This text explores the border between medical and social problems, the boundary between health and social change. The book studies the body as the mediator between individual and collective experience, finding that many health problems, for example the trauma of violence or depression in the course of chronic pain, are less individual medical problems than interpersonal experiences of social suffering. It argues for an ethnographic approach to moral practice in medicine, one that embraces the infrapolitical context of illness, responses to (...) it, the social institutions relating to it and the way it is configured in medical ethics. Modern socio?cultural studies of medicine demonstrate the symbolic character of much of medical reality. This symbolic reality can be appreciated as mediating the traditional division of medicine into biophysical and human sciences. Comparative studies of medical systems offer a general model for medicine as a human science. These studies document that medicine, from an historical and cross?cultural perspective, is constituted as a cultural system in which symbolic meanings take an active part in disease formation, the classification and cognitive management of (...) illness, and in therapy. Medicine's symbolic reality also forms a bridge between cultural and psychophysiological phenomena; the basis for psychosomatic and socioso?matic pathology and therapy. Trust is indispensable not only for interpersonal relationships and social life, but for good quality healthcare. As manifested in the increasing violence and tension in patient-physician relationships, China has been experiencing a widespread and profound crisis of patient–physician trust. And globally, the crisis of trust is an issue that every society, either developing or developed, has to face in one way or another. Yet, in spite of some pioneering works, the subject of patient-physician trust and mistrust – a crucial matter (...) in healthcare especially because there are numerous ethical implications – has largely been marginalized in bioethics as a global discourse. Drawing lessons as well as inspirations from China, this paper demonstrates the necessity of a trust-oriented bioethics and presents some key theoretical, methodological and philosophical elements of such a bioethics. A trust-oriented bioethics moves beyond the current dominant bioethical paradigms through putting the subject of trust and mistrust in the central agenda of the field, learning from the social sciences, and reviving indigenous moral resources. In order for global bioethics to claim its relevance to the things that truly matter in social life and healthcare, trust should be as vital as such central norms like autonomy and justice and can serve as a potent theoretical framework. To investigate the phenomenon of patient–physician mistrust in China, a qualitative study involving 107 physicians, nurses and health officials in Guangdong Province, southern China, was conducted through semi-structured interviews and focus groups. In this paper we report the key findings of the empirical study and argue for the essential role of medical professionalism in rebuilding patient-physician trust. Health professionals are trapped in a vicious circle of mistrust. Mistrust leads to increased levels of fear and self-protection by doctors which exacerbate difficulties (...) in communication; in turn, this increases physician workloads, adding to a strong sense of injustice and victimization. These factors produce poorer healthcare outcomes and increasingly discontented and angry patients, escalate conflicts and disputes, and result in negative media coverage, all these ultimately contributing to even greater levels of mistrust. The vicious circle indicates not only the crisis of patient-physician relationship but the crisis of medicine as a profession and institution. Underlying the circle is the inherent conflict of interest in the healthcare system by which health professionals and hospitals have become profit-driven. This institutional conflict of interest seriously compromises the fundamental principle of medical professionalism—the primacy of patient welfare—as well as the traditional Chinese ideal of “medicine as the art of humanity”. Patient trust can be restored through rectifying this institutional conflict of interest and promoting medical professionalism via a series of recommended practical measures. (shrink)

The last decade has seen an explosion of interest in the health and welfare of marginalized communities around the world. In one striking indicator, public and private development assistance for health programs increased from $8.65 billion in 1998 to $21.79 billion in 2007 [1]. There has been emergent academic interest as well, with growing ranks of undergraduate and graduate students and professionals adopting the field as their specialty. Despite the burgeoning interest, however, much about the field remains unclear. Reimagining Global (...) Health is an important contribution to this budding field for two reasons: it proposes a cohesive introductory text for a field in desperate need of one, and it seeks to “reimagine” some key concepts in global health in an effort to provide a bold new direction for the field. This article utilizes the ongoing debates over the role of certain agricultural insecticides in causing Colony Collapse Disorder —the phenomenon of accelerated bee die-offs in the United States and elsewhere—as an opportunity to contribute to the emerging literature on the social production of ignorance. In our effort to understand the social contexts that shape knowledge/nonknowledge production in this case, we develop the concept of epistemic form. Epistemic form is the suite of concepts, methods, measures, and interpretations that shapes the ways (...) in which actors produce knowledge and ignorance in their professional/intellectual fields of practice. In the CCD controversy, we examine how the privileging of certain epistemic forms intersects with the social dynamics of academic, regulatory, and corporate organizations to lead to the institutionalization of three interrelated and overlapping types of ignorance. We consider the effects of these types of ignorance on US regulatory policy and on the lives of different stakeholders. (shrink)

Although scholarship in the social construction of technology has contributed much to illuminating technological development, most work using this theoretical approach is committed to an agency-centered approach. SCOT scholars have made only limited contributions to illustrating the influence of social structures. In this article, the authors argue for the importance of structural concepts to understanding technological development. They summarize the SCOT conceptual framework defined by Trevor Pinch and Wiebe Bijker and survey some of the methodological and explanatory difficulties that arise (...) with their approach. Then the authors present concepts from organizational sociology and political economy that illuminate structural influences in shaping phenomena of interest to SCOT scholars. These structural concepts can be applied to the study of the design, development, and transformation of technology. The authors conclude that the limited amount of scholarship on structural factors in the social shaping of technological development presents numerous opportunities for research. (shrink)

Global Mental Health is a field of research and practice that addresses the expansion of universal and equitable mental health care worldwide. This article explores the ways the concept of culture is employed in Global Mental Health literature. Global Mental Health advocates and critics assume an ontological separation between ‘nature’ and ‘culture’ to typify mental illness, linking it predominantly to one or the other of these two categories. Advocates of Global Mental Health view mental disorders as a nature–culture hybrid, while (...) critics see them as typically cultural phenomena. The cultural critique of Global Mental Health can be strengthened by a sociological approach to both the role of critique and the uses of the concept of culture within social sciences. As an alternative to the ontologization of culture, we propose a different theoretical approach to the social issues involved in the expansion of international public health care in mental health: Arthur Kleinman's and Didier Fassin’s moral anthropological approaches. Recent studies of Reinhold Niebuhr's life and work demonstrate his continued importance in theology, ethics, and political thought. Historical studies by Heather Warren, Mark Kleinman, and Normunds Kamergrauzis provide new assessments of Niebuhr's role as a political and religious leader in his own time and trace the consequences of the movements in which he participated. They also show us more clearly how his work was connected to the ideas and programs of his contemporaries. Colm McKeogh offers a more systematic (...) treatment of Niebuhr's political realism in relation to just war theory. Niebuhr's intellectual legacy remains disputed, with Stanley Hauerwas and Langdon Gilkey offering radically different assessments of his theology. Hauerwas sees in Niebuhr a close connection to the religious liberalism of William James that precludes any authentic Christian witness. Niebuhr's empiricism reduces God to a necessary feature of human consciousness. Gilkey notes Niebuhr's early use of James' psychology, but discerns a theology of history that is central to Niebuhr's mature work. In that theology, the ground of hope necessarily lies beyond human consciousness, and indeed beyond history itself. (shrink)

In At the Will of the Body , Arthur Frank told the story of his own illnesses, heart attack and cancer. That book ended by describing the existence of a "remission society," whose members all live with some form of illness or disability. The Wounded Storyteller is their collective portrait. Ill people are more than victims of disease or patients of medicine they are wounded storytellers. People tell stories to make sense of their suffering when they turn their diseases (...) into stories, they find healing. Drawing on the work of authors such as Oliver Sacks, Anatole Broyard, Norman Cousins, and Audre Lorde, as well as from people he met during the years he spent among different illness groups, Frank recounts a stirring collection of illness stories, ranging from the well-known--Gilda Radner's battle with ovarian cancer--to the private testimonials of people with cancer, chronic fatigue syndrome, and disabilties. Their stories are more than accounts of personal suffering: they abound with moral choices and point to a social ethic. Frank identifies three basic narratives of illness in restitution, chaos, and quest. Restitution narratives anticipate getting well again and give prominence to the technology of cure. In chaos narratives, illness seems to stretch on forever, with no respite or redeeming insights. Quest narratives are about finding that insight as illness is transformed into a means for the ill person to become someone new. (shrink)
