Background: Older, frail patients with multimorbidity are at an especially high risk for disease severity and death from COVID-19. The social restrictions proved challenging for the residents, their relatives, and the care staff. While these restrictions clearly impacted daily life in Norwegian nursing homes, knowledge about how the pandemic influenced nursing practice is sparse. Aim: The aim of the study was to illuminate ethical difficult situations experienced by Norwegian nurses working in nursing homes during the COVID-19 pandemic. Research design and (...) participants: The research design involved semistructured individual interviews conducted with 15 nurses working in 8 nursing homes in 3 health regions in Norway, within both urban and rural areas. Ethical considerations: Oral and written information about the study was provided before the participants gave their written consent. The transcribed interviews were de-identified. The study was approved by the Norwegian Centre for Research Data. Findings: Four ethical difficult situations were identified: (a) turning the nursing home into a prison; (b) using medication to maintain peace and order; (c) being left alone with the responsibility; and (d) s. impact on decision-making. Conclusions: The nurses’ ethical challenges were intertwined with external factors, such as national and local guidelines, and the nurses’ own internalized factors, which were connected to their subjective professionality. This duality inflicted emotional distress and gave nurses few opportunities to perform nursing in a professionally sound and safe manner. (shrink)

The up‐ and down‐regulation of the salivary gland secretion protein (Sgs) genes during the third larval instar of Drosophila melanogaster are controlled by fluctuations of the titre of the steroid hormone 20‐hydroxyecdysone (20E). Induction of these genes by a low hormone titre is a secondary response to 20E mediated by products of 20E‐induced ‘early’ genes. Surprisingly, in the case of the Sgs‐4 gene this response also requires a direct contribution of the 20E‐receptor complex. A model is presented which proposes that (...) the Sgs genes, and other 20E‐regulated genes with similar temporal expression profiles, are regulated by complex hormone response units. The hormonal signal is effectively transmitted by these response units only after binding of additional factors, e.g. secretion enhancer binding proteins, which act together in a synergistic manner with the 20E receptor and early gene products to establish a stage‐ and tissuespecific expression pattern. Biomolecules and particularly proteins and DNA exhibit some mysterious features that cannot find satisfactory explanation by quantum mechanical modes of atoms. One of them, known as a Levinthal’s paradox, is the ability to preserve their complex three-dimensional structure in appropriate environments. Another one is that they possess some unknown energy mechanism. The Basic Structures of Matter Supergravitation Unified Theory (BSM-SG) allows uncovering the real physical structures of the elementary particles and their spatial arrangement in atomic nuclei. The resulting physical models (...) of the atoms are characterized by the same interaction energies as the quantum mechanical models, while the structure of the elementary particles influence their spatial arrangement in the nuclei. The resulting atomic models with fully identifiable parameters and angular positions of the quantum orbits permit studying the physical conditions behind the structural and bonding restrictions of the atoms connected in molecules. A new method for a theoretical analysis of biomolecules is proposed. The analysis of a DNA molecule leads to formulation of hypotheses about the energy storage mechanism in DNA and its role in the cell cycle synchronization. This permits shedding a light on the DNA feature known as a C-value paradox. The analysis of a tRNA molecule leads to formulation of a hypothesis about a binary decoding mechanism behind the 20 flavors of the complex aminoacyle-tRNA synthetases - tRNA, known as a paradox. Sin embargo, este tipo de obras ha recibido una atención muy limitada desde el punto de vista lingüístico, a pesar de que presentan características propias de las variedades habladas del árabe. Three principal varieties of mathematical structuralism are compared: set-theoretic structuralism (‘STS’) using model theory, Shapiro's ante rem structuralism invoking sui generis universals (‘SGS’), and the author's modal-structuralism (‘MS’) invoking logical possibility. Several problems affecting STS are discussed concerning, e.g., multiplicity of universes. SGS overcomes these; but it faces further problems of its own, concerning, e.g., the very intelligibility of purely structural objects and relations. MS, in contrast, overcomes or avoids both sets of problems. Finally, it is argued that the modality (...) of MS or a close cousin appears at crucial junctures in both STS and SGS, so that the above outcome is not obviously tendentious. (shrink)

The most distinctive feature of Murdoch's philosophical project is her attempt to reclaim the exploration of moral life as a legitimate topic of philosophical investigation. In contrast to the predominant focus on action and decision, she argues that “what we require is a renewed sense of the difficulty and complexity of the moral life and the opacity of persons. We need more concepts in terms of which to picture the substance of our being” (AD 293).1 I shall argue that to (...) fully appreciate the novelty of this proposal we need to recognize some elements of continuity with analytic methods of philosophical inquiry and themes that belong to continental traditions. On Murdoch's view, the most important question facing moral philosophy is that of whether and how we can become better. In order to describe moral progress and failure, struggle and ascent, we need ethical concepts capable of capturing mental events such as change in mind, self-examination, and redescription. These concepts are presently unavailable, and this lack seriously undermines our attempts to understand the phenomena of morality. Because of this conceptual loss, we succumb to the mistaken idea that moral life coincides with its scattered outer manifestations, i.e. public acts. Instead, Murdoch urges us “the moral life […] is something that goes on continually, not something that is switched off in between the occurrence of explicit moral choices” (SG 37).2 Murdoch's point is that we need to conceive the locus of agency more broadly if we want to understand the complicated machinery of action. According to phenomenal conservatism, seemings can provide prima facie justification for beliefs. In order to fully assess phenomenal conservatism, it is important to understand the nature of seemings. Two views are that (SG) seemings are a sui generis propositional attitude, and that (D2B) seemings are nothing over and above dispositions to believe. Proponents of (SG) reject (D2B) in large part by providing four distinct objections against (D2B). First, seemings have a distinctive phenomenology, but dispositions to believe do not. Second, seemings (...) can provide a non-trivial explanation for dispositions to believe, which wouldn’t be possible if seemings were dispositions to believe. Third, there are some dispositions to believe that are not seemings. Fourth, there are instances of seemings which are not dispositions to believe. I consider and reject each of these objections. The first and third objections rely on a misunderstanding of (D2B). The second objection fails because there are contexts in which an appeal to a previously unknown identity can provide an interesting explanation. The fourth objection overlooks the possibility of finkish and masked dispositions, phenomena which are widely accepted in the dispositions literature. The framework of causal Bayes nets, currently influential in several scientific disciplines, provides a rich formalism to study the connection between causality and probability from an epistemological perspective. This article compares three assumptions in the literature that seem to constrain the connection between causality and probability in the style of Occam's razor. The trio includes two minimality assumptions—one formulated by Spirtes, Glymour, and Scheines (SGS) and the other due to Pearl—and the more well-known faithfulness or stability assumption. In terms of (...) logical strength, it is fairly obvious that the three form a sequence of increasingly stronger assumptions. The focus of this article, however, is to investigate the nature of their relative strength. The comparative analysis reveals an important sense in which Pearl's minimality assumption is as strong as the faithfulness assumption and identifies a useful condition under which it is as safe as SGS's relatively secure minimality assumption. The use of aggregated quality of life estimatesin the formation of public policy and practiceguidelines raises concerns about the moralrelevance of variability in values inpreferences for health care. This variabilitymay reflect unique and deeply held beliefs thatmay be lost when averaged with the preferencesof other individuals. Feminist moral theorieswhich argue for attention to context andparticularity underline the importance ofascertaining the extent to which differences inpreferences for health states revealinformation which is morally relevant toclinicians and policymakers. Sixteenwomen between the ages of 45 and 55 wereenrolled in this pilot study. Theirpreferences regarding five health statesassociated with HRT (menopausal symptoms, sideeffects of HRT, breast cancer, myocardialinfarction, and osteoporosis) were assessed inquantitative terms known as utilities. Twostandard methods, the visual analog scale (VAS) and the standard gamble (SG), were used toassess utility and time preference (calculatedas a discount rate). The wide variability ofresponses underlines the importance oftailoring health care to individual women'spreferences. In the causal inference framework of Spirtes, Glymour, and Scheines, inferences about causal relationships are made from samples from probability distributions and a number of assumptions relating causal relations to probability distributions. The most controversial of these assumptions is the Causal Faithfulness Assumption, which roughly states that if a conditional independence statement is true of a probability distribution generated by a causal structure, it is entailed by the causal structure and not just for particular parameter values. In this paper we (...) show that the addition of the Causal Faithfulness Assumption plays three quite different roles in the SGS framework: it reduces the degree of underdetermination of causal structure by probability distribution; computationally, it justifies reliable causal inference algorithms that would otherwise have to be slower in order to be reliable; and statistically, it implies that those algorithms reliably obtain the correct answer at smaller sample sizes than would otherwise be the case. We also consider a number of variations on the Causal Faithfulness Assumption, and show how they affect each of these three roles. (shrink)

Human-Computer Interaction and games set a new domain in understanding people’s motivations in gaming, behavioral implications of game play, game adaptation to player preferences and needs for increased engaging experiences in the context of HCI serious games. When the latter relate with people’s health status, they can become a part of their daily life as assistive health status monitoring/enhancement systems. Co-designing HCI-SGs can be seen as a combination of art and science that involves a meticulous collaborative process. The design elements (...) in assistive HCI-SGs for Parkinson’s Disease patients, in particular, are explored in the present work. Within this context, the Game-Based Learning design framework is adopted here and its main game-design parameters are explored for the Exergames, Dietarygames, Emotional games, Handwriting games, and Voice games design, drawn from the PD-related i-PROGNOSIS Personalized Game Suite holistic approach. Two main data sources were involved in the study. In particular, the first one includes qualitative data from semi-structured interviews, involving 10 PD patients and four clinicians in the co-creation process of the game design, whereas the second one relates with data from an online questionnaire addressed by 104 participants spanning the whole related spectrum, i.e., PD patients, physicians, software/game developers. Linear regression analysis was employed to identify an adapted GBL framework with the most significant game-design parameters, which efficiently predict the transferability of the PGS beneficial effect to real-life, addressing functional PD symptoms. The findings of this work can assist HCI-SG designers for designing PD-related HCI-SGs, as the most significant game-design factors were identified, in terms of adding value to the role of HCI-SGs in increasing PD patients’ quality of life, optimizing the interaction with personalized HCI-SGs and, hence, fostering a collaborative human-computer symbiosis. This article examines the representation of the United Nations in speeches delivered by its Secretary-General. It focuses on the role of metaphor in constructing a common ‘imagining’ of international diplomacy and legitimizing an international organizational identity. The SG legitimizes the organization, in part, through the delegitimization of agents/actions/events constructed as threatening to the international community and to the well-being of mankind. It is a desire to combat the forces of menace or evil which are argued to motivate and determine the (...) organizational agenda. This is predicated upon an international ideology of humanity in which difference is silenced and ‘working towards the common good’ is emphasized. This is exploited to rouse emotions and legitimize institutional power. Polarization and antithesis are achieved through the employment of metaphors designed to enhance positive and negative evaluations. The article further points to the constitutive, persuasive and edifying1 power of topic and situationally motivated metaphors in speech-making. (shrink)

SG: Carol, thank you for the opportunity to speak (...) with you about your artwork. Having just lost my sister to cancer, your work reminds me of much pain and loss, but also of love and the desire to survive. Before we discuss the work itself, I would like to hear about your journey through cancer, treatment, and recovery. CC: The breast cancer patient’s journey is complex and perilous. When I was healthy, I was ignorant about the magnitude of suffering many cancer patients undergo. And, although I am a private person, I’m now publicly sharing my cancer experience in order to be a voice for those who have suffered through similar treatments—and worse—and those who sadly are no longer with us to tell their story. Being bald and nauseated are not the most awful possible side effects of the treatments, although they are the most well known. Some women survive the cancer but can die from the treatments, or others suffer through the treatments and die anyway. Often, it seems many believe patients can overcome their 572 Carol Collins adversity solely with positive thinking and persistence. However, not discussing the array of physical and psychological damage that even those with good prognoses incur propagates ignorance about this malady. I hope that my artwork and story will promote a deeper understanding of a breast cancer patient’s plight and will encourage more support for research for individualized therapies and prevention. My cancer journey began when I received a breast cancer diagnosis in January 2014. Life just before that point was good; the future was bright. Tendon pain, caused by my flat feet and a lot of exercising, was my only known ailment. But one day a routine mammogram revealed three tumors in my right breast. The radiology oncologist said the masses were highly likely to be cancerous, so he immediately did painful core biopsies at the sites of my three tumors. The next day, it became clear that we needed to act quickly. The biopsy results revealed I had a very aggressive invasive breast cancer with estrogen, progesterone, and HER2 receptors. Breast cancer in premenopausal women grows quickly due to the high level of hormones present. The oncologists and surgeons said I would, at a minimum, need a mastectomy and possibly lymph node resection, and they warned that I would likely need chemotherapy, which would destroy my eggs and ovaries. After learning of my limited options, I approved the removal of one breast and my sterilization. I knew I needed to think rationally and face the giant raven before me head on. However, I did not realize the magnitude and multitude of physical and psychological pains, illnesses, and losses that were soon to come. The first year of cancer treatments inflicted much physical pain. I received three extremely painful injections into my right breast with a radiolabeled colloid just prior to the mastectomy. The pain came from the injected dye, not from the needle. The modified radical mastectomy was more excruciatingly painful than I anticipated, even with painkillers and overnight hospitalization. Breast reconstruction was an option that added more pain and discomfort from weekly fillings of the installed “expander” and additional surgeries. Extreme pain after the mastectomy prevented me from lying down for months. Foolishly, I did not get an injection port installed in my chest. Instead, I endured numerous deep (and often unsuccessful) needle pricks into the seemingly few veins of my left arm (which still had its lymph nodes) that were appropriate for infusions over the period of a year. Shooting pains across my chest and arm were the norm. This paper gives a simple method for providing categorial brands of feature-based unification grammars with a model-theoretic semantics. The key idea is to apply the paradigm of fibred semantics (or layered logics, see Gabbay (1990)) in order to combine the two components of a feature-based grammar logic. We demonstrate the method for the augmentation of Lambek categorial grammar with Kasper/Rounds-style feature logic. These are combined by replacing (or annotating) atomic formulas of the first logic, i.e. the basic syntactic types, by (...) formulas of the second. Modelling such a combined logic is less trivial than one might expect. The direct application of the fibred semantics method where a combined atomic formula like np (num: sg & pers: 3rd) denotes those strings which have the indicated property and the categorial operators denote the usual left- and right-residuals of these string sets, does not match the intuitive, unification-based proof theory. Unification implements a global bookkeeping with respect to a proof whereas the direct fibring method restricts its view to the atoms of the grammar logic. The solution is to interpret the (embedded) feature terms as global feature constraints while maintaining the same kind of fibred structures. For this adjusted semantics, the anticipated proof system is sound and complete. (shrink)

This article explores a specific case of premodern social thought, the Middle Persian Zoroastrian system of estates, MP pēšagān, sg. pēšag, which originated in Sasanian Iran, and its link to the social position of priests in the empire. It is argued that Zoroastrian religious experts tried to impose a totalizing system of social organization and heuristic possibility in a situation characterized by competition for resources in a tributary society. Against a widely held belief, it will be shown that this system (...) was only loosely based on an Avestan predecessor—an observation that should caution against broader attempts to project Middle Persian concepts onto the Avesta and vice versa. The findings also contribute to our understanding of the relationship between the Sasanians and the Zoroastrian tradition. (shrink)

Given their salience in many sports and physical cultures, it is surprising that the practices, processes and production of intercorporeality and ‘doing together’ remain under-explored from a sociological perspective. The ongoing achievement of ‘togethering’ can be particularly important for the embodied partnership between a visually impaired (VI) runner and a sighted guide (SG) runner: a specific sporting dyad whose experiences are currently under-researched. To address this lacuna and contribute original insights to sensory sociological studies, here we explore the accomplishment of (...) running-together by VI runners and sighted guides, focusing on the auditory dimension. To illustrate how these runners put the sense of hearing ‘to work’ in achieving finely attuned intercorporeality, often at considerable speed, we draw on qualitative data from a research project on VI running in the UK, involving five VI runners and five SGs. Here, we focus on auditory attunement in two domains identified as highly salient in the running-partners’ intercorporeal experiences: (1) Listening out – auditory attention to non-linguistic sounds; and (2) Tuning in – the importance of attending to team-talk between VI runners and SG runners. Because woman has disappeared both figuratively and literally from society [see Sen, “More Than 100 Million Women Are Missing”], Irigaray conceives the contemporary ethical (...) project as a recall to difference rather than equality, to difference between women and men—that is, sexual difference. She characterizes relations between men and women as market relations in which women are commodities, objects, but never subjects of exchange, objects to men but not to themselves: women do not belong to themselves but exist “to keep relationships among men running smoothly” [TS 192]. Women under these conditions require imaginative ways to reconfigure the self, to subvert the melancholy and regression of masculinist economies and envisage a future in which women would not be ashamed of the feminine, would experience it as a positivity worth emulating.Irigaray contends that after the gains of egalitarian politics are carefully examined, the inclusion of women in the political arena has failed to take into account women’s distinct and different position from men, and from each other, as well as perpetuating the fiction of the “neutral” citizen, the ahistorical individual citizen of the nation-state. It is that fiction Irigaray dispels in her critique of liberal democratic politics and its creation, “citizens who are neuter in regard to familial singularity, its laws, and necessary sexual difference” [SG 112] in order to benefit the State and its laws. Irigaray suggests that “the return of women to collective work, to public places, to social relations, demands linguistic mutations” and profound transformations, an embodied imagination with force and agency in civil life [TD 65].Irigaray warns that if civil and political participation is construed in overly narrow terms, if focus is on economic or judicial “circuits” alone, we overlook the symbolic organization of power—women risk losing “everything without even being acknowledged” [TD 56]. Instead an interval of recognition can expand the political to include the concerns and activities of real women, lest silence imply consent to sexual neutrality, or more likely, to women’s obliteration under men’s interests and concerns. The patriarchal family is still the legal norm, even when certain exceptions are made, while enduring questions regarding women’s health and children’s physical welfare as priorities beyond market considerations are consigned to legislative obfuscation, still a political afterthought. Instead, in the US the liberal state removes the slender welfare net specific to women and children, Aid to Families with Dependent Children, and fails to provide medical coverage to those who are among the most vulnerable of its citizens. Women without access to the legal protection of sex-neutral citizenship, poor working women without language (the money for an effective “mouthpiece” to represent their distress in a court of law), are further disempowered by liberal politics’ insistence on sexual neutrality—that is, on repression or amnesia regarding the lived experiences of women.Sexual difference is key to any project of self-definition by women. Irigaray insists on the sexual nature of this self-definition, not solely for its obvious procreative necessity, but because the natural world is a source of renewal and fecundity which requires attentive interrogation and respect [SG 15]. We analyze the results from three different risk attitude elicitation methods. First, the broadly used test by Holt and Laury, HL, second, the lottery-panel task by Sabater-Grande and Georgantzis, SG, and third, responses to a survey question on self-assessment of general attitude towards risk. The first and the second task are implemented with real monetary incentives, while the third concerns all domains in life in general. Like in previous studies, the correlation of decisions across tasks is low and usually statistically (...) non-significant. However, when we consider only subjects whose behavior across the panels of the SG task is compatible with constant relative risk aversion, the correlation between HL and self-assessed risk attitude becomes significant. Furthermore, the correlation between HL and SG also increases for CRRA-compatible subjects, although it remains statistically non-significant. (shrink)

Patients fit with cochlear implants commonly indicate at the time of device fitting and for some time after, that the speech signal sounds abnormal. A high pitch or timbre is one component of the abnormal percept. In this project, our aim was to determine whether a number of years of CI use reduced perceived upshifts in frequency spectrum and/or voice fundamental frequency. The participants were five individuals who were deaf in one ear and who had normal hearing in the other (...) ear. The deafened ears had been implanted with a 18.5 mm electrode array which resulted in signal input frequencies being directed to locations in the spiral ganglion that were between one and two octaves higher than the input frequencies. The patients judged the similarity of a clean signal presented to their implanted ear and candidate, implant-like, signals presented to their normal-hearing ear. Matches to implant sound quality were obtained, on average, at 8 months after device activation and at 35 months after activation. At Time 1, the matches to CI sound quality were characterized, most generally, by upshifts in the frequency spectrum and in voice pitch. At Time 2, for four of the five patients, frequency spectrum values remained elevated. For all five patients F0 values remained elevated. Overall, the data offer little support for the proposition that, for patients fit with shorter electrode arrays, cortical plasticity nudges the cortical representation of the CI voice toward more normal, or less upshifted, frequency values between 8 and 35 months after device activation. Cortical plasticity may be limited when there are large differences between frequencies in the input signal and the locations in the SG stimulated by those frequencies. (shrink)

Le passage du visible aux invisibilia Dei est un moment (rationnel) qui a un ressort noétique interne, un fondement externe dans l’expressivité du créé, des vertus et des limites au sein d’une économie de la manifestation de Dieu par le sensible. La connaissance de Dieu par les sages des nations est orientée vers (...) une pédagogie salvifique de la foi, avec ses secours propres : la Loi, l’Incarnation et l’Évangile. L’exégèse et les usages de Rm 1, 19-20 demeurent toujours soumis à la thèse de Rm 1, 16 au sujet de « la puissance de la grâce évangélique ». La connaissance naturelle de Dieu, altérée par une faute envers Dieu, ne diminue en rien la nécessité de la grâce pour être sauvé. (shrink)
