BackgroundWhile evaluation of ethical aspects in health technology assessment has gained much attention during the past years, the integration of ethics in HTA practice still presents many challenges. In response to the increasing demand for expansion of health technology assessment methodology to include ethical issues more systematically, this article reports on a multi-stage study that aimed at construction of a framework for improving the integration of ethics in HTA.MethodsThe framework was developed through the following phases: 1) a systematic review and (...) content analysis of guidance documents for ethics in HTA; 2) identification of factors influencing the integration of ethical considerations in HTA; 3) preparation of an action-oriented framework based on the key elements of the existing guidance documents and identified barriers to and facilitators of their implementation; and 4) expert consultation and revision of the framework.ResultsThe proposed framework consists of three main components: an algorithmic flowchart, which exhibits the different steps of an ethical inquiry throughout the HTA process, including: defining the objectives and scope of the evaluation, stakeholder analysis, assessing organizational capacity, framing ethical evaluation questions, ethical analysis, deliberation, and knowledge translation; a stepwise guide, which focuses on the task objectives and potential questions that are required to be addressed at each step; and a list of some commonly recommended or used tools to help facilitate the evaluation process.ConclusionsThe proposed framework can be used to support and promote good practice in integration of ethics into HTA. However, further validation of the framework through case studies and expert consultation is required to establish its utility for HTA practice. (shrink)

BackgroundSocio-cultural perceptions surrounding death have profoundly changed since the 1950s with development of modern intensive care and progress in solid organ transplantation. Despite broad support for organ transplantation, many fundamental concepts and practices including brain death, organ donation after circulatory death, and some antemortem interventions to prepare for transplantation continue to be challenged. Attitudes toward the ethical issues surrounding death and organ donation may influence support for and participation in organ donation but differences between and among diverse populations have not (...) been studied.ObjectivesIn order to clarify attitudes toward brain death, organ donation after circulatory death and antemortem interventions in the context of organ donation, we conducted a scoping review of international English-language quantitative surveys in various populations.Study appraisalA search of literature up to October 2020 was performed, using multiple databases. After screening, 45 studies were found to meet pre-specified inclusion criteria.Results32 studies examined attitudes to brain death, predominantly in healthcare professionals. In most, around 75% of respondents accepted brain death as equivalent to death of the person. Less common perspectives included equating death with irreversible coma and willingness to undertake organ donation even if it caused death. 14 studies examined attitudes to organ donation following circulatory death. Around half of respondents in most studies accepted that death could be confidently diagnosed after only 5 min of cardiorespiratory arrest. The predominant reason was lack of confidence in doctors or diagnostic procedures. Only 6 studies examined attitudes towards antemortem interventions in prospective organ donors. Most respondents supported minimally invasive procedures and only where specific consent was obtained.ConclusionsOur review suggests a considerable proportion of people, including healthcare professionals, have doubts about the medical and ethical validity of modern determinations of death. The prognosis of brain injury was a more common concern in the context of organ donation decision-making than certainty of death. (shrink)

The book argues that the category of mother is distinct from the category of woman and that many of the problems mothers face-socially, economically, politically, culturally, and psychologically-are specific to women's work and identity as mothers. Indeed, mothers, arguably more so than women in general, remain disempowered despite forty years of feminism. Mothers, thus, need a feminism of their own, one that positions mothers' concerns as the starting point for a theory and politic of empowerment. O'Reilly terms this new mode (...) of feminism matricentic feminism and the book explores how it is represented and experienced in theory, activism, and practice. The chapter on maternal theory examines the central theoretical concepts of maternal scholarship to include Adrienne Rich's distinction between mothering and motherhood, Sara Ruddick's model of maternal thinking and practice and Patricia Hill Collins' concept of othermothering while the chapter on activism considers the 21st century motherhood movement. Feminist mothering is likewise examined as the specific practice of matricentric feminism and this chapter discusses varies theories and strategies on and for maternal empowerment. Matricentric feminism is also examined in relation to the larger field of Feminist Theory and Women's Studies; here O'Reilly persuasively shows how matricentric feminism has been marginalized in feminist scholarship and considers the reasons for such exclusion and how such may be challenged and changed. This article appeals to Thomas Aquinas in order to offer a construal of the nature of reason arguably preferable to that prominent in the Enlightenment. Thomas’s account neither espouses the notion that reason is devoid of any appetitive influence nor so conflates reason and will as to suggest that thinking becomes essentially a form of willing. His view does respect that the activity of willing is of fundamental import for the life of reason. Since the ultimate object of the will (...) is union with God, it follows that the virtue that specifically promotes the attainment of this end—the virtue of religion—has particular import because it aims at rectifying the will and is the most excellent among the moral virtues. In brief, this virtue promotes the optimal intellectual and moral flourishing of individuals as well as the realization of justice in society. (shrink)

This study elicits a concern to show forth those elements in the theology of Saint Thomas Aquinas that can menaingfully engage with those trends in contemporary hermeneutical philosophy and theology that highlight the conditioned nature of human understanding. The main point of reference in this regard os the hermeneutical philosophy of Hans-Georg Gadamer. At the heart of this hermeneutical enterprise is Thomas's construal of the relationship between intellect and will, a relationship that can be described as one of dynamic reciprocity. (...) A dynamic interaction between intellect and will obtains in both their natural and graced operations. Hence, the title of this book, The Hermeneutics of Knowing and Willing in the Thought of St. Thomas Aquinas. Some might be concerned that the notion of hermeneutics will import the spectre of relativism into Thomas's thought. By guiding the reader through Thomas's doctrine of man as made to the image of the Trinity, his Trinitarian theology, his Christology, and his treatment of grace, the theological virtues of faith and charity, and the Gifts of the Holy Spirit, the author shows how Thomas in fact offers us resources for a theological hermeneutics of objectivity. The criteria for this objectivity, so the author argues, are Trinitarian, Christological, Pneumatological, ecclesial, and Scriptural. It is to be hoped that this book will be read not only by those who have a particular interest in Thomas's theology but also by theologians outside of the Thomistic tradition, particularly those interested in hermeneutics. This volume of essays retrieves the largely unresearched thought and the original ideas of ancient women philosophers and carves out a space for them in the canon. The broad focus includes women thinkers in ancient Indian, Chinese, and Arabic philosophy as well as in the Greek and Roman philosophical traditions.

With a few exceptions, Thomists have by and large failed to engage with the historical and hermeneutical turns in philosophy and theology. This article offers an account of what the beginnings of a Thomistic engagement with recent hermeneutical philosophy might look like. In order to develop such an account, the author turns to arguably the most important contemporary hermeneutical philosopher, namely Hans-Georg Gadamer, as a dialogue partner. Despite claims to the contrary, this article argues that Gadamer does not successfully deal (...) with the specter of relativism. It goes on to show how that the thought of Thomas Aquinas can ably deal with this specter. There are two central elements in the argument put forward. Firstly, the author argues that the human being exists in between God as First Efficient Cause of all that exists and God as Final Cause of all that exists. This existence between God as First Efficient Cause of all that exists and God as Final Cause of all that exists furnishes the connatural context in which human knowing and willing unfolds. Secondly, he considers the dynamics of “interinvolvement” between intellect and will. The upshot of these arguments is a construal of reason that is at once capable of universal truth and hermeneutical in nature. Indeed, the universal range of reason and its hermeneutical dimension are in no way opposed to each other: they are rather dual aspects of one and the same reality. (shrink)

Research consent processes must provide potential participants with the necessary information to help them decide if they wish to join a study. On the Oxford ‘A’ Research Ethics Committee we’ve found that current research proposals mostly provide adequate detail (even if not in an easily comprehensible format), but often fail to support decision making, a view supported by published evidence. In a previous paper, we described how consent might be structured, and here we develop the concept of an Information and (...) Decision Aid (IDA) that can support decision making and be used to guide the dialogue between researcher and potential participant. Our proposal requires limited changes to current processes or paperwork and would provide an easily accessible document for others that the potential participant might approach for advice. It could later be integrated with the Informed Consent Form to ensure all matters of concern to the individual participant have been addressed before consent is formally signed off. (shrink)

The prefrontal cortex is widely believed to play an important role in facilitating people's ability to switch performance between different tasks. We present a biologically‐based computational model of prefrontal cortex (PFC) that explains its role in task switching in terms of the greater flexibility conferred by activation‐based working memory representations in PFC, as compared with more slowly adapting weight‐based memory mechanisms. Specifically we show that PFC representations can be rapidly updated when a task switches via a dynamic gating mechanism based (...) on a temporal‐differences reward‐prediction learning mechanism. Unlike prior models of this type, the present model develops all of its internal representations via learning mechanisms as shaped by the demands of continuous periodic task switching. This advance opens up a new domain of research into the interactions between working memory task demands and the representations that develop to meet them. Results on a version of the Wisconsin card sorting task are presented for the full model and a number of comparison networks that test the importance of various model features. Furthermore, we show that a lesioned model produces perseverative errors like those seen in frontal patients. (shrink)

The use of video for research purposes is something that has attracted ethical attention and debate. While the usefulness of video as a mechanism to collect data is widely agreed, the ethical sensitivity and impact of recording equipment is more contentious. In some clinical settings the presence of a camera has a dual role, as a portal to a reflecting team and as a recording device to obtain research data. Using data from one such setting, family therapy sessions, this article (...) shows how the role played by recording equipment is negotiated in the course of talk and other activities that constitute sessions. Analysis reveals that members of the therapy interaction orient in different ways and for different purposes to the value of recordings. The article concludes that there are layers of benefit to be derived from recording of clinical interactions, including for members themselves, and this has wider implications for the ways in which qualitative research designs in health sciences are evaluated. This paper reviews the fate of the central ideas behind the complementary learning systems (CLS) framework as originally articulated in McClelland, McNaughton, and O’Reilly (1995). This framework explains why the brain requires two differentially specialized learning and memory systems, and it nicely specifies their central properties (i.e., the hippocampus as a sparse, pattern-separated system for rapidly learning episodic memories, and the neocortex as a distributed, overlapping system for gradually integrating across episodes to extract latent semantic structure). We review the (...) application of the CLS framework to a range of important topics, including the following: the basic neural processes of hippocampal memory encoding and recall, conjunctive encoding, human recognition memory, consolidation of initial hippocampal learning in cortex, dynamic modulation of encoding versus recall, and the synergistic interactions between hippocampus and neocortex. Overall, the CLS framework remains a vital theoretical force in the field, with the empirical data over the past 15 years generally confirming its key principles. (shrink)

An animal confronts numerous challenges when constructing an optimal navigational route. Spatial representations used for path optimization are likely constrained by critical environmental factors that dictate which neural systems control navigation. Multiple coding schemes depend upon their ecological relevance for a particular species, particularly when dealing with the third, or vertical, dimension of space.

Scholars have characterised the trial of the life of pleasure in Philebus 20b-21d as digressive or pejorative. I argue that it is neither: it is a thought experiment containing an important argument, in the form of a reductio, of the hypothesis that a life could be most pleasant without cognition. It proceeds in a series of steps, culminating in the precisely chosen image of the jellyfish. Understanding the intended resonance of this creature, and the sense in which it is deprived, (...) is critical for reconstructing the argument, and yields new insight into Plato’s views on the minimal conditions for pleasure. Government responses to globalisation include developing educational leaders as reformers for workforce competitiveness in the knowledge economy. Qualitative research tracked interventions involving national leadership development bodies to acculturate leaders in secondary schools and universities. Acculturating leaders as reformers was mediated through interaction with professional cultures valuing autonomy. Yet mediation supported the government's global positioning through adapting reforms and independent innovation.

Background: The development of implicit tests for measuring biases and behavioral predispositions is a recent development within psychology. While such tests are usually researched within a social-cognitive paradigm, behavioral researchers have also begun to view these tests as potential tests of conditioning histories, including in the sexual domain. Objective: The objective of this paper is to illustrate the utility of a behavioral approach to implicit testing and means by which implicit tests can be built to the standards of behavioral psychologists. (...) Design: Research findings illustrating the short history of implicit testing within the experimental analysis of behavior are reviewed. Relevant parallel and overlapping research findings from the field of social cognition and on the Implicit Association Test are also outlined. Results: New preliminary data obtained with both normal and sex offender populations are described in order to illustrate how behavior-analytically conceived implicit tests may have potential as investigative tools for assessing histories of sexual arousal conditioning and derived stimulus associations. Conclusion: It is concluded that popular implicit tests are likely sensitive to conditioned and derived stimulus associations in the history of the test-taker rather than ‘unconscious cognitions’, per se. The remarks of my exciting and ebullient colleague, John King-Farlow do not clarify what I find most perplexing either in Wittgenstein’s analogy of the builders or in Rhees’ famous comments on it. Professor King-Farlow does, however, lay before us important and challenging allegations as to an incoherent or nonsensical quality of the discussion so far. Before one turns to these allegations, something should be said on behalf of Saint Augustine’s remarks in The Confessions of language-learning.

The posterior cortex, hippocampus, and prefrontal cortex in the Leabra architecture are specialized in terms of various neural parameters, and thus are predilections for learning and processing, but domain-general in terms of cognitive functions such as face recognition. Also, these areas are not encapsulated and violate Fodorian criteria for modularity. Anderson's terminology obscures these important points, but we applaud his overall message.

Several groups of large DNA viruses successfully utilise the rich resource provided by insect hosts. Defining the mechanisms that enable these pathogens to optimise their relationships with their hosts is of considerable scientific and practical importance, but our understanding of the processes involved is, as yet, rudimentary. Here we describe an informatics-based approach that uses comparison of viral genomic sequences to identify candidate genes likely to be specifically involved in this process. We hypothesise that such genes should satisfy two essential (...) criteria, namely, that they should be (i) present in those members of a virus family that infect insects, but absent from those that infect other hosts, and (ii) found in at least two unrelated taxa of insect viruses. These criteria currently identify six groups of viral genes, including one that encodes the fusolin/gp37 proteins. Demonstration that the fusolin/gp37 proteins can enhance oral infectivity of insect viruses provides a primary validation of this approach to the examination of insectvirus relationships.
