We examine ethical considerations in access to facial transplantation, with implications for promoting health equity. As a form of vascularised composite allotransplantation, FT is still considered innovative with a relatively low volume of procedures performed to date by a small number of active FT programmes worldwide. However, as numbers continue to increase and institutions look to establish new FT programmes, we anticipate that attention will shift from feasibility towards ensuring the benefits of FT are equitably available to those in need. This manuscript assesses barriers to care and their ethical implications across a number of considerations, with the intent of mapping various factors relating to health equity and fair access to FT. Evidence is drawn from an evolving clinical experience as well as published scholarship addressing several dimensions of access to FT. :Facial transplantation is emerging as a therapeutic option for self-inflicted gunshot wounds. The self-inflicted nature of this injury raises questions about the appropriate role of self-harm in determining patient eligibility. Potential candidates for facial transplantation undergo extensive psychosocial screening. The presence of a self-inflicted gunshot wound warrants special attention to ensure that a patient is prepared to undergo a demanding procedure that poses significant risk, as well as stringent lifelong management. Herein, we explore the ethics of considering mechanism of injury (...) in the patient selection process, referring to the precedent set forth in solid organ transplantation. We also consider the available evidence regarding outcomes of individuals transplanted for self-inflicted mechanisms of injury in both solid organ and facial transplantation. We conclude that while the presence of a self-inflicted gunshot wound is significant in the overall evaluation of the candidate, it does not on its own warrant exclusion from consideration for a facial transplantation. Most aspects of agriculture in Cuba prior to 1989 were comparable to California: a high energy input, conventional agriculture (based on what the Cubans now call the “classical model”) in which little was done to protect the nation's soils from erosion, loss of fertility, salinization, and other forms of degradation. In stark contrast the new “Alternative Model,” which has been rapidly replacing the previous model since 1989, emphasizes soil conservation and rehabilitation and the general improvement of the nation's soils as (...) the key to sustaining low-input production and attainment of food security. One of the first steps in implementing the new model was the launching of an ambitious program to reclassify, evaluate, and map the nation's soils in great detail, and to interpret the maps for management of sustainable production. A main feature of this program is coordinated fertility trials to determine, for each combination of crop and soil, the minimum quantity of plant nutrients needed to produce the crop. The build up and maintenance of soil fertility and productivity is being accomplished with various organic and mineral amendments and biofertilizers, produced or mined within the country (locally, where possible) and through rational management utilizing cover crops, green manures, crop successions (intercropping and rotations), and other appropriate technologies. Rehabilitation of degraded soils, tillage reduction, reforestation, vermiculture, vermicomposting, and other forms of waste cycling are other features of the new model that are important to soil conservation and maintenance for sustainable production. Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.
Many theists of a traditional bent have been bothered by the apparent tension between God's essential omnipotence and his essential moral goodness. Nelson Pike draws attention to the conflict between these two attributes in his article ‘Omnipotence and God's Ability to Sin’, and there have been many attempts to respond to it since that time. Most of these responses argue that the essential omnipotence and essential goodness of God are not logically incompatible, so that the traditional conception of God is (...) not incoherent; I think the arguments have been largely successful. However, some theists have found the typical responses to Pike less than convincing, and are tempted to surrender the claim that God has moral perfection essentially in favour of the more modest claim that God is morally perfect in the actual world though in some possible worlds God is morally defective. I argue in this paper that this fall-back position is incoherent. More accurately, I argue that a necessary being who is essentially omniscient and essentially omnipotent cannot be contingently morally perfect or contingently morally defective. Any such being is either essentially good or essentially evil. Since the latter alternative seems unattractive, I argue that theists should embrace the essential moral perfection of God. (shrink)
The research presented in this paper focuses on business ethical values inChina, a country in which the process of institutional transformation has left cultural values in a state of flux. A survey was conducted in China and the U.S. by using five business scenarios. Survey results show similarities between the Chinese and American decision choices for three out of five scenarios. However, the results reveal significant differences in rationales, even forsimilar decisions. ABSTRACTThirty years ago, the subfields of emotion and cognition operated relatively independently and the associated science reflected the tacit view that they were distinct constructs. Today, questions about the integration of cognition and emotion are among the most interesting questions in the field. I offer a personal view of the key changes that fuelled this shift over time and describe research from my group that unfolded in parallel and led to the identification of the positivity effect.
Clinical ethics can be viewed as a practical discipline that provides a structured approach to assist healthcare practitioners in identifying, analysing and resolving ethical issues that arise in practice. Clinical ethics can therefore promote ethically sound clinical and organisational practices and decision-making, thereby contributing to health organisation and system quality improvement. In order to develop students’ decision-making skills, as well as prepare them for practice, we decided to introduce a clinical ethics strand within an undergraduate medical curriculum. We designed a (...) programme of clinical ethics activities for teaching and assessment purposes that involved using ethical frameworks to analyse hypothetical and real-life cases in uni- and inter- professional groups. In this paper, we draw on medical student feedback collected over 6 years to illustrate the appeal to students of learning clinical ethics. We also outline the range of benefits for students, healthcare organisations, and the field of clinical ethics arising from tomorrow’s doctors experiencing clinical ethics early in their training. We conclude by briefly reflecting on how including clinical ethics within tomorrow’s doctors curricular can secure and continue future engagement in clinical ethics support services in the UK, alongside the dangers of preparing students for organisational cultures that might not exist. We anticipate the findings presented in the paper will contribute to wider debates examining the impact of ethics teaching, and its ability to inform future doctors’ practice. (shrink)
This paper examines faculty perceptions regarding ethical behavior among colleagues and students, and faculty practices with regard to teaching ethics in three institutions over a 4-year period. Faculty reported an uneven pattern of unethical behavior among colleagues over the period. A majority of business courses included ethics, however as both a specific topic on the syllabus and within course discussions. The percentage of courses with ethics discussions increased in 2006, however, the time allocated to these discussions decreased. These results suggest (...) that faculty are approaching ethics instruction less formally, raising concerns over the success of curriculum integration. (shrink)

The proposal that language has evolved to conform to general cognitive and learning constraints inherent in the human brain calls for specification of these mechanisms. We propose that just as cognition appears to be grounded in cross-modal perceptual-motor capabilities, so too must language. Evidence for perceptual-motor grounding comes from non-arbitrary sound-to-meaning correspondences and their role in word learning.
Design arguments make a case for the existence of God based on examples of apparent design or purposiveness in the natural world. Current versions of the argument proceed, not in terms of analogies between the universe and human artifacts, but as inductive arguments to the best explanation of the data. Theism is offered as the simplest hypothesis that can explain facts such as the mathematical elegance and intelligibility of the laws of the nature. Since most defenders of divine timelessness are even more firmly committed to omniscience, driving a wedge between the two is intended to convert such persons to a temporal view of God. However, I believe that both arguments fail to demonstrate an incompatibility between omniscience and timelessness, and that the objections themselves rest in large part on misunderstandings (...) regarding both the motivation for and substance of the doctrine of divine timelessness. The nineteenth century worked to ensure that the incomplete body did indeed retain a sense of self by creating prostheses to mimic corporeal wholeness. Paul Helm offers a welcome and stimulating treatment of the problem of God's timelessness from a standpoint which affirms the traditional theistic view that God is without time. Works devoted wholly to divine eternity are rare in any case, and the most well-known recent book on the subject, Nelson Pike's God and Timelessness, attempts to discredit the notion of God's atemporality. Although Pike has been joined in his view by many philosophers of religion in recent years, there seem to be (...) an increasing number of those sympathetic to divine timelessness. Helm demonstrates an excellent grasp of the issues in this debate and the literature on the topic, especially that of analytic philosophers of religion. One of the greatest strengths of Helm's own discussion is that it is very thorough and carefully argued while remaining largely free of technical jargon and formal notation. This is a very clear and accessible book; it would make a good text for a graduate seminar and will certainly be required reading for anyone working in this field. (shrink)

Historians influenced by post-structuralism and the linguistic turn and feminist historians concerned to incorporate the category of gender into historical analysis have recently challenged the categories, methodologies, and questions of labor history as it has been practiced in the United States for the past thirty years. Those operating under the influence of the linguistic turn have challenged labor history's foundational assumption of class as both a category of analysis and as a social formation constituted primarily by material and productive relations. (...) They have argued that class and class interest are constituted culturally and discursively rather than materially. Feminist critics have called attention to the importance of gender both as a subject of analysis and as an analytical lens through which to examine the ways in which class and class relations have been constituted on the basis of sexual difference. Their goal is to interrogate the ways in which notions of masculinity and femininity shaped relations of subjugation and domination and governed mechanisms of inclusion and exclusion in work and in the labor movement. These challenges to the field have led some to charge that labor history is undergoing an epistemological crisis. Recent work by labor historians of France in Lenard Berlanstein's collection, Rethinking Labor History: Essays on Discourse and Class Analysis and by feminist American labor historians in Ava Baron's Work Engendered: Towards a New History of American Labor has taken up these challenges and demonstrates the potential of these new analytical frameworks to awaken new debates and produce new knowledge in the field. Essays in both collections suggest that labor history is less in crisis than it is in the process of an epistemological and methodological rebirth. (shrink)

Emmanuel Levinas writes from the phenomenological tradition of Sartre, Merleau-Ponty, Husserl and Heidegger. Yet he differs in some important ways, especially from the latter two.
Recent research has demonstrated that word learners can determine word-referent mappings by tracking co-occurrences across multiple ambiguous naming events. The current study addresses the mechanisms underlying this capacity to learn words cross-situationally. This replication and extension of Yu and Smith (2007) investigates the factors influencing both successful cross-situational word learning and mis-mappings. Item analysis and error patterns revealed that the co-occurrence structure of the learning environment as well as the context of the testing environment jointly affected learning across observations. Learners (...) also adopted an exclusion strategy, which contributed conjointly with statistical tracking to performance. Implications for our understanding of the processes underlying cross-situational word learning are discussed. (shrink)
