Rationale In Belgium, several policies regulating reimbursement of acid suppressant drugs and evidence-based recommendations for clinical practice were issued in a short period of time, creating a unique opportunity to observe their effect on prescribing. Aims and objectives To describe the evolution of prescriptions for acid suppressants and explore the interaction of policies and practice recommendations with prescribing patterns. Method Monthly claims-based data for proton pump inhibitors (PPIs) and H-2-antihistamines by general practitioners, internists and "astroenterologists were obtained from the Belgian (...) national health insurance database (1997-2005). The evolution of reimbursed defined daily doses and expenses after introduction of reimbursement regulations and dissemination of practice recommendations was explored. Results Recommendations had no impact on prescribing. All changes can be related to concomitant policies. Lifting reimbursement restrictions for cheaper products did not control growth or save costs in the Iona term. Only restricting reimbursement of all PPIs managed to curb the growth. We observed an unintended increase of non-omeprazole PPIs by gastroenterologists. Conclusions Reimbursement policies influence prescribing, but their effect can be unintended. A dialogue between policymakers and guideline developers, and evidence-based policies that are linked to clinical guidelines, could be an effective way to pursue both cost-containment and quality of care. This essay highlights a methodological weakness in Foucault’s genealogy of neoliberalism often mistaken for a biographical shift in his philosophy. Naissance de la biopolitique is sometimes interpreted as evidence for Foucault’s conversion to neoliberalism, whereas its lack of critical acuity stems rather from its methodological limitations. Through a discussion of the “neoliberal conversion”-thesis, I highlight those limitations. Though Foucault’s appreciative tone in his neoliberalism lectures is surprising, his aim is mainly to defamiliarize readers from the dominant mode of neoliberal rationality (...) so that they can affirm the creative potential to foster new conducts, new identities, and new rationalities. Foucault did not convert to neoliberalism, but sought to destabilise it by revealing its historical contingency. However, as Foucault’s surprisingly positive tone shows, this strategy is insufficient for combatting neoliberalism. There is an elective affinity between Foucault’s own politics of creative self-reinvention and neoliberalism’s promise of a non-invasive, post-disciplinary government by indirect economic incentives alone. Foucault’s libertine stance toward subjectivity hence seems easily integrated into a neoliberal conduct of conducts. I propose to supplement Foucault’s genealogical method as he deploys it in Naissance de la biopolitique with an immanent critique of neoliberalism from the perspective of the governed. Rather than investigating neoliberal rationality as a promising endeavour for the future, we can study the real-life implementations of neoliberalism and their effects on subjective conducts. Neoliberal rationality is involved in a permanent struggle to shape the conduct of conducts along entrepreneurial and competitive lines, but subjects are refractory living beings whose conducts are not so simply transformed. Neoliberalism has had to give up its post-disciplinary aspirations and turn to the power of discipline and abandonment to enforce neoliberal norms of conduct. Neoliberalism has transformed into a negative biopolitics that sacrifices the lives of unproductive subjects as collateral damage to the overall welfare of the population. Immanent critique thereby reveals how subjects are pushed beyond their limits until neoliberal norms of conduct become unbearable. This intolerability of neoliberalism cannot be directly deduced from the writings of Becker, Friedman, or Hayek.111 It becomes manifest in the confrontation between neoliberal rationality and the material living bodies of finite human beings unable to be stretched in the ways neoliberalism requires. Valorising the biocultural heritage of common goods could enable peasant farmers to achieve socially and economically inclusive sustainability. Increasingly appreciated by consumers, peasant heritage products offer small farmers promising opportunities for economic, social and territorial development. Identifying the obstacles and levers of this complex, multi-scale and multi-stakeholder objective requires an integrative framework. We applied the panarchy conceptual framework to two cases of participatory research with small quinoa producers: a local fair in Chile and quinoa export production in Bolivia. In both (...) cases, the “commoning” process was crucial both to bring stakeholders together inside their communities and to gain outside recognition for their production and thus achieve social and economic inclusion. Despite the differences in scale, the local fair and the export market shared a similar marketing strategy based on short value chains promoting quality products with high identity value. In these dynamics of biocultural heritage valorisation, the panarchical approach revealed the central place as well as the vulnerability of the community territory. As a place of both anchoring and opening, the community territory is the privileged space where autonomous and consensual control over the governance of common biocultural resources can be exercised. A travers ces interrogations, il s'agit de mettre en lumière la fonction de cette matière particulière dont les aspects sont déclinés de manière originale dans l'ensemble de son oeuvre. En rassemblant la multiplicité des images de l'eau, l'auteur interroge la nature et la valeur des images en s'efforçant de montrer comment (...) Platon, écrivain et poète, met en oeuvre avec inventivité métaphores, allégories et mythes. De quoi alimenter un questionnement sur le réel, auquel l'eau donne un fil directeur. On y voit aussi se défaire une certaine idée du platonisme. Critique à l'égard de la rhétorique et des arts, Platon vise dans le miroir les effets illusoires de l'image et de l'imitation, mais Socrate utilise lui-même les images et les reflets tout au long des dialogues, et on en découvre avec étonnement toutes les variations. La métaphore hydrique conditionne l'univers de la parole et de la science, reflète le mouvement de la pensée et ses dérives. Dans les images métaphysiques et morales, ce sont les flux, les fleuves et le monde marin qui reflètent la nature et la qualité des âmes ; mythes et allégories déplient les symboles de la corruption, de l'oubli et de l'incontinence, où se jouent une idée de la matière et la valeur de l'existence. Sur le plan ontologique, l'eau dénoue les figures de l'instable qui mettent en oeuvre la réfutation des «liquides» - théoriciens du flux - et l'intelligibilité du devenir. Background In 2002, Belgium set a legal framework for euthanasia, whereby granting and performing euthanasia is entrusted entirely to physicians, and—as advised by Belgian Medical Deontology—in the context of a trusted patient–physician relationship. Euthanasia is, however, rarely practiced, so the average physician will not attain routine in this matter. Aim To explore how general practitioners in Flanders (Belgium) deal with euthanasia. This was performed via qualitative analysis of semistructured interviews with 52 general practitioners (GPs). Results Although GPs can understand a (...) patient's request for euthanasia, their own willingness to perform it is limited, based on their assumption that legal euthanasia equates to an injection that ends life abruptly. Their willingness to perform euthanasia is affected by the demanding nature of a patient's request, by their views on what circumstances render euthanasia legitimate and by their own ability to inject a lethal dose. Several GPs prefer increasing opioid dosages and palliative sedation to a lethal injection, which they consider to fall outside the scope of euthanasia legislation. Conclusions Four attitudes can be identified: (1) willing to perform euthanasia; (2) only willing to perform as a last resort; (3) feeling incapable of performing; (4) refusing on principle. The situation where GPs have to consider the request and—if they grant it—to perform the act may result in arbitrary access to euthanasia for the patient. The possibility of installing transparent referral and support strategies for the GPs should be further examined. Further discussion is needed in the medical profession about the exact content of the euthanasia law. (shrink)

For a more sustainable future of work, digital technologies should support human development instead of subordinating it to algorithmic control.

'The Matrix Project' comprises two contexts, namely photographic work of architect Thierry Lagrange and the self-reflective trajectory that he developed during his doctorate at KULeuven. Lagrange's practice-based research developed to an autonomous artistic project, in which he studied the signification and nature of observation within a creative process. Through his multifaceted practice as an architect, photographer and researcher, Lagrange developed 'The Matrix Method'; a method that stimulates creativity within different contexts and can be implemented for example in art education. The aim of this essay is to understand the relationship between knowledge and society and to reflect on the links between science and political decision making. The text evolved from a number of reflections the author made while president of the European Astronomical Society, president of the Swiss Academy of Sciences and vice-president of the European Academies Science Advisory Council (EASAC). The book starts by using astronomy as a showcase for what science brings to society in terms of intellectual enrichment, (...) of practical tools and of societal inputs. It then turns to looking generally at science as a human endeavour for which pleasure is a prime motivation and it describes the efforts made by researchers to rationalise their findings, thus making them universally acceptable. The author also describes the role of science in shaping our environment and discusses resulting responsibility of the scientists with respect to the evolution of the world. As part of an analysis of the relationship between science and policy the author describes the way in which scientists can (and must) bring knowledge in the political decision making process. The argument is closed with considerations on global governance, while the conclusion puts evidence based decision making processes in relation with the more emotional aspects of our behaviour. The readership of the book is intended to be all scientists involved in "science for policy" activities, as well as those who should become more active in this domain. This includes, but is not limited to, all members of science academies. Astronomers will find specific elements to help them think about their science. The intended readers of the book also include civil servants and policy makers who develop legislation and societal action in domains in which a solid knowledge base is important. Nourri de littérature classique et de philosophie orientale, éternel célibataire aux nombreuses amitiés masculines et féminines, brillant conférencier, il concentre en lui cette indépendance intemporelle et cet appel du Wild qui constituent l'âme de la nation américaine. Au risque d'écorner l'image d'Epinal que s'est faite de lui la postérité, cette biographie se propose de révéler les aspects les moins connus de la vie du grand écrivain, qui tint son Journal pendant un quart de siècle. Ainsi choisi et légitimé par l’entreprise encyclopédique et ses reprises, l’ouvrage de Cramer devient objet patrimonial au tournant du xixe siècle pour les mathématiciens, amateurs, professionnels ou enseignants qui travaillent sur les courbes algébriques. Le suivi sur le temps long des (...) mentions de l’Introduction dans les encyclopédies, dictionnaires ou histoires des mathématiques en langue française, ainsi que dans quelques textes d’enseignement, nous permettra de comprendre que ce processus de patrimonialisation n’induit pas sa conservation, sa protection, sa valorisation à de seules fins mémorielles, en figeant l’ouvrage dans un passé immuable. Au contraire, il permet une historicisation critique de la part des acteurs qui les met en position d’interroger et de réactualiser les connaissances et les pratiques que l’œuvre contient, au gré des mouvements d’émergence, de réorganisation et des spécialisations des savoirs mathématiques, quitte à en oublier certains aspects pour en mettre de nouveaux en valeur. Les rapports de pouvoir irradient notre société : sont-ils inéluctables? Nous nous apercevons qu'ils ont été construits pour asseoir la domination d'une minorité sur le reste de la population. Cet ouvrage démonte et analyse les mécanismes qui sont à l'origine des relations de pouvoir. Le point de départ de cette étude réside dans la volonté d'interroger la croyance, aussi répandue que massive, selon laquelle la domination serait naturelle aux hommes et aux animaux. En effet, lorsqu'on examine les opinions habituelles portant (...) sur la domination se distingue ordinairement la certitude quasi indéracinable qu'elle ne serait rien que l'expression d'une force naturelle et que, s'il y a des forts et des faibles, c'est parce que force et faiblesse, puissance et soumission seraient inscrites dans un ordre naturel des choses. À la faveur d'une alliance entre philosophie et biologie, il s'agit ici de se demander quelle est l'origine de la domination, et de comprendre les mécanismes des relations de pouvoir. Au fond, y a-t-il une légitimité de nature à poser ou réclamer l'inégalité? Où l'on voit que l'enjeu de ce livre réside dans l'effort pour résoudre scientifiquement et philosophiquement le problème du statut des phénomènes de domination dans le monde, et pour poser les bases à partir desquelles peut s'édifier une pensée de l'émancipation authentique. Ce livre collectif complète Le Territoire des philosophes (La Découverte, 2009), qui se focalisait sur les penseurs du XXe siècle. Dix-neuf auteurs, de Platon à Nietzsche, sont ici questionnés quant à leur usage théorique de l'espace et du lieu, alors même qu'ils sont, pour la plupart, célèbres pour d'autres concepts et apports. Les contributeurs s'efforcent de situer la pensée de leur auteur dans son temps et dans la longue géohistoire des idées. La philosophie est ici entendue à la fois comme un « art de vivre » et une manière de comprendre le monde, deux dimensions inséparables dans la saisie et de l'espace et du lieu. While the challenges workers face in the gig economy are now well-known, reflections on emancipatory solutions in political philosophy are still underdeveloped. Some have pleaded for enhancing workers' bargaining power through unionisation; others for enhancing exit options in the labour market. Both strategies, however, come with unin-tended side-effects and do not exhaust the full potential for worker self-government present in the digital gig economy. Using the republican theory of freedom as non-domination , I argue that G.D.H. Cole's 20th-century defence of (...) guild socialism offers a promising avenue for enhancing worker autonomy in the gig economy. Platform companies rely on relational and structural domination to undermine worker autonomy, but Cole's guild socialism was specifically designed to enhance autonomy in the workplace. Following Cole's advice today would amount to a defence of worker-owned cooperative platforms. By putting workers in control of platform design and governance, cooperative platforms create new opportunities for worker autonomy. However, platform coopera-tivism faces serious challenges if it plans on becoming a realistic alternative to the traditional gig economy. (shrink)

Formal topology aims at developing general topology in intuitionistic and predicative mathematics. Many classical results of general topology have been already brought into the realm of constructive mathematics by using formal topology and also new light on basic topological notions was gained with this approach which allows distinction which are not expressible in classical topology. Here we give a systematic exposition of one of the main tools in formal topology: inductive generation. In fact, many formal topologies can be presented in (...) a predicative way by an inductive generation and thus their properties can be proved inductively. We show however that some natural complete Heyting algebra cannot be inductively defined. (shrink)

In the beginning of the 1970s, Michel Foucault dismisses the terminology of ‘exclusion’ for his projected analytics of modern power. This rejection has had major repercussions on the theory of neoliberal subject-formation. Many researchers disproportionately stress how neoliberal dispositifs produce entrepreneurial subjects, albeit in different ways, while minimizing how these dispositifs sometimes emphatically refuse to produce neoliberal subjects. Relying on Saskia Sassen’s work on financialization, I argue that neoliberal dispositifs not only apply entrepreneurial norms, but also suspend their application for (...) groups that threaten to harm the population’s profitability. Neoliberal dispositifs not only produce entrepreneurial subjects, but also surplus populations that are expelled from the overall population to maintain its productivity. Here, the concept of ‘exclusion’ is appropriate if understood in Agamben’s sense of an inclusive exclusion. The surplus population is part of neoliberal dispositifs, but only as the element to be abandoned. (shrink)

Une analyse de la dimension egoiste de la personnalite de Montaigne, prise dans un sens moral, qui romprait avec la traditionnelle emprise de la sphere publique sur le prive, posant les jalons d'une pensee liberale. Il confronte la pensee du philosophe a celle d'Habermas, d'Arendt, de Derrida et de Rand pour en devoiler l'heritage.

Objective: Body dissatisfaction is a major risk factor underlying vulnerability to eating disorders. Laboratory settings suggest that body dissatisfaction could be improved by using evaluative conditioning (EC). The present study evaluates the feasibility of using an EC app in everyday life and the effects of its use on body dissatisfaction. Method: We designed a game-like app inspired by the Therapeutic Evaluative Conditioning app. 60 participants were randomly assigned to two conditions. Participants in the EC condition had to pair photographs of (...) their own body with positive photographs, while participants in the neutral condition had to pair photographs of their own body with neutral photographs. We tested the effect of use of the app on body dissatisfaction, drive for thinness, self-esteem, mood and eating behaviors. Results: Analysis revealed that participants in all conditions presented a significant decrease in body dissatisfaction and drive for thinness. However, contrary to our hypothesis, these reductions were not greater in the EC condition than in the neutral condition. Conclusions: This is the first study to evaluate the effects of an app-based EC intervention targeting body image. Results appear to be promising and the app could easily be implemented in an ecological setting as it is a low effort, attractive and accessible intervention. However, these findings do not support the idea that evaluative conditioning is responsible for the effects. Results are discussed. (shrink)

Digital innovations are rapidly changing the contemporary workplace. Big Tech companies marketing algorithmic management increasingly decide on the Future of Work. Political responses, however, often focus on managing the impact of these technologies on workers. They leave the question of how these technologies are designed or how workers can determine their own futures unanswered. This approach risks surrendering the Future of Work debate to techno-determinist imaginaries aligned with corporate interests. Using Cornelius Castoriadis’ early writings on worker struggles in French Tayloristic (...) factories and his later oeuvre on conflictual social imaginaries, I propose an agonistic approach to the Future of Work. Not merely companies implementing workplace technologies, but also workers themselves possess agency in determining the Future of Work. Workers’ frequent contestations against machinic surveillance and technologically mediated coordination of the labour process disclose an alternative Future of Work in which technologies are used to enhance worker autonomy rather than bureaucratic control. Worker struggles at Amazon Mechanical Turk illustrate this agonistic dynamic: Amazon mobilizes algorithmic management to fragment and control a workforce of globally dispersed remote workers, but the latter form online social communities and create counter-technologies to reappropriate control over the labour process. The primary method by which science endeavors to order the world is the analytic approach, consistent with Cartesian principles of dividing the problem in as many sections as required for an optimal solution, and progressing from the simplest to the most complex reasoning. When the interactions among the various elements of the system being studied are minimal, such a procedure indeed makes it possible to formulate laws that describe chains of causality. However, when the variables are interdependent and linked by (...) non-linear equations, the atomistic method cannot account for the phenomenon in its entirety. Spurred by this inadequacy, scientists sought to discover synthetic methods which, early in the twentieth century, led to the conceptualization of holistic theories in a number of disciplines. The most highly developed fields are the psychology of form, structural anthropology (1), and the various systems theories (2). More recently, the development of epigenetic concepts in biology has corresponded to the appearance of different theories of pattern in physics and mathematics (3, 4) and to the progressive emergence of the concept of self-organization (5, 6, 7). The objective is to explain the production of complex structures on the basis of interacting elements, none of which contains a guiding scheme that dictates these structures. In biology, the challenge is to locate the constraints that are expressed in a space of possibilities (8) in order to develop a theory of organization that is capable of predicting the finite number of forms that living creatures can assume (9). This school of thought can be described as structuralist, emergentist, self-organizational, or epigeneticist, depending on the theoretical version that is emphasized. (shrink)

Le livre reprend les interventions des conférenciers et intervenants, présente de manière synthétique et pédagogique les débats qui ont suivi. et en propose une relecture. Cette dernière est faite en termes de défis que l'ensemble de la société est invité à (...) relever pour que les sciences et les techniques soient vraiment au service d'une société toujours plus humaine.
