Assisted reproductive technology treatment tends to involve significant physical and emotional commitments that can impact maternal, infant, and family health and well-being. An in-depth understanding of experiences is necessary to provide adequate support for women and their families during pregnancy and transition to parenthood following ART treatment. The aim of this study was to explore first-time mothers’ experiences of pregnancy and transition to parenthood following successful ART treatment in Taiwan. Twelve first-time mothers who conceived and gave live birth using ART (...) treatment were purposively selected from a fertility centre in Taipei, Taiwan. Women’s experiences in pregnancy and in their transition to motherhood were explored using semi-structured in-depth interviews. All interviews were recorded, transcribed, and analysed using the Colaizzi strategy. The mothers’ accounts reflected three main themes: ‘being different from mothers who became pregnant naturally’, ‘ensuring health and safety of the foetus’, and ‘welcoming new lives with excitement’. The difference mothers felt about themselves was evident in four subthemes: becoming pregnant after a long wait, feeling vulnerable during pregnancy, relying on family’s assistance and support, and worrying about the impact of ART on health. The theme on ‘ensuring health and safety of the foetus’ encompassed three subthemes: activities to protect the unborn baby, monitoring foetal movement constantly to maintain peace of mind, and receiving foetal reduction for the sake of the pregnancy. Narratives around ‘welcoming new lives with excitement’ reflected four subthemes: overcoming hardship for worthwhile results, realising one’s life and dreams, proving to be fertile enough to give birth, and return to normal life track. Findings indicate the need for educational and psychosocial interventions to support women and their families physically and psychologically during ART treatment. The stigma related to infertility and the psychosocial support from family are aspects to consider while planning intervention programmes. (shrink)

During immune responses against invading pathogenic bacteria, the cytoskeleton network enables macrophages to implement multiple essential functions. To protect the host from infection, macrophages initially polarize to adopt different phenotypes in response to distinct signals from the microenvironment. The extracellular stimulus regulates the rearrangement of the cytoskeleton, thereby altering the morphology and migratory properties of macrophages. Subsequently, macrophages degrade the extracellular matrix (ECM) and migrate toward the sites of infection to directly contact invading pathogens, during which the involvement of cytoskeleton‐based (...) structures such as podosomes and lamellipodia is indispensable. Ultimately, macrophages execute the function of phagocytosis to engulf and eliminate the invading pathogens. Phagocytosis is a complex process that requires the cooperation of cytoskeleton‐enriched super‐structures, such as filopodia, lamellipodia, and phagocytic cup. This review presents an overview of cytoskeletal regulations in macrophage polarization, ECM degradation, migration, and phagocytosis, highlighting the pivotal role of the cytoskeleton in host defense against infection. University students having high entrepreneurial intention while not transferring into actual entrepreneurial behavior is a contradictory issue in need of in-depth research. To explore the successive development mechanism of the entrepreneurial process, this study constructed a moderated mediation model to examine whether entrepreneurial commitment from three dimensions mediated the relationship between entrepreneurial intention and behavior, and whether this mediating process was moderated by family support. A survey was conducted among university students from six major universities in south China using the (...) snowball sampling approach. A total of 469 valid responses were obtained. Structural equation modeling was adopted for data analysis. According to the results of the confirmatory factor analysis, it was found that entrepreneurial intention had both direct and indirect positive effects on entrepreneurial behavior, while entrepreneurial commitment worked as the mediator, and family support moderated the relationship between entrepreneurial intention and behavior. Results indicated that entrepreneurial commitment bridged the path from entrepreneurial intention to behavior, and family support created the boundary effect. This finding highlights the importance of guiding students through entrepreneurial commitment toward entrepreneurial behavior, and pays special attention to the crucial role of family support under the national strategy. Mei-Hua Chen and Hong-zen Wang Flexible Intimacies in the Global Intimate Economy: Evidence from Taiwan’s Cross-Border Marriages When Lin Ping was interviewed by the first author of this article at a detention center in the southern city of Tainan, Taiwan, in September 2006, she was forty-three. At that time, she had been married to a Taiwanese man (...) for five years and had successfully obtained a permit for employment at a factory, earning NT$27,000 a month for working ten hours a day. However, before having obtained the work permit, Lin managed to find jobs in informal sectors as a cleaner, a care worker, and a stall vendor’s helper, earning NT$15,000 a month. By the time of our interview, Lin had gone to work at a brothel because she could not put up with her husband anymore. Lin’s husband is twenty years older than she is and, as she described their relationship, the huge age gap makes communication between the couple difficult. They fought often and fiercely. During those fights, Lin’s husband would threaten to divorce her and to not renew her application for residency, which would result in her immediate deportation to China. This did happen once when Lin’s husband refused to renew her work permit. As a result, Lin overstayed her visa and lost her job at the factory. Being thus unemployed, Lin had to dip into her savings and risked going into debt. Her dire financial situation caused her anxiety. Losing the regular income provided by her factory job meant not only having nothing to live on but also being unable to send money back home to China. Given that her marriage failed to provide her the work permit she needed, Lin wanted to leave her husband Mei-Hua Chen and Hong-zen Wang 259 and “go out” for her own good. When asked what she thinks of the stigma attached to commercial sex, Lin replied, “I need to be able to feed myself first. But if you don’t have a work permit and a job, it [sex work] is the only way out. So, I thought I would do it.” She continued: “Once I had the money, I would be able to run a small business such as a noodle stall or selling steam dumplings.” Lin Ping’s stories serve to sketch out the interconnectedness between cross-border sex work and cross-border marriage. As her experiences show, cross-border sex work and marriage tend to overlap in certain spheres; however, they are more often than not conceptualized as two separate institutions. Both are grouped under already dichotomized migratory categories such as marriage migrants and economic migrants, real marriage and fake marriage, legal workers and workers without permits, and legal and illegal migrants, etc. Such dichotomization downplays the fact that many marriage migrants, like economic migrants, invest great economic interests in cross-border marriages; it also downplays the fact that migrant sex workers might also be involved in having intimate relationships with someone. Apart from the entanglement of monetary exchange and intimacy, cross-border marriage and cross-border sex work are also parts of a global intimate economy in which the Global North appropriates southern women’s reproductive and intimate labor. Seen in this light, it becomes urgent to theorize how women from the South manage to survive in the global intimate economy by shifting between cross-border marriage and cross-border sex work in the North. Admittedly, such a Global North-and-South framework fits awkwardly when it comes to understanding Taiwan’s situation because economic development is the sole barometer used to define the North and the South, and economically, Taiwan should be in the North camp. Politically, however, due to the majority of nations not recognizing its sovereignty, Taiwan is much more peripheral than most southern countries, let alone China. Therefore, compared to countries in Southeast Asia, Taiwan is economically North but politically South. This paper focuses on the problem of target tracking using k fittest robots in a group of n mobile robots with n>k. We present centralized and distributed coordination models with all-to-all and limited communications, respectively. For the case of all-to-all communication between robots, theoretical analysis is presented to prove the exponential stability of the whole system. In real applications of robotic networks, a robot may only be allowed to exchange information with a limited number of neighbors. In such a limited (...) communication case where centralized quantity is not available, a consensus filter is used to estimate the centralized quantities in a distributed way, and a distributed competitive target tracking is thus achieved. The stability of the distributed control is also proved in theory. Finally, illustrative examples are provided and analyzed to substantiate the efficacy of the proposed models for tracking moving target in a competition manner with all-to-all communications and limited communications. This paper studies a special scheduling problem under hierarchical management in nurse staff. This is a more complex rostering problem than traditional nurse scheduling. The first is that the rostering requirements of charge nurses and general nurses are different under hierarchical management. The second is that nurses are preferable for relative fair rather than absolute fair under hierarchical management. The model aims at allocating the required workload to meet the operational requirements, weekend rostering preferences, and relative fairness preferences. Two hybrid (...) heuristic algorithms based on multiobjective grey wolf optimizer and three corresponding single heuristic algorithms are employed to solve this problem. The experimental results based on real cases from the Third People’s Hospital, Panzhihua, China, show that MOGWO does not as good as it does on other engineering optimization. However, the hybrid algorithms based on MOGWO are better than corresponding single algorithms on generational distance and spacing of Pareto solutions. Furthermore, for relative fair rostering objective, NSGAII-MOGWO has more power to find the optimal solution in the dimension of relative fairness. (shrink)

Based on the regret regulation theory and entrepreneurial cognition theory, this study aims to investigate the relationship between entrepreneurs’ anticipated regret and entrepreneurial persistence through the mediator of entrepreneurial cognition. To that end, we distributed surveys to entrepreneurs who were supported by the “Xing Chuang Tian Di” project and used 248 questionnaire data to examine the hypotheses. The results show that entrepreneurs’ anticipated regret has a significant and direct impact on entrepreneurial persistence. Also, entrepreneurship cognition plays a mediating role between (...) anticipated regret and entrepreneurial persistence. In addition, the entrepreneurial environment plays a positive role in moderating the relationship between anticipated regret and entrepreneurial persistence. (shrink)

Objects: To evaluate the feasibility and effectiveness of in-bed wearable elbow robot training for motor recovery in patients with early and late subacute stroke.Methods: Eleven in-patient stroke survivors received 15 sessions of training over about 4 weeks of hospital stay. During each hourly training, participants received passive stretching and active movement training with motivating games using a wearable elbow rehabilitation robot. Isometric maximum muscle strength of elbow flexors and extensors was evaluated using the robot at the beginning and end of (...) each training session. Clinical measures including Fugl-Meyer Assessment of upper extremity, Motricity Index for upper extremities, Modified Ashworth Scale were measured at baseline, after the 4-week training program, and at a 1-month follow-up. The muscle strength recovery curve over the training period was characterized as a logarithmic learning curve with three parameters.Results: At the baseline, participants had moderate to severe upper limb motor impairment {FMA-UE [median ]: 28 } and mild spasticity in elbow flexors {MAS [median ]: 0 }. After about 4 weeks of training, significant improvements were observed in FMA-UE and MI, and the improvements were sustained at the follow-up. The elbow flexors MVC significantly increased by 1.93 Nm and the elbow extensor MVC increased by 0.68 Nm. Muscle strength recovery curve showed that patients with severe upper limb motor impairment had a greater improvement rate in elbow flexor strength than those with moderate motor impairment.Conclusion: In-bed wearable elbow robotic rehabilitation is feasible and effective in improving biomechanical and clinical outcomes for early and late subacute stroke in-patients. Results from the pilot study suggested that patients with severe upper limb motor impairment may benefit more from the robot training compared to those with moderate impairment. (shrink)

ObjectiveThis study used a moderated mediation model to test the mediating effect of anxiety on the relationship between negative perfectionism and sleep quality and the moderating effect of COVID-19 epidemic risk perception during the COVID-19 pandemic in Chinese international students.Materials and methodsA sample of 239 Chinese international students from the south of China, was surveyed with the Negative and Positive Perfectionism Scale, the Pittsburgh Sleep Quality Index, the General Anxiety Disorder Scale, and the COVID-19 Epidemic Risk Perception Inventory. Version 23.0 (...) of SPSS and version 3.4 of PROCESS were used to perform the correlation analyses, mediation analysis, and moderated mediation analysis.Results Negative perfectionism was significantly correlated with anxiety and poor sleep quality. Anxiety was significantly correlated with poor sleep quality. The mediating effect test showed that anxiety had a mediating effect between negative perfectionism and poor sleep quality. Epidemic risk perception moderated the mediating effect of anxiety between negative perfectionism and poor sleep quality.ConclusionNegative perfectionism affected sleep quality indirectly through anxiety. In particular, COVID-19 epidemic risk perception moderated the relationship between anxiety and sleep quality, such that the association was stronger when the COVID-19 epidemic risk perception was high. These results provide a more comprehensive understanding of the negative link between negative perfectionism and poor sleep quality. (shrink)

To demonstrate how green innovation effectively occurs, this study examines the effects of green intellectual capital on GI from the perspective of green supply chain integration. Based on a natural-resource-based view and knowledge-based view, the authors constructed an intermediary model of GIC-GSCI-GI, and analyzed the effects of green absorptive ability and relationship learning ability as moderators. An empirical survey of 328 Chinese manufacturing companies was conducted. Our results indicate that three dimensions of GIC positively impact GI. The mediating effects of (...) internal and external GSCI exist in the relationship between GIC and GI. The moderating effects of GAA and RLA in these effects were also verified. Our study provides further empirical evidence for the relationship between GIC and GI, highlights the effects of companies’ internal and external abilities on GI, and suggests new ways and implementation contexts for GI. Indonesia is recognized as a nurse exporting country, with policies that encourage nursing professionals to emigrate abroad. This includes the country’s adoption of international principles attempting to protect Indonesian nurses that emigrate as well as the country’s own participation in a bilateral trade and investment agreement, known as the Indonesia–Japan Economic Partnership Agreement that facilitates Indonesian nurse migration to Japan. Despite the potential trade and employment benefits from sending nurses abroad under the Indonesia–Japan Economic Partnership Agreement, Indonesia itself is suffering (...) from a crisis in nursing capacity and ensuring adequate healthcare access for its own populations. This represents a distinct challenge for Indonesia in appropriately balancing domestic health workforce needs, employment, and training opportunities for Indonesian nurses, and the need to acknowledge the rights of nurses to freely migrate abroad. Hence, this article reviews the complex operational and ethical issues associated with Indonesian health worker migration under the Indonesia–Japan Economic Partnership Agreement. It also introduces a policy proposal to improve performance of the Indonesia–Japan Economic Partnership Agreement and better align it with international principles focused on equitable health worker migration. Background Relational autonomy is an alternative concept of autonomy in which an individual is recognized as embedded into society and influenced by relational factors. Social context, including social location, political structure, and social forces, significantly influence an agent to develop and exercise autonomy skills. The relational approach has been applied in clinical practice to identify relational factors impacting patient autonomy and decision-making, yet there is a knowledge gap in how these factors influence the demonstration of relational autonomy in the context (...) of medical decisions of adults. Objective The present study targeted the existing knowledge of what and how relational factors impact individuals making medical decisions using the theoretical framework of relational autonomy. Methods A meta-synthesis study was utilized. Four electronic databases, including Embase, OVID Medline, CINAHL, and PubMed, were searched, along with gray literature and reference lists, to identify relevant studies. Results 23 studies reporting 21 qualitative and two mixed-method studies were reviewed. Four themes emerged from the qualitative findings: (1) supportive relationships facilitate an individual’s relational autonomy; (2) obtaining comprehensive information from broader sources helps individuals exercise relational autonomy; (3) undue family pressure impedes the exercising of patient relational autonomy; and (4) healthcare providers’ dominant voice hampers the demonstration of relational autonomy. Conclusions Applying relational autonomy to assist adults in making well-considered decisions is essential. The meta-synthesis suggests establishing a supportive relationship between individuals, healthcare providers, and family. A supportive relationship will allow healthcare providers to make judgments in line with an individual’s values and wishes with the aim of promoting relational autonomy. Advance care planning was proposed as the effective solution to obtain a consensus between individuals and their families while respecting an individual’s values and preferences. Furthermore, it is considered crucial for healthcare providers to appreciate an individual's values and incorporate their preferences into recommendations. (shrink)

Background: Cognitive impairment is one of the core symptoms of schizophrenia, which is considered to be significantly correlated to prognosis. In recent years, many studies have suggested that metabolic disorders could be related to a higher risk of cognitive defects in a general setting. However, there has been limited evidence on the association between metabolism and cognitive function in patients with early-stage schizophrenia.Methods: In this study, we recruited 172 patients with early-stage schizophrenia. Relevant metabolic parameters were examined and cognitive function (...) was evaluated by using the MATRICS Consensus Cognitive Battery (MCCB) to investigate the relationship between metabolic disorder and cognitive impairment.Results: Generally, the prevalence of cognitive impairment among patients in our study was 84.7% (144/170), which was much higher than that in the general population. Compared with the general Chinese setting, the study population presented a higher proportion of metabolic disturbance. Patients who had metabolic disturbance showed no significant differences on cognitive function compared with the other patients. Correlation analysis showed that metabolic status was significantly correlated with cognitive function as assessed by the cognitive domain scores (p < 0.05), while such association was not found in further multiple regression analysis.Conclusions: Therefore, there may be no association between metabolic disorder and cognitive impairment in patients with early-stage schizophrenia.Trial Registration: Clinicaltrials.gov, NCT02880462. As heritage is the precious treasure of human society, heritage also carries the genes of culture. It is of vital importance to effectively develop heritage tourism resources and explore the mechanisms that influence tourists’ cultural identity. This study has integrated the stimulus-organism-response framework with the attitude-behavior-context theory to construct a hypothetical model of heritage tourism aesthetics, tourist involvement, mental experience, and cultural identity so as to figure out their relationships. The questionnaires were collected to investigate the impact paths and mechanisms (...) between heritage aesthetics, tourist involvement, mental experience, and cultural identity. The structural equation model was used to examine the relationship between heritage tourism aesthetics, tourist involvement, mental experience, and cultural identity. The main findings include: the positive impact of aesthetics driving mental experience and cultural identity is significant; the impact of tourist involvement on mental experience and cultural identity is also significant; the impact of aesthetics on cultural identity is not significant, but mental experience mediates the relationship between aesthetics and cultural identity in heritage tourism. This study provides a new research framework and perspective for the aesthetics, tourist involvement, mental experience, and cultural identity of tourists in heritage tourism. This study also provides practical implications for government culture sectors to propagandize culture and for heritage destination managers to better manage heritage sites. (shrink)

Cambridge, Great Britain: Cambridge University Press, 1998. xv +227 pp. In a work brimming with unobtrusive erudition and centered on the figure of Huang Po (d. 850), Dale Wright offers a seasoned account of a topic that is still very much in need of clarification, (...) namely, the roles of language, conceptuality, textuality, interpretation, and historical development in Zen Buddhism. Some recent critics tend to see Zen as incoherent or even hypocritical in that this "special transmission outside the sutras, not dependent on language and texts, pointing directly to mind" (quoted, p. 64) in reality developed complex and varied textual and ritual traditions from [End Page 147] which the supposedly pure enlightenment experience cannot be siphoned out. Wright argues that the critics themselves are often naive, giving an inverted reflection of the naiveté of earlier scholars such as John Blofeld, who saw Zen as centered on an ineffable Supreme Experience, quite independent of the language used to point to it. Nonetheless, Wright himself is in basic agreement with the critics and in radical opposition to Blofeld (and to D. T. Suzuki). But he points out that the early Chinese Zen masters speak in many voices, often anticipating the most sophisticated insights of their critics.Wright's style is very unlike that of Bernard Faure, being slow, serene, reflective, scrupulous, and imbued with the deepest respect for the Zen tradition. Yet his thought rejoins Faure's at many points.The thrust of Wright's argument is to demystify Zen enlightenment by reinserting it in its varied linguistic, social, institutional, and historical contexts. He gives a realistic and holistic account of what Zen experience must have been like for disciples of Huang Po (pp. 187-192). His stress on the linguisticality of all experience certainly illuminates the texture of Huang Po's world. But I am left wondering if there is not another aspect of Zen that is missed here. Modern philosophers have derived much insight from the realization that thought and perception are deeply embedded in language. But I do not know that anyone has proven that there can never be a thinking or perception that is independent of language. Even the supposedly pan-textualist Derrida states somewhere that he does not exclude the possibility of nonlinguistic thought. In most fields of inquiry this abstruse question makes no practical difference, but in the case of Zen it is of crucial import. The presupposition of universal linguisticality, especially if it hardens into a dogma, may block access to the core of Zen experience.Wright argues that though some Zen masters sought prelinguistic immediacy, the majority were deeply aware of the inseparability of experience and the language in which it is grasped. But could it not be that prelinguistic immediacy was so much taken for granted in Zen that the masters scarcely needed to insist on it? Wright says that enlightenment itself is a linguistic event, since it is often occasioned by a verbal statement and given immediate expression in another verbal statement. But to recognize the indispensability of language for conveying experience is not necessarily to imply that the experience itself is dependent on language. Although a certain poem of Huang Po "strives to make its anti-textual point, the master must enter into the textual world to do so, thus abandoning the position of 'no dependence on texts'" (p. 22). Is it really necessary to see a contradiction here, given that, as Wright himself points out, "no unanimity on the meaning of the mandate against 'words and letters' existed" (p. 26)? Let us suppose that the point is nonattachment to texts and a realization of the intrinsic independence of enlightenment from the limited perspectives of textuality, language, and conceptual thought. This is quite compatible with intensive use of texts in practice.Blofeld explains that texts were useful to the learner but cast aside when enlightenment was reached. For the Western philosopher the most difficult idea to understand is the Zen (Ch'an) notion of ?Mind?, which is a key to understanding Zen Buddhism. In order to transmit the idea of ?Mind? Huang Po suggests that the only successful method for understanding it is intuition. Perhaps the difficulty for the Western philosopher arises from his compulsion to analyze and his wholesale rejection of intuition as a valid method of understanding. My biggest [End Page 130] challenge will be that I don't understand either of these languages, leaving me at the mercy of translators, in a field where meaning is often dependant on nuance.I was, therefore, very interested to read Huang Di Nei Jing Su Wen: Nature, Knowledge, Imagery in an Ancient Chinese Text by Paul Unschuld. Although it is considered a seminal text, the Huang Di Nei Jing Su Wen is not a unified manual of medicine but the accumulation of writing and experience that predated its compilation. In his efforts to translate this book, Unschuld attempted to take into consideration the broad variety of influences that would affect its intent and meanings.The body of work that is known by this name underwent many revisions through the years, attaining the form we recognize today approximately twelve hundred years ago. Even working within the original language this presents several challenges. The first is that language and meanings change, and this sometimes obscures the intended connotation even for a scholar conversant in the original language. Unschuld also took into consideration the fact that the book had many editors, each working within a different social, political, and religious milieu. Medical knowledge grew with each generation, based on and referring to the writings of the time, many of which are no longer extant but are assumed to have been available to readers of the Huang Di Nei Jing Su Wen.If one were to postulate that this work was perfectly understood in Chinese, Unschuld would still face a challenge in his attempt to translate it into English. There are concepts in Chinese that have no equivalent in English and therefore present a challenge for someone trying to remain true to the original intent. He explains that this difficulty begins with the title itself, which is normally given as "The Yellow Emperor's Classic of Chinese Medicine." Although Huang Di is usually translated as Yellow Emperor, there are implications of the word emperor that are not part of huang, and, in fact, Unschuld prefers to leave the word untranslated, as there is no English option that does not alter the intent of the original.Like most important books, the Huang Di Nei Jing Su Wen cannot be fully grasped in one reading. I expect to revisit it many times throughout my life, as my experience in life and my practice leaven my understanding. I truly believe that texts such as Unschuld's will be an important part of my developing perception. Knowledge that is examined divorced from its formative circumstances loses the heart of its meaning, and Unschuld has made important progress in revealing the keys to the essence of the Su Wen.Huang Di Nei Jing Su Wen: Nature, Knowledge, Imagery in an Ancient Chinese Text is a dense but fascinating read. Unschuld has done a good job of providing the information needed to be clear and thorough without getting bogged down in what are probably intriguing details. The layperson in Chinese history and textual analysis will likely find it occasionally rough going, but I found it an inspiration to learn more. Overall, I believe Paul Unschuld's work to shows him to be a worthy disciple of Wang Bing, one of the Su Wen's editors, who wrote: "I hope to elucidate the message of the sages and to make their mysterious sayings shine. They should resemble the stellar constellations hung up on high.... [T]hey should resemble the crystalclear water in a deep spring where scales and shells can be distinguished."
