In 2012, a U.S. Institute of Medicine report called for a different approach to health care: “Left unchanged, health care will continue to underperform; cause unnecessary harm; and strain national, state, and family budgets.” The answer, they suggested, would be a “continuously learning” health system. Ethicists and researchers urged the creation of “learning health organizations” that would integrate knowledge from patient‐care data to continuously improve the quality of care. Our experience with an ongoing research study on atrial fibrillation—a trial known (...) as IMPACT‐AFib—gave us some insight into one of the challenges that will have to be dealt with in creating these organizations. Although the proposed educational intervention study placed no restrictions on what providers and health plans could do, the oversight team argued that the ethical principle of beneficence did not allow the researchers to be “bystanders” in relation to a control group receiving suboptimal care. In response, the researchers designed a “workaround” that allowed the project to go forward. We believe the experience suggests that what we call “bystander ethics” will create challenges for the kinds of quality improvement research that LHOs are designed to do. (shrink)

The present article provides an analysis of the existing proof systems for dynamic epistemic logic from the viewpoint of proof-theoretic semantics. Dynamic epistemic logic is one of the best known members of a family of logical systems that have been successfully applied to diverse scientific disciplines, but the proof-theoretic treatment of which presents many difficulties. After an illustration of the proof-theoretic semantic principles most relevant to the treatment of logical connectives, we turn to illustrating the main features of display calculi, (...) a proof-theoretic paradigm that has been successfully employed to give a proof-theoretic semantic account of modal and substructural logics. Then, we review some of the most significant proposals of proof systems for dynamic epistemic logics, and we critically reflect on them in the light of the previously introduced proof-theoretic semantic principles. The contributions of the present article include a generalization of Belnap's cut-elimination metatheorem for display calculi, and a revised version of the display-style calculus D.EAK. We verify that the revised version satisfies the previously mentioned proof-theoretic semantic principles, and show that it enjoys cut-elimination as a consequence of the generalized metatheorem. Les fouilles récentes dans la partie nord du site archéologique de Kition-Bamboula ont conduit à la découverte d’un puits tout à fait remarquable. Par sa forme tout d’abord, puisqu’il s’agit d’un puits à roue élévatrice d’eau, d’un type qui n’était jusqu’alors documenté, à Chypre, que pour des périodes plus récentes. Par son comblement ensuite, puisque ce dernier contenait, outre divers débris, un dépôt de trois enfants (périnatals) accompagnés de mobilier. Par sa date, enfin, puisqu’il atteste une occupation de la zone (...) au ive siècle apr. J.‑C., alors qu’on pensait le site abandonné. Cet article propose une étude complète du puits et de son comblement (à l’exception des restes fauniques, présentés dans un article séparé), que prolonge une réflexion sur le paysage urbain de Kition au début de l’Antiquité tardive. Le dépôt du rempart Nord d'Amathonte a livré une série particulièrement abondante et variée de céramiques culinaires. Ces vases, qui se distinguent par une pâte spécifique, sont classés et étudiés en détail. Beaucoup de formes sont rares ou inédites. Le rapprochement avec des trouvailles provenant d'autres sites et avec des représentations figurées permet de proposer quelques hypothèses d'utilisation et met en relief la variété des modes de cuisson. Ces récipients, qui appartenaient à la batterie de cuisine du palais, montrent que (...) des repas collectifs étaient préparés sur l'acropole. Comme les broches à rôtir et les chaudrons, les céramiques culinaires faisaient partie des instruments de cuisine de l'élite et, à ce titre, elles ont toute leur place au palais d'Amathonte et dans la nécropole « royale » de Salamine, où elles sont également très nombreuses. En appendice, est présenté le résultat d'analyses physico-chimiques portant sur cette catégorie céramique. (shrink)

The Archaic deposit of the North Rampart at Amathus, VII. Other Cypriote wares and Levantine imports After two previous studies, concerning specific categories in the deposit (kitchen wares, and vases of the ‘ Amathus style’), this article treats all the other ceramic categories of Cypriote production, as well as the Levantine imports. It draws up the repertoire and thus proposes a synthesis of the Amathusian production of Late Archaic times. The abundant material collected from up against the North Rampart of (...) the Lower Town of Amathus has to be compared with the contemporary deposits of the cave of the sanctuary of Aphrodite and of the cemeteries : this assemblage, of appreciably different composition, stresses the importance of feasting in the palace at the end of the Archaic period. In his critical comment on our paper in this journal, Shaw argues that ‘false assumptions’ which we have criticised are in fact correct . He suggests that the risk of addiction to neuroenhancers may not be relevant, and that safety and research in regard to neuroenhancement do not pose unique ethical problems. Here, we demonstrate that Shaw ignores key empirical research results, trivialises addiction, commits logical errors, confuses addictions and passions, argues on a speculative basis, and fails to distinguish the (...) specific ethical conditions of clinical research from those relevant for research in healthy volunteers. The excavations carried out beside the north rampart of the lower town of Amathus have revealed the existence of a large archaic deposit, most probably originating from a destruction of the palace on the acropolis. This is the first part of a series of articles devoted to the material from this deposit. It deals with the context of its discovery, and considers the circular structure, which requires careful attention, filled by this material.

The phrase, ‘political obligation’, is far more popular in English than in other European languages. Whether this may be due to historical circumstances, or to a peculiar bent of the English mind, is a fascinating question; but it is not the one which I propose to discuss here today. I am mentioning it only to explain the choice of my subject, a subject which would probably sound rather uncommon to an Italian audience, but which, I am sure, has a familiar (...) ring to English ears. My subject, I would like to emphasise, is the nature of political obligation; my approach will be analytical rather than historical; and indeed, let me say at once that this lecture is a kind of examen de conscience in which I am myself involved, since I feel personally responsible for having introduced the phrase into my own native language, and for having used it during a life-time as if its meaning were self-evident, without ever raising the questions which I want to raise today. Beauchamp and Childress have performed a great service by strengthening the principle of respect for the patient's autonomy against the paternalism that dominated medicine until at least the 1970s. Nevertheless, we think that the concept of autonomy should be elaborated further. We suggest such an elaboration built on recent developments within the neurosciences and the free will debate. The reason for this suggestion is at least twofold: First, Beauchamp and Childress neglect some important elements of autonomy. Second, neuroscience itself needs (...) a conceptual apparatus to deal with the neural basis of autonomy for diagnostic purposes.
