BackgroundFear of cancer recurrence is common amongst cancer survivors. There is rapidly growing research interest in FCR but a need to prioritize research to address the most pressing clinical issues and reduce duplication and fragmentation of effort. This study aimed to establish international consensus among clinical and academic FCR experts regarding priorities for FCR research.MethodsMembers of the International Psycho-oncology Society Fear of Cancer Recurrence Special Interest Group were invited to participate in an online Delphi study. Research domains identified in Round (...) 1 were presented and discussed at a focus group to consolidate the domains and items prior to presentation in further survey rounds aimed at gaining consensus on research priorities of international significance.ResultsThirty four research items were identified in Round 1 and 33 of the items were consolidated into six overarching themes through a focus group discussion with FCR experts. The 33 research items were presented in subsequent rounds of the delphi technique. Twenty one participants contributed to delphi round 1, 16 in round 2, and 25 and 29 participants for subsequent delphi rounds. Consensus was reached for 27 items in round 3.1. A further four research items were identified by panelists and included in round 3.2. After round 3.2, 35 individual research items were ratified by the panelists. Given the high levels of consensus and stability between rounds, no further rounds were conducted. Overall intervention research was considered the most important focus for FCR research. Panelists identified models of care that facilitate greater access to FCR treatment and evaluation of the effectiveness of FCR interventions in real world settings as the two research items of highest priority. Defining the mechanisms of action and active components across FCR/P interventions was the third highest priority identified.ConclusionThe findings of this study outline a research agenda for international FCR research. Intervention research to identify models of care that increase access to treatment are based on a flexible approach based on symptom severity and can be delivered within routine clinical care were identified as research areas to prioritize. Greater understanding of the active components and mechanisms of action of existing FCR interventions will facilitate increased tailoring of interventions to meet patient need. (shrink)

While cancer survivors commonly experience fear and anxiety, a substantial minority experience an enduring and debilitating fear that their cancer will return; a condition commonly referred to as fear of cancer recurrence. Despite recent advances in this area, little is known about FCR among people from Indigenous or other ethnic and racial minority populations. Given the high prevalence and poor outcomes of cancer among people from these populations, a robust understanding of FCR among people from these groups is critical. The (...) current review identified and aggregated existing literature on FCR amongst adult cancer survivors from Indigenous and minority populations. The protocol of this review was registered with PROSPERO in July 2020. A systematic search of bibliographic databases was conducted for relevant articles published from 1997 to November 2019. Data from eligible articles were extracted and appraised for quality by two independent reviewers. Nineteen articles from four countries met the inclusion criteria, including 14 quantitative, 4 qualitative and 1 mixed-methods study. Only one article reported on an Indigenous population. Few studies reported on FCR prevalence or severity. While the variation in tools used to measure FCR hindered a robust estimate of severity, results suggested some differences in FCR severity between minority and dominant populations, although these may have been due to study metholodological differences. Few factors were reported as being associated with FCR in minorities across multiple studies. The qualitative synthesis found five themes associated with the lived experience of FCR: variations in the lived experience of FCR; spirituality and worldview impacting on FCR; the importance of staying positive; complexities around support; and increasing cancer knowledge. The findings of this review highlight differences in FCR across cultures and contexts, which reinforces the need for culturally-specific approaches to this condition. The dearth of research in this area is of concern given the significant burden of cancer in these populations. A deeper understanding of this condition among Indigenous and minority populations is critical to developing and delivering appropriate and effective psychosocial care for cancer survivors from these groups. Mireille Hadas-Lebel shines a spotlight on the complex life and works of Philo, the illustrious Alexandrian Jewish philosopher, offering a fascinating insight into a seminal religious thinker at the crossroads of Judaism and Hellenism.

Should citizens have equal say regarding the running of society? Following the principles of democracy, and most of political philosophy: yes (at least at a fundamental level, thus allowing for representatives and the like). Indeed, comparing the main alternative seemingly supports this intuition. Epistocracy would instead give power just to the most epistemically competent. Yet testing citizens’ political and economic knowledge looks apt to disproportionately disempower marginalised groups, making the position seem like a nonstarter and democracy the clear winner. Nevertheless, (...) this paper argues against giving citizens equal say, or at least, it offers the strongest possible motivation for this position. In particular, I introduce the progressive case for epistocracy, and what I term standpoint epistocracy. This account refigures the relevant notion of political competence such that it is not the most privileged classes who would most likely constitute our epistocracy, but rather, the least. The resulting picture considerably improves on traditional versions of epistocracy and also democracy. (shrink)

This book explores four principal accounts of the ontological basis of properties, including universals, trope theory, resemblance nominalism, and class nominalism, considering the assumptions and ontolological commitments which are required to make each into a plausible account of properties. -/- The latter half of the book investigates the applications of property theory and the different conceptions of properties which might be adopted with these in mind: first, the possibility and desirability of individuating properties, and (...) the distinction between sparse and abundant conceptions of properties; second, the distinction between intrinsic and extrinsic properties; third, the relationship between properties and their causal powers, and whether this is necessary or contingent; fourth, the relationship between properties and causation, modality, and laws of nature; finally, whether properties exist objectively, and if they do, what our epistemic position is with respect to such entities. Le Poidevin has recently presented an argument that gives rise to a serious problem for relationist theories of space. It appeals to the simple geometrical fact that if A, B and C are three points lying in a straight line, then AB and BC together entail AC. He suggests that an ontological relationship of supervenience must be appealed to to explain this entailment. Given this thesis of supervenience, relationism is implausible. I argue that the problem that Le Poidevin raises for (...) relationism should be rejected, because the thesis of supervenience is false. The latter rests upon what Le Poidevin refers to as ‘The explanatory principle', a principle which he claims to be a natural extension of the truthmaker principle. Contrary to this, I argue that given any plausible theory of truthmaking, the explanatory principle is false. With the rejection of this principle, Le Poidevin's argument against relationism collapses. (shrink)

Many views rely on the idea that it can never be rational to have high confidence in something like, “P, but my evidence doesn’t support P.” Call this idea the “Non-Akrasia Constraint”. Just as an akratic agent acts in a way she believes she ought not act, an epistemically akratic agent believes something that she believes is unsupported by her evidence. The Non-Akrasia Constraint says that ideally rational agents will never be epistemically akratic. In a number of recent papers, the (...) Non-Akrasia Constraint has been called into question. The most widespread statement of political philosophy is presented here in the simplified and trivialised form of “man is X; he must become Y. ” Man must do so at the same time for himself, for his own survival, but also for the good of all, of the Community, of the City: the plurality must absolutely, in any way whatsoever, give way to unity, subject to [sous peine] and under threat of chaos. The essential question found confronting political doctrines, moreover (...) since the prominence of the idea of democracy is the following: how to bring about the existence of a united society across a heterogeneous social body? The response from philosophy articulates itself around three principle schemas: the theoretical construction of a unified community under the order of similar laws to those of nature (Platonic schema); the search for the best regime, which will have as its ultimate end the moderation and perpetual regulation of conflicts, by an optimal combination of freedom and stability (Aristotelian schema); and the theorisation of the “end of politics” by the locating of a “social wrong” inscribed in a structure doomed to collapse by the practical negation of its ideological foundations (Marxian schema and derivatives). The mode of thought which imposes itself here is decisional. Beyond the third schema which constitutes in some way a meta-political critique, and necessitates that it alone has a particular analysis, in the two preceding, man constitutes a kind of material – raw or primary, depending – that philosophy will work on, and sculpt to give it a form that harmonises fully with the Whole that it prescribes. In political philosophy, we always turn more or less around the “Let’s make man” of Hobbes, that is, around the technical transformation of a material given. Or to say it otherwise: the creation of an oeuvre from crude and imperfect elements, an oeuvre thought like a masterpiece – that is to say, one that contains within it an idea of perfection and permanence – but doomed here to serial reproduction. Believing rationally is epistemically valuable, or so we tend to think. It’s something we strive for in our own beliefs, and we criticize others for falling short of it. We theorize about rationality, in part, because we want to be rational. But why? I argue that how we answer this question depends on how permissive our theory of rationality is. Impermissive and extremely permissive views can give good answers; moderately permissive views cannot.

Epistemologists often assume that rationality bears an important connection to the truth. In this paper I examine the implications of this commitment for permissivism: if rationality is a guide to the truth, can it also allow some leeway in how we should respond to our evidence? I first discuss a particular strategy for connecting permissive rationality and the truth, developed in a recent paper by Miriam Schoenfield. I argue that this limited truth-connection is unsatisfying, and the version of permissivism that (...) supports it faces serious challenges; so, for mainstream permissivism, the truth problem is still unsolved. I then discuss a strategy available to impermissivists, according to which rationality bears a quite strong connection to truth. I argue that this second strategy is successful. (shrink)

The subject of this book is young children's emotional-social learning and development within early childhood care and education settings in Aotearoa-New Zealand. The focus on emotional complexity fills a gap in early childhood care and education research where young children are frequently framed narrowly as 'learners,' ignoring the importance of emotional functioning and the feelings with which children make sense of themselves and the world. This book draws on original data in the form of narrative-like framed events to creatively illustrate (...) the complexities in children's diverse ways of feeling, thinking, playing, being, and becoming. Events illuminate the feelings and meanings of observed experiences in holistic and contextualised gestalts. Awareness of unconscious processes, the feeling of feelings, and cultural dimensions of development and meaning-making are addressed. The book emphasises the emergent and psychodynamic nature of children's development and learning with strong links to the role of play and playfulness in the events, drawing on two ethnographically inspired research projects that present theory, experience and practice in real-life events. The conventional view of Hobbes’s commonwealth is that it was inspired by contemporary theories of tyranny. This article explores the idea that a paradigm for Hobbes’s state could in fact be found in early modern readings of Aristotle on democracy, as found in Book Three of the Politics. It argues that by the late sixteenth century, these meditations on the democratic body politic had developed claims about unity, mythology, and personation that would become central to Hobbes’s own theory of the (...) commonwealth. Tracing the history of commentary on the relevant passages in Aristotle reveals new perspectives not only on the political theories of both Aristotle and Hobbes but also introduces modern readers to the richness of early modern commentaries on classical political texts. The article ends with some thoughts on why attention to traditions of commentary might be valuable for political theorists today. (shrink)

Credences, unlike full beliefs, can’t be true or false. So what makes credences more or less accurate? This chapter offers a new answer to this question: credences are accurate insofar as they license true educated guesses, and less accurate insofar as they license false educated guesses. This account is compatible with immodesty; : a rational agent will regard her own credences to be best for the purposes of making true educated guesses. The guessing account can also be used to justify (...) certain coherence constraints on rational credence, such as probabilism. The chapter concludes by discussing some advantages of the guessing account over rival accounts of accuracy. (shrink)

UK higher education reform (BIS, 2011) has been presented as a common‐sense movement towards efficiency. This article will argue that, in reality, the marketisation of higher education is a movement towards negative freedom, defined after Berlin (2007) as unrestricted choice. Using Shakespeare's Antony and Cleopatra as a means to explore the relationship between rationality and sensibility, it considers how negative freedom may undermine human connectivity and debase our relationships. In so doing, this article challenges the idea that importing the market (...) system into education will enhance the ‘student experience’. (shrink)

William James famously tells us that there are two main goals for rational believers: believing truth and avoiding error. I argues that epistemic consequentialism—in particular its embodiment in epistemic utility theory—seems to be well positioned to explain how epistemic agents might permissibly weight these goals differently and adopt different credences as a result. After all, practical versions of consequentialism render it permissible for agents with different goals to act differently in the same situation. -/- Nevertheless, I argue that epistemic consequentialism (...) doesn’t allow for this kind of permissivism and goes on to argue that this reveals a deep disanalogy between decision theory and the formally similar epistemic utility theory. This raises the question whether epistemic utility theory is a genuinely consequentialist theory at all. (shrink)

Do conceivability arguments work against physicalism if properties are causal powers? By considering three different ways of understanding causal powers and the modality associated with them, I will argue that most, if not all, physicalist powers theorists should not be concerned about the conceivability argument because its conclusion that physicalism is false does not hold in their favoured ontology. I also defend specific powers theories against some recent objections to this strategy, arguing that the conception of properties as powerful blocks (...) conceivability arguments unless a rather implausible form of emergence is true. The actions of able agents are often reliably successful. I argue that their success may be modally robust along two dimensions. The first dimension helps distinguish the exercise of abilities, which requires local control, from lucky success. The second concerns the global availability of acts: agents with the ability to φ can φ across a variety of circumstances. I introduce a framework that captures the two dimensions and their interaction, and show how it bears on a disagreement about the modal (...) force the robustness of ability requires: while local control involves a kind of local necessity, global availability does not. Toute phénoménologie, par définition, part de et en reste au monde tel qu'il s'offre à la conscience. Une "phénoménologie de la transcendance" semble donc une entreprise impossible, puisqu'il s'agirait de chercher dans l'expérience du monde "quelque chose" qui ne puisse en aucune manière que ce soit être rapporté au monde. L'expression de "phénoménologie de la transcendance" est ainsi formellement contradictoire: car si la transcendance était "phénomène", et pouvait faire l'objet d'une "-logie", d'une saisie par le logos, elle serait précisément de (...) l'ordre de ce qui peut être mis sous la proposition "il y a quelque chose plutôt que rien". This article takes as its starting point the central place given to contradiction by Hermann Goldschmidt in his book Contradiction Set Free, and it compares his approach with that of the philosopher Vladimir Jankélévitch. At the same time as Goldschmidt, Jankélévitch also assigned a central role to contradiction in thought, so much so that he often referred to his own philosophical method as “paradoxology.” For him, as for Goldschmidt, paradox is the driving force behind thought that is always on the (...) move. This article presents the main features of Jankélévitch’s paradoxology and illustrates it with two themes: forgiveness and Jewish identity. By highlighting this proximity between Goldschmidt’s approach and that of Jankélévitch, I suggest that they are both part of a more general movement in continental philosophy in the second half of the twentieth century, one of whose main challenges was to rethink philosophical rationality in depth. Le langage de l'enfant, le langage de l'écolier, de l'enfant du peuple, fait l'objet d'observations à la fois patientes et passionnées à la fin du XIXe siècle et toutes ces observations sont résumées dans l'expression qui devient courante alors de « vie du langage ». Quelque chose là ne se laisse pas dire, sinon par métaphore. Qu'entend-on par « vie du langage »? La référence au puissant paradigme de la vie est fort à la mode car l'évolutionnisme spencérien, puis darwinien, (...) a pris le pouvoir sur toutes les sciences dans la deuxième moitié du siècle. Pour autant, la vie en question n'est qu'une métaphore : il ne s'agit pas, en France du moins, d'appliquer littéralement au langage les caractéristiques des êtres vivants. Ce mode d'expression signale l'émergence d'une vision nouvelle des rapports entre langue et pensée, qui bouleverse les cadres classiques de la question. Il se découvre, dans le babillage de l'enfant, comme dans le langage automatique des fous, ou bien encore dans la misère linguistique du peuple, une insubordination de la langue à la pensée. Les gens ne pensent pas ce qu'ils disent, ils parlent sans penser ou pensent sans parler. En réalité, et en dépit de toutes les illusions qu'ils entretiennent à ce sujet, leur langue est assez rarement ajustée à leurs idées, elle les devance ou les ignore, elle « vit » à sa façon. Comment dès lors arriver à une langue « juste »? Comment éduquer le peuple, l'enfant du peuple, de manière à ce qu'il puisse maîtriser sa pensée? Telle est la question centrale de la pédagogie républicaine qui travaille, au tournant du XIXe et du XXe siècle, en étroite relation avec les sciences humaines naissantes. (shrink)

Advances in neuroscience have enabled the transition of transcranial direct current stimulation (tDCS) from research and clinical settings to public use. For this primarily home-based context, tDCS has been popularized as a do-it-yourself (DIY) approach to improved cognition and wellness. The line between wellness and health is blurry, however, and little is known about how engagement with therapeutic tDCS impacts users’ interactions with other interventions such as clinical consultations, pharmacotherapy, complementary medicine, and even other neurotechnology. To close this gap, we (...) collected data from the online content aggregator Reddit and analyzed posts pertaining to tDCS. Findings indicate that most users turn to Reddit to request information about tDCS as an adjunct, but not as a bypass, to ongoing or prior approaches. Posts suggest that mainstream medical care is viewed as necessary but not sufficient to address conditions such as depression and anxiety. Users discuss a mix of benefits and harms. This discourse provides valuable insights into the health practices, concerns and priorities of users, and new knowledge for informing applications of neurotechnology both inside and outside the therapeutic setting. Preterm birth is defined as delivery occurring before 37 weeks gestation.1 Infants born prematurely have increased risks of morbidity and mortality throughout life, especially during the first year. These risks increase as the gestational age at birth decreases.2 Additionally, there are significant racial and ethnic differences in preterm birth rates. In 2022, the rate of preterm birth among non-Hispanic black women was approximately 50% higher than that observed in non-Hispanic white women.1 The outcomes for these infants are also disparate–preterm birth (...) and low birth weight are the second-leading causes of infant mortality (deaths before 1 year of age). Notably, among preterm neonates, infants born to non-Hispanic black women have a mortality rate more than double that of infants born to non-Hispanic white mothers.3 Importantly, there is a small but significant group of neonates that can be classified as severely premature (those born <28 weeks). Infants born before 28 weeks have varied and uncertain outcomes. While this group accounts for only 0.4% of preterm births, it also accounts for 40% of deaths among preterm neonates.4 As noted by Romanis and Adkins, the advent of artificial amnion and placenta technology (AAPT) holds promise for its potential to address the physiologic complications and mortality risks of extreme prematurity by introducing amniotic substitutes that mimic in vivo placental oxygenation and substrate delivery. Objective To explore attitudes towards conscientious objections among medical students in the UK. Methods Medical students at St George's University of London, Cardiff University, King's College London and Leeds University were emailed a link to an anonymous online questionnaire, hosted by an online survey company. The questionnaire contained nine questions. A total of 733 medical students responded. Results Nearly half of the students in this survey stated that they believed in the right of doctors to conscientiously object to any procedure. (...) Demand for the right to conscientiously object is greater in Muslim medical students when compared with other groups of religious medical students. Discussion Abortion continues to be a contentious issue among medical students and this may contribute to the looming crisis in abortion services over the coming years. This project sheds some light on how future doctors view some of their ethical rights and obligations. Using empirical evidence, it reveals that conscientious objection is an issue in the UK medical student body today. These data could help anticipate problems that may arise when these medical students qualify and practise medicine in the community. Conclusion Clearer guidance is needed for medical students about the issue of conscientious objection at medical school. (shrink)

ABSTRACTThis paper argues that confabulation is motivated by the desire to have fulfilled a rational obligation to knowledgeably explain our attitudes by reference to motivating reasons. This account better explains confabulation than alternatives. My conclusion impacts two discussions. Primarily, it tells us something about confabulation – how it is brought about, which engenders lively debate in and of itself. A further upshot concerns self-knowledge. Contrary to popular assumption, confabulation cases give us reason to think we have distinctive access to why (...) we have our attitudes. (shrink)
