This study describes which clinical ethics approaches are available to support healthcare personnel in clinical practice in terms of their construction, functions and goals. Healthcare personnel frequently face ethically difficult situations in the course of their work and these issues cover a wide range of areas from prenatal care to end-of-life care. Although various forms of clinical ethics support have been developed, to our knowledge there is a lack of review studies describing which ethics support approaches are available, how they (...) are constructed and their goals in supporting healthcare personnel in clinical practice. This study engages in an integrative literature review. We searched for peer-reviewed academic articles written in English between 2000 and 2016 using specific Mesh terms and manual keywords in CINAHL, MEDLINE and Psych INFO databases. In total, 54 articles worldwide described clinical ethics support approaches that include clinical ethics consultation, clinical ethics committees, moral case deliberation, ethics rounds, ethics discussion groups, and ethics reflection groups. Clinical ethics consultation and clinical ethics committees have various roles and functions in different countries. They can provide healthcare personnel with advice and recommendations regarding the best course of action. Moral case deliberation, ethics rounds, ethics discussion groups and ethics reflection groups support the idea that group reflection increases insight into ethical issues. Clinical ethics support in the form of a “bottom-up” perspective might give healthcare personnel opportunities to think and reflect more than a “top-down” perspective. A “bottom-up” approach leaves the healthcare personnel with the moral responsibility for their choice of action in clinical practice, while a “top-down” approach risks removing such moral responsibility. Most questions commonly asked about international politics are ethical ones. Should the international community intervene in Bosnia? What do we owe the starving in Somalia? What should be done about the genocide in Rwanda? Yet, Mervyn Frost argues, ethics is accorded a marginal position within the academic study of international relations. The Magic of Unknowing is a unique philosophical and literary work. Cast in the dialogue form, it unfolds in the mood of soliloquy. Mervyn Sprung has created an imaginative meeting of the minds of great western philosophers: Aristotle, Descartes, Kant, Hume, Nietzsche, Wittgenstein and Pyrrho. During the past four years, the International Institute for Indigenous Resource Management has sponsored and co-sponsored a series of discursive roundtables on the ethical, legal, social, and cultural implications of genetic research on Indian tribes and Indian people. The deliberations of the tribal leaders, legal scholars, researchers, representatives of non-governmental organizations, and others who participated in these roundtables laid out a range of barriers to informed tribal participation in genetic research and proposed a policy, legal, and scientific research agenda to (...) fill gaps in knowledge and to otherwise support indigenous peoples' decision-making on genetic research issues.An understanding of Native American attitudes toward genetic research on their community cannot be fully understood absent an appreciation of the interrelationship among native peoples, genetic research, and the landscape, contaminated as it is by governmental and other factors. The deliberations described herein present invaluable insights into tribal attitudes toward genetic research that have application to ethical, legal and social aspects of forensic DNA databanks, sample collection and retention, and, in particular, issues of secondary non-forensic uses of such DNA. WHEN THE FIRST GIFFORD LECTURES were delivered in Scotland in 1888-89, the Scottish philosophical and theological worlds were undergoing significant changes. Through much of the eighteenth and nineteenth centuries in Britain, natural theology referred to the traditional arguments for the existence of God, particularly as put forth in the work of William Paley. But developments in the empirical sciences and in the empirical type of philosophy which dominated British thought during this period challenged these arguments and led to widespread (...) religious skepticism. Thomas Reid and William Hamilton were two Scots who had sought to overcome the religious skepticism of the empirical age, but the appeal to common sense belief on the one hand and to an unknowable absolute on the other hand provided little comfort for many who were dissatisfied with the options of religious scepticism and doctrinal orthodoxy. A new philosophical spirit, however, was making itself known. This is suggested in a note to the 1877 edition of David Masson's Recent British Philosophy, first published in 1865. (shrink)

The twentieth century saw the emergence of a new episteme of death that fundamentally revolutionized values relating to mortality and life. Previously this revolution has been seen primarily in terms of the sequestration and denial of death, but it is necessary to go farther and recognize that these are really just an aspect of the industrialization ‐the Fordism ‐ of death. This takes two major institutional forms: the militarization, and the medicalization of death. Both ensure that death is administered on (...) an industrial scale and in accord with institutional and bureaucratic imperatives and values. The total mobilization of the Great War was the prototype that revealed the potential of this approach. With the subsequent medical revolution of the middle decades of the century the approach was quickly rationalized and refined into a new episteme of administered death, with ‘administer’ being understood in its twin senses of ‘to manage’ and ‘to dispense’ — the two characteristic orientations to death in contemporary society. This new episteme quickly displaced traditional values derived predominantly from religious, philosophical, mythological and traditional sources and has advanced far beyond their responsive capacity, as the many interminable debates around issues of bioethics reveal. While this new episteme might enhance the human condition, it also has great potential for the impoverishment of the human spirit, and for the further reduction of human beings to the status of mere components and functions to be administered within medico‐technological systems that are themselves parts of an increasingly globalized economic system.
