BackgroundFew studies have examined factors associated with willingness of people living with HIV to participate in HIV treatment clinical trials in Sub-Saharan Africa. We assessed the factors associated with participation of PLHIV in HIV treatment clinical trials research at a large urban clinical and research facility in Uganda.MethodsA mixed methods study was conducted at the Infectious Diseases Institute, adult HIV clinic between July 2016 and January 2017. Data were collected using structured questionnaires, focused group discussions with respondents categorised as either (...) participated or never participated in clinical trials and key informant interviews with IDI staff. A generalized linear model with a logit link function was used for multivariate analyses while the qualitative data were summarized using a thematic approach.ResultsWe enrolled a total of 202 and analysed 151 participants, 77 of whom were male with mean age of 41 years. The majority 127 expressed willingness to participate in treatment clinical trials if given an opportunity. At bivariate analysis, willingness to participate was significantly associated with respondents’ perception of a satisfactory compensation package, special status accorded and belief that their health status would improve while on the clinical trial. At multivariate analysis, a satisfactory compensation package and special status accorded in clinical trials remained significant. The qualitative data analysis confirmed these findings as participants valued the privilege of jumping the clinic waiting queues and spending less time in clinic, the wide range of free tests offered to trial participants, unrestricted access to senior physicians and regular communication from study team. Additionally, free meals offered during clinic visits meant that participants were not in a hurry to go back home. Barriers to participation included the perception that new drugs were being tested on them, fear of side effects like treatment failure and the uncertainty about privacy of their data.ConclusionWe found overwhelming willingness to participate in HIV treatment clinical trials. This was largely extrinsically influenced by the perceived material and health-related benefits. Investigators should pay attention to participants’ concerns for benefits which may override the need to understand study procedures and risks. (shrink)

Gary Ebbs: Quine's Naturalistic Explication of Carnap's Logic of Science: If one studies Quine's epistemology without appreciating its deep connections to Carnap's logic of science, one can easily get the impression that unlike Carnap, Quine aims to preserve and clarify the traditional empiricist idea that our best theories of nature are justified by, or based on, our sensory evidence, and are for that reason likely to be true. Quine writes, for instance, that [The] human subject is accorded a certain experimentally (...) controlled input—certain patterns of irradiation in assorted frequencies, for instance—and in the fullness of time the subject delivers as output a description of the three‐dimensional external world and its history. The relation between the meager input and the torrential output is a relation that we are prompted to study for somewhat the same reasons that always prompted epistemology; namely, in order to see how evidence relates to theory, and in what ways one's theory of nature transcends any available evidence. Commenting on this and similar passages in Quine, Barry Stroud writes: … the question Quine poses in terms of the “underdetermination” of the “torrential output” by the “meager input” makes essential use of a notion of epistemic priority. It is because the “information” we get at “input” does not uniquely determine the truth of what we assert as “output” that we must explain how we get from the one to the other. I argue that despite its superficial plausibility, this interpretation of Quine is mistaken: following Carnap, yet also radically transforming his views, Quine takes our best current theory as our ultimate arbiter of truth. The goal of Quine's naturalistic account of the relationship between theory and evidence is not to show that our best theories of nature are justified by our sensory evidence, but to show that we can describe science from within science itself in a way that mirrors and thereby clarifies his doctrinal principle that we can only judge truth from the standpoint of our best current theory. Heracleides Ponticus, a pupil of the schools of Plato and Aristotle, who lived from about 390 to 310 B.C., shared the wide interests of many of his pre-Platonic predecessors. Diogenes Laertius gives a long list of his works, many of them now known only by their titles, which he divided into writings on ethics, physics, grammar, music, rhetoric, and history. Like most of his predecessors he gave some attention to the heavens and speculated about the nature of the moon , (...) comets , the infinity of the cosmos ; he was best known in antiquity, in this field, for his suggestion that the phenomena could be saved if the heavens were at rest and the earth revolved about the central axis .2 One of two pieces of evidence for his involvement in anything more than this general, inexact speculation in the field of astronomy is contained in the commentary on Plato's Timaeus written in Latin by Chalcidius somewhere at the beginning of the fourth century A.D.; it is this passage which this paper will discuss. Self-deception may be a natural consequence of active goal operation instead of an adaptation for negotiating the social world. In 1916 Bertrand Russell was prosecuted and fined for publishing (in defence of a conscientious objector) 'statements likely to prejudice the recruiting and discipline of His Majesty's forces.' He was almost immediately afterwards dismissed from his Lectureship at Trinity College, Cambridge, by the College Council. This expulsion provoked a storm of protest and the true facts of the case became obscured by misconceptions, prejudices and uninformed gossip, to the discredit of the College. In 1942, therefore G. H. Hardy the mathematician (...) printed for private circulation to another generation of Fellows at Trinity a full account of the incident in an attempt to explain what really happened. This is now made public. Besides provoking an authoritative record of a celebrated but misinterpreted episode in Russell's eventful academic career, this document contains interesting evidence about attitudes to pacifism in the First World War and in particular about the sympathies of such distinguished colleagues and contemporaries of Russell as Cornford, Housman, McTaggart and Whitehead. We present a dynamic model of the evolution of communication in a Lewis signaling game while systematically varying the degree of common interest between sender and receiver. We show that the level of common interest between sender and receiver is strongly predictive of the amount of information transferred between them. Our understanding of communication and its evolution has advanced significantly through the study of simple models involving interacting senders and receivers of signals. Many theorists have thought that the resources of mathematical information theory are all that are needed to capture the meaning or content that is being communicated in these systems. However, the way theorists routinely talk about the models implicitly draws on a conception of content that is richer than bare informational content, especially in contexts where false content (...) is important.
