ABSTRACT In Mexico informed consent is a legal requirement that ensures that patients who are invited to participate in clinical trials are provided with all the information needed to decide whether to participate, or not, in a research protocol. To improve our understanding of the problems physicians in developing countries encounter, when obtaining informed consent (IC), we examined their opinion on the importance of IC in clinical research, the quantity and quality of the information provided to the participant, and the (...) conditions in which the IC is obtained. Investigators considered that IC was useful to the patients, providing information that helped the patient to make a decision about his/her participation. Nevertheless, they felt that for some aspects of the research, like drug development in general, the use of placebos, and the randomization process, many of the patients were not capable of fully understanding the information provided, referring to the complexity of the information and illiteracy as the main reasons. Many investigators were not acquainted with some of the guidelines established in the Mexican General Law of Health,1 36% of them admitting to not having completed their IC letters. Most investigators gave only minutes to the patient to make a decision and 20% of ICs were obtained while the patient was hospitalized. Except for one investigator, all of them considered that specific training in medical ethics would be useful for the daily clinical work. (shrink)

The COVID-19 Vaccines Global Access Facility represents an unprecedented global collaboration facilitating the development and distribution of vaccines for COVID-19. COVAX pools and channels funds from state and non-state actors to promising vaccine candidates, and has started to distribute successful candidates to participating states. The WHO, one of the leaders of COVAX, recognised vaccine doses would initially be scarce, and therefore, prepared a two-staged allocation mechanism they considered fair. In the first stage, vaccine doses are distributed equally among participating countries, (...) while in the second stage vaccine doses will be allocated according to a country’s need. Ethicists have questioned whether this is the fairest distribution—they argue a country’s need should be taken into account from the start and correspondingly, have proposed a framework that treats individuals with equal moral concern, aims to minimise harm and gives priority to the worst-off. In this paper, we seek to explore these concerns by comparing COVAX’s allocation mechanism to a targeted allocation based on need. We consider which distribution would more likely maximise well-being and align with principles of equity. We conclude that although in theory, a targeted distribution in proportion to a country’s need would be more morally justifiable, when political realities are taken into account, an equal distribution seems more likely to avert a greater number of deaths and reduce disparities. The Diagnostic and Statistical Manual (DSM) of the American Psychiatric Association, currently in its fourth edition and considered the reference for the characterization and diagnosis of mental disorders, has undergone various developments since its inception in the mid-twentieth century. With the fifth edition of the DSM presently in field trials for release in 2013, there is renewed discussion and debate over the extent of its relative successes - and shortcomings - at iteratively incorporating scientific evidence on the often ambiguous nature (...) and etiology of mental illness. Given the power that the DSM has exerted both within psychiatry and society at large, this essay seeks to analyze variations in content and context of various editions of the DSM, address contributory influences and repercussion of such variations on the evolving landscape of psychiatry as discipline and practice over the past sixty years. Specifically, we document major modifications in the definition, characterization, and classification of mental disorders throughout successive editions of the DSM, in light of shifting trends in the conceptualization of psychopathology within evolving schools of thought in psychiatry, and in the context of progress in behavioral and psychopharmacological therapeutics over time. We touch upon the social, political, and financial environments in which these changes took places, address the significance of these changes with respect to the legitimacy (and legitimization) of what constitutes mental illness and health, and examine the impact and implications of these changes on psychiatric practice, research, and teaching. We argue that problematic issues in psychiatry, arguably reflecting the large-scale adoption of the DSM, may be linked to difficulties in formulating a standardized nosology of psychopathology. In this light, we highlight 1) issues relating to attempts to align the DSM with the medical model, with regard to increasing specificity in the characterization of discrete mental disease entities and the incorporation of neurogenetic, neurochemical and neuroimaging data in its nosological framework; 2) controversies surrounding the medicalization of cognition, emotion, and behavior, and the interpretation of subjective variables as 'normal' or 'abnormal' in the context of society and culture; and 3) what constitutes treatment, enablement, or enhancement - and what metrics, guidelines, and policies may need to be established to clarify such criteria. The return to religion has perhaps become the dominant cliche of contemporary theory, which rarely offers anything more than an exaggerated echo of a political reality dominated by religious war. Somehow, the secular age seems to have been replaced by a new era, where political action flows directly from metaphysical conflict. The Faith of the Faithless asks how we might respond. Following Critchley's Infinitely Demanding, this new book builds on its philosophical and political framework, also venturing into the questions of (...) faith, love, religion and violence. Should we defend a version of secularism and quietly accept the slide into a form of theism--or is there another way? From Rousseau's politics and religion to the return to St. Paul in Taubes, Agamben and Badiou, via explorations of politics and original sin in the work of Schmitt and John Gray, Critchley examines whether there can be a faith of the faithless, a belief for unbelievers. It is shown that the Hilbert-Bernays-Quine principle of identity of indiscernibles applies uniformly to all the contentious cases of symmetries in physics, including permutation symmetry in classical and quantum mechanics. It follows that there is no special problem with the notion of objecthood in physics. Leibniz's principle of sufficient reason is considered as well; this too applies uniformly. But given the new principle of identity, it no longer implies that space, or atoms, are unreal.

In considering the debate about the meaning of ‘disease’, the positions are generally presented as falling into two categories: naturalist, e.g., Boorse, and normativist, e.g., Engelhardt and many others. This division is too coarse, and obscures much of what is going on in this debate. I therefore propose that accounts of the meaning of ‘disease’ be assessed according to Hare’s (1997) taxonomy of evaluative terms. Such an analysis will allow us to better understand both individual positions and their inter-relationships. Most (...) importantly, it will show that it is unlikely that there is a single unique disease-concept at issue. Rather, different authors are, for the most part, considering different concepts. But this picture of a ‘block universe’, composed of a timeless web of ‘world-lines’ in a four-dimensional space, however strongly suggested by the theory of relativity, is a piece of gratuitous metaphysics. Since the concept of change, of something happening, is an inseparable component of the common-sense concept of time and a necessary component of the scientist's view of reality, it is quite out of the question that theoretical physics should require us to hold the Eleatic view that nothing happens (...) in ‘the objective world’. Here, as so often in the philosophy of science, a useful limitation in the form of representation is mistaken for a deficiency of the universe. (shrink)

This paper considers John Doris, Stephen Stich, Alexandra Plakias, and colleagues’ recent attempts to utilize empirical studies of cross-cultural variation in moral judgment to support a version of the argument from disagreement against moral realism. Crucially, Doris et al. claim that the moral disagreements highlighted by these studies are not susceptible to the standard ‘diffusing’ explanations realists have developed in response to earlier versions of the argument. I argue that plausible hypotheses about the cognitive processes underlying ordinary moral judgment and (...) the acquisition of moral norms, when combined with a popular philosophical account of moral inquiry—the method of reflective equilibrium—undercut the anti-realist force of the moral disagreements that Doris et al. describe. I also show that Stich's recent attempt to provide further theoretical support for Doris et al.'s case is unsuccessful. (shrink)

The purpose of this study was to compare exploratory behaviours in children with autism and typically developing preschool children and the course of their adaptation to novelty. A series of five repeated trials was conducted, during which children were allowed to freely explore the experimental room. The results revealed differences between study groups in the overall rate of exploratory activity, which was lower in children with autism. Patterns of time characteristics of exploratory activity showed both similarities and differences between the (...) groups. In both groups, the rate of simple exploratory behaviours decreased with time, while the levels of diversive exploration increased. Children with autism engaged in less complex object manipulation than their peers. Similarly, their adaptation and habituation to a novel environment proceeded in a different way in the low stimulation zone than in the high stimulation zone. In the low and medium stimulation zones, the rate of exploration decreased with time, while in the high stimulation zone it remained relatively constant. In typically developing children, habituation occurred in all stimulation zones. These results suggest the presence of some differences between the patterns of adaptation to novelty in the two groups, which emerge in a stimulation-rich environment. Due to the limitations of the study, in particular the small number of subjects, the present paper should be treated as a preliminary report. (shrink)

If science fiction is a genre, then attempts to think about the nature of science fiction will be affected by one’s understanding of what genres are. I shall examine two approaches to genre, one dominant but inadequate, the other better, but only occasionally making itself seen. I shall then discuss several important, interrelated issues, focusing particularly on science fiction : what it is for a work to belong to a genre, the semantics of genre names, the validity of attempts to (...) define genres, and the connections between genre and normativity. One important but neglected clue to the nature of genres lies in the kinds of disagreements they generate over the assignment of works to genres. I conclude by explaining why these disagreements tell us something about the nature of genres, and discussing in some detail two famous cases of disagreement about whether some work or works are science fiction. Ethical discussions about Artificial Intelligence (AI) often overlook its potentially large impact on nonhuman animals. In a recent commentary on our paper about AI’s possible harms, Leonie Bossert argues for a focus not just on the possible negative impacts but also the possible beneficial outcomes of AI for animals. We welcome this call to increase awareness of AI that helps animals: developing and using AI to improve animal wellbeing and promote positive dimensions in animal lives should be a vital ethical (...) goal. Nonetheless, we argue that there is some value in focusing on technology-based harms in the context of AI ethics and policy discourses. A harms framework for AI can inform some of our strongest duties to animals and inform regulation and risk assessment impacts designed to prevent serious harms to humans, the environment, and animals. Simon Evnine examines various epistemic aspects of what it is to be a person. Persons are defined as finite beings that have beliefs, including second-order beliefs about their own and others' beliefs, and are agents, capable of making long-term plans. It is argued that for any being meeting these conditions, a number of epistemic consequences obtain. First, all such beings must have certain logical concepts and be able to use them in certain ways. Secondly, there are at least two (...) principles governing belief that it is rational for persons to satisfy and are such that nothing can be a person at all unless it satisfies them to a large extent. These principles are that one believe the conjunction of one's beliefs and that one treat one's future beliefs as, by and large, better than one's current beliefs. Thirdly, persons both occupy epistemic points of view on the world and show up within those views. This makes it impossible for them to be completely objective about their own beliefs. Ideals of rationality that require such objectivity, while not necessarily wrong, are intrinsically problematic for persons. This "aspectual dualism" is characteristic of treatments of persons in the Kantian tradition. In sum, these epistemic consequences support a traditional view of the nature of persons, one in opposition to much recent theorizing. (shrink)

Which political principles should govern global politics? In his new book, Simon Caney engages with the work of philosophers, political theorists, and international relations scholars in order to examine some of the most pressing global issues of our time. Are there universal civil, political, and economic human rights? Should there be a system of supra- state institutions? Can humanitarian intervention be justified?

Many environmental scientists, scholars and activists characterise our situation as one of alienation from nature, but this notion can easily seem meaningless or irrational. In this book, Simon Hailwood critically analyses the idea of alienation from nature and argues that it can be a useful notion when understood pluralistically. He distinguishes different senses of alienation from nature pertaining to different environmental contexts and concerns, and draws upon a range of philosophical and environmental ideas and themes including pragmatism, eco-phenomenology, climate (...) change, ecological justice, Marxism and critical theory. His novel perspective shows that different environmental concerns - both anthropocentric and nonanthropocentric - can dovetail, rather than compete with, each other, and that our alienation from nature need not be something to be regretted or overcome. What it means for conspiracy theories to become mainstream In debates about conspiracy theories, it is often claimed that conspiracy thinking is on the rise or has even become mainstream. In this article, I want to explore this claim conceptually, and argue that there are at least three ways to interpret the claim that ‘conspiracy thinking has become mainstream’. First, there is the individual level, where it is a matter of counting heads. Mainstream then means that the majority believes in (...) conspiracy theories. But there are two other levels. There is the institutional level, which is about whether conspiracy theories circulate within social institutions in an acceptable way. Is it acceptable to express conspiracy theories as a politician, scientist or journalist? Mainstream here means that dominant institutions in society view conspiracy thinking as an acceptable form of communication. Finally, there is the cultural level. Here the question is whether conspiracy thinking is a culturally acceptable way of speaking and thinking about society. Is it socially accepted as normal behavior, or is it rather a form of deviant behavior, accompanied by (informal) sanctions or demands for additional justification or proof? Mainstream here means that conspiracy thinking is seen as a normal way of speaking, without being accompanied by strange looks, shunning or the need to justify oneself as a conspiracy thinker. By making this distinction, we can think more clearly about whether conspiracy thinking is actually on the rise, and how we should deal with it. (shrink)

Everyone agrees that the world is accelerating. With advances in communication, transportation and information processing technologies, it is clear that the pace of events in global politics is speeding up at an alarming rate. The implications of this new speed however, continue to be a significant source of debate. Will acceleration lead to a more interconnected, productive, peaceful, and humane world; or a nightmarish descent into ecological devastation, economic exploitation and increasingly violent warfare? The Politics of Speed attempts to map (...) the contours of the new global space of speed, and investigates key issue areas âe" including democratic governance, warfare, capitalism, globalization and transnational activism âe" to uncover the ways in which acceleration is shaping the world. The book uses contemporary political theory (especially the works of Deleuze and Guattari) to develop an ontological account of speed, showing how its effects are frequently far more complex and surprising than we might expect. The result is an attempt to craft a way of engaging with global acceleration that might help avoid the dangers of speed, while embracing the possibilities it provides us with to produce a safer, more egalitarian, democratic and pluralistic world. The later Wittgenstein’s perspective on mathematical sentences as norms is motivated for sentences belonging to Hilbertian axiomatic systems where the axioms are treated as implicit definitions. It is shown that in this approach the axioms are employed as norms in that they function as standards of what counts as using the concepts involved. This normative dimension of their mode of use, it is argued, is inherited by the theorems derived from them. Having been motivated along these lines, Wittgenstein’s perspective on (...) mathematical language may appeal also to those who are not friends of or experts on Wittgenstein’s later philosophy of mathematics. This book is the first treatment at length of negative, or apophatic, theology within the analytic tradition. Apophatic theology holds that there is a significant sense in which we cannot say what God is. Important negative theological elements are present in a host of Christian thinkers, from Gregory of Nyssa to Aquinas, and yet apophaticism is neglected in philosophical theology as practiced within the analytic tradition. By contrast, Hewitt shows how apophatic theology is integral to how Christians have thought about (...) God, and how it can be defended against standard attacks in the philosophical literature. Hewitt diagnoses the unease with apophaticism amongst contempory philosophical theologicans as rooted in a certain picture of how language functions, here called referentialism. Arguing that this picture is not compulsory, an account of language which sits more comfortably with negative theology (originating from work of later Wittgenstein) is invoked, and applied to key themes in philosophical theology including divine personhood, the Trinity, the Incarnation and the afterlife. Our engagement with time is a ubiquitous feature of our lives. We are aware of time on many scales, from the briefest flicker of change to the way our lives unfold over many years. But to what extent does this encounter reveal the true nature of temporal reality? To the extent that temporal reality is as it seems, how do we come to be aware of it? And to the extent that temporal reality is not as it seems, why does (...) it seem that way? These are the central questions addressed by Simon Prosser in Experiencing Time. He defends the B-theory of time, according to which the apparently dynamic quality of change, the special status of the present, and even the passage of time are all illusions. Simone Weil is one of the most profound thinkers of the twentieth century. Her writings encompass an extraordinary breadth of subjects, including philosophy, religion, sociology, and politics. The badness of having conflicting emotions is a familiar theme in academic ethics, clinical psychology, and commercial self-help, where emotional harmony is often put forward as an ideal. Many philosophers give emotional harmony pride of place in their theories of practical reason.1 Here we offer a defense of a particular species of emotional conflict, namely, ambivalence. We articulate an conception of ambivalence, on which ambivalence is unresolved inconsistent desire (§1) and present a case of appropriate ambivalence (§2), before considering two (...) alternative defenses of emotional conflict (§3). We then argue that inconsistent desires can be fitting (§4) and that it can be reasonable not to resolve inconsistent desires (§5), before considering an objection (§6) and concluding the discussion (§7).
