This article argues that standard models of person-centred care (PCC) and shared decision making (SDM) rely on simplistic, often unrealistic assumptions of patient capacities that entail that PCC/SDM might have detrimental effects in many applications. We suggest a complementary PCC/SDM approach to ensure that patients are able to execute rational decisions taken jointly with care professionals when performing self-care. Illustrated by concrete examples from a study of adolescent diabetes care, we suggest a combination of moral and psychological considerations to support (...) the claim that standard PCC/SDM threatens to systematically undermine its own goals. This threat is due to a tension between the ethical requirements of SDM in ideal circumstances and more long-term needs actualized by the context of self-care handled by patients with limited capacities for taking responsibility and adhere to their own rational decisions. To improve this situation, we suggest a counseling, self-care, adherence approach to PCC/SDM, where more attention is given to how treatment goals are internalized by patients, how patients perceive choice situations, and what emotional feedback patients are given. This focus may involve less of a concentration on autonomous and rational clinical decision making otherwise stressed in standard PCC/SDM advocacy. (shrink)

This essay takes a Traditional Christian, that is, Orthodox look at the integration of psychotherapy into pastoral counseling, as endorsed by many Western mainline Christianities. It examines how the Christian pastor can guide his sheep through the bioethical problems they encounter in their pursuit of salvation. The first part explores whether the turn to psychology and psychotherapy can be welcomed as a return to the Traditional therapeutic understanding of theology and of the Church as a spiritual hospital for (...) fallen souls. The second part examines two typical ways in which non-Orthodox Christianities embrace psychotherapy in pastoral counseling, as directed to unburdening parishioners from their doubts about bioethical decision making in the context of a Christian life. This part highlights the theological assumptions (about the pastor, his care, and the notion of God informing both) framing that embrace and shows them to be incompatible with the Traditional commitment to man's Divine vocation. The turn to psychotherapy here, rather than inaugurating a return to the therapeutic spirit of Traditional theology, offers a secularized substitute. The third part investigates the possibility of a Traditional Christian way of practicing psychotherapy along with its significance for Orthodox pastoral care in bioethical counseling. In this article the authors discuss psychological counselling as it emerges as a gendered profession in the transitional economy of Russia. Based on qualitative, semi-structured interviews with 23 female and three male practising counsellors, the article analyses their perceptions of their profession, focusing in particular on two key issues: their reasons for entry into the profession; and their expectations of their work as a profession. The authors argue that both female and male counsellors’ perceptions of their entry into this profession (...) revealed highly gendered stances, and furthermore, that these were strongly imbricated in the legacies of Soviet employment policies and gender ideology. These subjects concern the significance of psychotherapy and psychology for (1) reaching out to parishioners who are estranged from the church, (2) criticizing religion as a whole, and (3) remedying certain distortions for which pastoral care and counseling have been criticized in some Christianities. The purpose of the current study was to investigate the effects of COVID-19 on high school students' psychological symptoms and to understand how ready counselors and school counseling services are based on the data we have. Therefore, this research is designed under two different studies: Study 1: Effects of COVID-19 pandemic on students' psychological symptoms and Study 2: Views and expectations of students and school counselors about school counseling services. The first study was a quantitative study and included (...) 549 high school students. A structural equation model was created to examine the effects of COVID-19 pandemic on participants' psychological symptoms. The Impact of Event Scale-Revised scores showed that 107 individuals had a score of 50 and above pointing out that individuals in this group had severe impact of event/trauma symptomologies. The SEM analysis indicated that IES-R scores had a total effect of 0.79 on anxiety, 0.75 on depression, 0.74 on negative self-concept, 0.68 on somatization, and 0.66 on hostility scores. Furthermore, female students had significantly higher scores on anxiety, depression, negative self-concept, somatization, hostility, and impact of events variables than male students. Study 2 was a qualitative design and consisted of five school counselors and five students from different schools. The results indicated that students' difficulties during the coronavirus disease 2019 outbreak were educational, cognitive, emotional, physiological, relational, technological, and related to routines. Academic, social, emotional, and behavioral issues came to the fore among the difficulties that can be experienced if students start face-to-face education. On the other hand, the school counselors listed that family relations, personal–social, emotional, and academic themes were the difficulties experienced by the students at the beginning of the COVID-19. In addition, when COVID-19 started, the services offered by school counselors were discussed under services for the student, services for the family, and services for the teacher. Finally, according to the opinions of the school counselors, if students start face-to-face education, they may experience emotional, academic, and relational difficulties. In summary, it is vital that student personality services be prepared and implemented by school counseling services for schools based on the results. (shrink)

The aim of this study is to investigate the necessity of revising the Ethics Code of the Psychology and Counseling Organization of the Islamic Republic of Iran with respect to people’s rights and dignity and to avoid unfair discriminations toward sexual orientation and gender identity. It is said that confused diagnoses; wrong decision making; unethical practice; and the subsequent harm caused to lesbian, gay, bisexual, and transgender clients result from the lack of a clear code and relevant guidelines. In addition, certain conditions in Iran lead professional psychologists and researchers to avoid researching sexual minorities and gender identity, which negatively impacts the Ethics Code of the PCOIRI. Iranian and Western researchers have identified four primary sources to which psychologists and counselors can refer when faced with ethical dilemmas: developmental psychology, professional ethics codes, philosophy of psychology literature, and available researches and compilations on professional ethics. Finally it is suggested that, when dealing with homosexual clients and the related ethical dilemmas, Iranian psychologists and counselors need clear and specific guidelines and a professional ethics code developed from more appropriate reference points than legal and religious sources. ABSTRACT A new phenomenon in the philosophical discipline, philosophical practice—or counselling—is introduced to the reader through the description of three different approaches to philosophical practice. What is common to these approaches is that an academically trained philosopher philosophises together with a ‘sane’person—the visitor—on questions and problems relevant to the visitor. The essential difference between this and psychological counselling is that philosophers do not use therapeutic methods or make diagnoses. The philosophical practitioner triggers in his dialogue partner a didactic philosophical process, (...) which enables the visitor to become an authentic thinker and eventually to arrive at philosophical answers by her/himself. In this paper I will move away from what has become the "traditional" approach to writing and thinking about philosophical counseling - I will not compare and contrast the virtues of the philosophical and psychological paradigms, nor will I attempt to defend philosophical counseling against its critics. Instead, I will use the methods and practices employed by philosophical counselors as a paradigm to inform and govern another philosophical practice, that is, clinical medical ethics. ObjectiveTertiary education can be stressful for many young people, who consistently report high levels of distress. The issue has major implications for campus health services and mental health policymaking more widely. The present study proposes to map student counseling services in Europe.MethodsThe sample of institutions was sourced, using standardized data extraction, from the European Tertiary Education Register. Then, each institution’s website was analyzed for information about the availability of student counseling centers and the services provided. Data extracted from (...) the ETER database were: ETER ID, national identifier, institution name, English institution name, number of students, legal status, institution category, and institutional website. Data extracted from institutions’ websites concerned the availability of students’ psychological centers and the services provided. Analyses were carried out using the SPSS Statistics software package, version 26.ResultsOverall, it was found that most institutions do not provide mental health counseling services for their students. Institutions of medium dimensions showed a higher probability of reporting students’ psychological centers than small institutions. Moreover, private institutions and public institutions were more likely to report having such centers, while private government-dependent institutions were less likely. Universities of applied sciences and universities were more likely to report having them, while other institutions were less likely. Regarding provision according to geographic area, compared to Northern Europe, every other European region was less likely to report featuring such centers. Most institutions reported offering counseling, career counseling, or not otherwise specified psychological services, but only a small number reported providing services such as psychotherapy, psychiatric services, or counseling for learning-specific disabilities.ConclusionIt is critically important to catalog European data on student counseling centers and services, to encourage tertiary education institutions to invest in such services as key sites for mental health promotion. In lieu of an abstract, here is a brief excerpt of the content:48 SOME CQUHSEL ON HUMEAN RELATIONS In a paper published eight years ago I tried to bring out a neglected feature of Hume's theory of relations, namely the difference between philosophical and natural re1 2. lations. Now Ijnlay, without referring to my work, has expanded some of its themes in an extremely interesting and, I think, important way. At least he has made me rethink the whole distinction between (...) philosophical and natural relations and, what goes with relation theory, Hume's analytic-synthetic distinction. The strengths of his paper are, embarrassingly, the weaknesses of my own. I did not articulate clearly the connection between relation theory in Hume and the analytic-synthetic distinction. I shall turn to these strengths presently. My focus in this paper, however, will be on a central weakness in Imlay 's; and the point, it turns out, is one I gave full credence to earlier. Imlay does not distinguish what in Hume can and must be distinguished if we are to make any sense (let alone full sense) of his theory of causation. The distinction is be3 tweer, necessity and certainty. In trying to explicate Hume's distinction between intuitive and demonstrative inference on the one hand, and probability on the other, Imlay finally reaches an impasse. For, if the absence of the corresponding impression does not deprive mathematical necessity of... existence, why should it do so in the case of causal necessity? This, as Imlay recognizes, is at least related to standard criticisms of Hume, most forcibly articulated by Prichard. Imlay fortunately does not recommend—as Prichard does — throwing out the baby with the bath. If I understand him correctly, what he advocates is that we simply admit that Hume's attempts to ground necessity are inadequate. Before proceeding to details, let me issue two brief caveats. 49 First. It is of course not surprising that Hume fails to make a clear analytic-synthetic distinction. Has anyone? Lacking the logical tools of a Russell, or the mathematical understanding of a Leibniz, Hume labors under insuperable difficulties which, in many instances, are selfcreated. He is not clear as to whether necessity attaches to 'objects' or ideas (let alone sentences or propositions); he confuses, by invoking pseudo-psychological criteria involving conceivability, analytic with synthetic a priori truths; he has what many, including myself, consider to be a ludicrous theory about mathematics and mathematical objects. With these horrors to work with it is small wonder that necessity never gets grounded. Indeed, it is miraculous that the discussion even gets off the ground. But, Prichard's sneeringly just comments notwithstanding, both he and Imlay miss something, and I hope to show precisely what that is. Second. Hume is notoriously obscure in his discussion of what we would call ordinary physical objects. Sometimes (when he is in a psychologist or even ordinaryperson mood) he speaks as if physical objects cause impressions. Sometimes (when in the philosopher's closet) he speaks as if it makes no sense to speak of anything but impressions and collections of them, ordinary physical objects being relegated to the fictions of the vulgar. His theory of relations rather obviously suffers from the same disease; it is not clear what the relata are. Are impressions related temporally? Spatially? Is it relations between objects 'out there' of which he speaks when he discusses causation? Imlay is quite aware of the confusion ; he just lets it go. For now, I shall more or less follow suit. Later, as we shall see, one's choice makes a difference. In the long run, I think any coherent view of Hume will have to treat his analysis of physical objects in a way similar to Berkeley's · I. Hume, Prichard, and Imlay At first glance it would seem easy to allay at least Prichard' s fears. What, after all, is the connection between Hume's theory of relations, his attempt at distinguishing the necessary from the probable or possible, and his theory of causation? Hume claims that causation is a relation that, unlike some others, eg., resemblance, cannot be 'seen' to hold between its relata. But all of us believe that causal connections are necessary. There is currently a movement advocating “philosophical counseling.” My own development as a philosopher, then a human services professional, then a psychoanalyst, charts how I came to believe that philosophical training was underrated, and training in psychology was overrated, as an appropriate intellectual foundation for psychotherapy. However, these fields are not distinct. Laws governing the practice of psychology are arrogant in their scope, and make virtually everything out to be the practice of psychology. The scope and (...) nature of philosophy isn’t any clearer. The kind of thinking encouraged in psychology is liable to be exactly the wrong sort of thing for training therapists. Unfortunately, philosophers are liable to not be good therapists either. The lack of neat distinctions between philosophical counseling and psychotherapy provides an argument against a monopoly on therapy-like activities by psychologists. On the liberal side this is an argument in favor of freedom of speech, of belief, and trade, for the applied philosopher. On the conservative side, it may also be an argument for certification (as opposed to licensure) for both psychologists and philosophers, in the interest of protecting the vulnerable by promoting truthful self-representation. Many philosophical counselors seem to be counselors who use or point to phil­osophical texts or use abstract indeed logical or rational methods when working with a client. I want to introduce the idea of a counseling philosopher, who uses the client’s own concrete experiences as the basis for philosophizing with the client about the nature of the client’s dilemma - using ‘the between’ (Buber) as that special creative space where one em­ploys the art of philosophizing to the unique situation. Otherwise, the particularity of that client gets subsumed under theory or methods, much like what has happened in psychology and which gave rise to Achenbach’s criticisms of psychology/psychiatry. The dialogical of which Buber and Friedman speak is the give and take between client and counseling philosopher of understanding and expanding perceptions confirmed through the actual relationship. Philosophy as an art (and not a method) helps us restore the trust Buber talks about which allows us to engage the world directly and not through categories of thought grounded in psychological or philosophical texts and theories. Can philosophy help ordinary people confront their personal or interpersonal problems of living? Can it help a couple whose marriage is on the rocks, or someone going through a midlife crisis, or someone depressed over the death of a significant other, or who suffers from anxiety about making a life change? These and many other behavioral and emotional problems are ordinarily referred to psychologists, psychiatrists, clinical social workers, or other mental health specialists. The discipline of Mental Health Counseling, referred to in this essay as Counseling, has no substantial philosophy. In the United States, much of Counseling philosophy is rooted in the American Counseling Association’s code of ethics. However, this is a code of material conduct, not a substantial Counseling philosophy, and by this orientation the distinction between doing and the knowledge that informs activity is not understood important. Counseling theories thereby adhere in a third principle that (...) is left to disseminate in foreign powers. This is to say, Counseling is commonly understood as a name for a loose set of theoretical practices bound by ethical standards, that these practices are reckoned to lay apart from one another while all referencing or otherwise answering to psychology. Due to this deferment, the apparent rise of mental health issues could be attributed to a weakening of intentional focus, since psychology, by its own scientific standard, is less a standard of care than an experimental method oriented in discovering and implementing an objective reality through which it then offers corrective protocols. This essay draws upon philosophical efforts more rigorous than a granting of epistemological deferment to propose that Counseling is a practice unto itself, in-itself, of a true substance, concerned, involved with, and related to psychology but not subject to it. The first part introduces some examples of the concepts that sharply distinguish philosophical counselling from psychological and psychotherapeutic approaches. It also considers those that mix these different approaches. The second part deals with the question of whether or not philosophical counselling can be (...) considered to be a therapy. Some philosophical counsellors work on the belief that there is a synchrony between modern philosophical counselling and the classical conception of philosophy as therapy. Many, however, are of the opinion that it is not possible to speak of it in terms of therapy. The third part gives examples of the way in which philosophical counselling is understood to be contemplation and on the other hand of those who employ approaches based on critical thinking in philosophical counselling. The last two and a half decades has seen the emergence of philosophical counseling. While it is practiced in many countries comparatively little has been said on its general character. In this paper I will seek to understand philosophical counseling by exploring its points of convergence to and deviation from its complimentary parts – philosophy and counseling. The practical and applied orientation of philosophical counseling seems worlds apart from what many consider to exemplify philosophy – theoretical, (...) intellectual and abstract concern with foundational questions. Given this it is worth exploring how philosophical counseling coincides with what is understood in academic circles as philosophy. On the other hand, the field of counseling has largely been the domain of psychology. Given the influence and orientation of the practice it would seem likely that philosophical counseling would be significantly different from the common conception of psychological counseling, but in what ways? Understanding how the practice lies in relation to the discipline of philosophy and psychological counseling will go some way to grasping its nature. (shrink)

In this study, religious counseling and guidance services in our country, applications, opportunities and methods were examined in terms of the psychology of religion. Including the theoretical framework and survey application, this study basically consists of two parts, the mental state of people under threat, religious services focuses on the issues of how to provide assistance. The article looks at critical thinking in philosophical counselling and the concepts upon which it is based. In conceptions that place critical thinking as the basis of philosophical counselling, an important role is played by the Socratic approach to philosophising. The Socratic method in thinking allocates a fundamental role to conversation, and thus to intersubjectivity, and is therefore an alternative to individual ways of thinking. Conversation as philosophical reflection corresponds to the Socratic intersubjective understanding of truth. The author adopts the (...) view of German philosopher H. Schnädelbach who distinguishes between dialogic and doctrinal approaches. The dialogic approach is found in the Socratic-Platonic tradition, while the doctrinal approach is found in Aristotelean approaches. Doctrinal philosophising is premised in the ideal of intersubjectivity which can be achieved by anyone. Philosophical thought as reflection is always implicitly dialogic at the very least. The article considers definitions of critical thinking and provides examples of critical thinking based philosophical counselling from the thinking of Tim LeBon and Elliot D. Cohen, which link both philosophical and psychological approaches. In conclusion it is critical of an excessive focus on rationality in counselling. (shrink)

Notwithstanding recent successes of philosophical counseling, which appear to be leading to its legitimization as a professional practice in America and abroad, many forces concen to condition its emergent structure and function. This paper briefly elucidates some of the influences to which philosophical counseling is subject, that lie beyond its unilateral control. These include its portayal by the media to the public, its scope of practice, its relations with psychology and psychiatry, its foreseeable effects in particular cases, (...) and its perception by (and of) analytical philosophy. Following participation in Dr. Yair Caspi’s “Psychology in Judaism” workshop, the writers contemplate whether the teaching of Caspi’s model in academic settings could become simultaneously a fresh addition to interdisciplinary approaches to the teaching of Judaism in Israeli Academic life, and an academic addition to the contemporary trend to Jewish renewal in Israeli society. The model is based on weekly facilitated workshops in which participants both reflect on and discuss their lives and also explore unique interpretations of Jewish texts (...) and ideas, constantly seeking the borderline where their lives inform the text and vice versa. The process leads to the re-enchantment of ancient Jewish concepts, the reinvention of the Jewish religious experience in a contemporary paradigm and idiom, and the possibility of deep transformative insight.
