Childbirth in the United States is in crisis. This is especially true for Black and brown mothers. This childbirth emergency constitutes a failure of the social contract: because society has failed to provide minimally decent care for all birthing mothers, but especially for Black and brown mothers, it is necessary to allow mothers to choose home birth. I amplify the voices of Black and brown scholars and midwives to defend home birth, and I argue that (...) class='Hi'>home birth is safe and empowering and that it is rational for those who desire it to choose it. Native women residing in the Subarctic and Arctic are currently struggling for the right to decide whether they will be hospitalized or have a midwife present for the birth of their children. The argument presented in this review paper outlines the cultural and clinical factors in favour of recognizing and legalizing traditional midwifery in the North and critically examines the statistical and safety concerns raised by those arguing against giving Northern Native women an alternative to evacuation from their home (...) communities. The home birth movement in the United States is an alternative health belief system that promotes a model of pregnancy and childbirth contradictory to the conventional biomedical model. The alternative model stresses normalcy and non-intervention and is informed by an ideology that promotes individual authority and responsibility for health and health care. It is founded in an epistemological system that assigns primacy and goodness to the Natural, fuses moral and practical injunctions in the arena of health behavior, and (...) valorizes subjective as well as objective sources of knowledge. Differences of opinion with the conventional medical model of childbirth do not spring from misunderstanding of this model, but from disagreement with it. In the United States, clinical interventions such as epidurals, intravenous infusions, oxytocin, and intrauterine pressure catheters are used almost routinely in births in the hospital setting, despite evidence that the overutilization of such interventions likely plays a key role in increasing the need for cesarean section (CS). In 2010, according to the U.S. Centers for Disease Control and Prevention, approximately 32.8 percent of births in the U.S. were by CS. The U.S. National Institutes of Health has reported that CS increases (...) avoidable maternal and neonatal morbidity and mortality. To increase understanding of what might motivate the overuse of CS in the U.S., we investigated the factors that influenced women’s decision making around childbirth, because women’s conscious and unconscious choices about giving birth could influence whether they would choose or allow delivery by CS.In this article, we report findings about women’s decisions related to place of birth—at home or in a hospital. We found that choosing a place of birth was significant in how women in our study attempted to mitigate their perceptions of the risks of childbirth for themselves and their infant. Concern for the safety of the infant was a central, driving factor in the decisions women made about giving birth, and this concern heightened their perceptions of the risks of childbirth. Heightened perceptions of risk about the safety of the fetus during childbirth were found to affect women’s ability to accurately assess the risk of using clinical interventions such as the time of admission, epidural anesthesia, oxytocin, or cesarean birth, which has important implications for clinical practice, prenatal education, perinatal research, medical decision making, and informed consent. Background Changes in policies and practices related to repeat cesareans and home birth in the U.S. have been influenced by different interpretations of the risk of poor outcomes. Methods This article examines two cases—vaginal birth after cesarean (VBAC) and home birth to illustrate how an emphasis on relative over absolute risk has been used to characterize outcomes associated with these practices. The case studies will rely on reviews of the research literature and examination of data on birth trends (...) and outcomes. Results Childbirth involves some unique challenges in assessing health risks, specifically the issues of: (1) timing of risks (lowering health risk in a current birth can increase it in subsequent births); (2) the potential weighing of risks to the mother’s versus the infant’s health; (3) the fact that birth is a condition of health and many of the feared outcomes (for example, symptomatic uterine rupture) involve very low absolute risk of occurrence; and (4) a malpractice environment that seizes upon those rare poor outcomes in highly publicized lawsuits that receive widespread attention in the clinical community. In the cases of VBAC and home birth, the result has been considerable emphasis on relative risks, typically an adjusted odds ratio, with little consideration of absolute risks. Conclusion Assessments of the safety of interventions in childbirth should involve careful consideration and communication of the multiple dimensions of risk, particularly a balancing of relative and absolute risks of poor health outcomes. Henry Home (1696-1782) has been called "perhaps the most complete 'Enlightenment man' among the eighteenth-century Scottish thinkers." Kinsman and friend of David Hume, mentor and patron of Adam Smith, John Millar, and Thomas Reid, he was a key figure in that circle of luminaries. He read law, was called to the bar in 1723, was raised to the Bench of the Court of Session in 1752, with the title Lord Kames (the name of his family estate), and joined the (...) High Court of the Justiciary in 1763. Publishing broadly in law, history, philosophy, and criticism, Kames made significant contributions to the Enlightenment's science of human nature. The Essays is commonly considered Kames's most important philosophical work. In the first part, he sets forth the principles and foundations of morality and justice, attacking Hume's moral skepticism and addressing the controversial issue of the freedom of human will. In the second part, Kames focuses on questions of metaphysics and epistemology to offer a natural theology in which the authority of the external senses is an important basis for belief in the Deity. The text of this volume is based on the third edition of 1779, while the appendix presents substantial variant readings in the first and second editions. (shrink)

p166 In February 1965, the United States escalated the war against South Vietnam radically, and also, on the side, began regular bombing of the North at a much lower level. That was a big public issue in the United States: Should we bomb North Vietnam? The bombing of the South was ignored. The same shows up in the internal planning, for which we now have an extremely rich record, not only from the Pentagon Papers, but from tons of declassified documents (...) that have been released in the last couple of years. It turns out again, one of the very few interesting revelations of the Pentagon Papers that there was no planning for the escalated bombing of the South. There was very meticulous planning about the bombing of the North carefully calibrated, when should we do it, and a lot of agonizing about it. The bombing of the South at triple the scale of the North is barely discussed. There are a few casual decisions here and there. The same shows up in McNamara's recent memoirs. He discusses at great length the bombing of the North. The bombing of the South he literally doesn't mention. He mentions what he did on January 21, 1965, a really important day: there was a big discussion about whether to bomb North Vietnam. He doesn't mention what we know from other documents, that on that same day, he authorized for the first time the use of jet planes to escalate the bombing of South Vietnam over and above the massive bombing that had been going on for years that's not even mentioned. Richard Bett presents a ground-breaking study of Pyrrho of Elis, who lived in the late fourth and early third centuries BC and is the supposed originator of Greek scepticism. In the absence of surviving works by Pyrrho, scholars have tended to treat his thought as essentially the same as the long subsequent sceptical tradition which styled itself 'Pyrrhonism'. Bett argues, on the contrary, that Pyrrho's philosophy was significantly different from this later tradition, and offers the first detailed account of that (...) philosophy in this light. Bett considers why Pyrrho was adopted as the figurehead for that tradition: his answer suggests that we should distinguish two phases within Pyrrhonism, of which the initial phase is much closer to Pyrrho's own thought than is the better-known later phase. Bett also investigates the origins and antecedents of Pyrrho's ideas; in particular, Plato is singled out as an important inspiration. The result is the first comprehensive picture of this key figure in the development of philosophy. The new claims that Bett puts forward have major implications for the history and interpretation of ancient Greek thought. (shrink)

Physics first became established in Australia and Japan at the same period, during the final quarter of the nineteenth and the first years of the twentieth century. A comparison of the processes by which this happened in these two developing countries on the Pacific rim shows that, despite the great cultural differences that existed, and that might have been expected to have been a source of major differences in national receptiveness to the new science, there were in fact many parallels (...) between the patterns of development in the two cases. Identifying these enables us to draw attention to a number of significant features of the physics discipline more generally at this period. Such differences as emerge in the early history of physics in the two countries seem to have arisen more from the different political situations that prevailed than from anything else; in particular they reflect the fact that Australia was a part of the British Empire while Japan was an independent political power. Discussions on the nature of reality between Albert Einstein and Rabindranath Tagore, Bengali poet, philosopher and Nobel laureate, have provoked interest among both physicists and philosophers since their first publication in 1930/31. This article points out their relevance to past and present debates about the meaning of quantum mechanics.

In 1914, the physics discipline had reached a very similar stage of development in Australia and Japan. A generation later the paths of development had considerably diverged. A systematic comparison of the evolution of physics in the two countries during these years identifies factors—political, economic and cultural—that led to this divergence, but it also uncovers a number of underlying parallels.

We argue that the inference of CP violation in experiments involving the K0-K0 system in weak interactions of particle physics is facilitated by the assumption of particle trajectories for the decaying particles and the decay products. A consistent explanation in terms of such trajectories is naturally incorporated within the Bohmian interpretation of quantum mechanics.

Leonhard Euler was the leading eighteenth-century critic of Isaac Newton's projectile theory of light. Euler's main criticisms of Newton's views are surveyed, and also his alternative account according to which light is a wave motion propagated through the aether. Important changes are identified as having occurred between 1744 and 1746 in Euler's thinking about the way in which a wave such as he supposed light to be is propagated through a medium. Paradoxically, in view of Euler's overtly anti-Newtonian stand, these (...) amount to Euler abandoning his early, Malebranchian notions about the physical basis of wave propagation, in favour of the ideas set out by Newton in Book II of his Principia. We tend to think that a person who is both reasonable and moral can have a good life. What constitutes a life that is not only good but superlative, or even “marvellous” or “holy”? Those who have such lives are called sages, heroes or saints, and their lives can display great integrity as well as integration with a transformative “Spiritual Presence.” Does it follow that saints are perfect people? Is there a common vision that impels them to seek holiness? In (...) a controversial interpretation of mysticism Horne suggests that there is no single formula for the meaning of life and no one story that displays it to us. Mysticism, rather than being just a visionary perception, then becomes a problem-solving process that brings about a creative transformation of the personality at a critical stage in life. He suggests also that saints may be imperfect morally and describes true saints as double-minded: they are serious in a playful way. Mysticism and Vocation illuminates our understanding of saintly lives by explicating their mystical characteristics and extending discussions of mysticism to explain its role in active lives. In discussing important decisions that go beyond conventional morality, it adds to recent philosophical arguments by Charles Taylor, Alasdair MacIntyre, Iris Murdoch, Bernard Williams and others to the effect that morality should be defined more broadly to deal with the human condition. Mysticism is condemned as often as it is praised. Much of the condemnation comes from mysticism’s apparent disregard of morality and ethics. For mystics, the experience of “union” transcends all moral concern. In this careful examination of the works of such practitioners or examiners of mysticism as Paul Tillich, Thomas Merton, Evelyn Underhill, and Martin Buber, the author posits a spectrum of uneasy relationships between mysticism and morality. Many of us have a prereflective sense--or at least, a hope--that there are reasons to be moral which apply to an agent regardless of what his or her existing motivations may be. The view that there are no such reasons may, then, be regarded as a form of moral scepticism. The philosophical position which seems most fit to refute this form of moral scepticism, and hence to support our prereflective sense, is a Kantian view of morality, according to which we (...) are required to be moral solely in virtue of being rational. I examine a number of recent arguments for this position; I conclude that all of them fail, and for similar reasons. Nor, I argue, is there any non-Kantian view which could succeed at the same task. It is thus very tempting to infer that this form of moral scepticism is immune to any simple refutation. However, perhaps the best way to combat scepticism is not to argue for its negation--as do the positions examined so far--but to show that there is something problematic in the sceptic's whole approach to the issue. At this point I turn for enlightenment to an analogy with epistemological scepticism. Here, too, if we try to argue for the negation of scepticism, we seem bound to fail; but here the more subtle strategy of attacking the sceptic's whole approach has actually been attempted. I sketch a number of versions of this more subtle strategy; I then sketch some analogous responses to moral scepticism, and consider whether any of them are successful. I argue that the attempt fails--that even if epistemological scepticism is indeed amenable to this kind of treatment , moral scepticism is not. This conclusion is only strengthened when I argue that another, related strategy, which employs an analogy between morality and mathematics, also falls short at the same point. Moral scepticism therefore survives intact. The appropriate reaction, I suggest, is neither complete despair nor complete equanimity. (shrink)

We argue that the Aharonov-Anandan-Vaidman model, by using the notion of so-called “protective measurements,” cannot claim to have dispensed with the ldcollapse of the wave function,” because it does not succeed in avoiding the quantum measurement problem. Its claim to be able to distinguish between two nonorthogonal states is also critically examined.
