immigrant‐receiving nation and many graduate students in nursing and other disciplines pursue immigrant health research. As these students often start with inadequate understanding of the policy, theoretical, and research contexts in which their work should be situated, we became concerned that the theses and dissertations were less sophisticated than were both possible and desirable. This led to development of a PhD‐level course titled Migration and Health in the Canadian Context. In this study, we provide an analytic overview (...) including course description, objectives, assignments, and specific class topics. Areas of focus include historical and theoretical considerations; determinants of immigrant health; refugee health; cultural competence and cultural safety; research challenges, approaches, and skills; policy‐relevant research; and educational imperatives in the health and related disciplines. Salient research is introduced in each of these classes. While Canada is the main focus, comparative data are provided and there is relevance for nurse‐researchers in other immigrant‐receiving countries. (shrink)
