Dostoevsky wrote that love in action is a harsh and terrible thing compared to love in dreams. That reality is particularly evident in medicine, where there is an almost universal, involuntary participation of physicians and other healthcare workers in the suffering of their patients. This paper explores this phenomenon through the paradigm of ‘mystery’ as explained by the French existentialist philosopher Gabriel Marcel. A mystery is different from a problem in the sense that the former requires the active immersion of (...) the person involved in order to be truly experienced. It is a ‘meta-problem’ that cannot be analyzed objectively and separately from the person that it affects, without changing the nature of the thing experienced. The authors contend that the human suffering encountered in medicine is one such phenomenon, and the paper draws on illustrations of this concept in art and literature. Awareness of the subtle but important difference between mystery and problem may help physicians better understand their personal entanglement with the suffering of patients. (shrink)

This article examines some of these issues, paying attention to the place that the recognition of identities occupies both in her republican and in her liberal theories, as well as the type of justification advanced in both cases. Supporters of neutrality as benign neglect argue that a neutral state should not grant any type of recognition to cultural or religious groups. Liberal multiculturalists argue instead that due to the non-neutral nature of public institutions, democratic states must adopt policies that recognize and accommodate the distinctive needs of ethnocultural groups. This article examines a different way of conceiving the principle of neutrality. In this conception, developed by Alan Patten in the framework of liberal multiculturalism, a state can only be (...) neutral when it extends equivalent levels of assistance/hindrance to rival conceptions of the good. Neutrality is thus reinterpreted to become a principle justifying the religious exemptions. Les théories normatives qui justifient les politiques multiculturelles sont souvent dénoncées comme étant relativistes, conservatrices et anti-libérales. De telles politiques menaceraient en effet la cohésion sociale et promouvraient la fragmentation sociale et l’inégalité juridique en plaçant les cultures au-dessus de la politique et les groupes au-dessus des individus. Elles se fonderaient sur un respect inconditionnel du droit à la différence, en mettant l’accent sur les droits des minorités ethniques au détriment de la majorité et en s’attaquant à l’égalité de tous (...) les citoyens devant la loi. Dans ce cadre, le multiculturalisme est souvent présenté comme incompatible aussi bien avec le libéralisme qu’avec le républicanisme. Se voulant une introduction à six théories politiques de la diversité, cet ouvrage présente la relation entre le libéralisme, le républicanisme et le multiculturalisme sous un éclairage différent. Au moyen d’une reconstruction et d’une clarification des théories politiques retenues il montre, d’une part, que le libéralisme et le républicanisme sont tous deux compatibles avec la prise en compte gouvernementale de la diversité culturelle et religieuse, notamment en raison de l’adaptation de leurs principes fondamentaux à la réalité pluriculturelle contemporaine. D’autre part, il montre que les théories politiques examinées sont difficilement concevables à la lumière de certaines critiques formulées par les détracteurs des politiques multiculturelles. PurposeTo examine the contribution of “values-based reasoning” in evaluating older adults’ capacity to make medical decisions.Design and MethodsOlder men with schizophrenia (n=20) or dementia (n=20), and a primary care comparison group (n=19), completed cognitive and psychiatric screening and an interview to determine their capacity to make medical decisions, which included a component on values. All of the participants were receiving treatment at Veterans Administration (VA) outpatient clinics.ResultsParticipants varied widely in the activities and relationships they most valued, the extent to which (...) religious beliefs would influence healthcare decisions, and in ratings of the importance of preserving quality versus length of life. Most participants preferred shared decision making with doctor, family, or both. Individuals with schizophrenia or dementia performed worse than a primary care comparison group in reasoning measured by the ability to list risks and benefits and compare choices. Individuals with dementia performed comparably to the primary care group in reasoning measured by the ability to justify choices in terms of valued abilities or activities, whereas individuals with schizophrenia performed relatively worse compared to the other two groups. Compared to primary care patients, participants with schizophrenia and with dementia were impaired on the ability to explain treatment choices in terms of valued relationships.ConclusionMedical decision making may be influenced by strongly held values and beliefs, emotions, and long life experience. To date, these issues have not been explicitly included in structured evaluations of medical decision-making capacity. This study demonstrated that it is possible to inquire of and elicit a range of healthcare related values and preferences from older adults with dementia or schizophrenia, and individuals with mild to moderate dementia may be able to discuss healthcare options in relation to their values. However, how best to incorporate a values assessment into a structured capacity evaluation deserves further research attention. (shrink)

Pojem řádu má své veliké dějiny od předsókratiků až k H. Kringsovi, který v roce 1941 vydává knihu Ordo. U nás se pojmu řádu a řádovosti věnoval po celý život ze­jména brněnský a olomoucký myslitel J. L. Fischer (1894–1973). Idea řádu byla také jedním z pilířů jeho skladebné filosofie, jež měla základní problémy moderní společnosti vyřešit účinněji než marxismus. Současně s „laickým“ Fischerem usilovala v kritických 30. letech dvacátého století o nastolení kýženého řádu vyhraněně katolická skupina sou­středěná kolem tehdy nově (...) založené revue Řád. Oběma pólům šlo sice o podobný cíl (skoncovat s vládou neblahého kapitalismu), ale neporozuměly si pro různost svých věr. Toto osudové neporozumění bychom měli především na univerzitách v Brně a Olomou­ci napravovat využitím odkazu W. C. Smithe, který zásadně rozlišil setrvačnou tradiční víru (belief) a otevřenou víru všech opravdu tvůrčích lidí (faith). Takovou víru měl totiž podle autora také a snad především nový řád usilovně hledající Josef Ludvík Fischer. (shrink)

The project review as outlined in this article explores the questions: What is transformative research and what is transformation as far as the community stakeholders are concerned? To what extent has the transformative research achieved its intended outcomes? The Bokamoso project (founded by Lesego Serolong as facilitator and investor) is an integrated development project designed to create employment and to enable the community to learn while they make a living through a diverse range of farming activities. The participatory research as (...) discussed here is part of the University of South Africa’s research and community engagement programme, and is set in a remote, arid zone. This research coupled with community engagement aims to transform lives and create opportunities by enhancing their participation in education, training and employment creation. The project (which has been running since 2015) supports indigenous ways of being, knowing and doing and has succeeded in enabling the farmers trained through Bokamoso to be less dependent on social benefits (government grants). Free logic is an important field of philosophical logic that first appeared in the 1950s. J. Karel Lambert was one of its founders and coined the term itself. The essays in this collection explore the philosophical foundations of free logic and its application to areas as diverse as the philosophy of religion and computer science. Amongst the applications on offer are those to the analysis of existence statements, to definite descriptions and to partial functions. Although worship of the relics of the Buddha — and its corollary, st?pa worship — is a widespread feature of Buddhist devotional practice among both lay Buddhists and monks, there is in some quarters a view that, while recommended to lay followers, it is forbidden to monks. This controversy started very early after the Buddha’s parinibb?na and has reverberated throughout the centuries till the present time. Its source is in the Mah?parinibb?na Sutta, and it stems from the ambiguity in the (...) meaning of the compound sar?ra-p?j? in the Buddha’s reply to?nanda’s two questions concerning the actions to be taken after the Master’s death with respect to his body. The resolution of the controversy depends on a correct understanding of the nature of the Buddha’s replies to the two questions. This paper analyses the relevant passages of the sutta and the way they have been translated, correctly or incorrectly, into Western languages and into Chinese, and finally arrives at a solution derived entirely from within the text of the Mah?parinibb?na Sutta itself. This paper focuses on Augustine’s concept of self-knowledge or self-awareness in non-rational animals through examining the relation between external senses, internal sense and rationality. The explanation of what causes motion in non-rational living beings is quite puzzling in the case of animal’s self-perception – for what reason do they move, sense or live. This motivation is also connected to the self-preservation principle, which is one of the two sources of confusion regarding self-perception in animals; the other one is the ability (...) of internal sense or anima to perceive themselves, other animals and humans as alive – in spite of the fact that this can in no way be sensory information. In the Philebus, Plato elaborates an image of our soul as a book where a scribe and a painter are constantly at work. This book examines the implicit premises of this image and aims at overcoming the general polarity of ancient phantasia and modern imagination.

The COVID-19 pandemic has highlighted the difficult task of balancing access to misinformation with respect for patient decision-making. Due to its innate antagonism, the paradigm of “physician paternalism” versus “patient autonomy” may not adequately capture the clinical relationship. The authors hypothesized that most patients would, in fact, prefer significant physician input as opposed to unopinionated information when making medical decisions. There is a lack of empirical data corroborating this in the United States. To that end, a survey was distributed to (...) 650 individuals through Amazon Mechanical Turk, of which 499 responses met pre-determined quality criteria. Most respondents believed their doctor's insight would be better than their own if injured or gravely ill. When asked to affirm preferences separately, a significantly higher proportion of respondents preferred guidance from their doctor when making medical decisions compared to being presented with unopinionated information ( p < 0.001). Encouragingly, 93.1% believed that the doctor's primary goal was their health. When asked directly to compare physician guidance to unopinionated information, 69.1% respondents stated they would prefer physician guidance. We found a consistent association between educational/economic background and affirmative responses ( p < 0.001), suggesting particular attention should be paid to patients that are disadvantaged with respect to these demographic factors. The belief in a shared goal, and a consistent preference for physician input, suggests that patients endorse a more collaborative view of the clinical dynamic than is suggested by the paternalism-autonomy paradigm. This pilot study suggests physicians should not be afraid to communicate conviction with regard to treatment decisions. (shrink)

Public debate on acceptable farm animal husbandry suffers from a confusion of tongues. To clarify positions of various stakeholder groups in their joint search for acceptable solutions, the concept of animal welfare was split up into three notions: no suffering, respect for intrinsic value, and non-appalling appearance of animals. This strategy was based on the hypothesis that multi-stakeholder solutions should be based on shared values rather than on compromises. The usefulness of such an artificial value distinction strategy was tested in (...) a small series of experiments. The results demonstrate that the chosen concept to distinguish between values is effective in a stakeholder context. Farmers’ views on doing good to animals appeared to be largely based on their value to prevent suffering and predominantly focused on the provision of regular care. Their priority for this value is clearly shared with other stakeholders, providing a basis for joint solutions. The concept of intrinsic value does not play a discernable role in farmers’ considerations. Based on the varying views on welfare, it can be inferred that there is a gradual rather than a principal difference between government legislation and farmers’ values, whereas public perception and acceptance of farm practices remains complicated. Distinction between value groups and focusing on a selected notion (such as no suffering) proved to be effective in bringing representatives of stakeholder groups together, but is unlikely to bridge the emotional gap between commercial farm practices and public ideals.
