Since the onset of the SARS-CoV-2 pandemic, an array of off-label interventions has been used to treat patients, either provided as compassionate care or tested in clinical trials. There is a challenge in determining the justification for conducting randomised controlled trials over providing compassionate use in an emergency setting. A rapid and more accurate evaluation tool is needed to assess the effect of these treatments. Given the similarity to the Ebola Virus Disease (EVD) pandemic in Africa in 2014, we suggest (...) using a tool designed by the WHO committee in the aftermath of the EVD pandemic: Monitored Emergency Use of Unregistered and Investigational Interventions (MEURI). Considering the uncertainty around SARS-CoV-2, we propose using an improved MEURI including the Plan–Do–Study–Act tool. This combined tool may facilitate dynamic monitoring, analysing, re-evaluating and re-authorising emergency use of unproven treatments and repeat it in cycles. It will enable adjustment and application of outcomes to clinical practice according to changing circumstances and increase the production of valuable data to promote the best standard of care and high-quality research—even during a pandemic. Developers are often the engine behind the creation and implementation of new technologies, including in the artificial intelligence surge that is currently underway. In many cases these new technologies introduce significant risk to affected stakeholders; risks that can be reduced and mitigated by such a dominant party. This is fully recognized by texts that analyze risks in the current AI transformation, which suggest voluntary adoption of ethical standards and imposing ethical standards via regulation and oversight as tools to compel developers (...) to reduce such risks. However, what these texts usually sidestep is the question of how aware developers are to the risks they are creating with these new AI technologies, and what their attitudes are towards such risks. This paper asks to rectify this gap in research, by analyzing an ongoing case study. Focusing on six Israeli AI startups in the field of radiology, I carry out a content analysis of their online material in order to examine these companies’ stances towards the potential threat their automated tools pose to patient safety and to the work-standing of healthcare professionals. Results show that these developers are aware of the risks their AI products pose, but tend to deny their own role in the technological transformation and dismiss or downplay the risks to stakeholders. I conclude by tying these findings back to current risk-reduction recommendations with regards to advanced AI technologies, and suggest which of them hold more promise in light of developers’ attitudes. This book demonstrates that we need not choose between seeing so-called Presocratic thinkers as rational philosophers or as religious sages. In particular, it rethinks fundamentally the emergence of systematic epistemology and reflection on speculative inquiry in Hesiod, Xenophanes and Parmenides. Shaul Tor argues that different forms of reasoning, and different models of divine disclosure, play equally integral, harmonious and mutually illuminating roles in early Greek epistemology. Throughout, the book relates these thinkers to their religious, literary and historical surroundings. This article argues that Empedocles’ trust in Strife (DK31 B115.14 = LM22 D10.14) is not, as the prevailing interpretation has it, only a past misjudgement and failure. Rather, trust in Strife still, and to his own lament, infects Empedocles’ mind and informs his life. This detail then offers a fresh perspective on Empedocles’ self-conception and on how, through the daimōn’s cosmic peregrinations, Empedocles raises and pursues questions of agency and responsibility. Furthermore, it sheds light on Empedocles’ understanding of his own (...) ethical standing: common assumptions to the contrary notwithstanding, he does not see himself as an unqualifiedly successful embodiment of his own ideals. Perceptual learning can be defined as practice-induced improvement in the ability to perform specific perceptual tasks. We previously proposed the Reverse Hierarchy Theory as a unifying concept that links behavioral findings of visual learning with physiological and anatomical data. Essentially, it asserts that learning is a top-down guided process, which begins at high-level areas of the visual system, and when these do not suffice, progresses backwards to the input levels, which have a better signal-to-noise ratio. This simple concept has proved (...) powerful in explaining a broad range of findings, including seemingly contradicting data. We now extend this concept to describe the dynamics of skill acquisition and interpret recent behavioral and electrophysiological findings. (shrink)

The current study examines moral dilemmas related to the war against terror: the amounts of force used to arrest or harm a “most wanted” terrorist: the greater the use of force, the higher the risk for harming civilians and the lower the risk to the soldiers and vice versa. The study focuses on the association between moral decisions, confidence, and level of difficulty in making the decisions and political attitudes among Israeli Defense Forces soldiers. In addition, the study examines the (...) differences between regular and reserve combat infantry soldiers regarding these variables. According to study hypotheses results indicate more right-wing political attitudes predicted greater use of force and fewer difficulties in making moral decisions, and reserve soldiers reported higher use of fire, higher confidence, and lower level of difficulty in making moral decisions compared with regular soldiers. The current study uses a repeated measures design to compare two-time points across the COVID-19 pandemic. The first was conducted at the end of the “first wave” [T1] and the second was carried out on October 12-14 2020 in Israel. The participants completed the same questionnaire at both time points. The study examined the predictions of hope and morale at T2 by psychological and demographic predictors at T1. Results indicated the following: The three types of resilience significantly and positively predicted (...) hope and morale. Well-being significantly and positively predicted hope and morale. Younger age significantly and positively predicts higher hope, but not morale. A higher level of religiosity significantly and positively predicts higher hope and morale. More right-wing political attitudes significantly and positively predict higher hope, but not moral. More economic difficulties due to the pandemic, significantly and negatively predict hope and morale. We concluded that hope and morale can serve as significant indicators of the population's ability to cope with the COVID-19 pandemic. Furthermore, they can serve as a “thermometer” for the general mood of the population and can be used by decision-makers to assess coping ability at varied stages of the pandemic. (shrink)

Following the lead of Duncan Pritchard’s “Wittgensteinian Pyrrhonism,” this paper takes a further, comparative and contrastive look at the problem of justification in Sextus Empiricus and in Wittgenstein’sOn Certainty. I argue both that Pritchard’s stimulating account is problematic in certain important respects and that his insights contain much interpretive potential still to be pursued. Diverging from Pritchard, I argue that it is a significant and self-conscious aspect of Sextus’ sceptical strategies to call into question large segments of our belief systemen (...) masseby exposing as apparently unjustifiable fundamental propositions which are closely related in their linchpin role to Wittgenstein’s hinge propositions. In the first instance, the result is a more complex account of both a deeper affinity between Wittgenstein’s approach to hinge propositions and Sextus’ approach to what I termarchaipropositions and a divergence between the two. In the second instance, I suggest how the comparison withOn Certaintycan be illuminating for the interpreter of Sextus. In particular, it can help us to see how the Pyrrhonist’s everyday conduct—common assumptions to the contrary notwithstanding—involves rational procedures of justification, in line with a naturalism reminiscent of Wittgenstein. Furthermore, it can help us to reflect on the Pyrrhonist’s attitude to what Wittgenstein would have called her ‘worldview’. Throughout, I suggest that the comparison with Wittgenstein is interesting, although it must be cashed out differently, not only on the interpretation—or, perhaps, strand—of ancient Pyrrhonism which has the sceptic exempt ordinary beliefs from her suspension of judgement, but also on the interpretation which has her disavow all beliefs categorically. This article explores the ethical challenges of providing Medical Assistance in Dying (MAID) in a paediatric setting. More specifically, we focus on the theoretical questions that came to light when we were asked to develop a policy for responding to MAID requests at our tertiary paediatric institution. We illuminate a central point of conceptual confusion about the nature of MAID that emerges at the level of practice, and explore the various entailments for clinicians and patients that would flow from different (...) understandings. Finally, we consider the ethical challenges of building policy on what is still an extremely controversial social practice. While MAID is currently available to capable patients in Canada who are 18 years or older—a small but important subsection of the population our hospital serves—we write our policy with an eye to the near future when capable young people may gain access to MAID. We propose that an opportunity exists for MAID-providing institutions to reduce social stigma surrounding this practice, but not without potentially serious consequences for practitioners and institutions themselves. Thus, this paper is intended as a road map through the still-emerging legal and ethical landscape of paediatric MAID. We offer a view of the roads taken and considered along the way, and our justifications for travelling the paths we chose. By providing a record of our in-progress thinking, we hope to stimulate wider discussion about the issues and questions encountered in this work. (shrink)
