BackgroundAmidst expanding roles in education and policy making, questions have been raised about the ability of Clinical Ethics Committees s to carry out effective ethics consultations. However recent reviews of CECs suggest that there is no uniformity to CECons and no effective means of assessing the quality of CECons. To address this gap a systematic scoping review of prevailing tools used to assess CECons was performed to foreground and guide the design of a tool to evaluate the quality of CECons.MethodsGuided (...) by Levac et al’s methodological framework for conducting scoping reviews, the research team performed independent literature reviews of accounts of assessments of CECons published in six databases. The included articles were independently analyzed using content and thematic analysis to enhance the validity of the findings.ResultsNine thousand sixty-six abstracts were identified, 617 full-text articles were reviewed, 104 articles were analyzed and four themes were identified – the purpose of the CECons evaluation, the various domains assessed, the methods of assessment used and the long-term impact of these evaluations.ConclusionThis review found prevailing assessments of CECons to be piecemeal due to variable goals, contextual factors and practical limitations. The diversity in domains assessed and tools used foregrounds the lack of minimum standards upheld to ensure baseline efficacy.To advance a contextually appropriate, culturally sensitive, program specific assessment tool to assess CECons, clear structural and competency guidelines must be established in the curation of CECons programs, to evaluate their true efficacy and maintain clinical, legal and ethical standards. IntroductionClinical ethics committees support and enhance communication and complex decision making, educate healthcare professionals and the public on ethical matters and maintain standards of care. However, a consistent approach to training members of CECs is lacking. A systematic scoping review was conducted to evaluate prevailing CEC training curricula to guide the design of an evidence-based approach.MethodsArksey and O’Malley’s methodological framework for conducting scoping reviews was used to evaluate prevailing accounts of CEC training published in six databases. Braun and Clarke’s thematic (...) analysis approach was adopted to thematically analyse data across different healthcare and educational settings.Results7370 abstracts were identified, 92 full-text articles were reviewed and 55 articles were thematically analysed to reveal four themes: the design, pedagogy, content and assessment of CEC curricula.ConclusionFew curricula employ consistent approaches to training. Many programmes fail to provide CEC trainees with sufficient knowledge, skills and experience to meet required competencies. Most programmes do not inculcate prevailing sociocultural, research, clinical and educational considerations into training processes nor provide longitudinal support for CEC trainees. Most CEC training programmes are not supported by host institutions threatening the sustainability of the programme and compromising effective assessment and longitudinal support of CEC trainees. While further reviews are required, this review underlines the need for host organisations to support and oversee a socioculturally appropriate ethically sensitive, clinically relevant longitudinal training, assessment and support process for CEC trainees if CECs are to meet their roles effectively. In any society influenced by a plurality of cultures, there will be widespread, systematic differences about at least some important values, including moral values. Many of these differences look like deep disagreements, difficult to resolve objectively if that is possible at all. One common response to the suspicion that these disagreements are unsettleable has always been moral relativism. In the flurry of sympathetic treatments of this doctrine in the last two decades, attention has understandably focused on the simpler case in (...) which one fairly self-contained and culturally homogeneous society confronts, at least in thought, the values of another; but most have taken relativism to have implications within a single pluralistic society as well. I am not among the sympathizers. That is partly because I am more optimistic than many about how many moral disagreements can be settled, but I shall say little about that here. For, even on the assumption that many disputes are unsettleable, I continue to find relativism a theoretically puzzling reaction to the problem of moral disagreement, and a troubling one in practice, especially when the practice involves regular interaction among those who disagree. David Owen wants to understand what Hume means by reason, given its pivotal importance in the wide range of issues that Hume discusses in his philosophical works. In order to achieve that understanding, Owen places Hume in the historical context of writers such as Descartes and Locke, what was later referred to as the way of ideas. Owen objects to stating Humes views in terms of contemporary semantic frameworks. After a careful review of the many contexts in which Hume discusses (...) reason in book 1 of the Treatise, Owen concludes that Hume rejected the notion of reason as an independent faculty and offered instead an account of reason as based on the imagination. Owens scholarship is meticulous and his attempt to understand Humes historical context commendable. The difficulties are that he tells us nothing new and his context is much too narrow. Hume was neither a scholastic nor a contemporary professional responding only to the works of other professional philosophers on some technical point. At the very least, Humes discussion of causal reasoning was very much influenced by the transition from Aristotelian physics and its attendant conception of causality to Newtons physics and its attendant conception of causality. This has enormously increased our power to act which has, in turn, brought us both all the great benefits of the modern world and the crises we now face. Modern science and technology have made possible modern industry and agriculture, the explosive growth of the world’s population, global (...) warming, modern armaments and the lethal character of modern warfare, destruction of natural habitats and rapid extinction of species, immense inequalities of wealth and power across the globe, pollution of earth, sea and air, even the aids epidemic (aids being spread by modern travel). All these global problems have arisen because some of us have acquired unprecedented powers to act, via science and technology, without also acquiring the capacity to act wisely. (shrink)
Nicholas Wolterstorff discusses the ethics of belief which Locke developed in Book IV of his Essay Concerning Human Understanding, where Locke finally argued his overarching aim: how we ought to govern our belief, especially on matters of religion and morality. Wolterstorff shows that this concern was instigated by the collapse, in Locke's day, of a once-unified moral and religious tradition in Europe into warring factions. His was thus a culturally and socially engaged epistemology. This view of Locke invites a (...) new interpretation of the origins of modern philosophy. He maintained that instead of following tradition we ought to let 'reason be our guide.' Accordingly, after discussing Hume's powerful attack on Locke's recommended practice, Wolterstorff argues for Locke's originality and emphasizes his contribution to the 'modernity' of post-sixteenth-century philosophy. In this paper I want to discuss David Hume's views about morals, politics and citizenship and the role of philosophers and philosophizing in modern civil society - what I shall call his theory of civic morality. This is a subject which has been neglected by philosophers, presumably because it is of limited philosophical interest. But it is of considerable interest to the historian who wants to understand Hume's development as a philosopher, to locate his thought within a specific, Scottish context (...) and to arrive at some understanding of his surprisingly close and cordial relations with the literary and social world of enlightened Edinburgh. These are large claims and I cannot hope to substantiate them fully in a short paper. My purpose is first, to show that, historically speaking, Hume's preoccupation with civic morality was of central rather than peripheral interest to him as a philosopher and that it helps to explain his otherwise rather puzzling decision to give up philosophizing systematically in the manner of Hobbes and Locke, in favour of polite essay-writing in the manner of Addison and Steele. My second purpose is to suggest that Hume's interest in civic morality, his neo-Addisonian mode of philosophizing about it and the nature of his understanding of politics, citizenship and philosophizing in a modern age was, unlike his thought about religion, responsive to and consonant with some of the most important ideological preoccupations of his Scottish contemporaries. It was, I suspect, a shared interest which helped to contain some of the anxieties Hume's notorious religious scepticism caused his contemporaries. Without it, he could not possibly have emerged as one of the leaders of Edinburgh's intellectual life in the age of the Scottish Enlightenment. (shrink)
A major part of the mind–body problem is to explain why a given set of physical processes should give rise to perceptual qualities of one sort rather than another. Colour hues are the usual example considered here, and there is a lively debate as to whether the results of colour vision science can provide convincing explanations of why colours actually look the way they do. The internal phenomenological structure of colours is considered here in some detail, and a comparison is (...) drawn with sounds and their synthesis. This paper examines the type of explanation that is needed, and it is concluded that it does not have to be reductive to be effective. What needs to be explained more than anything is why inverted hue scenarios are more intuitive than other sensory inversions: and the issue of physicalism versus dualism is argued to be of only marginal relevance. Postmodernism and poststructuralism challenge fundamental positions in social theory. This book sets out some of the components of a postmodern social theory of health and healing, deriving from theorists including Derrida, Deleuze and Guattari, Foucault, Cixous and Kristeva. Nicholas J. Fox observes that the knowledge of the medical profession about the body, illness and health supplies the basis for medical dominance. The body of the patient is inscribed by discourses of professional `care,' an interaction which subjectifies the patient. Deleuze and Guattari's radical challenge to psychoanalysis and familial repetitions within the healer/patient contact allows a re-reading of central ideas in medical sociology. While focusing upon the possibilities of postmodern social theory, the book demands a reappraisal of issues of structure, identity and knowledge in modernist medical sociology. Modernist sociology, Fox suggests, has been complicit in the creation of `the patient,' and of 'health' and 'illness.' Why should I be just? What have I to gain if I am decent, honest, moral, upright, fair and truthful? Other people benefit if I am just, but do I? And doesn't it seem clear that sometimes the benefit that other people receive from my being just is a benefit received at my expense? Perhaps then I have no adequate reason to be just. Perhaps if I have any sense I will not bother.
This chapter outlines a novel solution to the problem of the many, according to which objects can be simultaneously constituted by many collections of particles. To support this proposal, it develops a conception of objects that implies it. On this view, objects are fundamentally subjects of change: the changes an object can survive are explanatorily prior to its constitution. Short-term traffic flow prediction is an effective means for intelligent transportation system to mitigate traffic congestion. However, traffic flow data with temporal features and periodic characteristics are vulnerable to weather effects, making short-term traffic flow prediction a challenging issue. However, the existing models do not consider the influence of weather changes on traffic flow, leading to poor performance under some extreme conditions. In view of the rich features of traffic data and the characteristic of being vulnerable to external weather conditions, (...) the prediction model based on traffic data has certain limitations, so it is necessary to conduct research studies on traffic flow prediction driven by both the traffic data and weather data. This paper proposes a combined framework of stacked autoencoder and radial basis function neural network to predict traffic flow, which can effectively capture the temporal correlation and periodicity of traffic flow data and disturbance of weather factors. Firstly, SAE is used to process the traffic flow data in multiple time slices to acquire a preliminary prediction. Then, RBF is used to capture the relation between weather disturbance and periodicity of traffic flow so as to gain another prediction. Finally, another RBF is used for the fusion of the above two predictions on decision level, obtaining a reconstructed prediction with higher accuracy. The effectiveness and robustness of the proposed model are verified by experiments. (shrink)
In VAGUENESS AND DEGREES OF TRUTH, Nicholas Smith develops a new theory of vagueness: fuzzy plurivaluationism. -/- A predicate is said to be VAGUE if there is no sharply defined boundary between the things to which it applies and the things to which it does not apply. For example, 'heavy' is vague in a way that 'weighs over 20 kilograms' is not. A great many predicates -- both in everyday talk, and in a wide array of theoretical vocabularies, from (...) law to psychology to engineering -- are vague. -/- Smith argues, based on a detailed account of the defining features of vagueness, that an accurate theory of vagueness must involve the idea that truth comes in degrees. The core idea of degrees of truth is that while some sentences are true and some are false, others possess intermediate truth values: they are truer than the false sentences, but not as true as the true ones. Degree-theoretic treatments of vagueness have been proposed in the past, but all have encountered significant objections. In light of these, Smith develops a new type of degree theory. Its innovations include a definition of logical consequence that allows the derivation of a classical consequence relation from the degree-theoretic semantics, a unified account of degrees of belief and their relationships with degrees of truth and subjective probabilities, and the incorporation of semantic indeterminacy -- the view that vague statements need not have unique meanings -- into the degree-theoretic framework. -/- As well as being essential reading for those working on vagueness, Smith's book provides an excellent entry-point for newcomers to the area -- both from elsewhere in philosophy, and from computer science, logic and engineering. Our world is enormously sophisticated and nature's complexity is literally inexhaustible. As a result, projects to describe and explain natural science can never be completed. This volume explores the nature of complexity and considers its bearing on our world and how we manage our affairs within it. Rescher's overall lesson is that the management of our affairs within a socially, technologically, and cognitively complex environment is plagued with vast management problems and risks of mishap. In primitive societies, failure to understand (...) how things work can endanger a family or, at worst, a clan or tribe. In the modern world, man-made catastrophes on the model of Chernobyl can endanger millions, possibly even risking the totality of human life on our planet. Rescher explains "technological escalation" as a sort of arms race against nature in which scientific progress requires more powerful technology for observation and experimentation, and, conversely, scientific progress requires the continual enhancement of technology. The increasing complexity of science and technology along with problems growing faster than solutions confront us with major management and decision problems. This study is the first of its kind. There have been many specialized studies of complexity in physics and computation theory, but no overall analysis of the phenomenon. Although Rescher offers a sobering outlook, he also believes that complexity entails mixed blessings: our imperfect knowledge provides a rationale for putting forth our best efforts. Rescher urges us to gear the conduct of life's practical affairs to the demands of a complex world. This highly readable and accessible volume will be of interest to those interested in philosophy, the philosophy of science, science policy studies, and future studies. (shrink)
