Syndromic surveillance uses new ways of gathering data to identify possible disease outbreaks. Because syndromic surveillance can be implemented to detect patterns before diseases are even identified, it poses novel problems for informed consent, patient privacy and confidentiality, and risks of stigmatization. This paper analyzes these ethical issues from the viewpoint of the patient as victim and vector. It concludes by pointing out that the new International Health Regulations fail to take full account of the ethical challenges raised by syndromic (...) surveillance. (shrink)

The main focus of Lean philosophy is to create value for the stakeholders and end customers by reducing the non-value added activities or waste, and laying a strong foundation towards sustainable growth, by focusing on continuous process improvement. The overall philosophy provides a focused approach for continuous process improvement and the targeting of a variety of tools and methods to bring improvements. This paper presents case studies for Lean scheduling of manufacturing processes and exemplify how Lean scheduling processes allow organisations to maintain the continuous improvement philosophy. This paper also addresses the key elements of rescheduling processes i.e. how to schedule and when to schedule. Making research data readily accessible during a public health emergency can have profound effects on our response capabilities. The moral milieu of this data sharing has not yet been adequately explored. This article explores the foundation and nature of a duty, if any, that researchers have to share data, specifically in the context of public health emergencies. There are three notable reasons that stand in opposition to a duty to share one’s data, relating to: (i) data property and ownership, (ii) (...) just distribution of benefits and burdens and (iii) the contemporary ethos of science. We argue each reason can be successfully met with corresponding rationale in favour of data sharing. Further support for data sharing has been echoed in policies of health agencies, funding bodies and academic institutions; in documents on the ethical conduct of biomedical research; and in discussions on the nature of public health. From this, we ascertain that sharing data is the morally sound default position. This article then highlights the key roles reciprocity and solidarity play in supporting the practice of data sharing. We conclude with recommendations to regard public health research data as a common-pool resource in order to build a framework for stable data sharing management. (shrink)

This paper proposes a generalization of the Kripke semantics of intuitionistic logic IL appropriate for intuitionistic Łukasiewicz logic IŁL — a logic in the intersection between IL and (classical) Łukasiewicz logic. This generalised Kripke semantics is based on the poset sum construction, used in Bova and Montagna (Theoret Comput Sci 410(12):1143–1158, 2009) to show the decidability (and PSPACE completeness) of the quasiequational theory of commutative, integral and bounded GBL algebras. The main idea is that w \Vdash \sigma—which for IL is (...) a relation between worlds w and formulas \sigma, and can be seen as a function taking values in the booleans w \Vdash \sigma \in {B}—becomes a function taking values in the unit interval w \Vdash \sigma \in [0,1]. An appropriate monotonicity restriction (which we call sloping functions) needs to be put on such functions in order to ensure soundness and completeness of the semantics. (shrink)

Michael Hornsby-Smith offers an overview of Catholic social thought particularly in recent decades. While drawing on official teaching such as papal encyclicals and the pastoral letters of bishops' conferences, he takes seriously the need for dialogue with secular thought. The 2006 book is organized in four stages. Part I outlines the variety of domestic and international injustices and seeks to offer a social analysis of the causes of these injustices. Part II offers a theological reflection on the characteristics of (...) the kingdom of God which Christians are urged to seek. Part III reviews Catholic social thought in six main areas: human rights, the family and bioethical issues, economic life, social exclusion, authentic development, and war and peace. Mistakes and errors happen in most spheres of human life and activity, including in medicine. A mistake can be as simple and benign as the collection of an extra and unnecessary urine sample. Or a mistake can cause serious but reversible harm, such as an overdose of insulin in a patient with diabetes, resulting in hypoglycemia, seizures, and coma. Or a mistake can result in serious and permanent damage for the patient, such as the failure to consider epiglottitis in an (...) initial differential diagnosis, resulting in a chronic vegetative state for a seven-year-old boy. The constructionist thesis of history states, in general, that the historian must construct a theory to explain the past. Some, including Leon Goldstein, attempt to push this formulation beyond a description of historical methodology. They argue that since the real past is inaccessible to present observation, the real past can have no relevance for historiography. The distinctions made between the present, the real past, and the historical past generate problems with the concepts of past and present knowledge, theoretical infrastructure and (...) experience, verification and truth, conflicting historical theories, and observation and knowledge. Goldstein's formulation of the constructionist thesis assumes the conflicting positions that experiential perception is paradigmatic of all methods of acquiring knowledge, and that knowledge is itself a kind of experience. As well as conflicting with commonsense views, his thesis is internally incoherent. (shrink)

Utilitarianism claims to be a rational moral theory in at least three ways. First, it claims to give us an objective standard of morality, a way of deciding moral issues, not in the light of what each of us happens to like or dislike, but on publicly verifiable grounds. Secondly, by offering only one criterion of morality it assures consistency. If we accept a system which invokes two or more independent principles, there is always the possibility of insoluble conflict. For (...) example, if we take our stand on the Ten Commandments, we cannot, without divine revelation, be sure that we shall not one day find ourselves in a situation in which we must break one or other of them; and our system gives us no guide as to which we ought to break. In a recent paper in this journal, Dagsvik derives the class of independent random utility representations that are “equivalent” to the independence-from-irrelevant-alternatives assumption by Luce. In this short note, we clarify the relations between this paper by Dagsvik, and a paper in Lindberg’s 2012 thesis.

This paper draws on a wide range of researches to stress the importance of social context to the sociological understanding of religious experiences. It argues that individualistic definitions fail to take into account real group experiences such as those resulting from the reforms of Vatican II. For the sociologist, it is important to explore general patterns of group experiences and the meanings attributed to them. The paper discusses some of the methodological and conceptual problems in this area before considering evidence (...) for the patterning of religious experience according to differences of generation, gender, class, level of urbanization, institutional involvement, and status inconsistency. The paper concludes by locating religious experiences in the context of modernity. In contrast to related theories of secularization, it draws attention to the recent work of Hervieu‐Léger which suggests that utopian future expectations create space which can only be met by new forms of religious experience. The Struldbrugs (...) were allowed to be born totally exempt from the “calamity of human Nature,” in that their minds were free “and disingaged, without the Weight and De pression of Spirits caused by the continued Apprehension of Death.” They were thus condemned “to a perpetual continuance in the World.” In his travels, Gulliver found some Struldbrugs well over 1,000 years old. Not everyone finds a in suffering. Indeed, even those who do subscribe to this interpretation recognize the responsibility of each individual to show not only sensitivity and compassion but render assistance to those in distress. Pharmacologic hypnosis, morphine intoxication, and terminal sedation provide their own type of medical to the terminally ill patient suffering unremitting pain. More and more states are enacting legislation that recognizes this need of the dying to receive relief through regulated administration of controlled substances. Wider legislative (...) recognition of this need would go far toward allowing physicians, in the exercise of their reasonable medical judgment, to administer a range of narcotics and barbiturates to the terminally ill without fear of legal sanctions. Sadly, social attitudes and governmental concerns about the spread of drug addiction provide an undeniable policy nexus that impedes unduly a rational approach or exception for the treatment of pain experienced by the dying. The development of culture-independent strategies to study microbial diversity and function has led to a revolution in microbial ecology, enabling us to address fundamental questions about the distribution of microbes and their influence on Earth’s biogeochemical cycles. This article discusses some of the progress that scientists have made with the use of so-called “omic” techniques (metagenomics, metatranscriptomics, and metaproteomics) and the limitations and major challenges these approaches are currently facing. These ‘omic methods have been used to describe the taxonomic structure (...) of microbial communities in different environments and to discover new genes and enzymes of industrial and medical interest. However, microbial community structure varies in different spatial and temporal scales and none of the ‘omic techniques are individually able to elucidate the complex aspects of microbial communities and ecosystems. In this article we highlight the importance of a spatiotemporal sampling design, together with a multilevel ‘omic approach and a community analysis strategy (association networks and modeling) to examine and predict interacting microbial communities and their impact on the environment.
