When making substituted judgments for incapacitated patients, surrogates often struggle to guess what the patient would want if they had capacity. Surrogates may also agonize over having the (sole) responsibility of making such a determination. To address such concerns, a Patient Preference Predictor (PPP) has been proposed that would use an algorithm to infer the treatment preferences of individual patients from population-level data about the known preferences of people with similar demographic characteristics. However, critics have suggested that even if such (...) a PPP were more accurate, on average, than human surrogates in identifying patient preferences, the proposed algorithm would nevertheless fail to respect the patient’s (former) autonomy since it draws on the ‘wrong’ kind of data: namely, data that are not specific to the individual patient and which therefore may not reflect their actual values, or their reasons for having the preferences they do. Taking such criticisms on board, we here propose a new approach: the Personalized Patient Preference Predictor (P4). The P4 is based on recent advances in machine learning, which allow technologies including large language models to be more cheaply and efficiently ‘fine-tuned’ on person-specific data. The P4, unlike the PPP, would be able to infer an individual patient’s preferences from material (e.g., prior treatment decisions) that is in fact specific to them. Thus, we argue, in addition to being potentially more accurate at the individual level than the previously proposed PPP, the predictions of a P4 would also more directly reflect each patient’s own reasons and values. In this article, we review recent discoveries in artificial intelligence research that suggest a P4 is technically feasible, and argue that, if it is developed and appropriately deployed, it should assuage some of the main autonomy-based concerns of critics of the original PPP. We then consider various objections to our proposal and offer some tentative replies. (shrink)

A structured character is a semantic value of a certain sort. Like the more familiar Kaplanian characters, structured characters determine the contents of expressions in contexts. But unlike Kaplanian characters, structured characters also have constituent structures. The semantic theories with which most of us are acquainted do not mention structured characters. But I argue in this paper that these familiar semantic theories fail to make obvious distinctions in meaning---distinctions that can be made by a theory that uses structured characters. Thus (...) David Chalmers uses Bayesian theories of credence to argue against referentialism about belief. This paper argues that Chalmers’s Bayesian objections to referentialism are similar to older, more familiar objections to referentialism. There are familiar responses to the old objections, and there is a predictable way to modify those old responses to meet Chalmers’s Bayesian objections. The new responses to the new objections are no less plausible than the old responses to the old objections. Public trust of conventionally produced food is now a pivotal issue for the Canadian food supply chain as consumers are increasingly demanding traceability, transparency and sustainability of the agri-food system. To ensure that Canadians understand what farmers do, how they do it, and why—there has been significant human and financial investment by both the agri-food industry and government over the last decade. Farmers, civil servants, and non-farming agricultural professionals alike are being encouraged to join the national conversation promoting the legitimacy (...) of conventional agriculture. As part of this large-scale effort, women in agriculture are advocating, in gendered ways, for the safety and legitimacy of the agri-food system and its conventional farming practices. This is being done by utilizing their motherhood capital. This motherhood capital legitimizes the authority granted to mothers as expert decisionmakers regarding their children’s food consumption. Through the usage of their motherhood capital, women are being positioned—and are positioning themselves—as an important voice in re-narrating the story of conventional agriculture through the circulation of their maternal foodwork experiences. Using their authority as mothers they are advocating for the safety, necessity, and trustworthiness of conventional agriculture to counter narratives of mistrust and risk. (shrink)

Through an analysis of the political economy of participatory praxis within current neoliberal contexts, we focus on (...) three main areas: (i) reconceptualizing the pragmatics of participation; (ii) the microeconomic implications of participatory research, including ethical issues in payment for research participation; and (iii) the value and limits of using research as a tool for activism and empowerment. We conclude with a brief discussion of what we see to be some of the most salient social justice implications arising from feminist and participatory approaches to health research within neoliberal political spaces. Ethik-Komitees gehören zum festen Bestandteil des Ethikmanagements und der Organisationsethik in klinischen Einrichtungen des Gesundheitswesens. Entsprechende Ethikstrukturen und die damit verbundenen Angebote stoßen hinsichtlich ihrer Wirksamkeit allerdings an ihre Grenzen. Ihre Arbeitsweisen sind häufig reaktiv und eine Verankerung in den entsprechenden Organisationsebenen fehlt. Ausgehend von diesen Limitationen der klinischen Ethikberatung hat sich die multiprofessionelle „Arbeitsgruppe Ethik“ am Universitätsklinikum Tübingen um die Konzeption und Implementierung eines neuen Ansatzes zur nachhaltigen Integration von ethischen Reflexions- und Entscheidungsprozessen auf den Stationen des UKT bemüht. Mit dem Tübinger Modell der Ethikbeauftragten der Station verfolgt sie ein Pilotprojekt, das speziell geschulte Pflegekräfte aus allen Stationen des UKT als AnsprechpartnerInnen für ethische Fragen einsetzt. Damit stellen die Ethikbeauftragten eine Erweiterung zu etablierten Strukturen der Ethikberatung dar und ergänzen vorhandene Top-Down-Strategien. Der vorliegende Beitrag stellt die Zielsetzungen des Tübinger Modells dar und schildert erste Erfahrungen in der Umsetzung. Neben der Einbettung in organisationale Strukturen der Ethikberatung werden die stationsinternen und stationsübergreifenden Aufgaben der Ethikbeauftragten dargestellt. Zudem wird das Qualifikationsprogramm für Ethikbeauftragte sowie ein Train-the-Trainer-Konzept vorgestellt, welche eine vertiefende Entwicklung von pflege- und medizinethischer Kompetenzen unterstützen und Sicherheit in den stationsbezogenen Reflexions- und Entscheidungsprozessen vermitteln. Nearly one-quarter of Costa Rica's export earnings derive from an expanding tourist sector, one that is increasingly diversified in a mix of tourist niches. Ecotourism is the fastest growing niche and its promises are featured in a range of sites and practices, including the largest multinational hospitality and hotel corporations. These companies promote a vision of sustainability that relies on expanding consumption of ‘environmental' amenities through profit-driven global corporations – a vision that is, to some, antithetical to the very meaning (...) of ecotourism. Our study explores the historical evolution of tourist development in Costa Rica, specifically large-scale coastal development, as a means for national development. In this article, I present discussions of conditions for reviving public morale and, in the process, public morality, which would ultimately be a political goal, using examples from the Victorian era in Britain and what Americans refer to as the Progressive Era at the beginning of the twentieth century in the United States. I begin with an older book by Gertrude Himmelfarb that emphasises the revitalisation of public morality in Victorian Britain. A book by Robert D. Putnam and Shaylyn Romney (...) Garrett makes similar claims for the effects of the Progressive Era in the US, and for how a similar approach could be useful in the present era. Both books emphasise cultural critique and discount the effects of causality going in the opposite direction, starting with economic revival, and I discuss this dilemma in this article. (shrink)

This book explores a relatively little investigated area of creole languages, word-formation. It provides the most comprehensive account so far of the word-formation patterns of an English-based creole language, Sranan, as found in its earliest sources, and compares them with the patterns attested in the input languages. One of the few studies of creole morphology based on historical data, the book discusses the theoretical problems arising with the historical analysis of creole word-formation and provides an analysis along the lines of (...) Booij’s Construction Morphology in which the assumed boundaries between affixation, compounding and syntactic constructions play a very minor role. It shows that Early Sranan word-formation is characterised by the absence of superstrate derivational affixes, the use of free morphemes as derivational markers and of compounding as the major word-formation strategy. The emergence of Early Sranan word-formation involved multiple sources and different mechanisms. The findings render untenable theoretical accounts of creole genesis based on one explanatory factor, such as superstrate or substrate influence. (shrink)

Navigating limits to confidentiality with adolescent clients can be ethically and professionally challenging. This study follows on from a previous quantitative survey of psychologists about confidentiality dilemmas with adolescents. The current study used qualitative methods to explore such dilemmas in greater depth. Twenty Australian psychologists were interviewed and asked to describe an ethically challenging past case. Cases were then used to facilitate discussion about the decision-making process and outcomes. Interviews were transcribed and analyzed using interpretive content and thematic analysis. Three (...) key findings are discussed. First, it is of little use to perceive confidentiality dilemmas as binary choices because psychologists described 5 distinct options. These can be conceptualized on a spectrum of varying degrees of client autonomy, ranging from “no disclosure” to “disclosure without the client’s knowledge or consent”. Second, confidentiality dilemmas often involve balancing multiple and conflicting risks regarding both immediate and future harm. Third, a range of strategies are employed by psychologists to minimize potential harms when disclosing information. These are primarily aimed at maintaining the therapeutic relationship and empowering clients. These findings and the case studies described provide a valuable resource for teaching and professional development. (shrink)

Research examining the possible effects of deceptive research participation on participants' perceptions of psychology has yielded equivocal results. The present study's goal was to clarify the possible effects of participation in mildly deceptive research on participants' impressions of scientific and applied psychology. Participants (N = 112) were randomly assigned to one of six experimental conditions: active groups receiving negative, positive, or no feedback, or passive groups receiving negative, positive, or no feedback. Following participation, participants completed measures of impressions of psychotherapy (...) and psychotherapists, researchers, and instructors. The manipulation did not affect attitudes toward psychology on any of the dependent measures, although gender effects resulted on one measure. Participants in general reported very positive attitudes toward the science and practice of psychology. Recommendations are offered for future research on the effects of more extensive deceptions.
