Ethical care is beginning to be recognised as care that accounts for the views of those at the receiving end of care. However, in the context of palliative and supportive district nursing care, the patients’ and their carers’ views are seldom heard. This qualitative research study explores these views. Data were collected through semi-structured interviews with 26 patients with palliative and supportive care needs receiving district nursing care, and 13 of their carers. Participants were recruited via community nurses and hospices (...) between September 2010 and October 2011. Post-structural discourse analysis is used to examine how discourses operate on a moral level. One discourse, ‘busyness’, is argued to preclude a moral form of nursing care. The discourse of friendship is presented to contrast this. Discussion explores Gallagher’s ‘slow ethics’ and challenges the currently accepted ways of measuring to improve quality of care concluding that quality cannot be measured. (shrink)

Technology and its interfaces with nursing care, patients and carers, and the home are many and varied. To date, healthcare services research has generally focussed on pragmatic issues such access to and the optimization of technology, while philosophical inquiry has tended to focus on the ethics of how technology makes the home more hospital like. However, the ethical implications of the ways in which technology shapes the subjectivities of patients and carers have not been explored. In order to explore this, (...) poststructural theory, in particular the work of Butler, Foucault, and Deleuze, is used to theorize the relationship between subjectivity and materiality as ethically mandated on producing rather than precluding the development of subjectivities in novel ways. This theoretical understanding is then utilized through a process of ‘plugged in’ as described by Jackson and Massie that aims to link empirical data, research, and philosophical inquiry. Through this process, it is suggested that power, which the empirical data demonstrate, is frequently exercised through medical discourses and restricts patients' and carers' ability to shape the material environment of the home as a place to live and be cared for in palliative stages of illness. Alternative discourses are suggested both from the empirical data as well as other research, which may offer patients and carers the possibility of reclaiming power over the home and their subjectivities. Finally, the dichotomy between the home and hospital, mediated via technology, is posited as being problematic. It is argued the dichotomy is false and should be moved away from in order to allow an ethical embrace of technology in palliative care. (shrink)

Quality of care is a prominent discourse in modern health‐care and has previously been conceptualised in terms of ethics. In addition, the role of knowledge has been suggested as being particularly influential with regard to the nurse–patient–carer relationship. However, to date, no analyses have examined how knowledge (as an ethical concept) impinges on quality of care. Qualitative semi‐structured interviews were conducted with 26 patients with palliative and supportive care needs receiving district nursing care and thirteen of their lay carers. Poststructural (...) discourse analysis techniques were utilised to take an ethical perspective on the current way in which quality of care is assessed and produced in health‐care. It is argued that if quality of care is to be achieved, patients and carers need to be able to redistribute and redevelop the knowledge of their services in a collaborative way that goes beyond the current ways of working. Theoretical works and extant research are then used to produce tentative suggestions about how this may be achieved. (shrink)

I criticize a recent paper by Thomas Tymoczko in which he attributes fundamental philosophical significance and novelty to the lately-published computer-assisted proof of the four color theorem (4CT). Using reasoning precisely analogous to that employed by Tymoczko, I argue that much of traditional mathematical proof must be seen as resting on what Tymoczko must take as being "empirical" evidence. The new proof of the 4CT, with its use of what Tymoczko calls "empirical" evidence is therefore not so novel as he (...) maintains. Finally, without attempting to give a full account of the notion of empirical mathematical evidence, I sketch a view showing how the use of calculation injects an empirical ingredient into proof. Ce livre ne vise pas à donner un aperçu de ce que devrait être une religion. Il ne vise pas davantage à mettre en valeur l'argumentaire de l'athéisme. Il entend plutôt montrer qu'une personne qui se dit parfaitement agnostique et adepte d'une philosophie qui valorise la rationalité avant tout peut parfaitement être croyante, voire même pratiquante, sans qu'il y ait là la moindre contradiction. Dans une telle démarche, l'armature cognitive à laquelle de nombreux croyants ont jugé approprié d'arrimer leur foi (...) risque de se voir quelque peu déboulonnée. Et nombre d'arguments qui ont fortement contribué à miner les croyances religieuses pourraient s'en trouver passablement affaiblis. Dans les deux cas, tout découlerait de la simple reconnaissance du fait que la foi n'est en rien un savoir. Croire, expose Maurice Lagueux, en un Dieu transcendant ne veut pas dire connaître quoi que ce soit à propos d'un monde transcendant qui, par définition, échappe à nos capacités cognitives et est conçu comme le lieu de mystères insondables. Croire veut dire avoir sincèrement confiance en ce qui pourrait nous réserver l'existence de ce Dieu qui, toutefois, demeure inconnaissable. On peut se reconnaître résolument croyant sans s'estimer moins agnostique pour autant. (shrink)

Au début du XIXe siècle, la science, en apportant à la fois des connaissances et une méthode, a permis la naissance de la médecine moderne. Débarrassé des idées reçues et des creuses spéculations, le médecin s'est mis à confronter les signes (...) cliniques avec les lésions anatomiques puis avec les données biologiques, pour définir et classer les affections une à une. Démarche d'une prodigieuse efficacité : en deux siècles, l'espérance de vie est passée de vingt-sept à soixante-dix-sept ans. Mais ces progrès ont fait négliger l'aspect psychologique de la santé. Drogues licites et illicites, maladies sexuellement transmissibles, accidents, suicides : la mortalité des jeunes augmente aujourd'hui. Alors que la victoire semblait à portée de main, le médecin découvre qu'un corps ne petit être sain si l'esprit est malade. Devant le mal-être contemporain, en conservant la rigueur intellectuelle et méthodologique qui lui a valu ses succès, la médecine individuelle et curative doit aussi devenir collective et préventive, tenir compte d'une nouvelle donne économique et sociologique. Ces deux essais tournent autour des deux thèmes de prédilection de Maurice Clavelin. Le premier, Galilée ou la naissance de la science moderne, interroge l'un des tournants les plus importants de l'histoire des sciences : le " moment Galilée ". L'auteur montre que ce n'est pas une simple critique des idées traditionnelles ou une meilleure attention portée aux données de l'observation qui caractérise la science de ce génie. Rallié aux théories de Copernic, Galilée crée une vraie science mathématisée du (...) mouvement, véritable révolution conceptuelle qui ouvre une ère nouvelle à la spéculation sur la Nature, et signe ainsi l'acte de naissance de la science moderne. Le second aborde sous un angle inédit le rapport entre science et philosophie. Il montre qu'au début du XXe siècle, et après une longue stagnation, la théorie empiriste de la connaissance reprend forme et rigueur sous l'influence combinée de la logique symbolique et de l'interprétation logiciste des mathématiques. He presents an early account of the impersonal “neuter” in subject-less experiences like “alienation,” “alteration,” “dispersion,” “disappearance,” and “absence.” These experiences of strangeness threaten thought, which is only “itself and for-itself its own experience.” Relatedly, they also reveal “the neutrality of being or neutrality as being.” With reference to both Emmanuel Levinas and Martin Heidegger, Blanchot clarifies the (...) meaning and exigency of a neuter that can take “upon itself the reality of alienation.”. This article is based on the findings of a study that elicited the views of terminally ill patients, their carers and bereaved carers on the palliative care services they received. It explores the range of ethical issues revealed by the data. Although the focus of the original study was on community services, the participants frequently commented on all aspects of their experience. They described some of its positive and negative aspects. Of concern was the reported lack of sensitivity to the (...) role of the family among health professionals. The family, as carers, service users and advocates, represent a challenge to professional boundaries and the ethical norms of confidentiality and best interest. The accounts reveal the complexity of the ethical issues that characterize terminal care, issuing specific ethical challenges to nurses and other health professionals involved in this field.
