Measures are designed to evaluate the processes and outcomes of care associated with the delivery of clinical (and non-clinical) services. They allow for intra- and interorganizational comparison to be used continuously to improve patient health outcomes. The use of performance measures always means to abstract the complex reality (medical scenarios and procedures) in order to provide an understandable and comparable output. Measures can focus on global performance. The more detailed data are available the more specific judgements with respect to the (...) appropriateness of clinical decision-making and implementation of evidence are feasible. Externally reported measures are intended both to inform and lead to action. By providing this information, deficiencies in patient care and unnecessary variations in the care process can be uncovered. Such variations have contributed to disparities in morbidity and mortality. The developments in information technology, especially world-wide interconnectivity, standards for electronic data exchange and facilities to store and manage large amounts of data, offer the opportunity to analyse health-relevant information in order to make the delivery of healthcare services more transparent for consumers and providers. Global performance measures, such as the overall life expectancy (mortality) in a country, can give a rough orientation of how well health systems perform but they do not offer general solutions nor spe-cific insights into care processes that have to be improved. In contrast to population-based measures, case-based performance measures use a defined group of patients depending on specific patient characteristics and features of disease. By means of these measures we are able to compare the number of patients that receive a necessary medical procedure against those patients who do not. The use of case-based measures is a bottom-up approach to improve the overall performance in the long run. They are not only a tool for global orientation but can offer a straightforward link to the areas of deficient care and the underlying procedures. Performance measures are relevant to providers as well as consumers, from their own individual perspective. Cased-based measures focus on the management of individual patient. This approach to performance measurement can inform physicians in a meaningful and constructive way by monitoring their individual performance and by pointing out possible areas of improvement. Aber wie ist überhaupt unsere reflexive Einsicht erklärbar, daß unser Bewußtsein solche Charakteristika aufweist? An prominenten Beispielen läßt sich zeigen, daß dazu keineswegs die Annahme ausreicht, sie seien intrinsische Bestimmungen unserer Bewußtseinszustände. Vielmehr ist eine radikalere internalistische Konzeption der Inhalte unseres Bewußtseins erforderlich, wie sie Hector-Neri Castañeda entwickelt hat. ZusammenfassungDie traditionsreiche Deutsche Gesellschaft für Innere Medizin war durch ihr Agieren in der NS-Zeit belastet. Vor ihrem ersten Nachkriegskongress 1948 in Karlsruhe fiel dem Bonner Klinikdirektor Paul Martini die Rolle eines Reorganisators zu. Martini, der während der NS-Zeit in Opposition zum Regime gestanden hatte, wählte einen Kurs umfassender Integration. Verfolgte des NS-Staats versuchte er ebenso in die DGIM einzubinden wie einstige gemäßigte Nationalsozialisten. Zugleich bemühte er sich um die Bewahrung des gesamtdeutschen Charakters der Kongresse sowie um einen zügigen Anschluss an (...) die internationale Forschung. Dennoch führte der eingeschlagene Weg die DGIM in nur scheinbar unpolitische Wissenschaftlichkeit und Jahrzehnte weitgehender Kritiklosigkeit gegenüber ihrer NS-Geschichte. (shrink)

There is a widely accepted view in action theory (most prominently defended by Donald Davidson) according to which (1) actions are events, (2) reasons are intentional attitudes of the agent (pairs of beliefs and desires), and (3) acting for a reason entails that the reason rationalizes as well as causes the action. In the first part of my contribution I list seventeen difficulties for this standard account; in the second part I give an outline of how a more plausible conception (...) of reasons and actions could look like. According to this conception, which is based on Gilbert Ryle’s criticism of a mechanistic understanding of psychological concepts, agency is due to a special kind of disposition of the agent, namely the disposition to behave as if the agent were permanently deliberating about what to do. The conception has surprising consequences for the ontological status of intentional attitudes and actions and for the relationship between action and responsibility. (shrink)

Phenomena covered by the term duality occur throughout the history of mathematics in all of its branches, from the duality of polyhedra to Langlands duality. By looking to an “internal epistemology” of duality, we try to understand the gains mathematicians have found in exploiting dual situations. We approach these questions by means of a category theoretic understanding. Following Mac Lane and Lawvere-Rosebrugh, we distinguish between “axiomatic” or “formal” (or Gergonne-type) dualities on the one hand and “functional” or “concrete” (or Poncelet-type) (...) dualities on the other. While the former are often used in the pursuit of a “two theorems by one proof”-strategy, the latter often allow the investigation of “spaces” by studying functions defined on them, which in Grothendieck's terms amounts to the strategy of proving a theorem by working in a dually equivalent framework where the corresponding proof is easier to find. We try to show by some examples that in the first case, dual objects tend to be more ideal (epistemologically more remote) than original ones, while this is not necessarily so in the second case. Das Handbuch Angewandte Ethik soll folgenden beiden Interessen Rechnung tragen: sowohl dem allgemeinen Informationsbedarf, beispielsweise von Journalisten, Politikern, Rechtswissenschaftlern und Fachleuten der jeweils betroffenen Disziplinen, als auch dem spezifischen der Kolleginnen und Kollegen, Studierenden und Lehrenden in der Angewandten Ethik. Entsprechend öffnen die neun Teile des Buches mehrere Zugangswege in das Handbuch. Nicht nur der Aufbau des Handbuchs ist mehrdimensional, auch die Beiträge stammen aus unterschiedlichen Bildungshintergründen und Arbeitskontexten. This book addresses the endangerment of children’s bodies in affluent societies. Bodily integrity is an important part of a child’s physical and mental well-being, but it can also be violated through various threats during childhood; not only affecting physical health but also causing mental damage and leading to distortions in the development of the self. Gegenstand des Bandes ist eine Profilierung der Kulturphilosophie mit dem Ziel ihrer notigen Selbstbehauptung gegenuber einem gegenwartigen Naturalismus. Ein zentrales Ratsel der Gegenwartsphilosophie ist das Verhaltnis unseres Bewusstseins, der Perspektive der ersten Person machtig und Urheber der Kultur zu sein, zu den Erkenntnissen der Kognitions- sowie der Evolutionswissenschaften seit Darwin. In diesen scheint von der Natur her auf die Kultur ein neues Licht zu fallen. Dies ist ebenso faszinierend wie interessant. Es ist aber auch eine Herausforderung: Der Begriff der "Welt" (...) wird naturwissenschaftlich praokkupiert, Extrapolationen ins Ontologische wie die "kausale Geschlossenheit der Welt" breiten sich auf Kosten fruherer, starker zuruckgenommener Erkenntnis- und Wissenschaftstheorien aus. Wie steht es demgegenuber mit den Beitragen von Kulturphilosophie und Kulturwissenschaften / Geisteswissenschaften? Deutlich wird in den Beitragen im Anschluss an Dilthey, Cassirer, Simmel und weitere Autoren: Die Notwendigkeit und der Eigencharakter kultureller Arbeit mussen offentlich wie in den Wissenschaften wieder bewusst werden. School as counter-public" is the hermeneutic key with which Ralf Koerrenz interprets the school model of the Jena Plan. Similar to the Dalton-Plan or the Winnetka-Plan, the Jena Plan is one of the most important concepts of alternative schools developed in the first half of the 20th century as part of the international movement for alternative education, the?World Education Fellowship?. 0Peter Petersen's "Jena Plan" concept must be understood from his educational philosophical foundations. The didactic levels of action at school (...) (teaching, learning) as well as the reflection of theory in pedagogical practice are made understandable by "school as a counter-public". Not least with a view to the today's Jena Plan schools, the question is asked for a context-independent core of what makes a school a Jena Plan school. Davidson's account of action explanation is at the core of his theory of action and hence of his philosophy in general. Usually, Davidson is regarded as the typical exponent of a standard causal understanding of action explanation. A closer look at his writings reveals, however, that his approach is much more sophisticated, for example, by distinguishing between causes and causal powers and counting reasons among the latter, which allows him to give satisfactory answers to a number of standard problems in (...) action theory, for example, the validity of the logical connection argument, the problem of causal exclusion, and the problem of wayward causal chains. Moreover, it becomes clear in which way his understanding of action explanations fits with his thesis about the anomalous nature of the mental and his theory of radical interpretation in the philosophy of language. This paper distinguishes bruteness from fundamentality by developing a theory of stochastic grounding that makes room for non-fundamental bruteness. Stochastic grounding relations, which only underwrite incomplete explanations, arise when the fundamental level underdetermines derivative levels. The framework is applied to fission cases, showing how one can break symmetries and mitigate bruteness whilst avoiding arbitrariness and hypersensitivity.

The Lost Mirror traces cultural patterns in which the interpretation of learning and education was developed against the backdrop of Hebrew thought. The appreciation of learning is deeply rooted in the Hebrew way of thinking. Learning is understood as an open and history-conscious engagement of man with culture. The consciousness of history is shaped by the motif of the unavailability of the "other" and the difference to this "other". This "other" is traditionally remembered as "God", but may also be reflected (...) in the motifs of the other person or the other society. The lost Mirror reminds us of a deficit, which is that in our everyday thinking and everyday action, we usually hide, forget and partly suppress the meaning and presence of the unavailable other. The book approaches this thinking through portraits of people such as Hannah Arendt, Leo Baeck, Walter Benjamin, Agnes Heller, Emanuel Levinas and others. (shrink)

Ausgehend von den Grundelementen der psychoanalytischen Situation werden Basiskompetenzen des Analytikers postuliert und beschrieben, die die Herstellung und Bewahrung der analytischen Situation garantieren. Im Zentrum der Überlegung steht dann die analytische Deutungsfunktion, um die immer wieder gerungen werden muss, weil sie auch der Dynamik von Übertragung und Widerstand unterworfen ist. ABSTRACTOne prestudy based on a corpus analysis and four experiments in which participants had to invent novel names for persons or objects investigated how the valence of a face or an object affects the phonological characteristics of the respective novel name. Based on the articulatory feedback hypothesis, we predicted that /i:/ is included more frequently in fictional names for faces or objects with a positive valence than for those with a negative valence. For /o:/, the pattern should reverse. An analysis (...) of the Berlin Affective Word List – Reloaded yielded a higher number of occurrences of /o:/ in German words with negative valence than in words with positive valence; with /i:/ the situation is less clear. In Experiments 1 and 2, participants named persons showing a positive or a negative facial expression. In his new book, Fred Schauer adopts a prototypical approach to the law in order to reestablish the importance of “The Force of Law”, and I strongly support his claim that there are interesting things to be said about the relationship between law and force. One aspect concerns the special kind of force to which the law is related. In the tradition of political philosophy, this kind of force has often been characterized with the state's monopoly on legitimate force. Whereas (...) the essay will support the idea that the law has a monopoly of force, it will challenge the idea that it is its legitimacy that makes it characteristic. It is a monopoly not so much on legitimate, but on ultimate force. The robustness of the force the law is—prototypically—related to, however, should not obscure the fact that the relation between law and force is quite delicate and precarious. Three strategies of the law to manage this fundamental precariousness are pointed out. (shrink)

The mechanics of claims focusses predominantly on the claim to life. The claim to life is rooted in the autonomy principle, just like other specific claims. Still, the mechanics of claims does not have a systematic place for the fundamental negation of the status as an autonomous being as such. It is, however, the proctiction of the status as such, which is at the center of the protection of human dignity in German constitutional law. Looked at it from this perspective, (...) the protection of human dignity as the protection of the status of an autonomous human being, appears to be a blind spot of the mechanics of claims. The comment attempts to show, how this blindspot leads to inconsistencies in the mechanics of claims, and how they might be ameliorated if human dignity is considered as an absolute right independent of the claims to life.
