© 2019 The Author. Background: The phenomenon of being 'with woman' is fundamental to midwifery as it underpins its philosophy, relationships and practices. There is an identified gap in knowledge around the 'with woman' phenomenon from the perspective of midwives providing care in a variety of contexts. As such, the aim of this study was to explore the experiences of being 'with woman' during labour and birth from the perspective of midwives' working in a model where care is provided by (...) a known midwife. Methods: A descriptive phenomenological design was employed with ten midwives working in a 'known midwife' model who described their experiences of being 'with woman' during labour and birth. The method was informed by Husserlian philosophy which seeks to explore the same phenomenon through rich descriptions by individuals revealing commonalities of the experience. Results: Five themes emerged 1) Building relationships; 2) Woman centred care; 3) Impact on the midwife; 4) Impact on the woman; and 5) Challenges in the Known Midwife model. Midwives emphasised the importance of trusting relationships while being 'with woman', confirming that this relationship extends beyond the woman - midwife relationship to include the woman's support people and family. Being 'with woman' during labour and birth in the context of the relationship facilitates woman-centred care. Being 'with woman' influences midwives, and, it is noted, the women that midwives are working with. Finally, challenges that impact being 'with woman' in the known midwife model are shared by midwives. Conclusions: Findings offer valuable insight into midwives' experiences of being 'with woman' in the context of models that provide care by a known midwife. In this model, the trusting relationship is the conduit for being 'with woman' which influences the midwife, the profession of midwifery, as well as women and their families. Descriptions of challenges to being 'with woman' provide opportunities for professional development and service review. Rich descriptions from the unique voice of midwives, provided insight into the applied practices of being 'with woman' in a known midwife model which adds important knowledge concerning a phenomenon so deeply embedded in the philosophy and practices of the profession of midwifery. ‘Substance’ (substantia, zelfstandigheid) is a key term of Spinoza’s philosophy. Like almost all of Spinoza’s philosophical vocabulary, Spinoza did not invent this term, which has a long history that can be traced back at least to Aristotle. Yet, Spinoza radicalized the traditional notion of substance and made a very powerful use of it by demonstrating – or at least attempting to demonstrate -- that there is only one, unique substance -- God (or Nature) -- and that all other things are (...) merely modes or states of God. Some of Spinoza’s readers understood these claims as committing him to the view that only God truly exists, and while this interpretation is not groundless, we will later see that this enticing and bold reading of Spinoza as an ‘acosmist’ comes at the expense of another audacious claim Spinoza advances, i.e., that God/Nature is absolutely and actually infinite. But before we reach this last conclusion, we have a long way to go. So, let me first provide an overview of our plan. In the first section of this paper we will examine Spinoza’s definitions of ‘substance’ and ‘God’ at the opening of his magnum opus, the Ethics. Following a preliminary clarification of these two terms and their relations to the other key terms defined at the beginning of the Ethics, we will briefly address the Aristotelian and Cartesian background of Spinoza’s discussion of substance. In the second section, we will study the properties of the fundamental binary relations pertaining to Spinoza’s substance: inherence, conception, and causation. The third section will be dedicated to a clarification of Spinoza’s claim that God, the unique substance, is absolutely infinite. This essential feature of Spinoza’s substance has been largely neglected in recent Anglo-American scholarship, a neglect which has brought about an unfortunate tendency to domesticate Spinoza’s metaphysics to more contemporary views. The fourth section will study the nature of Spinoza’s monism. It will discuss and criticize the interesting yet controversial views of the eminent Spinoza scholar, Martial Gueroult, about the plurality of substances in the beginning of the Ethics; address Spinoza’s claim in Letter 50 that, strictly speaking, it is improper to describe God as “one”; and, finally, evaluate Spinoza’s kind of monism against the distinction between existence and priority monism recently introduced into the contemporary philosophical literature. The fifth and final section will explain the nature, reality, and manner of existence of modes. We therefore have an ambitious plan; let’s get down to business. (shrink)

Empirische Daten belegen, dass die moderne Geburtsmedizin Schwangerschaft und Gebären vorrangig als krankhaft wahrnimmt, was zu einem Legitimationsdefizit führt: Eine primär invasive Geburtsmedizin verletzt das Prinzip der Nichtschädigung, verursacht vermeidbare Kosten und ist nicht ohne weiteres durch das Autonomieprinzip gedeckt. Von den unmittelbar Beteiligten ist diese Pathologisierung als eine solche jedoch kaum wahrgenommen worden. Daher stellt sich die Frage, wie es zur sozialen Akzeptanz einer derart drastischen Wahrnehmungsverschiebung kommen konnte. Da Begriffe unsere Wahrnehmung strukturieren, interessiert uns vor allem die konzeptionelle (...) Dimension dieser geburtsmedizinischen Verkehrung des Normalen ins Krankhafte, die in einer Reflexion auf die zwei Quellen legitimen ärztlichen Handelns erkennbar wird. Sowohl der Krankheitsbegriff als auch das moderne Verständnis von Selbstbestimmung weisen eine interne Unschärfe auf, die pathologisierende Tendenzen begünstigt. Der moderne Krankheitsbegriff erschwert eine klare Abgrenzung zwischen krank und gesund. Und Autonomie ist ein mehrdeutiges Konzept, das in einer spezifischen Auslegung die Pathologisierung fördert. Da beide Konzepte ärztliches Handeln rechtfertigen, bleibt ihr delegitimierendes Potential in der Praxis unscheinbar. Erst reflexiv und polylogisch lässt sich dieser blinde Fleck beobachten, so dass eine Korrektur der Pathologisierung möglich wird. (shrink)

ZusammenfassungDass unsere Gesellschaften den Tod verdrängen, ist eine weit verbreitete Überzeugung. Sie lässt sich jedoch soziologisch weder belegen noch widerlegen: A) Die Moderne ist sich des Todes wohl bewusst. Zum einen kennt sie aufgrund ihrer funktionalen Differenzierung viele Todesbilder. Zum anderen ist sie stark individualisiert, so dass der Tod eine neue Bedeutung gewinnt. Er wird zum irreversiblen Verlust einer unersetzbaren und in sich wertvollen Person. B) Die Moderne wird aber auch als todesverdrängend erlebt, insofern viele ihrer Todesbilder diesen individuellen Tod (...) ignorieren. Diese Ambivalenzen lassen sich in einer Betrachtung moderner Medizin genauer aufklären: A) Diese thematisiert den Tod durchaus, aber vorrangig erfahrungswissenschaftlich, so dass die Person und der ihr eigene Tod gerade ausgeklammert werden. Da die subjektive Dimension auch für Schmerzphänomene konstitutiv ist, grenzt eine solche Medizin diese ebenfalls aus. Sie gelten als tendenziell sinnlos und werden ignoriert oder aber beseitigt. B) Zugleich ist moderne Medizin stets schon mehr als reine Naturwissenschaft, da sie auch einer Logik individuellen Fallverstehens folgt. Beide Logiken sind spannungsvoll, können einander ergänzen, aber auch konfligieren. Während eine dominante Orientierung an den Naturwissenschaften weder den Schmerzen noch dem individuellen Tod gerecht wird, ermöglicht das individuelle Fallverstehen eine reflexive Kritik. Derzeit ist es vor allem die Palliativmedizin, die sich für eine neue Balance dieser Handlungslogiken einsetzt, wobei offen ist, ob sie einen verdrängenden Umgang mit dem Tod überwinden kann. (shrink)
