Aim: Understanding the effects of psychotherapy is a crucial concern for both research and clinical practice, especially when outcome tends to be negative. Yet, while outcome is predominantly evaluated by means of quantitative pre-post outcome questionnaires, it remains unclear what this actually means for patients in their daily lives. To explore this meaning, it is imperative to combine treatment evaluation with quantitative and qualitative outcome measures. This study investigates the phenomenon of non-improvement in psychotherapy, by complementing quantitative pre-post outcome scores (...) that indicate no reliable change in depression symptoms with a qualitative inquiry of patients’ perspectives. Methods: The study took place in the context of a Randomized Controlled Trial evaluating time-limited psychodynamic and cognitive behavioural therapy for major depression. A mixed methods study was conducted including patients’ pre-post outcome scores on the BDI-II-NL and post treatment Client Change Interviews. Nineteen patients whose data showed no reliable change in depression symptoms were selected. A grounded theory analysis was conducted on the transcripts of patients’ interviews. Findings: From the patients’ perspective, non-improvement can be understood as being stuck between knowing versus doing, resulting in a stalemate. Positive changes (mental stability, personal strength and insight) were stimulated by therapy offering moments of self-reflection and guidance, the benevolent therapist approach and the context as important motivations. Remaining issues (ambition to change but inability to do so) were attributed to the therapy hitting its limits, patients’ resistance and impossibility and the context as a source of distress. “No change” in outcome scores therefore seems to involve a “partial change” when considering the patients’ perspectives. Conclusion: The study shows the value of integrating qualitative first-person analyses into standard quantitative outcome evaluation and particularly for understanding the phenomenon of non-improvement. It argues for more multi-method and multi-perspective research to gain a better understanding of (negative) outcome and treatment effects. Implications for both research and practice are discussed. This article discusses the trajectory of Simone de Beauvoir’s concern with the issue of torture. It argues that Beauvoir’s interest in torture extends back at least to World War II and that her activities and writings against torture during the French-Algerian War of 1954–1962 were pivotal in prompting her to reject ethical philosophical language and to embrace, in its place, a new concept of politics based on need. It further suggests that exploring the development of Beauvoir’s ideas about torture helps (...) elucidate her belated turn to feminism and that in Beauvoir’s disarray and disillusionment regarding the use of torture by the French during the French-Algerian War and her compatriots’ complaisance with regard to this, there are lessons of sorts, though not necessarily entirely comforting ones, for Americans and others facing similar situations. There is a widespread conviction that people distinguish two kinds of acts: on the one hand, acts that are generalisably wrong because they go against universal principles of harm, justice, or rights; on the other hand, acts that are variably right or wrong depending on the social context. In this paper we criticise existing methods that measure generalisability. We report new findings indicating that a modification of generalisability measures is in order. One of the new ways used by companies to demonstrate their social responsibility is to encourage employee volunteering, whereby employees engage in socially beneficial activities on company time, while being paid by the company. The reasoning is that it is good for employee motivation (internal effects) and good for the company reputation (external effects). This article reports an empirical investigation of the internal effects of employee volunteering conducted amongst employees of the Dutch ABN-AMRO bank. The study showed that (a) socio-demographic (...) characteristics from employee volunteers markedly differ from those of non-volunteers and community volunteers and (b) employee volunteering seems to have positive effects on attitudes and behavior towards the organization. Background: Discussing treatment risks has become increasingly important in medical communication. Still, despite regulations, physicians must decide how much and what kind of information to present. Objective: To investigate patients’ preference for information about a small risk of a complication of colonoscopy, and whether medical and personal factors contribute to such preference. To propose a disclosure policy related to our results. Design: Vignettes study. Setting: Department of Gastroenterology, Academic Medical Centre, the Netherlands. Patients: 810 consecutive colonoscopy patients. Intervention: A home-sent (...) questionnaire containing three vignettes. Vignettes varied in the indication for colonoscopy, complication severity and level of risk. Patients were invited to indicate their wish to be informed and the importance of such information. In addition, sociodemograhic, illness-related and psychological characteristics were assessed. Main outcome measurements: Wish to be informed and importance of information. Results: Of 810 questionnaires, 68% were returned. Patients generally wished to be informed about low-risk complications, regardless of the indication for colonoscopy or the severity of the complication. The level of risk did matter, though (OR = 2.48, SE = 0.28, p = 0.001). The information was considered less important if done for population screening purposes or diagnosis of colon cancer, if the complication was less severe (bleeding) and if the risk was smaller (0.01% and 0.1%). Patients’ information preference was also related to age, mood and coping style. Limitations: Difficulty of vignettes. Conclusions: Patients generally wish to be informed about all possible risks. However, this might become uninformative. A stepwise approach is suggested. (shrink)

En este trabajo se presentan criterios que permiten estimar parámetros de desinfección mediante el uso de luz ultravioleta UV-C de onda corta, en agua, aire y superficies. Se indican métodos para evaluar la dosificación en función de la potencia de la lámpara empleada. Este tipo de estrategia permiten resultados de hasta 99.9% de desinfección, inactivando diferentes tipos de microorganismos. Estas referencias sirven de base para el diseño de dispositivos de utilidad en la presente emergencia por COVID-19, cuyo origen, al ser (...) viral, es susceptible al mismo método de desinfección debido al proceso de dimerización del ADN, donde el daño producido en la estructura celular, afecta la capacidad de reproducción y de funcionalidad. Alix Cohen argues that the function of feeling in Kantian psychology is to appraise and orient activity. Thus she sees feeling and agency as importantly connected by Kant’s lights. I endorse this broader claim, but argue that feeling, on her account, cannot do the work of orientation that she assigns to it.

Understanding Sankara brings together the essays of the late Richard De Smet, SJ (1916-1997) on the great Indian Advaitin. With the help of his discovery of a doctrine of laksana (analogy) in Sankara, De Smet challenges the traditional interpretation of the acarya as an illusionistic mayavadin. He also attempts a dialogue between Sankara's Advaita and Christianity, especially as represented by Thomas Aquinas. The android Data from Star Trek admired human emotion whereas Spock viewed emotion as irrational and maladaptive. The theory that emotions fulfil adaptive functions is widely accepted in academic psychology but little is known about laypeople’s theories. The present study assessed the extent to which laypeople share Data’s view of emotion as helpful or Spock’s view of emotion as a hindrance. We also assessed how help and hinder theory endorsement were related to reasoning, emotion regulation, and well-being. Undergraduates completed a (...) stressful timed reasoning task and questionnaires that assessed their theories of emotion, emotion regulation strategies, happiness, and social support. Overall, participants viewed emotion more as a help than a hindrance. The more they endorsed the view that emotion helps, the better their reasoning scores. Endorsing a help theory also predicted the use of reappraisal which, in turn, predicted greater happiness and social support. In contrast, endorsing the view that emotion hinders was associated with emotion suppression and less social support. Thus, people’s theories about the functionality of emotion may have important implications for their reasoning and emotional well-being. (shrink)

Discussions surrounding patient engagement and empowerment often use the terms “patient” and “consumer” interchangeably. But do the two terms hold the same meaning, or is a “patient” a passive actor in the health care arena and a “consumer” an informed, rational decision-maker? Has there been a shift in our usage of the two terms that aligns with the increasing commercialization of health care in the U.S. or has the patient/consumer dynamic always been a part of the buying and selling of (...) health care in the American system? Recent discussions of the issue exist in the popular press and in social media forums such as TEDMED, but few direct analyses of the ethical, legal, and policy ramifications of this possible shift in terminology are available in the academic literature. This paper analyzes our usage of the terms and any recent changes in the dynamic and discusses the ethical, legal, and policy implications of this simple terminology for the physicianpatient relationship. Varios estudios han reportado una asociación entre competencias emocionales y logros académicos en educación superior, y sobre la importancia de implementar estrategias para que los jóvenes afronten las demandas educativas. En esta línea, el objetivo del presente estudio consistió en explorar las características de la competencia de regulación emocional que inciden en el rendimiento académico de un grupo de jóvenes universitarios, con la finalidad de diseñar una propuesta psicoeducativa orientada a fortalecer la autogestión emocional de esta población. Para ello, se (...) realizó una fase diagnóstica en la que participaron ochenta estudiantes universitarios, 33,8 % de sexo masculino y 66,2 % de sexo femenino, con edades comprendidas entre 18 y 25 años(M = 21,09; DE = 1,92), a quienes se les aplicó el instrumento Difficulties in Emotion RegulationScale (DERS) y la Escala de Valoración de Aprendizajes de la Pontificia Universidad Católica delEcuador (PUCE). Los resultados mostraron la presencia de dificultades en conductas dirigidas a metas, así como una asociación significativa entre rendimiento académico y falta de aceptación y de claridad emocional. Con estos hallazgos, se empleó la metodología del Marco Lógico para diseñar una propuesta de siete talleres para fomentar el desarrollo de competencias relacionadas con reconocimiento y gestión de emociones, manejo de estrés y ansiedad, toma decisiones y manejo de conflictos, motivación intrínseca y autoconocimiento, gestión y administración del tiempo. This investigation examined how people’s beliefs about the functionality of emotion shape their emotional response and regulatory strategies when encountering distressing events. In Study 1, we present data supporting the reliability and validity of an 8-item instrument, the Help and Hinder Theories about Emotion Measure (HHTEM), designed to assess an individual’s beliefs about the functionality of emotion. Participants who more strongly endorsed a Help Theory reported greater wellbeing, emotional acceptance, and use of reappraisal to regulate emotion. Participants who more strongly (...) endorsed a Hinder Theory reported less wellbeing and more expressive suppression and substance use. In Study 2, we demonstrate that encouraging participants to view emotion as helpful affected their physiological and regulatory response to a distressing event. Participants in the Help Theory condition showed greater physiological reactivity (SCL) during a distressing film than control participants but were more accepting of their emotional response. Shortly after the film, SCL decreased for participants in the Help Theory condition. Compared to control participants, they engaged in less suppression and reported less lingering effect of the film on their mood. Together, these studies suggest that people’s theories about the functionality of emotion influence their reactivity, the strategies they adopt to regulate emotion, and their ability to rebound after distressing events.
