Informed consent is a key condition for prenatal screening programmes to reach their aim of promoting reproductive autonomy. Reaching this aim is currently being challenged with the introduction of non-invasive prenatal testing (NIPT) in first-trimester prenatal screening programmes: amongst others its procedural ease—it only requires a blood draw and reaches high levels of reliability—might hinder women’s understanding that they should make a personal, informed decision about screening. We offer arguments for a renewed recognition and use of informed consent compared to (...) informed choice, and for a focus on value-consistent choices and personalized informational preferences. We argue for a three-step counselling model in which three decision moments are distinguished and differently addressed: (1) professionals explore women’s values concerning whether and why they wish to know whether their baby has a genetic disorder; (2) women receive layered medical-technical information and are asked to make a decision about screening; (3) during post-test counselling, women are supported in decision-making about the continuation or termination of their pregnancy. This model might also be applicable in other fields of genetic (pre-test) counselling, where techniques for expanding genome analysis and burdensome test-outcomes challenge counselling of patients. In the debate surrounding the introduction of non-invasive prenatal testing in prenatal screening programmes, the concept of routinisation is often used to refer to concerns and potential negative consequences of the test. A literature analysis shows that routinisation has many different meanings, which can be distinguished in three major versions of the concept. Each of these versions comprises several inter-related fears and concerns regarding prenatal screening and particularly regarding NIPT in three areas: informed choice, freedom to choose and consequences for (...) people with a disability. Three of the strongest arguments raised under the flag of routinisation are assessed for their validity: the threat that NIPT poses to informed choice, the potential increase in uptake of first-trimester prenatal screening and its consequences for social pressure to participate in screening or terminate affected pregnancies, and the negative consequences for disabled people. These routinisation arguments lack empirical or normative ground. However, the results of this analysis do not imply that no attention should be paid to possible problems surrounding the introduction of NIPT. At least two problems remain and should be addressed: there should be an ongoing debate about the requirements of informed choice, particularly related to an expanded scope of prenatal screening. Also, reproductive autonomy can only be achieved when expecting parents’ options are variegated, real and valuable, so that they can continue to choose whether or not to screen or to terminate a pregnancy. In this article I give a naturalistic-cum-formal analysis of therelation between beauty, empirical success, and truth. The analysis is based on the onehand on a hypothetical variant of the so-called `mere-exposure effect' which has been more orless established in experimental psychology regarding exposure-affect relationshipsin general and aesthetic appreciation in particular (Zajonc 1968; Temme 1983; Bornstein 1989;Ye 2000). On the other hand it is based on the formal theory of truthlikeness andtruth approximation as presented in my From Instrumentalism to Constructive Realism (...) (2000).The analysis supports the findings of James McAllister in his beautifulBeauty and Revolution in Science (1996), by explaining and justifying them.First, scientists are essentially right in regarding aesthetic criteria useful for empiricalprogress and even for truth approximation, provided they conceive of them as less hard thanempirical criteria. Second, the aesthetic criteria of the time, the `aesthetic canon', maywell be based on `aesthetic induction' regarding nonempirical features of paradigms of successfultheories which scientists have come to appreciate as beautiful. Third, aestheticcriteria can play a crucial, schismatic role in scientific revolutions. Since they may well be wrong, they may, in the hands of aesthetic conservatives, retard empirical progress and hence truth approximation, but this does not happen in the hands of aesthetically flexible, `revolutionary' scientists.We may find totallyopposite things beautiful: a simplemathematical principle as well as a series of unrepeatable complex contingencies. Three related intuitions are explicated in this paper. The first is the idea that there must be some kind of probabilistic version of the HD-method, a 'Hypothetico-Probabilistic method', in terms of something like probabilistic consequences, instead of deductive consequences. According to the second intuition, the comparative application of this method should also be functional for some probabilistic kind of empirical progress, and according to the third intuition this should be functional for something like probabilistic truth approximation. In all three cases, (...) the guiding idea is to explicate these intuitions by explicating the crucial notions as appropriate 'concretizations' of their deductive analogs, being 'idealizations'. It turns out that the comparative version of the proposed HP-method amounts to the likelihood comparison method applied to the cumulated evidence. This method turns out to be not only functional for probabilistic empirical progress but also for probabilistic truth approximation. The latter is based on a probabilistic threshold theorem constituting for this reason the analog of the deductive success theorem. (shrink)

Coalitional games raise a number of important questions from the point of view of computer science, key among them being how to represent such games compactly, and how to efficiently compute solution concepts assuming such representations. Marginal contribution nets , introduced by Ieong and Shoham, are one of the simplest and most influential representation schemes for coalitional games. MC-nets are a rulebased formalism, in which rules take the form pattern → value, where “pattern ” is a Boolean condition over agents, (...) and “value ” is a numeric value. Ieong and Shoham showed that, for a class of what we will call “basic” MC-nets, where patterns are constrained to be a conjunction of literals, marginal contribution nets permit the easy computation of solution concepts such as the Shapley value. However, there are very natural classes of coalitional games that require an exponential number of such basic MC-net rules. We present read-once MC-nets, a new class of MC-nets that is provably more compact than basic MC-nets, while retaining the attractive computational properties of basic MC-nets. We show how the techniques we develop for read-once MC-nets can be applied to other domains, in particular, computing solution concepts in network flow games on series-parallel networks. (shrink)

Surprisingly enough, modified versions of the confirmation theory of Carnap and Hempel and the truth approximation theory of Popper turn out to be smoothly synthesizable. The glue between confirmation and truth approximation appears to be the instrumentalist methodology, rather than the falsificationist one.By evaluating theories separately and comparatively in terms of their successes and problems (hence even if they are already falsified), the instrumentalist methodology provides – both in theory and in practice – the straight route for short-term empirical progress (...) in science in the spirit of Laudan. However, it is argued that such progress is also functional for all kinds of truth approximation: observational, referential, and theoretical. This sheds new light on the long-term dynamic of science and hence on the relation between the main epistemological positions, viz., instrumentalism (Toulmin, Laudan), constructive empiricism (van Fraassen), referential realism (Hacking and Cartwright), and theory realism of a non-essentialist nature (Popper), here called constructive realism.In From Instrumentalism to Constructive Realism (2000) the above story is presented in great detail. The present synopsis highlights the main ways of theory evaluation presented in that book, viz. evaluation in terms of confirmation (or falsification), empirical progress and truth approximation. This paper primarily deals with the conceptual prospects for generalizing the aim of abduction from the standard one of explaining surprising or anomalous observations to that of empirical progress or even truth approximation. It turns out that the main abduction task then becomes the instrumentalist task of theory revision aiming at an empirically more successful theory, relative to the available data, but not necessarily compatible with them. The rest, that is, genuine empirical progress as well as observational, referential and theoretical (...) truth approximation, is a matter of evaluation and selection, and possibly new generation tasks for further improvement. The paper concludes with a survey of possible points of departure, in AI and logic, for computational treatment of the instrumentalist task guided by the ‘comparative evaluation matrix’. (shrink)

Empathy and sympathy are feeling-acts which bring the self into direct encounter with other persons. In empathy a self grasps the affective act of another self; in sympathy x n persons apprehend a common object while immersed in similar feeling acts. Since touch is the paradigmatic sense for bringing what is felt into proximity with feeling, structural affinities between touch and these feeling acts can be shown. This relationship has been obscured by classical theories of touch in which it is (...) interpreted on analogy with the other senses. When the subject of touch is seen as the living body as a whole, the full range of its possible relationships to affective states can be explored. In this connection the theories of touch of Aristotle, Berkeley and Condillac are critically evaluated. While none recognizes the uniqueness of touch, each sees difficulties in incorporating touch in a general theory of sense. In this paper, we address the problem of truth approximation through theory change, asking whether revising our theories by newly acquired data leads us closer to the truth about a given domain. More particularly, we focus on “nomic conjunctive theories”, i.e., theories expressed as conjunctions of logically independent statements concerning the physical or, more generally, nomic possibilities and impossibilities of the domain under inquiry. We define both a comparative and a quantitative notion of the verisimilitude of such theories, and identify (...) suitable conditions concerning the (partial) correctness of acquired data, under which revising our theories by data leads us closer to “the nomic truth”, construed as the target of scientific inquiry. We conclude by indicating some further developments, generalizations, and open issues arising from our results. (shrink)

We investigate the effect on the complexity of adding the universal modality and the reflexive transitive closure modality to modal logics. From the examples in the literature, one might conjecture that adding the reflexive transitive closure modality is at least as hard as adding the universal modality, and that adding either of these modalities to a multi-modal logic where the modalities do not interact can only increase the complexity to EXPTIME-complete. We show that the first conjecture holds under reasonable assumptions (...) and that, except for a number of special cases which we fully characterize, the hardness part of the second conjecture is true. However, the upper bound part of the second conjecture fails miserably: we show that there exists a uni-modal, decidable, finitely axiomatizable, and canonical logic for which adding the universal modality causes undecidability and for which adding the reflexive transitive closure modality causes high undecidability. A person's cognitive map, or knowledge of large‐scale space, is built up from observations gathered as he travels through the environment. It acts as a problem solver to find routes and relative positions, as well as describing the current location. The TOUR model captures the multiple representations that make up the cognitive map, the problem‐solving strategies it uses, and the mechanisms for assimilating new information. There is an increasing recognition internationally of the critical impact of communication within healthcare. The link between ineffective communication, patient dissatisfaction and critical incidents is well established. Family Planning New South Wales has sought to address patient-centred care and communication in its policy platform. This article reports on research conducted within FPNSW, which analysed the discourse features that constituted effective doctor–patient1 communication in sexual and reproductive health consultations. The principal aim of the research was to understand how effectively messages were (...) conveyed and received and to what degree patients were active participants in their own sexual healthcare. Analysed consultations were characterised by extremely high levels of communicative competence on the part of the doctors who integrated medical expertise with the development of interpersonal relationships with patients, thus positioning patients as active contributors to the consultations and to decisions about their ongoing treatment. The detailed linguistic analysis identified characteristic features of patient-centred communication that are essential to patient-centred care. These interactions demonstrate that communicating care is just as important as delivering care and involves a drawing together of the medical and the interpersonal in consultations. The article details strategies for interweaving medical knowledge and establishing rapport that can inform practitioner communication practices across different healthcare contexts. (shrink)
