Shortages of new therapeutics to treat coronavirus disease (COVID-19) have forced clinicians, public health officials, and health systems to grapple with difficult questions about how to fairly allocate potentially life-saving treatments when there are not enough for all patients in need (1). Shortages have occurred with remdesivir, tocilizumab, monoclonal antibodies, and the oral antiviral Paxlovid (2) -/- Ensuring equitable allocation is especially important in light of the disproportionate burden experienced during the COVID-19 pandemic by disadvantaged groups, including Black, Hispanic/Latino and (...) Indigenous communities, individuals with certain disabilities, and low-income persons. However, many health systems have resorted to first-come, first-served approaches to allocation, which tend to disadvantage individuals with barriers in access to care (3). There is mounting evidence of racial, ethnic, and socioeconomic disparities in access to medications for COVID-19 (4, 5). -/- One potential method to promote equitable allocation is to use a weighted lottery, which is an allocation strategy that gives all eligible patients a chance to receive the scarce treatment while also allowing the assignment of higher or lower chances according to other ethical considerations (6). We sought to assess the feasibility of implementing a weighted lottery to allocate scarce COVID-19 medications in a large U.S. health system and to determine whether the weighted lottery promotes equitable allocation. Few figures of the Middle Ages command the attention of so many modern disciplines as Robert Grosseteste (c. 1170-1253). Theology, Philosophy, History, and Science are all areas which his life and thought continue to have significance and to inspire re-interpretation. Accompanied by a series of original commentaries, this new edition of Grosseteste's work, with English translation, draws together the perspectives of modern scientists and medieval specialists. Volume I of a six volume series, Knowing and Speaking presents two of the earliest (...) of Grosseteste's works, the treatises On the Liberal Arts and On the Generation of Sounds. These are accompanied by a significant Middle English treatise on the Seven Liberal Arts whose anonymous fifteenth-century author translated and excerpted passages from Grosseteste's treatises in a re-imagining of their structure and function.0Each work is treated separately within the volume, which is constructed in three parts. On the Liberal Arts sets Grosseteste's thoughts on the arts subjects and emphasises moral concerns about the purpose of learning. On the Generation of Sounds builds on the theories and statements of On the Liberal Arts in connection to the production of sound, elaborating the earlier position, relating the generation of sounds to human vocal and speech production. White has an incredibly sensitive ear for distinctions among ordinary concepts in the philosophy of mind. As a botanizer of terms and their uses, he effectively rivals Ryle, upon whose general approach to the philosophy of mind he builds. Thus he has excellent chapters in which he distinguishes such related conceptions as care, interest, attention, notice, and consciousness. In another chapter he distinguishes five irreducible uses of "think," while in the next chapter he comes up with five distinct notions (...) of feeling, the fifth of which has four subdivisions. The chapter on historical theories of mind is also neatly put together, and White takes this opportunity to identify himself as a somewhat modified Aristotelian. The ontological point of this claim is never made clear, however, as White prefers to remain in the linguistic heaven in which he can safely perform his "conceptual analysis" without descending to the level of nitty-gritty problems like ontic commitment. In Chapter 5 of his book, Res Cogitans, Zeno Vendler argued for the thesis that what we know when we know that p, e.g. that Los Angeles is south of San Francisco, and what we believe when we believe that p cannot be the same despite being expressed in the same words, on the ground that ‘believe’ is what he called a subjective verb and ‘know’ what he called an objective verb. For this he gave two main criteria that subjective (...) verbs can take only ‘subjective that- clauses,’ by which he means clauses which express propositions, whereas objective verbs can take only ‘objective that-clauses,’ by which he means clauses which express facts; that subjective verbs cannot take, whereas objective verbs can take, wh- nominalisations - in practice these are the interrogatives what, who where, when, how - which Vendler also takes as a mark of the objective. Previous research on academic dishonesty has consistently used self-report measures to establish prevalence of dishonest behavior. In this study we use actual plagiarism behavior to establish its prevalence, as well as relationships between integrity-related personal selection and workplace deviance measures. This research covers new ground in two respects: (a) That (...) the academic dishonesty literature is subject to revision using criterion variables to avoid self bias and social desirability issues and (b) we establish the relationship between actual academic dishonesty and potential workplace deviance/white-collar crime. If stem cell-based therapies are developed, we will likely confront a difficult problem of justice: for biological reasons alone, the new therapies might benefit only a limited range of patients. In fact, they might benefit primarily white Americans, thereby exacerbating long-standing differences in health and health care.

Purpose: To show that the idea of non-dualistic thinking is of great value for some of the core problems of media philosophy (which often lacks the radical approach of Josef Mitterer's concept). Method: Non-dualistic philosophy, introduced by Mitterer, has a lot in common with other thinkers' discontent with the traditional way of describing the subject-object relation. Their differences and the impasses of phenomenological, structuralist and psychoanalytic media theory shall be examined to show whether and to what extent non-dualism could do (...) better. Findings: As sociologists and some media-philosophers are already adopting some principles of non-dualism without a real framework, non-dualistic thinking offers a new insight into the way that consent/dissent is manufactured in a world where most descriptions are given in a mediated (i.e., non-interpersonal) form. Implications: If we re-establish the personal freedom of gathering knowledge individually instead of "getting nearer to the truth," the way of looking at the escapist power of media also has to be reinterpreted. The early distinction between Lumiére-like "documentation" (= truth) and Méliès-like "fiction" (= perception open to alternatives) could be another dualism, which should be expurgated by the application of Mitterer's concepts. Recent legislation, such as the Patient Self-Determination Act, establishes advance directives as an acceptable procedural means of incorporating patients’ preferences for life-sustaining treatments into their medical care. Advance directives can enhance medical decision making since they provide patients with an opportunity to communicate their preferences before suffering from an acute illness that may preclude their ability to do so.Although patients expect discussions about life-sustaining therapies to be initiated by their physicians, very little is known about what prompts physicians to discuss (...) advance directives with their patients. As in other areas of clinical decision making, there is evidence that patients’ sociodemographic factors influence whether discussions about advance directives occur between patients and their physicians. In one study of persons with the acquired immunodeficiency syndrome, those who had not had discussions about advance directives with their physicians tended to be non-white, have no prior hospitalization, and were more likely to have been cared for in a health maintenance organization than in a teaching hospital. Many clinical and psychological constructs are conceptualized to have multivariate higher-order constructs that give rise to multidimensional lower-order traits. Although recent measurement models and computing algorithms can accommodate item response data with a higher-order structure, there are few measurement models and computing techniques that can be employed in the context of complex research synthesis, such as meta-analysis of individual participant data or integrative data analysis. The current study was aimed at modeling complex item responses that can arise when underlying domain-specific, (...) lower-order traits are hierarchically related to multiple higher-order traits for individual participant data from multiple studies. We formulated a multi-group, multivariate higher-order item response theory (HO-IRT) model from a Bayesian perspective and developed a new Markov chain Monte Carlo (MCMC) algorithm to simultaneously estimate the (a) structural parameters of the first- and second-order latent traits across multiple groups and (b) item parameters of the model. Results from a simulation study support the feasibility of the MCMC algorithm. From the analysis of real data, we found that a bivariate HO-IRT model with different correlation/covariance structures for different studies fit the data best, compared to a univariate HO-IRT model or other alternate models with unreasonable assumptions (i.e., the same means and covariances across studies). Although more work is needed to further develop the method and to disseminate it, the multi-group multivariate HO-IRT model holds promise to derive a common metric for individual participant data from multiple studies in research synthesis studies for robust inference and for new discoveries.
