The paper by Mand et al raises important questions about the predictive genetic testing of children. They focus on those claims made by professionals that are open to empirical enquiry and give too little weight to those claims that do not require empirical support. The authors remind us that some commentators oppose empirical enquiry because of the concern that gathering evidence of the consequences of such testing may itself be harmful or unethical. They respond by asserting that the relevant research (...) questions are ‘eminently testable’ but fail to discuss the challenges of such research, including the questions of timescale, consent or the nature of the data whose collection (they assert) would resolve the perceived difficulties with the genetic testing of children. They conclude that empirical research is required without resolving the weighty arguments against predictive testing of young children or explaining how the evidence they wish to collect could overcome the force of the arguments that favour caution. Non-invasive prenatal testing is an emerging form of prenatal genetic testing that provides information about the genetic constitution of a foetus without the risk of pregnancy loss as a direct result of the test procedure. As with other prenatal tests, information from NIPT can help to make a decision about termination of pregnancy, plan contingencies for birth or prepare parents to raise a child with a genetic condition. NIPT can also be used by women and couples to test purely ‘for (...) information’. Here, no particular action is envisaged following the test; it is motivated entirely by an interest in the result. The fact that NIPT can be performed without posing a risk to the pregnancy could give rise to an increase in such requests. In this paper, we examine the ethical aspects of using NIPT ‘purely for information’, including the competing interests of the prospective parents and the future child, and the acceptability of testing for ‘frivolous’ reasons. Drawing on several clinical scenarios, we claim that arguments about testing children for genetic conditions are relevant to this debate. In addition, we raise ethical concerns over the potential for objectification of the child. We conclude that, in most cases, using NIPT to test for adult-onset conditions, carrier status or non-serious traits presenting in childhood would be unacceptable. (shrink)

A biblical approach to reconciling God’s sovereignty with creaturely responsibility should avoid the extremes of global occasionalism and completely autonomous creatures. This paper evaluates the standard intermediary solutions offered by conservationists and concurrentists. It argues that while each contributes insights which a satisfactory account should retain, none is fully adequate. Even Leibniz’s sophisticated response, which accounts for providence, miracles, and moral responsibility, unacceptably abridges creaturely power to implement decisions. My alternative proposal seeks to explain how creatures can retain full responsibility (...) for their actions even though God reserves the right to override creaturely power with His greater power. Christian philosophers have engaged naturalism in three main ways: direct refutation; systematic comparison; and sustained development of compelling alternative accounts. While all of these options have value, I argue that it is, and especially, that are most likely to win converts, and that we are witnessing an encouraging strategic shift in that direction. Options and bring Christian philosophers into closer dialogue with their naturalistic counterparts, building mutual respect and a greater opportunity for Christian philosophers to gain a full and fair (...) hearing. According to Christian physicalism (CP), a human person does not have an immaterial soul, but is identical to or constituted by a physical object. This chapter focuses on several reasons to think CP does not adequately account for the stewardship obligations. If CP is properly confined to the resources actually available to a physicalist anthropology it seems unable to account for the capacities of stewards, including a first‐person perspective, knowledge of the natural world, reasoning, and the ability to act. To (...) the extent that CP does seem able to account for these capacities, it relies heavily on a nonexplanatory notion of emergence and apparently nonphysical notions of teleology and life. But on physicalism, concepts emerge as the result of causal interactions with the brain, and this means that those concepts are limited to what can be explained by the individual learning history of a human organism or natural selection operating on the human species. John Searle claims that reasoning requires libertarian free will. He hopes this can be reconciled with a naturalistic neuroscience through a sophisticated theory of emergence, which includes indeterminism, and topdown causation. This is allegedly naturalistic because each mental state is causally reducible to a realizing neuronal state. I argue that Searle’s theory fails to overcome four main problems and cannot account for reasoning without implicit appeal to nonnaturalistic entities.

Modern military organizations are paternalistic organizations. They typically recognize a duty of care toward military personnel and are willing to ignore or violate the consent of military personnel in order to uphold that duty of care. In this paper, we consider the case for paternalism in the military and distinguish it from the case for paternalism in medicine. We argue that one can consistently reject paternalism in medicine but uphold paternalism in the military. We consider two well-known arguments for the (...) conclusion that military organizations should not be entitled to use experimental drugs on troops without first obtaining the informed consent of those troops. We argue that both of these are unsuccessful, in the absence of an argument for the rejection of paternalism in the military altogether. The case for military paternalism is widely accepted. However, we consider three ways in which it could be challenged. (shrink)

This paper argues that qualitative research is both useful and necessary, as it provides an essential means of gaining a richer understanding of patients' perceptions, social processes and meanings. In their paper in this edition of Clinical Ethics, Hallowell and Lawton raise many issues relating to the way that qualitative research is treated by RECs in the UK. In this paper I discuss just three key topics stimulated by their paper: the way that methodology relates to ethics, the experience and (...) character of the researcher, and the normative/descriptive distinction. I strongly support Hallowell and Lawton's appeal for greater communication between qualitative researchers and RECs. In other respects, I would argue that they do not go far enough. The recent review of research ethics structures and the review process provides an opportunity to move away from the present 'one size fits all' approach to ethical review. I would argue that this change could only benefit qualitative research and researchers. (shrink)

Substance dualism is compatible not only with Cartesian dualism but also with a number of nonCartesian alternatives, including several varieties of Thomistic dualism, William Hasker's emergent subject dualism, and the holistic anthropology of E. J. Lowe. Due to recent developments within the philosophy of mind, a renewed interest in historical and contemporary theories of the soul, and a more careful evaluation of what does and does not follow from neuroscience, substance dualism is back on the table for a serious critical (...) reevaluation. This chapter reviews the developments in the philosophy of mind and in scholarship on the soul. It shows that substance dualists and their many critics have been brought together by a shared focus on the nature of mental subjects. The chapter also presents an overview of the key concepts discussed in the subsequent chapters of this book. (shrink)

The present study examined the neural substrate of two classes of quantifiers: numerical quantifiers like ” at least three” which require magnitude processing, and logical quantifiers like ” some” which can be understood using a simple form of perceptual logic. We assessed these distinct classes of quantifiers with converging observations from two sources: functional imaging data from healthy adults, and behavioral and structural data from patients with corticobasal degeneration who have acalculia. Our findings are consistent with the claim that numerical (...) quantifier comprehension depends on a lateral parietal-dorsolateral prefrontal network, but logical quantifier comprehension depends instead on a rostral medial prefrontal-posterior cingulate network. These observations emphasize the important contribution of abstract number knowledge to the meaning of numerical quantifiers in semantic memory and the potential role of a logic-based evaluation in the service of non-numerical quantifiers. The acquisition of expertise in formal problem solving has been assumed to involve either a shift from backwards to forwards inference, or a shift from unguided to guided forwards inference. In a longitudinal study, the acquisition of formal problem-solving expertise was investigated. Participants were tested as novices before undertaking controlled practice in the problem domain which involved transformation rule problems , and were finally tested as experts. The direction of inference in problem solutions was found to be inadequate to describe (...) the strategic differences between novices and experts. Therefore, a new solution coding system was applied, based on atomic components of problem solution. Analysis of novice and expert solutions revealed no systematic strategy in the novice stage solutions were confused and contained unproductive steps and backtracking. Several strategies were found in the expert solutions, but they did not agree with previously reported results. It was therefore proposed that the acquisition of expertise does not involve a change from one specific solution strategy to another, but rather the development of an efficient strategy, which can differ between participants. The marketing of health care services raises the prospect that an ethic of strangers will govern relations between providers and patients. A fiduciary model that emphasizes honesty and public accountability, as well as the patient's good and avoiding unnecessary services, can keep marketing consistent with the ethical tradition of medicine.
