The modern ethical framework demands informed consent for research participation that includes disclosure of material information, as well as alternatives. The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic (COVID-19) results in illness that often involves rapid deterioration. Despite the urgent need to find therapy, obtaining informed consent for COVID-19 research is needed. The current pandemic presents three types of challenges for investigators faced with obtaining informed consent for research participation: (1) uncertainty over key information to informed consent, (2) time (...) and pressure constraints, and (3) obligations regarding disclosure of new alternative therapies and re-consent. To mitigate consenting challenges, primary investigators need to work together to jointly promote urgent care and research into COVID-19. Actions they can take include (1) prior plan addressing ways to incorporate clinical research into clinical practice in emergency, (2) consider patients vulnerable with early deliberation on the consent process, (3) seek Legally Authorized Representatives (LARs), (4) create a collaborative research teams, (5) aim to consent once, despite evolving information during the pandemic, and (6) aim to match patients to a trial that will most benefit them. The COVID-19 pandemic both exacerbates existing challenges and raises unique obstacles for consent that require forethought and mindfulness to overcome. While research teams and clinician-investigators will need to be sensitive to their own contexts and adapt solutions accordingly, they can meet the challenge of obtaining genuinely informed consent during the current pandemic. Recent EEG studies on the early postmortem interval that suggest the persistence of electrophysiological coherence and connectivity in the brain of animals and humans reinforce the need for further investigation of the relationship between the brain’s activity and the dying process. Neuroscience is now in a position to empirically evaluate the extended process of dying and, more specifically, to investigate the possibility of brain activity following the cessation of cardiac and respiratory function. Under the direction of the Center for Healthy (...) Minds at the University of Wisconsin-Madison, research was conducted in India on a postmortem meditative state cultivated by some Tibetan Buddhist practitioners in which decomposition is putatively delayed. For all healthy baseline and postmortem subjects presented here, we collected resting state electroencephalographic data, mismatch negativity, and auditory brainstem response. In this study, we present HB data to demonstrate the feasibility of a sparse electrode EEG configuration to capture well-defined ERP waveforms from living subjects under very challenging field conditions. While living subjects displayed well-defined MMN and ABR responses, no recognizable EEG waveforms were discernable in any of the tukdam cases. (shrink)

This chapter describes the potential far‐reaching consequences of contemplative higher education for the fields of science and medicine.

The authors of the numerous medieval and early modern Sanskrit-medium commentaries on the various recensions and sub-recensions of the Vālmīkirāmāyaṇa frequently found themselves in a somewhat awkward hermeneutical position. The epic itself, like many Indic texts, is highly revered both as a religious text, one of the earliest and most influential Vaiṣṇava texts, and as a literary work that is not only a great poem but indeed the very first poem and the fons et origo of all subsequent poetry. Moreover, (...) like Vyāsa, the author of the Rāmāyaṇa’s sister epic, the Mahābhārata, Vālmīki was regarded not merely as an inspired poet and sage, but as a ṛṣi, that is to say an inerrant seer whose speech, in his case inspired directly by the creator divinity Brahmā and the god’s gift of a divine vision, must therefore be accepted as absolutely true and authoritative. The problem facing the work’s commentators is that Vālmīki’s text portrays its hero, Rāma, as not only a god in the form of a man, but as one who, ignorant of his own divinity, suffers all of the mental, emotional, and physical pain to which ordinary mortals are prey. In this Rāma differs sharply from the subsequent Vaiṣṇava avatāra and central figure of the Mahābhārata, Kṛṣṇa, who, fully aware of his godhood, rarely suffers in any way mentally or physically. But the Rāmāyaṇa’s commentators are living and writing in a world in which the development of the medieval bhakti movements has led poets and theologians to conceive of and write about Rāma as an omnipotent and omniscient figure very much like Kṛṣṇa. The present essay discusses the lexical, grammatical, and hermaneutical strategies the commentors adopted to negotiate the tension between Vālmīki’s apparent depiction of the suffering of his hero and a proposed deeper meaning in which the avatāra conforms more fully to what became the medieval and modern theology of god on earth. Gödel’s Ontological argument is distinctive because it is the most sophisticated and formal of ontological arguments and relies heavily on the notion of _positive property_. Gödel uses a third-order modal logic with a property abstraction operator and property quantification into modal contexts. Gödel describes _positive property_ as "independent of the accidental structure of the world"; "pure attribution," as opposed to privation; "positive in the 'moral aesthetic sense.'" _Pure attribution_ seems likely to be related to the Leibnizian concept of perfection. By (...) a careful examination of the formal semantics of third-order modal logic with property abstraction together with a Completeness result for third-order modal logic with property abstraction for faithful models that I previously developed in 2000 in my work, _Gödel’s Ontological Argument_, I argue that it is not possible to develop a sufficient applied third-order modal semantics for Gödel’s ontological argument. As I explore possible approaches for an applied semantics including anti-Realist accounts of the semantics of modal logic compatible with Actualism, I argue that Gödel makes implicit philosophical assumptions which commit him to both possibilism and modal realism. Beyond the empressionistic similarities Chinese and Jews sometimes detect in each other, a study of the scriptural texts of the two traditions reveals surprising parallels. Eleven such parallels in ideas and values are examined and it is suggested that the comparative study of cultural traditions developed independently of each other is a worthwhile undertaking. This serves to apprehend that which is genuinely universal beyond the diversity of expression.

The marketing of health care services raises the prospect that an ethic of strangers will govern relations between providers and patients. A fiduciary model that emphasizes honesty and public accountability, as well as the patient's good and avoiding unnecessary services, can keep marketing consistent with the ethical tradition of medicine.

Graphical AbstractIn article 2000078, Alison Patteson et al. review the various functions of filamentous and non-filamentous vimentin. While the mechanical and intracellular roles of vimentin – such as providing protection of the nucleus and elasticity to the cytoskeleton – are well documented, recent studies point towards additional, non-mechanical functions for vimentin. These functions include signaling roles in wound healing, lipogenesis and sterol processing as well as a role in pathogen infection.

Intermediate filaments (IFs) formed by vimentin are less understood than their cytoskeletal partners, microtubules and F‐actin, but the unique physical properties of IFs, especially their resistance to large deformations, initially suggest a mechanical function. Indeed, vimentin IFs help regulate cell mechanics and contractility, and in crowded 3D environments they protect the nucleus during cell migration. Recently, a multitude of studies, often using genetic or proteomic screenings show that vimentin has many non‐mechanical functions within and outside of cells. These include signaling (...) roles in wound healing, lipogenesis, sterol processing, and various functions related to extracellular and cell surface vimentin. Extracellular vimentin is implicated in marking circulating tumor cells, promoting neural repair, and mediating the invasion of host cells by viruses, including SARS‐CoV, or bacteria such as Listeria and Streptococcus.
