BackgroundFrench military doctors are currently deployed in the Sahel to support the armed forces of Operation Barkhane, in medical or surgical units. As well as supporting French soldiers, their other missions are diverse and complex: medical assistance to civilians and persons under control (PUC), advice to commanding officers. These tasks can create ethical dilemmas when decisions are forced upon doctors that may be in conflict with medical values or fundamental principles. Little is known about the specific dilemmas experienced by French (...) military doctors in overseas operations. We therefore conducted a qualitative study among doctors and surgeons recently deployed to the Sahel to explore and better understand this question.MethodSemi-structured, face-to-face interviews were conducted with 20 French military doctors or surgeons deployed since January 2016 in medical or surgical facilities in Mali and Chad.ResultsAll interviewed doctors reported having faced several ethical dilemmas during missions. All reported dilemmas involved the treatment of civilians (while delivering community medical assistance) or of PUC. The dilemmas involved choices as to which patients to treat, the use of care as a means to an end by military authorities, and the level of care attainable in the absence of any possible hospital follow-up. Questions of delivering care at the risk of their own safety or the mission’s and of treating openly hostile patients were also brought up. Several dilemmas stemmed from the dual loyalty problem, namely the conflict between military doctors’ duty of care to patients and to the military institution, but this was not the only factor involved. Contextual factors (restricted resources and security constraints) and psychological factors (especially hostility towards the enemy) were also associated with many of the reported dilemmas.ConclusionThis is the first reported study focusing on the ethical dilemmas encountered by French military doctors in overseas operations. It provides unique insights into their ethical experiences and should prove useful in improving operational training for healthcare personnel deployed on overseas missions. (shrink)

The outcome of very preterm infants is marked by the development of complications that can have an impact on the quality of life of the children and their families. The concept of quality of life and its evaluation in the long term raise semantic and ethical problems for French physicians in perinatal care. Our reflection aims to gain a better understanding of the representations surrounding quality of life in neonatal medicine.

CB1 receptors are functionally present within brain mitochondria, although they are usually considered specifically targeted to plasma membrane. Acute activation of mtCB1 alters mitochondrial ATP generation, synaptic transmission, and memory performance. However, the detailed mechanism linking disrupted mitochondrial metabolism and synaptic transmission is still uncharacterized. CB1 receptors are among the most abundant G protein-coupled receptors in the brain and impact on several processes, including fear coping, anxiety, stress, learning, and memory. Mitochondria perform several key physiological processes for neuronal homeostasis, including (...) production of ATP and reactive oxygen species, calcium buffering, metabolism of neurotransmitters, and apoptosis. It is therefore possible that acute activation of mtCB1 impacts on these different mitochondrial functions to modulate synaptic transmission. In reviewing and integrating across the literature in this area, we describe the possible mechanisms involved in the regulation of brain physiology by mtCB1 receptors. Activation of mitochondrial CB1 but not of plasma membrane CB1 decreases brain mitochondrial activity, glutamatergic synaptic transmission, and memory performance. Several processes can be involved in this alteration of brain physiology, including mitochondrial ATP and ROS production, axonal mitochondrial transport, Ca2+ homeostasis, and/or metabolism of the neurotransmitter glutamate. This study aims to investigate the relationship between job burnout and employee performance in the feed industry during the ASF outbreak. Further, the researchers employed a descriptive-correlational research design in order to analyze the acquired data and produce pertinent findings. Thus, the researchers gathered data from one hundred two (102) feed industry employees. The Maslach Burnout Inventory (MBI) and Individual Work Performance Questionnaire (IWPQ) were employed to ascertain the extent of job burnout experienced by the respondents and evaluate employee performance, (...) respectively. Based on the statistical analysis, there is a significant relationship between job burnout and employee performance. Moreover, the study's findings were thoroughly analyzed and discussed. (shrink)

Within the research community, it is generally accepted that consent processes for research should be culturally appropriate and tailored to the context, yet researchers continue to grapple with what valid consent means within specific stakeholder groups. In this study, we explored the consent practices and attitudes regarding essential information required for the consent process within hospital-based trial communities from four referral hospitals in Vietnam. We collected surveys from and conducted semi-structured interviews with study physicians, study nurses, ethics committee members, and (...) study participants and family members regarding their experiences of participating in research, their perspectives toward research, and their views about various elements of the consent process. In our findings, we describe three interrelated themes related to the consent process: words and regulation; reimbursement, suspicions, and joining; and responsibilities. In general, stakeholders had highly varied perspectives of nghiên cứu and researchers used varying levels of detail regarding all aspects of the study in the consent process to build trust with and/or promote potential research participants’ choices about taking part in research. Findings additionally highlight how researchers felt that offering financial reimbursements in a hospital setting, where payment for services was routine, would be unfamiliar to participants and could raise suspicions about the research. Participants, however, focused their discussions on reimbursement or alternative reasons for joining the study, such as health related benefits or altruism. Finally, participants often relied on their physician to help them decide about joining a study or not. Further research is needed to understand how researchers and participants make sense of and practice consent, and how that impacts participants’ decision-making about research participation. To promote valid consent within this context, it is important to engage with hospital-based trial communities as a whole. The data from this study will inform future research on consent, guide the revisions of consent related policies within our research sites and point to several larger issues surrounding researcher-participant expectations, communication, and trust. Dans le traité De la divination dans le sommeil, Aristote entend réfuter la thèse d’une origine divine des rêves qui semblent annoncer les événements futurs. Pour cela, il ne se contente pas de soutenir une thèse différente. Il dénonce également la faiblesse épistémologique de la pratique divinatoire, en proposant un modèle scientifique alternatif : une méthode de classification des rêves et de division par épuisement, qui exclut les rêves inspirés. Cette méthode conduit à souligner le caractère irréductible des faits de (...) hasard et à admettre le caractère inexplicable d’un grand nombre de rêves « véridiques ». La notion de coïncidence ( sumptôma ) joue sur ce point un rôle essentiel. Ce rôle est à la fois descriptif et critique : il permet de définir la classe des rêves dont la dimension véridique est absolument fortuite, et il dénonce l’illusion commune qui les relie à des événements réels. De ce point de vue, le chapitre 1 du De divinatione apporte une contribution, modeste mais spécifique, à la théorie des modalités physiques. (shrink)

In the last lines of Politics VII, 3, Aristotle states that the happy city acts nobly. This implies that the city has a practical life, and that this life has its end in itself. This claim seems to contradict the famous distinction, which has been made elsewhere by Aristotle, between the practical and theoretical lives. It is argued in this paper that there is, here, neither contradiction nor inconsistency in Aristotle’s conception of human action. Some readings, according to which this (...) passage deals with the role of the philosophers in the happy city, are also ruled out. The proposed solution consists in showing that this text refers to a broader conception of action, one that is beyond the sphere of human practical activities: any genuine praxis has its goal in itself, even when it produces external effects. Bien que cette formule, en première approche, semble aller dans le sens d’une téléologie cosmique ou globale, elle conduit en fait à privilégier une téléologie relative, c’est-à-dire locale, qui opère à l’échelle des êtres vivants. Elle s’applique en effet, (...) non pas de manière absolue, mais relativement aux possibilités définies par la nature de l’espèce (propriétés spécifiques et génériques). Toutefois, elle n’exprime pas seulement le rapport à la finalité organique stricto sensu, mais aussi les conditions nécessaires externes du mouvement animal, conditions qui définissent, pour les espèces et les genres, un régime commun d’organisation. First collection of essays entirely devoted to the inscription of Diogenes of Oinoanda. The texts of Diogenes of Oinoanda (2nd century AD) who invited his readers to an Epicurean life is the largest ancient inscription ever discovered. Over 70 new finds have increased the number of known wall blocks and fragments to nearly 300, offering new insights into Diogenes' distinctive presentation of philosophy. This collection of essays discusses the philosophical significance of these discoveries and is the first of this kind (...) entirely devoted to Diogenes of Oinoanda. Particular attention is paid to his philosophical aims and polemical strategies. Diogenes was apparently well aware of still ongoing philosophical debates, engaging in polemics against Presocratic philosophers, Platonics, and especially Stoics. His views about important issues like happiness, fear, old age, and the afterlife are explained on the bases of Epicurean physics and theology, ethics, politics, theory of knowledge, and psychology. "The aim of this paper is to inscribe the argument on the Practical Syllogism of De motu animalium, chapter 7, in its immediate context. Now this framework is by no means a logical one. The De motu is a psychological or zoological treatise, whose scope belongs to natural philosophy. Moreover De motu's Practical Syllogism cannot be restricted to practical, i.e. anthropological, cases, like akrasia. It is inserted in a physiological development regarding the biological explanation of animal emotions, reactions and actions. (...) Thus, the necessity of the conclusion – here the action itself – does not mean that the conditions for acting are reducible to propositions or to logical connections: rather it stands for the necessity that governs the agent-patient relation in animal movement.". Pourquoi Aristote semble-t-il refuser aux artefacts le statut de substances? Cette question fait l’objet d’interprétations divergentes. Partant d’un passage généralement négligé de Métaphysique H 3 et en s’appuyant sur plusieurs textes de Metaph. Z 7-9, on défend la thèse selon laquelle il ne suffit pas d’évoquer le fait que les formes des êtres naturels sont éternelles. La différence entre les artefacts et les êtres naturels repose surtout sur le statut ontologique et la fonction de leurs formes respectives. Le livre H (...) de la Métaphysique, pris dans son ensemble, fournit plusieurs arguments permettant d’établir que les formes d’artefact ne sont pas des formes substantielles, des formes de plein droit, mais seulement des « différences » de la matière prochaine et de simples déterminations « analogues » à la substance proprement dite. Elles ont un statut intermédiaire entre la matière prochaine et la forme substantielle ; l’artefact n’est lui-même qu’un composé accidentel. (shrink)

Les atomistes de l'Antiquité doivent résoudre le problème suivant : comment expliquer la formation des mondes et des corps composés à partir de corps élémentaires - les atomes - qui n'ont en eux-mêmes aucune propriété cosmogonique? La solution d'Épicure, selon la Lettre à Hérodote, § 45, consiste à adopter une conception subtile de la causalité atomique : les atomes ne sont pas seulement des composants, ou des causes matérielles, mais aussi, par l'intermédiaire d'un processus de sélection spontanée, les véritables agents (...) de la production des mondes et des composés. Ancient atomists face the following difficulty : how can the becoming of worlds and compounds be explained by the elementary bodies, when these - the atoms - do not possess any cosmogonie property? Epicurus ' answer, according to Letter to Herodotus, § 45, is a subtle account of atomic causality : atoms are not only components or material causes, but also, through a spontaneous process of selection, the real agents of the production of worlds and compounds. En Éthique à Nicomaque II 4-6, Aristote entend donner une définition cohérente de la vertu éthique. Cependant, de manière assez surprenante, la dernière étape de la définition mentionne le phronimos et par son intermédiaire la sagesse pratique ( phronêsis ), qui est une vertu intellectuelle et non une vertu éthique : la vertu éthique est « une disposition à décider ( hexis proairetikê ), située dans un juste milieu relatif à nous, déterminé par la raison, et comme le déterminerait le (...) prudent ». Pour résoudre cette énigme, on soutient ici que la recherche d’une définition de la vertu éthique est en partie motivée par le débat autour de l’unité de la vertu. En disant que la vertu éthique est proairetikê, Aristote réserve une place à la sagesse pratique dans sa définition. Ce faisant, il renforce sa propre conception de l’unité (ou réciprocité) des vertus : une distinction sans séparation. (shrink)

Rare Disease research has seen tremendous advancements over the last decades, with the development of new technologies, various global collaborative efforts and improved data sharing. To maximize the impact of and to further build on these developments, there is a need for model consent clauses for rare diseases research, in order to improve data interoperability, to meet the informational needs of participants, and to ensure proper ethical and legal use of data sources and participants’ overall protection. A global Task Force (...) was set up to develop model consent clauses specific to rare diseases research, that are comprehensive, harmonized, readily accessible, and internationally applicable, facilitating the recruitment and consent of rare disease research participants around the world. Existing consent forms and notices of consent were analyzed and classified under different consent themes, which were used as background to develop the model consent clauses. The IRDiRC-GA4GH MCC Task Force met in September 2018, to discuss and design model consent clauses. Based on analyzed consent forms, they listed generic core elements and designed the following rare disease research specific core elements; Rare Disease Research Introductory Clause, Familial Participation, Audio/Visual Imaging, Collecting, storing, sharing of rare disease data, Recontact for matching, Data Linkage, Return of Results to Family Members, Incapacity/Death, and Benefits. The model consent clauses presented in this article have been drafted to highlight consent elements that bear in mind the trends in rare disease research, while providing a tool to help foster harmonization and collaborative efforts. Despite a growing body of research by management scholars to understand and explain failures in ethical decision making (EDM), misconduct prevails. Scholars have identified character, founded in virtue ethics, as an important perspective that can help to address the gap in organizational misconduct. While character has been offered as a valid perspective in EDM, current theorizing on how it applies to EDM has not been well developed. We thus integrate character, founded in virtue ethics, into Rest’s (1986) EDM model to (...) reveal how shifting attention to the nature of the moral agent provides critical insights into decision making more broadly and EDM specifically. Virtue ethics provides a perspective on EDM that acknowledges and anticipates uncertainties, considers its contextual constraints, and contemplates the development of the moral agent. We thus answer the call by many scholars to integrate character in EDM in order to advance the understanding of the field and suggest propositions for how to move forward. We conclude with implications of a character-infused approach to EDM for future research. (shrink)

Cet article cherche à montrer qu’amour et politique sont étroitement liés dans le De natura rerum de Lucrèce. D’une part, l’amour-passion, au Livre IV, se révèle aussi vain que le désir du pouvoir politique; d’autre part, le livre V oppose implicitement le désir sexuel des premiers êtres humains au « bien commun » qui gouverne les organisations sociales. En outre, la description, au livre IV, d’une sexualité exempte de passion, ou amour libre, caractérisée par un désir mutuel et un plaisir (...) partagé, fait office de contre-modèle ou de paradigme alternatif face à l’aliénation et à la violence politiques et sociales. En d’autres termes, l’objet de cet article est de montrer qu’amour, sexe et politique font l’objet chez Lucrèce d’une même approche, où l’on peut voir à la fois une composante importante de sa thérapie de l’âme et une profonde défiance à l’égard de la vie sociale et des activités politiques. On s’efforce ici de montrer que l’empirisme épicurien est, non seulement fondé sur les sensations, mais aussi sur des notions. Parce qu’il construit un projet scientifique cohérent, l’épicurisme antique entend se présenter comme un empirisme rationnel, fondé sur un usage méthodique des notions, sous la condition d’un accord constant avec l’expérience. Il conviendra donc de prendre en compte le double objectif de l’épistémologie d’Épicure et de ses successeurs : donner un fondement solide aux hypothèses scientifiques ; parvenir à une méthode (...) du jugement capable d’offrir une vision du monde qui contribue à la poursuite du bonheur. It is argued that the therapy of the soul relies on an overall explanation of the various emotional states, and that fear is at the core of this framework in so far as it is the root of most psychic troubles. The first part deals with fear in the context of the general theory of passions. The second part shows that this basic emotion is the root of (...) injustice and political excessive ambition, in Lucretius’De rerum natura. The third part puts the emphasis on the role of anxiety in passionate love, in the same text. Fear, in its various forms, appears to be the main cause of a number of pathological emotions. It may be concluded that the Epicurean therapy of emotions, far from dealing with separate passions from case to case, is based on a holistic approach of psychic states. (shrink)

Ele descreve o processo de reminiscência ou rememoração, e trata assim da organização voluntária das representações mentais como de seus encaminhamentos involuntários e patológicos. Correlativamente, ele explica em que sentido o homem pode ser agente de suas próprias representações e o princípio de suas associações. (...) Aristóteles aborda, por conseguinte, a propósito da memória, questões fundamentais para a definição dos estados psíquicos, para a teoria do conhecimento, bem como para a concepção do eu ou da identidade pessoal. A análise da memória e da reminiscência conduz assim a uma reavaliação da concepção aristotélica do caráter, fazendo deste último o substrato e o agente de uma história pessoal que não pode ser abstraída de suas condições psicofísicas.
