An individual's health, genetic, or environmental-exposure data, placed in an online repository, creates a valuable shared resource that can accelerate biomedical research and even open opportunities for crowd-sourcing discoveries by members of the public. But these data become “immortalized” in ways that may create lasting risk as well as benefit. Once shared on the Internet, the data are difficult or impossible to redact, and identities may be revealed by a process called data linkage, in which online data sets are matched (...) to each other. Reidentification, the process of associating an individual's name with data that were considered deidentified, poses risks such as insurance or employment discrimination, social stigma, and breach of the promises often made in informed-consent documents. At the same time, re-ID poses risks to researchers and indeed to the future of science, should re-ID end up undermining the trust and participation of potential research participants. The ethical challenges of online data sharing are heightened as so-called big data becomes an increasingly important research tool and driver of new research structures. Big data is shifting research to include large numbers of researchers and institutions as well as large numbers of participants providing diverse types of data, so the participants’ consent relationship is no longer with a person or even a research institution. In addition, consent is further transformed because big data analysis often begins with descriptive inquiry and generation of a hypothesis, and the research questions cannot be clearly defined at the outset and may be unforeseeable over the long term. In this article, we consider how expanded data sharing poses new challenges, illustrated by genomics and the transition to new models of consent. We draw on the experiences of participants in an open data platform—the Personal Genome Project—to allow study participants to contribute their voices to inform ethical consent practices and protocol reviews for big-data research. In this article, the authors bring two personal journeys together: one author’s liberationist journey, sparked by a search for justice and liberation in the slums of Guatemala City, and the other’s lifelong commitment to practical theology and spatial justice in South Africa. A practical theology of liberation is the result of life experiences in countries of the Global South amidst the search for justice and liberation. The worlds that come together in this article are René Girard’s mimetic theory, liberation theology (...) and practical theology. This article raises the question of the cross-contextuality of practical theology and the theological application of mimetic theory to develop innovative theological methodologies that respond to the collective woundedness of the urban context in the Global South.Contribution: The main contribution of this article is the use of three seemingly different theological approaches in conjunction. The health emergency due to the COVID-19 pandemic revealed limitations in health systems worldwide, making it necessary to establish a bioethical framework that provides tools to drive health professionals’ decision-making amid scarce health resources. Bioethical models such as principlism, utilitarianism, and personalism seek to focus clinical decisions on respect for people’s rights and dignity, thus protecting the medical practice. Personalism provides a person-centered approach to respect for human dignity during health emergencies to avoid giving material meaning to the individual. Decision (...) criteria are required to face bioethical conflicts in clinical practice, reducing the legal, emotional, and ethical burden of decision-making in pandemic situations. (shrink)

Para conocer las buenas prácticas éticas, se realizó un estudio exploratorio-descriptivo, basado en una metodología de enfoque mixto, divida en dos etapas. En la primera de carácter cualitativo se aplicó una entrevista a profundidad, y en la segunda se planteó el instrumento de recolección de datos: la encuesta. Este trabajo intenta avanzar en el debate sobre la ceguera en la construcción del conocimiento, originada por la incapacidad de visualizar otras formas de vida que se desarrollan fuera del sistema hegemónico, marcado esencialmente por la ideología capitalista y eurocéntrica. Para esta empresa, optamos por presentar a modo de ejemplo una serie de enunciados que mostrarán discursos marcados por prácticas y formas de vida de habitantes de sectores rurales del Centro y Sur de Chile. El procedimiento utilizado consistió en visibilizar (...) nuestras propias concepciones paradigmáticas para “ver” en todo el sentido de la palabra, discursos diversos; todo esto desde una perspectiva hermenéutica. Entre las conclusiones destaca la idea del “andar”, la paciencia y las recurrentes metáforas construidas desde la relación con la naturaleza. (shrink)

This paper contends that Marx develops in Volume III of Capital an incisive conceptual framework in which excessive credit creation, indebtedness and speculation play a critical and growing role in the reproduction of social capital on an extended basis; however, given the decentralised and anarchic nature of capitalist production, the credit system does so in a highly erratic and contradictory manner which only postpones the inevitable day of reckoning. The paper also highlights Marx's relatively neglected but highly important analysis of (...) the separation of ownership from management in the advanced capitalism of his day, England, and its modern-day implications for excessive risk-taking and debt-fuelled speculation up until the eve of the crash. More importantly, the paper argues that in Volumes II and III, Marx implicitly connected the expanding role of credit (which he associated with the development of capitalism) to a significant reduction in the turnover period of capital, thereby boosting the rate of surplus-value, and countering in a highly erratic and contradictory manner, the fall in the rate of profit. The growing role of credit has been relatively ignored in the Marxian literature as an important counteracting factor to the law of the declining rate of profit. It is not mentioned at all by Marx in his famous Chapter XIV, Vol. III of Capital where he discusses other important counteracting forces to the falling rate of profit, nor by Engels (in this particular context) who edited both Volumes II and III. Background Advance Care Planning via Group Visits (ACP-GV) is a patient-centered intervention facilitated by a clinician using a group modality to promote healthcare decision-making among veterans. Participants in the group document a “Next Step” to use in planning for their future care needs. The next step may include documentation of preferences in an advance directive, discussing plans with family, or anything else to fulfill their ACP needs. This evaluation seeks to determine whether there are identifiable subgroups of group participants with (...) differing needs prior to delivery of the ACP-GV program and, if so, to use information about the subgroups to enhance the program offered to veterans in United States Department of Veterans Affairs (VA) healthcare settings. Methods We conducted a secondary analysis of national data from a quality improvement evaluation. Patient- and provider-level data from administrative healthcare records for VA users in all 50 states, territories, and the District of Columbia provides data on veterans attending ACP-GV during federal fiscal years 2018–2022 (N = 26,857). Latent class analysis seeks to identify the various subgroups of veterans based on their level of ACP self-efficacy before attending ACP-GV and any demographic differences across the resulting subgroups of veterans attending ACP-GV. ACP self-efficacy is derived from seven items obtained from a participant worksheet used during the group. Results Analysis revealed two distinct groups of veterans, distinguishable by their pre-ACP-GV levels of one aspect of ACP self-efficacy: prior knowledge of ACP. Veterans with higher prior knowledge of ACP are associated with an identified next step focused on checking their current AD status and updating it, and veterans with lower ACP prior knowledge are associated with identifying a next step to discuss ACP more fully with family. Differences in age, sex, race, ethnicity, and marital status exist across subgroups of veterans. Conclusion Greater attention must be paid to ACP and veterans’ prior knowledge of ACP to consistently encourage annual review and status updates. (shrink)

The aim of this study was to analyze the explanatory level of the variables advertising influence, verbal messages, social models and social situations as regards body dissatisfaction, weight worry, and bodily discomfort. The study was conducted in a convenience sample of 206 Mexican women with an average age of 22.12 years. Structural equation modeling was used to analyze the direct and indirect effects of the independent variables on the dependent ones in three hypothetical models proposed. In the case of the (...) model proposed for body dissatisfaction, it was found that the set of variables had 79% of variance explained and showed adequate goodness-of-fit indices. The model for weight worry had 62% of variance explained and an acceptable goodness of fit. Finally, the model for bodily discomfort had 72% of variance explained and showed adequate goodness of fit. It is concluded that social influences have a significant impact on body image. Verbal messages had the strongest impact on the variables studied. (shrink)

Objective: Deep brain stimulation targeted to the ventral intermediate nucleus of the thalamus is effective for motor symptoms in essential tremor, but there is limited data on cognitive outcomes. We examined cognitive outcomes in a large cohort of ET DBS patients.Methods: In a retrospective analysis, we used repeated-measures ANOVA testing to examine whether the age of tremor onset, age at DBS surgery, hemisphere side implanted with lead, unilateral vs. bilateral implantations, and presence of surgical complications influenced the cognitive outcomes. Neuropsychological (...) outcomes of interest were verbal memory, executive functioning, working memory, language functioning, visuospatial functioning, and general cognitive function.Results: We identified 50 ET DBS patients; 29 males; the mean age of tremor onset was 35.84 years with a median age of 38 years. The mean age at DBS was 68.18 years. There were 37 unilateral 30 left, seven right, and 13 bilateral brain implantations. In the subgroup analysis, there was a significant interaction between assessment and age of tremor onset ; F = 4.47; p = 0.043 for working memory. The post hoc testing found improvements for younger onset ET. Similarly, there was a significant interaction between assessment and complications vs. no complications subgroups; F = 4.34; p = 0.043 for verbal memory with worsening scores seen for ET patients with complications. The remaining tests were not significant.Conclusion: In this large cohort of ET patients with, DBS was not accompanied by a significant decline in many cognitive domains. These outcomes were possibly related to the selection of patients with normal cognitive functioning before surgery, unilateral DBS implantations for the majority, and selection of patients with optimal response to DBS. Background The experiences of clinical ethicists have recently begun to garner some attention, but very little is known about the emotional impact of clinical ethics work on clinical ethicists. The purpose of our study was to explore the experiences of clinical ethicists in the United States in order to better understand the impact that their professional responsibilities have on them as people, as well as to learn about the ways in which organizational structures and coping mechanisms help them navigate the (...) emotional impact of their work.Methods This was a primary analysis of 34 semi-structured interviews with clinical ethicists across the United States. The interviews were conducted from May-September 2023 and were recorded, transcribed, and de-identified. The qualitative themes explored in this paper are part of a larger study informed by principles derived from grounded theory.Results Four major themes emerged: particular features of clinical ethics work seem to significantly impact the emotional state of clinical ethicists, lack of clarity about the role of clinical ethicists appears to significantly impact their emotional experience, organizational structures appear to shape clinical ethicists’ professional and emotional experiences, and clinical ethicists employ a variety of professional and personal coping mechanisms when experiencing adverse emotions because of their professional responsibilities.Conclusion Our findings suggest that clinical ethicists are profoundly impacted by their work and that their emotional experience is shaped by a wide range of factors both at the individual and systemic level. Resumen: Este artículo se propone configurar la trama saber-tropo como estrategia para desvelar ataduras coloniales que aún persisten en las vidas cotidianas de sujetos situados y encarnados, cuyas existencias se desarrollan en condiciones de precariedad material, exclusión, explotación y no reconocimiento. Con un marcado carácter epistémico-metodológico, y tomando como base los postulados de la tropología, se discute sobre la noción de tropo y saber, además de señalar las posibles relaciones que entre ellos se pueden dar, para finalmente, en términos demostrativos, (...) plantear un procedimiento orientado a identificar y analizar los tropos que permiten ir desvelando lastres coloniales en las narraciones cotidianas. La relevancia de esta propuesta se asienta en la necesidad de ir avanzado en construir herramientas metodológicas que permitan comprender los principios que configuran el pensamiento decolonial. Desde allí, se propone al tropo como expresión y discusión figurativa del saber, debido a que este puede ser utilizado persuasivamente para afectar la forma en que se habita el mundo, al tiempo que permite comprender el accionar social y, consecuentemente, las matrices coloniales con las que debemos romper.: This article proposes to configure knowledge-trope plot as a strategy to uncover colonial ties that persist in the daily lives of situated and incarnated subjects, whose existences develop in conditions of material precariousness, exclusion, exploitation and non-recognition. With a marked epistemic-methodological character, and based on the postulates of the tropology, the notion of trope and knowledge is discussed, as well as pointing out the possible relationships that can occur between them, and finally, in demonstrative terms, to propose a procedure aimed at identifying and analysing the tropes that allow us to reveal colonial ballasts in everyday narratives. The relevance of this proposal is based on the need to construct methodological tools that allow us to understand the principles of decolonial thinking. From there, the trope is proposed as an expression and figurative discussion of knowledge, because it can be used persuasively to affect the way in which the world is inhabited, while at the same time it allows us to understand social action and, consequently, colonial matrices with which we must break. Recent literature have underlined the connections between children’s reading skills and capacity to create and use mental representations or mental images; furthermore data highlighted the involvement of visuospatial abilities both during math learning and during subsequent developmental phases in performing math tasks. The present research adopted a longitudinal design to assess whether the processes of mental imagery in preschoolers (ages 4–5 years) are predictive of mathematics skills, writing and reading, in the early years of primary school (ages 6–7 years). The (...) research lasted for two school years; in the first phase, the general group of participants consisted of 100 children, and although all participants agreed to be part of the research, in the second phase, there was a mortality rate of 30%. In order to measure school learning and mental imagery processes four batteries of tests were used. The mental imagery battery evaluated mental generation, inspection and transformation processes. Data underlined that the different aspects in which mental imagery processes are articulated are differently implied in some skills that constitute school learning. These findings emphasise the potential usefulness of a screening for mental imagery ability for schoolchildren to adopt effective measures to increase their mental imagery abilities. (shrink)

A questão central é compreender de que formas as práticas populares se relacionam com o desenvolvimento e a manifestação das identidades cristãs. Como práticas populares elegemos as práticas de judaização, entre os não judeus adeptos da fé em Cristo, existentes no contexto das comunidades cristãs. A judaização é assim compreendida porque foge à instrução/controle das autoridades e da elite intelectual cristã. O recorte espaço-temporal do presente (...) estudo são as comunidades cristãs da província da Ásia Proconsular, no território imperial romano, no século II d.C. O conceito de etnicidade é empregado no exame das cartas do bispo Inácio de Antioquia a comunidades cristãs da Ásia Menor de forma a iluminar possíveis motivações para uma manutenção da judaização entre não judeus crentes em Jesus em detrimento da forte recomendação de seu abandono pelas autoridades cristãs. El presente artículo traza la semblanza intelectual y profesional del religioso José Mª Salvador y Barrera, natural de Marchena, que llegó a ser Rector del Colegio del Sacro Monte de Granada, Obispo de Tarazona, Obispo de Madrid-Alcalá, Arzobispo de Valencia y Senador, y que dedicó todos sus esfuerzos a defender la enseñanza religiosa y la acción social de la Iglesia desde los altos puestos que ocupó. Fue además un excelente orador y persona de gran cultura, siendo reconocida su labor por (...) las Reales Academias de la Historia y de Ciencias Morales y Políticas, que le eligieron como académico de número. El obispo Salvador fue realmente uno de los prelados más competentes y aperturistas de los dos primeros decenios del siglo XX. (shrink)

A team of young scholars from the University of Babes-Bolyai, Cluj-Napoca, Romania, has received funding to edit Book I of the commentary on the Sentences by the Cistercian Iacobus de Altavilla, or James of Eltville, who is attested as lecturing on the Sentences at Paris in 1369. The project has made it possible to correct Friedrich Stegmüller's findings, removing from his list several wrongly attributed manuscripts and adding to the list a number of authentic copies or fragments. The present article (...) provides an inventory with short descriptions of the twenty-one manuscripts that preserve the whole or parts of James of Eltville's commentary, and presents new information about the manuscript tradition based on the preliminary results of the Romanian project. Despite the lack of invariance problem (the many‐to‐many mapping between acoustics and percepts), human listeners experience phonetic constancy and typically perceive what a speaker intends. Most models of human speech recognition (HSR) have side‐stepped this problem, working with abstract, idealized inputs and deferring the challenge of working with real speech. In contrast, carefully engineered deep learning networks allow robust, real‐world automatic speech recognition (ASR). However, the complexities of deep learning architectures and training regimens make it difficult to use them to (...) provide direct insights into mechanisms that may support HSR. In this brief article, we report preliminary results from a two‐layer network that borrows one element from ASR, long short‐term memory nodes, which provide dynamic memory for a range of temporal spans. This allows the model to learn to map real speech from multiple talkers to semantic targets with high accuracy, with human‐like timecourse of lexical access and phonological competition. Internal representations emerge that resemble phonetically organized responses in human superior temporal gyrus, suggesting that the model develops a distributed phonological code despite no explicit training on phonetic or phonemic targets. The ability to work with real speech is a major advance for cognitive models of HSR. (shrink)

The progression of research and scholarly inquiry does not occur in isolation and is wholly dependent on accurate reporting of methods and results, and successful replication of prior work. Without mechanisms to correct the literature, much time and money is wasted on research based on a crumbling foundation. These guidelines serve to outline the respective responsibilities of researchers, institutions, agencies, and publishers or editors in maintaining the integrity of the research record. DBS Think Tank IX was held on August 25–27, 2021 in Orlando FL with US based participants largely in person and overseas participants joining by video conferencing technology. The DBS Think Tank was founded in 2012 and provides an open platform where clinicians, engineers and researchers can freely discuss current and emerging deep brain stimulation technologies as well as the logistical and ethical issues facing the field. The consensus among the DBS Think Tank IX speakers was that DBS expanded in (...) its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. After collectively sharing our experiences, it was estimated that globally more than 230,000 DBS devices have been implanted for neurological and neuropsychiatric disorders. As such, this year’s meeting was focused on advances in the following areas: neuromodulation in Europe, Asia and Australia; cutting-edge technologies, neuroethics, interventional psychiatry, adaptive DBS, neuromodulation for pain, network neuromodulation for epilepsy and neuromodulation for traumatic brain injury.
