Background Despite decades of anti-racism and equity, diversity, and inclusion (EDI) interventions in academic medicine, medical racism continues to harm patients and healthcare providers. We sought to deeply explore experiences and beliefs about medical racism among academic clinicians to understand the drivers of persistent medical racism and to inform intervention design. Methods We interviewed academically-affiliated clinicians with any racial identity from the Departments of Family Medicine, Cardiac Sciences, Emergency Medicine, and Medicine to understand their experiences and perceptions of medical racism. (...) We performed thematic content analysis of semi-structured interview data to understand the barriers and facilitators of ongoing medical racism. Based on participant narratives, we developed a logic framework that demonstrates the necessary steps in the process of addressing racism using if/then logic. This framework was then applied to all narratives and the barriers to addressing medical racism were aligned with each step in the logic framework. Proposed interventions, as suggested by participants or study team members and/or identified in the literature, were matched to these identified barriers to addressing racism. Results Participant narratives of their experiences of medical racism demonstrated multiple barriers to addressing racism, such as a perceived lack of empathy from white colleagues. Few potential facilitators to addressing racism were also identified, including shared language to understand racism. The logic framework suggested that addressing racism requires individuals to understand, recognize, name, and confront medical racism. Conclusions Organizations can use this logic framework to understand their local context and select targeted anti-racism or EDI interventions. Theory-informed approaches to medical racism may be more effective than interventions that do not address local barriers or facilitators for persistent medical racism. (shrink)

There is little information regarding arterial elasticity in heart transplant recipients. This study aimed to characterize small and large artery elasticity in (...) heart transplant recipients in comparison with an asymptomatic population free of overt cardiovascular disease. A second goal was to identify demographic and clinical factors associated with arterial elasticity in this unique population.Methods: Arterial pulse waveform was registered noninvasively at the radial artery in 71 heart transplant recipients between 2008 and 2010. SAEs and LAEs were derived from diastolic pulse contour analysis. Comparisons were made to a healthy cohort of 1,808 participants selected from our prevention clinic database. Multiple regression analyses were performed to evaluate associations between risk factors and SAE and LAE within the heart transplant recipients.Results: LAE and SAE were significantly lower in heart transplant recipients than in the normal cohort. Female sex and history of ischemic cardiomyopathy were significantly associated with reduced LAE and SAE. Older age and the presence of moderate cardiac allograft vasculopathy were also significantly associated with reduced SAE. Transplant duration was associated with increased SAE.Conclusion: Heart transplants are associated with peripheral endothelial dysfunction and arterial stiffness, as demonstrated by a significant reduction in SAE and LAE when compared with a healthy population. Furthermore, cardiac allograft vasculopathy is associated with a significant reduction in SAE, suggesting a potential use in screening heart transplant recipients at risk for developing this complication.Keywords: heart transplant, endothelial function, arterial elasticity, cardiac allograft vasculopathy. In this study we examine college cheating behaviors of business students compared to non-business students, and investigate possible antecedents to cheating in an effort to better understand why and when students cheat. We specifically examine power values; we found that they were positively related to academic cheating in our sample, and that choice of major (business or non-business) partially mediated the relationship between power values and cheating. We also considered the extent to which students provide justifications for their cheating, and (...) found that business students were more likely to justify (rationalize) their cheating behaviors. Finally, we update the literature in terms of the ways students cheat. We assess newer forms of academic cheating, as increased accessibility to information via the Internet and smartphones may have changed the ways and ease with which students cheat – a particularly relevant topic currently, as many classes have moved online during the COVID-19 pandemic. In our study, cheating was especially prevalent when taking quizzes or tests or completing homework online. We found that only 10% of participants reported never engaging in any of the cheating behaviors we examined. (shrink)

Mitochondrial/nuclear transfer (M/NT) to avoid the transmission of serious mitochondrial disease raises complex and challenging ethical, legal and social issues (ELSI). In February 2015, the United Kingdom became the first country in the world to legalize M/NT, making the heated debate surrounding this technology even more relevant. This critical interpretive review identified 95 relevant papers discussing the ELSI of M/NT, including original research articles, government-commissioned reports, editorials, letters to editors and research news. The review presents and synthesizes the arguments present (...) in the literature in relation to the most commonly raised themes: terminology; identity, relationships and parenthood; potential harm; reproductive autonomy; available alternatives; consent; impact on specific interest groups; resources; “slippery slope”; creation, use and destruction of human embryos; and beneficence. The review concludes by identifying those ELSI that are specific to M/NT and by calling for follow-up longitudinal clinical and psychosocial research in order to equip future ELSI debate with empirical evidence. In April 2020, almost six out of 10 people around the world were in lockdown due to the COVID-19 pandemic. Being locked down usually has a deleterious effect on the confined individual's mental health. In this exceptionally challenging context, finding ways to minimize negative mood about the pandemic is essential. Pandemic-related negative states (“negative mood”) and recovery experiences were investigated in a sample of 264 individuals who completed daily surveys four times per day over 7 consecutive days. MSEMs analyses revealed (...) that negative mood persisted from moment-to-moment through the day, thus showing a response lag effect. Further analyses revealed that when someone experienced pandemic-related psychological detachment, relaxation, mastery, control, pleasure, or relatedness at specific periods of the day, mood had improved at the next measured time period, suggesting a protective effect. However, the pattern displayed by singles with dependents suggests that some recovery experiences at specific periods during the day seem to have a backfiring effect and worsen subsequent mood. These findings bring new insight into the role of recovery experiences during lockdowns and suggest that many could benefit from such experiences throughout the day when self-isolating. However, for individuals with multiple risk factors such as being single with dependents, some recovery experiences, at specific periods during the day, might not bring the desired outcome and future research is needed to examine if guilt or domestic burden may explain this finding. Results contribute to our understanding of how to take care of one's mental health during the current pandemic, and concrete recommendations adapted to individual contexts are provided. (shrink)

The Moral Nexus develops and defends a new interpretation of morality—namely, as a set of requirements that connect agents normatively to other persons in a nexus of moral relations. According to this relational interpretation, moral demands are directed to other individuals, who have claims that the agent comply with these demands. Interpersonal morality, so conceived, is the domain of what we owe to each other, insofar as we are each persons with equal moral standing. The book offers an interpretative argument (...) for the relational approach. Specifically, it highlights neglected advantages of this way of understanding the moral domain; explores important theoretical and practical presuppositions of relational moral duties; and considers the normative implications of understanding morality in relational terms. The book features a novel defense of the relational approach to morality, which emphasizes the special significance that moral requirements have, both for agents who are deliberating about what to do and for those who stand to be affected by their actions. The book argues that relational moral requirements can be understood to link us to all individuals whose interests render them vulnerable to our agency, regardless of whether they stand in any prior relationship to us. It also offers fresh accounts of some of the moral phenomena that have seemed to resist treatment in relational terms, showing that the relational interpretation is a viable framework for understanding our specific moral obligations to other people. The importance of teleology in the 18 th century has mainly been studied from the point of view of a few specific authors. Leibniz is one of the most convinced advocates of the use of final causes in both physics and metaphysics. Despite its significance for the history of teleology, very few studies were in fact devoted to the influence of the Leibnizian doctrine during the 18 th century. However, Leibniz’s reception allows us not only to understand his own views (...) on teleology in comparison with those that 18 th century thinkers attributed to him, but also to feed the conceptual debate on the legitimate articulation of finality and efficiency in natural philosophy in that period. Consequently, studies on 18 th century physico-theology may help us to clarify the following questions: what is the difference between explanations concerning organic functions and in the architectonic use of the principle of optimum in physics? And how exactly do “the details of physics” illustrate the general harmony of the world? In order to clarify these kinds of questions, the present issue explores how Leibniz’s 18 th century readers integrated in their own philosophical doctrines multiple interpretative possibilities opened up by Leibniz’s texts. Why do so many people remain so passive in the face of today’s massive, looming economic, political, and ecological crises, such as climate change? Despite some notable rhetorical and regulatory examples, attempts to stem climate change have, as a rule, not come to frame the activities of most citizens. The inability to confront the imperative of social transformation today is a complex, manifold problem. At root, it has to do with fundamental systemic features of a global social system that we (...) all contribute to reproducing in our everyday lives. While these features do not preclude political engagement, innovation, and action, they do undermine the bases of movements towards truly systemic transformation. This article focuses on one such feature, reification, as a social-structural foundation of passivity that impedes the social innovations required to tackle the climate crisis. (shrink)

Ce recueil de textes propose de contribuer à l’histoire des grandes académies scientifiques européennes. Plutôt que de vouloir tenter d’en dégager une homogénéité discutable, les études réunies tentent, au contraire, de montrer comment la pratique académique trouve son unité dans un ensemble de querelles.

Appliquée à la lecture des ouvrages de Théophraste sur les plantes, la notion de genre facilite l’analyse d’un de ses deux critères classificatoires : la distinction entre « mâles » et « femelles ». Le maitre de la botanique grecque projette sur le monde végétal les représentations culturelles du masculin et du féminin en pays grec. Les plantes étant chargées de sens différents selon les cultures, cette constatation invite à tenir compte du genre des plantes dans l’étude de leur symbolique, (...) notamment dans la sphère du religieux. Les agronomes romains n’emploient pas eux les qualificatifs « mâles » et « femelles » pour décrire deux plantes s’unissant en vue de leur reproduction. Plusieurs critères entrent en jeu dans le choix de ces dénominations : la ressemblance entre un aspect de la plante et une partie de l’anatomie féminine ou masculine ; l’analogie entre une caractéristique de la plante et une attitude culturellement considérée comme féminine ou masculine. En somme, ce sont majoritairement des critères relevant du genrequi déterminent la catégorisation des végétaux en « mâles » et « femelles ». Mais ces catégories sont rarement essentialisées. Elles fonctionnent comme un couple polarisé, permettant de distinguer deux espèces similaires en leur reconnaissant des qualités relatives et non absolues. (shrink)

Il s’agit principalement pour eux de repositionner la cosmologie par rapport aux sciences de la nature. Au lieu de considérer la métaphysique comme étant au fondement des théories scientifiques, comme le supposent Descartes, Wolff et, d’une certaine manière, Kant, ou simplement d’interdire l’idée même d’une cosmologie, comme le stipulerait à la même époque d’Alembert, Maupertuis et (...) Euler, renversent l’ordre des disciplines pour donner aux sciences physiques une primauté sur la métaphysique. Ce repositionnement entraîne évidemment plusieurs questions : d’abord, de quelle manière les théories scientifiques peuvent-elles valider ou infirmer un principe métaphysique? Plus important encore, quel rôle incombe à la métaphysique de la nature, étant donné que les sciences possèdent non seulement une autonomie théorique, mais même une priorité sur les réflexions ontologiques. Il semblerait d’ailleurs que la métaphysique se réduise, pour ces deux philosophes, au seul domaine du cosmologique, en particulier aux questions qui concernent la force et le principe de moindre action, au statut ontologique de l’espace et du temps et aux déterminations de la matière que sont l’étendue, l’inertie, l’impénétrabilité et le mouvement. | : In this paper, I show that Maupertuis and Euler offer a contrasting conception of metaphysics of nature. It consists mainly for them in repositioning cosmology in relation to natural sciences. Instead of considering metaphysics to be at the foundation of scientific theories, as was assumed by Descartes, Wolff, and, in a certain way, Kant, or simply prohibiting the very idea of a cosmology, as d’Alembert would stipulate at the same period, Maupertuis and Euler invert the order of disciplines to give priority to physical sciences over metaphysics. This repositioning leads of course to several questions : first, how does scientific theories validate or deny metaphysical principles? Even more important, which role falls to metaphysics of nature given that sciences not only possess a theoretical autonomy, but also a priority over ontological reflections? Besides, it seems that metaphysics would be reduced, for these two philosophers, to the realm of cosmology alone, in particular to questions concerning force and the principle of least action, the ontological status of space and time, and the first constituents of matter, such as extension, inertia, impenetrability, and motion. Leibniz’s universal characteristic is a fundamental aspect of his theory of cognition. Without symbols or characters it would be difficult for the human mind to define several concepts and to achieve many demonstrations. In most disciplines, and particularly in mathematics, the mind must then focus on symbols and their combinatorial rules rather than on mental contents. For Leibniz, mental perception is most of the time too confused for attaining distinct notions and valid deductions. In this paper, I argue that the (...) functions of symbolization differ depending upon the kind of concepts that are replaced with characters. In my view, most commentators did not sufficiently underline the distinction between two main functions of formal substitution in Leibniz’s characteristic: either increasing our knowledge or simply structuring it. In the first case, we complete our knowledge because formal substitution makes sensible and imaginary concepts more distinct. In the second case, symbolization helps to organize contents that are already conceived of by reason. Thus the process of substitution is not always identically applicable, for symbols replace different types of concepts. (shrink)

Ethics and Decision Making in Counseling and Psychotherapy has a distinct and timely focus on counseling as a profession. Chapters address the mental health professions, values in counseling, decision making, ethical principles, ethical standards, technology, ethical climate, and office/administrative practices. The early chapters present a foundation for ethical practice of the profession and provides solid building blocks to the more advanced perspectives in later chapters. Chapters on specialty practice are lively and contemporary overviews of these practice areas in counseling that (...) reflect more recent trends to increased differentiation of these areas of practice. Une telle démarche, que l’organisation actuelle de la recherche a rendue utopique, a joué un rôle déterminant dans leur formation intellectuelle, leur affirmation de soi et, pour certaines d’entre elles, dans l’orientation de leurs travaux.

Dans le contexte de grande liberté qui caractérise l’Université des années 1970-1980, une douzaine d’enseignantes-chercheuses toulousaines venues de disciplines très différentes ont fondé le grief ettenté l’expérience (qu’elles jugent très heureuse!) d’une approche interdisciplinaire de l’histoire des femmes et du genre. Une telle démarche, que l’organisation actuelle de la recherche a rendue utopique, a joué un rôle déterminant dans leur formation intellectuelle, leur affirmation de soi et, pour certaines d’entre elles, dans l’orientation de leurs travaux.

I investigate the consequences of Nietzsche's perspectivism for notions of truth and objectivity, and show how the metaphor of visual perspective motivates an epistemology that avoids self-referential difficulties. Perspectivism's claim that every view is only one view, applied to itself, is often supposed to preclude the perspectivist's ability to offer reasons for her epistemology. Nietzsche's arguments for perspectivism depend on “internal reasons”, which have force not only in their own perspective, but also within the standards of alternative perspectives. Internal reasons (...) allow a perspectivist argument against dogmatism without presupposing aperspectival criteria for theory choice. Nietzsche also offers “internal” conceptions of truth and objectivity which reduce them to a matter of meeting our epistemic standards. This view has pluralistic implications, which conflict with common sense, but it is nevertheless consistent and plausible. Nietzsche's position is similar to Putnam's recent internalism, and this is due to their common Kantian heritage. (shrink)

In the Traité des systèmes, Condillac analyses a plurality of systematic metaphysical doctrines, specifically, those of Spinoza, Malebranche and Leibniz. Commentators have mainly interpreted this plurality as a way for Condillac to illustrate his criticisms and to raise doubts about the main contemporary systems while using the same criteria for interpretation. However, I note that his analytic tools differ according to which philosopher or philosophical system he analyses. This paper focuses on the criticism of Leibniz’s system, and aims to show (...) that Condillac operated from subsequent reconstructions. In order to raise criticisms against the Leibnizian system, Condillac gave it an architectonic structure that is borrowed mainly from Wolff. (shrink)

Il implique notamment que toute substance s’individualise par la totalité infinie de ses propriétés. Sur le plan épistémologique, ce principe pose toutefois problème: comment un entendement fini parvient-il à se représenter les individus, sachant que seul l’entendement de Dieu possède la capacité de connaître la série infinie des propriétés qui individualise la substance? L’auteur vise ici un double objectif : d’une part, démontrer (...) qu’il existe chez Leibniz une philosophie de la connaissance suffisamment distincte des autres disciplines, notamment de la logique et de la métaphysique. D’autre part, examiner l’impact de cette épistémologie sur la théorie de l’individuation. This paper discusses the problem of sensible qualities, an important, but underestimated topic in Leibniz's epistemology. In the first section, the confused character of sensible ideas is considered. Produced by the sensation alone, ideas of sensible qualities cannot be part of distinct descriptions of bodies. This is why Leibniz proposes to resolve sensible qualities by means of primary or mechanical qualities, a thesis which is analysed in the second section. Here, I discuss his conception of nominal definitions as distinct empirical (...) representations. The provisional and modifiable status of nominal definitions is then explained in the third section. Since nominal descriptions always contain sensible determinations, Leibniz claims that empirical knowledge is indefinitely changeable according to progress in sciences. In the final section, I address the criterion of coherence that enables us to approve of hypotheses that are based on both sensible and empirical properties. (shrink)

It is tempting to think that all of normativity, such as our reasons for action, what we ought to do, and the attitudes that it is fitting for us to have, derives from what is valuable. But value-first approaches to normativity have fallen out of favour as the virtues of reasons- and fittingness-first approaches to normativity have become clear. On these views, value is not explanatorily prior to reasons and fit; rather the value of things is understood in terms of (...) the pro-attitudes it is fitting to have, or that we have reasons to have, towards these things. The value-first accounts of reasons and fit that have been proposed have been direct accounts of reasons and fit. On these views, the fittingness of an attitude is explained in terms of the value of having it or the value of the object of that attitude. This chapter details and surveys these problems, explaining which are yet to be overcome, and explaining which problems direct value-first accounts can overcome. This chapter then explores the prospects of an indirect value-first account of reasons and fit. It argues that it avoids the problems that direct value-first accounts face and may be as explanatorily powerful as fittingness- and reasons-first accounts of value and normativity more generally. Is the essay literature or philosophy? A form of art or a form of knowledge? The contemporary essay is torn between its belletrist ancestry and its claim to philosophical legitimacy. The Spanish philosopher Eduardo Nicol captured the genre's uncertain status when he dubbed it “almost literature and almost philosophy” (Nicol 1961:207). The problem is hardly a new one. It goes back to what Plato called the “ancient quarrel” between poetry and philosophy, and more recently to the German Romantic theorist, Friedrich (...) Schlegel, who called for a mode of criticism which would be at once philosophical and poetic. But today, when the status of critical discourse is up for grabs, reflecting the crisis of knowledge in the universities, the question of the essay takes on a new urgency. Now the predominant form of writing in the human sciences, it cannot avoid the challenge to define itself according to the prevailing standards of scientific knowledge and method. (shrink)
