BackgroundHuntington’s disease has a poor prognosis. For HD patients in the Netherlands, one way of dealing with their poor prognosis is by drawing up an advance euthanasia directive. Little is known about the perspectives of HD patients on their AED.AimTo gain insight into patients’ views on and attitudes towards their AED, and changes over time.MethodsA longitudinal qualitative interview study using 1 to 6 semi-structured interviews over a period of maximum three years. Nine HD patients who either had an AED or (...) were thinking about drawing it up participated in this study.ResultsWe identified two themes that characterize patients’ perspectives on their AEDs: general character of the AED; uncertainty around their AED. Ad The conditions that the participants described in their AED were generally not very specific for the person. Mostly they were general notions of unbearable suffering. Familiarity with HD in the family could play a role in drawing up an AED. Ad Participants generally were aware of the tentative character of their AED and could have doubts concerning their own willingness or the willingness of others in the future. Sometimes these doubts were so great, that it prevented them from drawing up an AED. However, patients did not alter their AED during the follow-up period or changed in their view or attitude on their AED.ConclusionHD patients that draw up an AED usually describe general conditions for euthanasia and recognize that these conditions may change as the disease progresses. An AED or the wish to draw one up may be a good conversation starter for conversations about goals and preferences for future care. (shrink)

Burnout is characterized by emotional exhaustion and caused by exposure to excessive and prolonged stress related to job conditions. Moreover, burnout is highly prevalent among health care professionals. The aim of this study is, first, to examine the mediating role of social support over the effect of burnout in health care professionals and, second, to explore potential gender differences. A convenience sample of 1,035 health professionals from Ecuador, including 608 physicians and 427 nurses, was surveyed using the Maslach Burnout Inventory, (...) Social Support Survey, and General Health Questionnaire as measures of burnout, social support, and general health, respectively. Social support was found to mediate the negative effects of burnout on health regardless of gender. Differences across the three dimensions of burnout and health are further discussed, along with their implications for designing effective burnout interventions for health care professionals in Ecuador. The article is devoted to Miguel Servet (Michael Servetus, Spain – Miguel Serveto, also known as Miguel Servet, Miguel Serveto, Revés, or Michel de Villeneuve) – the XVI century Spanish thinker who was recognized as a heretic and burnt at the stake in Geneva in 1553.The author discusses the specifics of Servet’s philosophical system, the scientific background of his system, and his key ideas that have become a matter for auto-da-fė. The author argues that Servet’s concept was aimed to reconstruct (...) the “pure” Christian dogma, although he was not a professional theologist. The author claims that Servet used the deep knowledge in geography, astronomy, medicine, linguistics to create his complex system which was aimed to rescue the mankind from total destruction (Apocalypse). The author claims that Servet’s system could be considered as an example of interdisciplinary approach in the pre-disciplinary epoch. Moreover, Servet’s approach to the problem of personal salvation and the transformation of the humane nature is based on empirical scientific data. This article combines the model of critical spatiality with collective memory and religious imagination in order to create a theoretical basis from which to study biblical texts. The combination of critical spatiality, collective memory and religious imagination amount to remembered space as hermeneutical method. In order to be able to employ remembered space as hermeneutical method, as applied to Psalm 90 in this article, a number of aspects must be considered. These aspects include the different dimensions of space as per (...) spatial theory, the bases of ancient Near Eastern worldview and spatial orientation, as well as the theoretical principles of collective memory.Contribution: After considering all of these aspects, the article argues that remembered space provides a fresh perspective on the interpretation of biblical texts, highlighting the spatial notions, collective memory and imaginative outcomes underlying biblical texts. As a case in point, Psalm 90 employed remembered space through imaginative remembering to reimagine stability, identity and faith in a post-exilic time of crisis, instability and trauma. (shrink)

The article discusses the problems of philosophical geography, philosophical multipolarity, georationality. The debates on these issues are becoming interdisciplinary. Specialists in Eastern philosophies and cross-cultural communications as well as epistemologists, scientific methodologists, cognitive scholars, synergists became participants of the discussion held at the Institute of Philosophy of the Russian Academy of Sciences. The problem of Eurocentrism in academic philosophy has become the main topic of discussion. The opponents of the “regional” multipolarity argued that the Western European tradition of rationality was (...) an example of the rationality per se. The proponents, who relied heavily on the ad professionem arguments, advocated multipolarity and demonstrated the irreducibility of the world-pictures, languages and ontologies of philosophical systems. A point of view was expressed that Eurocentrism had an ideological background. The disputants noted that there was need to redefine or clarify such fundamental concepts as logic, rationality, scientific rationality, universalism, processuality. The redefinition of the concept of rationality is for the studies of various types of rationality outside European cultures. Methodologists of science drew attention that the cognitive problems of intercultural communications turned out to be similar to the problems faced by theory of complexity. The analysis of philosophical, logical and other cases in various “zones of exchange” reveals a plurality of directions in the global and local socio-cultural spaces. The panelists raised the question of the role of philosophy in the global world and its responsibility in uniting nations. The authors of the article draw attention to many functions that the Western European conceptual system performs in the formation of philosophical language, mentality and communication. The dynamics of philosophical discussions, the dialogue of the West and the East lead to the clarification of philosophical positions, the emergence of new concepts and meanings as well as the formation of discourses of the future. In many areas of the social and behavioral sciences, the nature of the experiments and theories that best capture the underlying constructs are themselves areas of active inquiry. Integrative experiment design risks being prematurely exploitative, hindering exploration of experimental paradigms and of diverse theoretical accounts for target phenomena.

BackgroundClinical ethics consultations can be complex interventions, involving multiple methods, stakeholders, and competing ethical values. Despite longstanding calls for rigorous evaluation in the field, progress has been limited. The Medical Research Council proposed guidelines for evaluating the effectiveness of complex interventions. The evaluation of CEC may benefit from application of the MRC framework to advance the transparency and methodological rigor of this field. A first step is to understand the outcomes measured in evaluations of CEC in healthcare settings. ObjectiveThe primary (...) objective of this review was to identify and map the outcomes reported in primary studies of CEC. The secondary objective was to provide a comprehensive overview of CEC structures, processes, and roles to enhance understanding and to inform standardization.MethodsWe searched electronic databases to identify primary studies of CEC involving patients, substitute decision-makers and/or family members, clinicians, healthcare staff and leaders. Outcomes were mapped across five conceptual domains as identified a priori based on our clinical ethics experience and preliminary literature searches and revised based on our emerging interpretation of the data. These domains included personal factors, process factors, clinical factors, quality, and resource factors. ResultsForty-eight studies were included in the review. Studies were highly heterogeneous and varied considerably regarding format and process of ethical intervention, credentials of interventionist, population of study, outcomes reported, and measures employed. In addition, few studies used validated measurement tools. The top three outcome domains that studies reported on were quality, process factors, and clinical factors. The majority of studies examined multiple outcome domains. All five outcome domains were multidimensional and included a variety of subthemes.ConclusionsThis scoping review represents the initial phase of mapping the outcomes reported in primary studies of CEC and identifying gaps in the evidence. The confirmed lack of standardization represents a hindrance to the provision of high quality intervention and CEC scientific progress. Insights gained can inform the development of a core outcome set to standardize outcome measures in CEC evaluation research and enable scientifically rigorous efficacy trials of CEC. (shrink)

Aging is associated with decreased functional connectivity in the main brain networks, which can underlie changes in cognitive and emotional processing. Neurofeedback is a promising non-pharmacological approach for the enhancement of brain connectivity. Previously, we showed that a single session of infra-low frequency neurofeedback results in increased connectivity between sensory processing networks in healthy young adults. In the current pilot study, we aimed to evaluate the possibility of enhancing brain connectivity during aging with the use of infra-low frequency neurofeedback. Nine (...) females aged 52 ± 7 years with subclinical signs of emotional dysregulation, including anxiety, mild depression, and somatoform symptoms, underwent 15 sessions of training. A resting-state functional MRI scan was acquired before and after the training. A hypothesis-free intrinsic connectivity analysis showed increased connectivity in regions in the bilateral temporal fusiform cortex, right supplementary motor area, left amygdala, left temporal pole, and cerebellum. Next, a seed-to-voxel analysis for the revealed regions was performed using the post- vs. pre-neurofeedback contrast. Finally, to explore the whole network of neurofeedback-related connectivity changes, the regions revealed by the intrinsic connectivity and seed-to-voxel analyses were entered into a network-based statistical analysis. An extended network was revealed, including the temporal and occipital fusiform cortex, multiple areas from the visual cortex, the right posterior superior temporal sulcus, the amygdala, the temporal poles, the superior parietal lobule, and the supplementary motor cortex. Clinically, decreases in alexithymia, depression, and anxiety levels were observed. Thus, infra-low frequency neurofeedback appears to be a promising method for enhancing brain connectivity during aging, and subsequent sham-controlled studies utilizing larger samples are feasible. As improvements in neuroscience have enabled a better understanding of disorders of consciousness as well as methods to treat them, a hurdle that has become all too prevalent is the denial of coverage for treatment and rehabilitation services. In 2011, a settlement emerged from a Vermont District Court case, Jimmo v. Sebelius, which was brought to stop the use of an “improvement standard” that required tangible progress over an identifiable period of time for Medicare coverage of services. While the use (...) of this standard can have deleterious effects on those with many chronic conditions, it is especially burdensome for those in the minimally conscious state, where improvements are unpredictable and often not manifested through repeatable overt behaviors. Though the focus of this paper is on the challenges of brain injury and the minimally conscious state, which an estimated 100,000 to 200,000 individuals suffer from in the United States, the post-Jimmo arguments presented can and should have a broad impact as envisioned by the plaintiffs who brought the case on behalf of multiple advocacy groups representing patients with a range of chronic care conditions. A Lorentz-invariant gauge theory for massive fermions on R × S 3 spacetime is built up. Using the symmetry of S 3,we obtain Dirac-type equation and derive the expression of the fermionic propagator. Finally, starting from the SU(N) gauge-invariant Lagrangian, we obtain the set of Dirac-Yang-Mills equations on R × S 3 spacetime, pointing out major differences from the Minkowskian case.

The tetradic Lorentz-gauge invariant formulation of the SU(2) × U(1) theory in S3 × R space-time is presented and the general gauge covariant Dirac-Klein-Gordon-Maxwell-Yang-Mills equations are derived. A direct comparison of these equations to those of the SU(2) × U(1) gauge theory on Minkowskian background points out major differences effectively induced by the minimally coupling to S3 × R gravity.

Introduction: The actions taken by the government to deal with the consequences of the coronavirus diseases 2019 pandemic caused different levels of restriction on the mobility of the population. The need to continue offering physical exercise to individuals after stroke became an emergency. However, these individuals may have barriers to adhere to the programs delivered remotely. There is a lack of evidence related to adherence, attendance, safety, and satisfaction of remote exercise programs for this population.Objective: The aim was to evaluate (...) adherence and barriers to attend a remote physical exercise program for individuals after stroke. We aimed to identify adherence and attendance rate of the remote physical exercise program ; to identify the safety of a remote physical exercise program ; and to identify the overall experience to participate in a remote program.Materials and methods: This is a longitudinal study, including 36 stroke survivors who already attended a face-to-face physical exercise program prior to the COVID-19 pandemic. The remote physical exercise program included sessions for 2 days/week for a duration of 22 weeks, with a total of 44 sessions, which were delivered asynchrony via recorded video sessions. As outcome measures, we performed two questionnaires to identify attendance, barriers, safety, and overall experience related to the program.Results: The adherence rate was 86. The attendance rate was 19, with a total of 8 sessions. The main barriers for lower attendance rate were as follows: lack of motor skills and physical fitness to workout in 80 reports, followed by no exercise companion in 44 reports. The remote physical program has been shown to be safe, and the overall experience was positive from the perspectives of the participants and the family members.Conclusion: Although the adherence rate was high, the attendance rate was low on the remote physical exercise program. The main barriers to attending the program remotely reflect the need of tailoring individually an asynchrony mode of delivering the sessions to individuals after stroke. Our results also indicate how the COVID-19 impacted the health conditions of stroke survivors. The program was safe, and the overall experience indicated a change in the mental, physical, and social health of individuals after stroke and their family members. (shrink)

We present the model construction technique called Linear Realizability. It consists in building a category of Partial Equivalence Relations over a Linear Combinatory Algebra. We illustrate how it can be used to provide models, which are fully complete for various typed λ-calculi. In particular, we focus on special Linear Combinatory Algebras of partial involutions, and we present PER models over them which are fully complete, inter alia, w.r.t. the following languages and theories: the fragment of System F consisting of ML-types, (...) the maximal theory on the simply typed λ-calculus with finitely many ground constants, and the maximal theory on an infinitary version of this latter calculus. This paper presents a streamlined taxonomy for categorizing offensive language in Arabic, specifically Modern Standard Arabic (MSA) and the Levantine dialect. Addressing a gap in the existing literature, which has mainly focused on Indo-European languages, our taxonomy divides offensive language into seven levels (six explicit and one implicit). We adapted our framework from the simplified offensive language (SOL) taxonomy by (Lewandowska-Tomaszczyk, Barbara, Slavko Žitnik, Anna Bączkowska, Chaya Liebeskind, Jelena Mitrovic & Giedre Valunaite Oleškeviciente. 2021a. Lod-connected offensive language ontology and tagset (...) enrichment. In Shubert R. Carvalho & Renato R. Souza (eds.), Proceedings of the workshops and tutorials held at ldk 2021 co-located with the 3rd language, data and knowledge conference, Vol. 3064, 135–150. CEUR Workshop Proceedings), customizing it to reflect the unique linguistic and cultural nuances of Arabic. To validate this taxonomy, we created a new dataset from various social media platforms, primarily focusing on Twitter. This dataset was manually curated by human annotators and is described in detail within the paper, serving as both a validation tool for our taxonomy and a foundation for future research on offensive language detection in Arabic. Initial analysis of the dataset reveals complex patterns of offensive expressions in MSA and Levantine Arabic, underscoring the need to account for linguistic and cultural variations in studying online abuse. Our taxonomy and dataset are vital for advancing research in Arabic sociocultural studies, natural language processing, and linguistic analysis, and contribute to the study of low-resource languages. This article examines colonialism, the regime of whiteness, and feminism; it sketches possible genealogies of theories and practices in order to design an aesthetic of radicality or a radical aesthetic that is insurgent and defiant, based on histories and knowledge. We know that aesthetics is a colonial formation that historically and currently privileges the white European bourgeois who could speculate on the beautiful and the good, while genocidal practices and slave trade were carried out from European soil in other parts (...) of the world. Similarly, the Shoah, the still unthinkable genocide that happened on European soil in the twentieth century and shaped the European present, is still, at its base, covered by rhetorically empty constructions of aesthetic perception, practices, and theories. A radical aesthetics must realign all these categories, not only with a cynical gesture of inversion but to situate them categorically and ethically elsewhere. (shrink)

El propósito de este ensayo es discutir si existe una tal cosa como una moral cristiana “específica” y cómo hemos de definirla y comprenderla. Sugiero que si existe, ella no debería ser entendida como un sistema moral que sería válido en un sentido realista, “objetivista” o convencional, sino que más bien su especificidad debería basarse en la clase de motivación a la que daría lugar. Esto quiere decir que lo que pudiéramos definir como una moral cristiana genuina es una nueva (...) ontología, una nueva manera de concebir a los seres humanos, más que un sistema normativo cuya interpretación descansaría en la autoridad y no en su validez interna o intrínseca. Su fundamentación ha de ser, entonces, “postconvencional”. Intento mostrar este punto apoyada en distintas fuentes teológicas, tales como en la teología paulina y en su impacto en distintos teólogos contemporáneos de la moral. Todos los teólogos contemporáneos citados por mí están de acuerdo en que la revelación evangélica hace posible una motivación moral autónoma que se apoya en el discernimiento personal, más que en una pretendida opinión autorizada. Over the last decade many emerging economies, and in particular Brazil, have established themselves as major players in global food animal production. Within these countries much of the increase in food animal production has been achieved by the adoption of intensive housing systems similar to those found in most industrialized countries. However, it is now well established that many of these systems are associated with numerous welfare problems, particularly with respect to restriction of movement. Previous work has shown that people (...) living in industrialized’ countries broadly support farm animal welfare reform, and that similar criticisms may be voiced from citizens living in developing countries as they become more aware of confinement housing and potentially contentious husbandry practices. Given the developments that have taken place in other countries, there are lessons that could be learned and applied by emerging economies that would undoubtedly ease or prevent the challenges observed in other countries. Thus, we briefly describe the vehicles used by different countries when addressing animal welfare that may provide insights into identifying possible challenges and potential solutions for Brazil and other emerging economies. Where available we review the associated science and identify gaps where more research is needed. We conclude by providing a possible roadmap on how farm animal welfare reform may be addressed in emerging countries. Solutions will need to be tailored, culturally relevant, and science must play a key role in supporting animal welfare reform in the emerging countries. (shrink)

Corporate social responsibility has grown on the corporate agenda and is at the heart of today’s corporate culture. While much research has examined CSR strategies and effects, the effects of post-crisis CSR communication have received relatively little academic attention. Therefore, this paper uses two experimental studies to examine several key contingency factors that influence consumers’ responses to post-crisis CSR initiatives. Results suggest that consumers demonstrate more favorable responses when a company launches a CSR initiative congruent with the crisis issue, or (...) when the crisis is the result of an accident rather than a transgression. Further, the congruence between the crisis issue and the pre-crisis CSR initiative moderates the consistency effects between pre- and post-crisis initiatives. Such findings should be understood by considering the mediating role of corporate CSR motives’ consumer attributions, which was evidenced in this study. This study theoretically contributes to an improved understanding of the underlying mechanism of the post-crisis CSR information process and managerially contributes to the strategic development of effective post-crisis CSR initiatives given a particular situation.
