My commentary on mcmahon addresses the fact that only peripheral data have been considered for explaining differential sensibility in somato- and viscerosensory systems. This fails to take it into account that central processing for visceral and somatic inputs is now known to depend on different functional pathways. My commentary on berkley points out that the hypothalamus-pituitary axis is more responsive to stress in females than in males.

The World Health Organization, in its last review of its International Classification of Diseases, established a new classification for chronic pain. Among the principal categories, of particular interest is chronic primary pain as a new type of diagnosis in those cases in which the etiology of the disease is not clear, being termed as chronic primary visceral pain when it is situated in the thorax, abdomen, or pelvis. Due to the novelty of the term, the objective (...) of the systematic review was to examine the psychopathological and neuropsychological disorders associated with chronic primary visceral pain. We carried out a search of the scientific literature following the PRISMA directives using the Pubmed, Medline, PsycInfo and Scopus databases. A total of 33 articles were selected after applying the inclusion and exclusion criteria. The analysis of the studies showed that most persons with chronic primary visceral pain suffer from at least one psychological disorder; the most prevalent being anxiety, depressive or somatoform disorders. The most frequent psychopathological symptoms are anxiety, depression and somatization. Similarly, the findings are insufficient to determine the existence of deficits in the domains of executive functioning, memory and intelligence. However, the existence of attention biases does seem to be clear. This review supposes a starting point for conceptualizing chronic primary visceral pain. It is necessary to continue further research so as to obtain a better understanding of this pathology and the disorders associated. (shrink)

Regulators of G protein signaling (RGS) proteins provide timely termination of G protein‐coupled receptor (GPCR) responses. Serving as a central control point in GPCR signaling cascades, RGS proteins are promising targets for drug development. In this review, we discuss the involvement of RGS proteins in the pathophysiology of the gastrointestinal inflammation and their potential to become a target for anti‐inflammatory drugs. Specifically, we evaluate the emerging evidence for modulation of selected receptor families: opioid, cannabinoid and serotonin by RGS proteins. We (...) discuss how the regulation of RGS protein level and activity may modulate immunological pathways involved in the development of intestinal inflammation. Finally, we propose that RGS proteins may serve as a prognostic factor for survival rate in colorectal cancer. The ideas introduced in this review set a novel conceptual framework for the utilization of RGS proteins in the treatment of gastrointestinal inflammation, a growing major concern worldwide. There are some conspicuous differences between the sensibilities of cutaneous and visceral tissues: (1) Direct trauma, which readily produces pain when applied to the skin, is mostly without effect in healthy visceral tissue. (2) Pain that arises from visceral tissues is initially often poorly localised and diffuse. (3) With time, visceral pains are often referred to more superficial structures. (4) The site of referred pain may also show hyperalgesia. (5) In disease states, the (...) afflicted viscera may also become hyperalgesic. In this target article, I consider to what extent differences in the physiology, anatomy, and chemistry of peripheral processing systems explain these different sensibilities. In almost every aspect, there are subtle differences in the properties of the processing mechanisms for cutaneous and visceral information. These may arise because of distinct developmental cues operating in the two domains. Many of the differences between visceral and cutaneous afferents are quantitative rather than qualitative. The quantitative differences, for example in the density of afferent innervation, can be large. The quantitative differences in the numbers of afferents alone may be a sufficient explanation for some aspects of the differential sensibility, for example, the poor localisation of sensation and the apparent insensitivity to focal yet tissue- damaging stimuli. In addition, the few clear qualitative differences apparent in the innervations of the two tissue types may be of special importance. That the encoding of visceral nociceptive events may occur by an intensity mechanism rather than a specificity mechanism could be the key difference in viscerosensory and somatosensory processing. A scientific understanding of pain requires an experiential‐phenomenological approach and method, one that precedes mechanistic explanations provided by neuroscience, molecular neurobiology, and even the rest of psychology. A key challenge in this approach is to find ways to observe and characterize the experience of pain. An experiential method applied to both clinical and experimental pain has found three common factors in all instances of pain: a somatic or visceral experience that is comprised of 1) unique (...) sensory qualities that are like those which occur during tissue damaging stimulation, 2) a closely related meaning of intrusion and/or threat, and 3) a feeling of unpleasantness and/or other possible negative emotion(s) in response to 1 and 2. These three factors comprise an experiential definition of pain and they are each correlated with brain activities and functional connections that serve them. Both the sensory and the motivational-affective component of pain must be taken into account in studies on sex differences as well as on neuropathic, postoperative, sympathetic, and visceral pain. In all these cases, therapeutic strategies should be aimed at controlling the peripheral, central, and psychological mechanisms underlying the global pain experience. This paper examines the inferential framework employed by Palaeolithic cognitive archaeologists, using the work of Wynn and Coolidge as a case study. I begin by distinguishing minimal-capacity inferences from cognitive-transition inferences. Minimal-capacity inferences attempt to infer the cognitive prerequisites required for the production of a technology. Cognitive-transition inferences use transitions in technological complexity to infer transitions in cognitive evolution. I argue that cognitive archaeology has typically used cognitive-transition inferences informed by minimal-capacity inferences, and that this reflects a tendency to favour (...) cognitive explanations for transitions in technological complexity. Next I look at two alternative explanations for transitions in technological complexity: the demographic hypothesis and the environmental hypothesis. This presents us with a dilemma: either reject these alternative explanations or reject traditional cognitive-transition inferences. Rejecting the former is unappealing as there is strong evidence that demographic and environmental influences play some causal role in technological transitions. Rejecting the latter is unappealing as it means abandoning the idea that technological transitions tell us anything about transitions in hominin cognitive evolution. I finish by briefly outlining some conceptual tools from the philosophical literature that might help shed some light on the problem. (shrink)

This article examines the inherently ethical nature of resource allocation in policing. Decision-makers must make trade-offs between values such as efficiency vs. equity, individual vs. collective benefit, and adopt principles of distribution which allocate limited resources fairly. While resource allocation in healthcare has been the subject of extensive discussion in both practitioner and academic literature, ethical resource allocation in policing has received almost no attention. We first consider whether approaches used in healthcare settings would be suitable for policing. Whilst there (...) are some high-level similarities in the relevant ethical considerations and processes—such as aiming for efficient and equitable resource deployment—there are also fundamental differences between the two settings, which we argue necessitate a different approach. These differences include: (1) greater diversity and incommensurability of the benefits of policing, (2) more frequent non-linearity in returns on policing investment, (3) greater “ethical divisibility” (permissible scaling down) of programs in policing, (4) a requirement to assess “need” for policing resources at the collective (rather than individual) level, and (5) clearer primacy of equity considerations in policing. Having drawn out the implications of these differences, we sketch some tentative proposals for ethical resource allocation in policing. (shrink)
