Most studies evaluating companion effects on medical triadic interaction focus on the doctors' part, e.g., how the companion's presence diverts doctors' attention away from the patient. In contrast to this mainstream approach, the current research proposes an alternative framework by focusing on the patient parties—especially on how companion participation reshapes the discourse sequences where patient parties provide information, and how it affects patient full turns and priority in providing complete first-hand information to doctors. By examining fifteen geriatric triadic conversations (...) collected in a teaching hospital in southern Taiwan, this proposed framework concludes that in the companion's presence, the information providing sequences are restructured into eight patterns, among which sole information provider is preferred to joint providers. Furthermore, the more companions participate in providing information, the less patients by themselves volunteer information or respond to doctors' questions prior to companions' verbal involvement. A more striking companion effect shows that even in the triads with a low-participation companion, whenever the companion does participate, the patient rarely has a full turn or priority to complete an information unit. The patient's turns are either taken, are simultaneous with, or cut-off by the companion's participation. (shrink)

Today many adult children find themselves in the position of caring for elderly parents and attending to the other demands of life. Because of the unique balance of power in the adult child/elderly parent relationship as well as other negative influences, many adult children find caring for parents a frustrating task. This article argues a solution to this dilemma can be found in a renewed appreciation of filial piety as it specifically relates to caring for elderly parents. Using the moral (...) insights of Aristotle and Aquinas, this paper develops a contemporary theoretical framework for geriatric filial piety that incorporates the traditional virtues of gratitude, respect, honor and obedience. It also illustrates their practical application so that adult children can find caring for their elderly parents a meaningful activity. (shrink)

With improvement in medical services in the last few years, there has been a constant rise in the geriatric population throughout the world, more so in the developing countries. The elderly are highly prone to develop psychiatric disorders, probably because of age related changes in the brain, concomitant physical disorders, as well as increased stress in later life. Psychiatric disorders in this population may have a different presentation than in other groups and some of psychopathologies might be mistaken for (...) normal age related changes by an unwary clinician. The aim of this work focuses on the relationship among the Dark Triad, perceptions of organizational politics, political skills, and counterproductive work behavior. This study empirically tests the mediating role of perceptions of organizational politics in the relationship between the Dark Triad and counterproductive work behavior. Furthermore, the study investigates the moderating role of political skills in strengthening the link between the Dark Triad and the perceptions of organizational politics. A sample of 149 participants was randomly selected. (...) To analyze the data of the present work, we employed a structural equation model using partial least square and PROCESS. From empirical findings, we imply an inference that perception of organizational politics partially mediates the Dark Triad's influence on the counterproductive work behavior. Moreover, the results identify the moderating role of political skills in strengthening the link between the Dark Triad and the perceptions of organizational politics. We investigate the relationship between the dark triad personality traits (Machiavellianism, narcissism, and psychopathy) of managers and the practice of reporting manipulation using a primary survey of 837 professionals working in accounting and finance departments. We find that (a) managers who exhibit dark personality traits are associated with a higher prevalence of fraudulent accounting practices in their accounting and finance departments and (b) traditional risk management mechanisms are only partially effective in mitigating this effect. Internal audits are effective in (...) curtailing the negative behavior of managers with dark triad traits only if these internal audits are outsourced and performed by independent external personnel but not if they are conducted by internal personnel. This suggests that managers with dark triad traits are able to manipulate other employees quite effectively. Consequently, having external personnel perform the auditing task provides a safeguard against such unethical practices and manipulation. This finding has strong practical implications, as it provides support for outsourcing internal audits rather than keeping them in-house. (shrink)

Two doctors attempt to answer this question, one a specialist in geriatric medicine, the other a psychiatrist interested in the psychiatric problems of the elderly and the old. Both, however, come to the same general conclusion: attitudes of the doctors themselves and of society must be changed. These attitudes can determine not only whether an old person lives or dies but how he lives. Old people should not have to survive in mentally suspended animation with all objectives gone but (...) should be helped to achieve the goal of an independent life until death in their own homes. The third paper in this miniature symposium is provided by David Hobman, of Age Concern, who amplifies the specialist views of the first two writers in his discussion of attitudes to the elderly in modern society. He hints, however, that these may shortly change as the elderly become a sophisticated and powerful lobby which governments are forced to heed. (shrink)

The world of the early Christian centuries in which the Trinity was developed as a tenet of belief included several religious and philosophical systems with similar beliefs. Triads and Trinity examines three possible areas of impact: Judaism, the religion of Egypt, and various Greek traditions. Whereas a pluralistic concept of God was inherited by Judaism, it eventually accepted a firm monotheism.

Despite being a collection of holistic assessment tools, the comprehensive geriatric assessment primarily focuses on the social category of age during the assessment and disregards for example gender. This article critically reviews the standardized testing process of the comprehensive geriatric assessment in regard to diversity-sensitivity. I show that the focus on age as social category during the assessment process might potentially hinder positive outcomes for people with diverse backgrounds of older patients in relation to other social categories, such (...) as race, gender or socio-economic background and their influence on the health of the patient as well as the assessment and its outcomes. I suggest that the feminist perspective of intersectionality with its multicategorical approach can enhance the diversity-sensitivity of the comprehensive geriatric assessment, and thus improve the treatment of older patients and their quality of life. By suggesting an intersectional-based approach, this article contributes to debates about justice and diversity in medical philosophy and advocates for the normative value of diversity in geriatric medicine. Routine management of geriatric problems often raises ethical problems, particularly regarding autonomy of the old person. The carers or children may be unaware of the sensitivity of role reversal in dealing with the financial affairs; the need for a residential carer may compromise the old person’s privacy. Attending a day centre confers much benefit, but one must understand the old person’s resistance to change in the proposal of a new daily regimen. Similarly his or her autonomy must be the (...) priority in planning for admittance to an old age home, and not the assumption that the family knows best. A common dilemma is the assessment of an old person’s competency in decision making, either about management of his affairs, or regarding consent to treatment, or participation in research. Because cognitive capacity is not always identical with competency, meaningful tools have recently been developed in which the emphasis is on the specific situation to be investigated. (shrink)

The article begins with a response to a recent contribution by Jensen, in which he has criticized several aspects of the use of triads of elements, including Döbereiner’s original introduction of the concept and the modern use of atomic number triads by some authors including myself. Such triads are groups of three elements, one of which has approximately the average atomic weight of the other two elements, as well as having intermediate chemical reactivity. I also examine Jensen’s attempted reconstruction Mendeleev’s (...) use of triads in predicting the atomic weights of three hitherto unknown elements, that were subsequently named gallium, germanium and scandium. The present article then considers the use of atomic number triads, in conjunction with the phenomenon of first member anomaly, in order to offer support for Janet’s left-step periodic table, in which helium is relocated into group 2 of the table. Such a table features triads in which the 2nd and third elements of each group, without fail, fall into periods of equal length, a feature that is absent in the conventional 18-column or the conventional 32-column table. The dual sense of the term element, which is the source of much discussion in the philosophy of chemistry, is alluded to in further support of such a relocation of helium that may at first appear to contradict chemical intuition. The Dark Triad traits – narcissism, Machiavellianism and psychopathy – have been found to be associated with intra- or interpersonal deception production frequency. This cross-sectional study (N = 207) investigated if the Dark Triad traits are also associated with deception detection accuracy, as implicated by the recent conception of a deception-general ability. To investigate associations between maladaptive personality space and deception, the PID-5 maladaptive personality traits were included to investigate if besides Machiavellianism, Detachment is negatively associated with response (...) bias. Finally, associations between the Dark Triad traits, Antagonism, Negative Affectivity and confidence judgments were investigated. Participants watched videos of lying vs. truth-telling senders and judged the truthfulness of the statements. None of the Dark Triad traits was found to be associated with the ability to detect deception. Detachment was negatively associated with response bias. Psychopathy was associated with global confidence judgments. The results provide additional support that dark and maladaptive personality traits are associated with judgmental biases but not with accuracy in deception detection. The internal consistencies of 4 of the 8 subscales of the used personality short scales were only low and nearly sufficient (αs =.65–.69). (shrink)

While there is a growing literature on “dark traits” (i.e., socially aversive traits), there has been a lack of integration with the burgeoning research literature on positive traits and fulfilling and growth-oriented outcomes in life. To help move the field toward greater integration, we contrasted the nomological network of the Dark Triad (a well-studied cluster of socially aversive traits) with the nomological network of the Light Triad, measured by the 12-item Light Triad Scale (LTS). The LTS is (...) a first draft measure of a loving and beneficent orientation toward others (“everyday saints”) that consists of three facets: Kantianism (treating people as ends unto themselves), Humanism (valuing the dignity and worth of each individual), and Faith in Humanity (believing in the fundamental goodness of humans). Across four demographically diverse samples (N = 1,518), the LTS demonstrated excellent reliability and validity, predicting life satisfaction and a wide range of growth-oriented and self-transcendent outcomes above and beyond existing measures of personality. In contrast, the Dark Triad was negatively associated with life satisfaction and growth-oriented outcomes, and showed stronger linkages to selfish, exploitative, aggressive, and socially aversive outcomes. The present cross-sectional study (N = 205) tested the hypothesis that the Dark Triad traits – narcissism, Machiavellianism, and psychopathy – and the PID-5 maladaptive personality traits – Negative Affectivity, Detachment, Antagonism, Disinhibition and Psychoticism – are associated with specific deception-related perceptions: perceived cue-based deception detectability, perceived deception production, and deception detection ability. Participants completed personality and deception measures in an online setting. All three Dark Triad traits and Antagonism were associated with perceived deception production ability, but not (...) It is commonly believed thatgeriatric medicine generates a distinctive setof ethical problems. Implicated are such issuesas resource allocation, competence and consent,advance directives, medical futility anddeliberate death. It is also argued that itwould be unjust to allow the elderly to competewith younger populations for expensive andscarce health care resources because theelderly “have already lived,” and that treatingthem the same as these other populations woulddiminish the available resources unfairly,prolong a life of inevitably failing health andresult in increased health care expenditures.In fact, however, (...) this perception of ethicaluniqueness is mistaken. Differences in medicalconditions, demographics and aetiology shouldnot be allowed to obscure the fact that ethicalissues in geriatric medicine are essentiallythe same as those faced in any other area ofhealth care, and that the solutions that areadopted in the geriatric context must beconsistent with the ethical principles that arefollowed elsewhere. The paper argues that theroot of the mistaken perception lies in theabandonment of the Hippocratic mandate ofmedicine and in an unreflective adherence tothe belief that medical advances are inevitablybeneficial. We study the relationship between risk managers’ dark triad personality traits (Machiavellianism, narcissism, and psychopathy) and their selective hedging activities. Using a primary survey of 412 professional risk managers, we find that managers with dark personality traits are more likely to engage in selective hedging than those without. This effect is particularly pronounced for older, male, and less experienced risk managers. The effect is also stronger in smaller firms, less centralized risk management departments, and family-owned firms.

Respecting and encouraging autonomy in the elderly is basic to the practice of geriatrics. In this paper, we examine the practice of cardiopulmonary resuscitation (CPR) and "artificial" feeding in a geriatric unit in a general hospital subscribing to jewish orthodox religious principles, in which the sanctity of life is a fundamental ethical guideline. The literature on the administration of food and water in terminal stages of illness, including dementia, still shows division of opinion on the morality of withdrawing nutrition. (...) We uphold the principle that as long as feeding by naso-gastric (N-G) or percutaneous endoscopic gastrostomy (PEG) does not constitute undue danger or arouse serious opposition it should be given, without causing suffering to the patient. This is part of basic care, and the doctor has no mandate to withdraw this. The question of CPR still shows much discrepancy regarding elderly patients' wishes, and doctors' opinions about its worthwhileness, although up to 10 percent survive. Our geriatric patients rarely discuss the subject, but it is openly ventilated with families who ask about it, who are then involved in the decision-making, and the decision about CPR or "do-not-resuscitate" (DNR) is based on clinical and prognostic considerations. Following Deleuze’s analysis of expressivity, this article approaches the two opening aphorisms of the Dao De Jing 道德經 as two movements of a triadic differentiation of expressive elements. These aphorisms are further presented within four Deleuzean paradoxes which necessarily accompany linguistic expression. Simultaneously, the opening lines are also analyzed as philosophical problems which constitute the core of the Daoist project itself within which the unthinkable or ineffable must be conceived of not as conditioned by privation or negation, but as being (...) located within the movement of differentiation which is by nature positive and creative. As the paper shows, the linguistic involvement here is not one of discrimination but of a fundamentally dynamic immersion in problematic thought which underlies the mechanism of sense-making. This dynamism is presupposed by the disequilibrium between what expresses itself and its expressions, which is the birthplace of sense as the expressed. Our analysis of the latter is unfolded through an analogy drawn between Daoist and Spinozian expressionism supported by Deleuze’s study on Spinoza. (shrink)

Headteachers in the community schools are responsible for academic and administrative activities. They are considered role models for integrity practices. However, the situation is different on some occasions. For instance, political interest plays more important roles than qualifications, training, work experience, etc., in school affairs. In this context, a qualitative study was carried out in the purposefully selected two schools – an urban and a rural – to explore the elements that influence school integrity practices. Data collected from the schools (...) were analyzed and interpreted. The findings show that nepotism, unhealthy party politics, and impunity influence the school integrity practices in Nepal. Claims that talk of life’s meaning is misguided, unmanageable or, worse, nonsensical, are overblown. Such claims especially track the cosmically focused the meaning of life. “The meaning of life” is perfectly intelligible, and is centered on a cluster of ideas encapsulated by what I call the “meaning triad.” One component of this triad—I-MEANING—provides the hermeneutical and conceptual resources for understanding the question “What is the meaning of life?” as asking for a single thing, in contrast to amalgam and (...) pluralist views. Within psychiatry, patients, family, and professionals are involved and interrelated. Yet it is not easy for healthcare professionals to involve family actively in patient care. Taking a feminist perspective, we investigate why health-care professionals experience ambivalence in involving family in attempts to reduce seclusion, suggesting how they can improve family involvement by adopting a relational view on autonomy. Professionals should view patients not only in terms of individual autonomy and rights, but also in terms of relations and dependencies that need (...) to be considered in fostering patient care. This daily diary study examined the within-person coupling between four emotion regulation strategies and both subjective well-being and perceived stress in daily life of geriatric nurses. Participants (N = 89) described how they regulated their emotions in terms of cognitive reappraisal and suppression. They also indicated their subjective well-being and level of perceived stress each day over three weeks. At the within-person level, cognitive reappraisal intended to increase positive emotions was positively associated with higher subjective well-being and negatively associated (...) with perceived stress. Suppression of the expression of positive emotions was negatively associated with subjective well-being and positively associated with perceived stress. However, cognitive reappraisal intended to down-regulate negative emotions and suppression as a strategy to inhibit the expression of negative emotions were not associated with daily well-being or perceived stress. Off-days were rated as days with higher subjective well-being and lower perceived stress in contrast to working days. At the between-person level, individuals who reported more daily negative affect reported increased suppression of positive emotions, corroborating the within-person findings. Moreover, findings indicated that nurses with more years of experience in the job reported higher subjective well-being and less perceived stress. These results provide insights into important daily emotional processes of geriatric nurses, both at workdays and in their leisure time. (shrink)

The idealistic, political and military causes and effects of the Warsaw Uprising are discussed by the author against a historical background and on the basis of his own experience as a participant in the fighting. Portrayed are its instigators’ and participants’ reasoning and ambitions as well as the revolt’s ultimate political and military defeat, whose tragic aftermath evoked heated discussions and mutual accusations among Poles. Klejn also dwells on the deep meaning of the uprising, whose ideals gradually led to the (...) 1989 changes in Poland. In his opinion, the tragic fate of Warsaw and its inhabitants was decided, but the consequences and conclusions of this longest uprising of the Second World War have become values that shaped the modern Polish nation and that constitute its contribution for the newly created European order.
