Objective Gather information from physicians about factors contributing to unnecessary noninvasive imaging and impact of possible solutions. Methods Qualitative study of 14 physicians using a phenomenological approach and the Theoretical Domains Framework. Results Most participants ( n = 9) self-reported that >10% of the imaging tests they order are unnecessary. External sources of pressure included: peer-review, patient demands, nursing expectations, specialist requests (social demands), as well as prior experience with patient advocates, and the compensation and pension system (environmental context). Internal (...) sources of pressure included reliance on anecdote (emotion), self-doubt about diagnoses (beliefs about capabilities), and fear of missing a diagnosis and of professional liability (beliefs about consequences). Participants expressed both optimism and concern about potential solutions, such as adopting decision support software. Conclusion Physicians are under pressure from multiple sources to order unnecessary imaging. Peer review, nursing expectations, and perceptions about Veteran compensation and pension are newly reported contributing factors. (shrink)

Timothy Findley's Not Wanted on the Voyage is a radical postmodern retelling of the biblical flood narrative, offering an invitation to empathy as well as a dark indictment of tyrannic religious structures. Findley begins by establishing a space of empathy with (and openness to) the experiences of animals and other marginalized groups within the context of religiously backed oppression. Since its introduction, multivariate pattern analysis, or ‘neural decoding’, has transformed the field of cognitive neuroscience. Underlying its influence is a crucial inference, which we call the decoder’s dictum: if information can be decoded from patterns of neural activity, then this provides strong evidence about what information those patterns represent. Although the dictum is a widely held and well-motivated principle in decoding research, it has received scant philosophical attention. We critically evaluate the dictum, arguing that it is false: decodability is (...) a poor guide for revealing the content of neural representations. According to Chalmers (2018), the meta-problem of consciousness is 'the problem of explaining why we think that there is a problem of consciousness'. In this paper I argue that the key to understanding both consciousness itself and addressing the meta-problem is to understand what acquaintance is and what its objects are. Unfortunately, I think there are still some serious mysteries lurking here, which I present briefly in this commentary. In particular, on the view of acquaintance I favour, it is unclear (...) how our acquaintance with the contents of experience can serve as data for our theory of conscious experience. Scientists and philosophers alike debate whether various systems such as plants and bacteria exercise cognition. One strategy for resolving such debates is to ground claims about nonhuman cognition in evidence from mathematical models of cognitive capacities. In this article, I show that proponents of this strategy face two major challenges: demarcating phenomenological models from process models and overcoming underdetermination by model fit. I argue that even if the demarcation problem is resolved, fitting a process model to behavioral data is, on (...) its own, not strong evidence for any cognitive process, let alone processes shared with humans. J. Christopher Maloney argues that free will is compatible with necessary laws of science and immutable history. For free will emerges from an akratic will that asymptotically approaches the ability to choose to act otherwise than it willfully does.

This paper is a sequel to an earlier result of the authors that in making inferences from certain probabilistic knowledge bases the maximum entropy inference process, ME, is the only inference process respecting “common sense.” This result was criticized on the grounds that the probabilistic knowledge bases considered are unnatural and that ignorance of dependence should not be identified with statistical independence. We argue against these criticisms and also against the more general criticism that ME is representation dependent. The American Institute of Certified Public Accountants monitors the misconduct of its members using the AICPA Code of Professional Conduct. To accomplish this task, the AICPA relies on various stakeholders to report known violations of its CPC. We examine the full population of sanctions imposed by the AICPA on its members under its CPC from 2008–2013 to identify recent trends in the misconduct of accounting professionals. While we find that multiple stakeholders identify and report violations, we also find that the (...) most common types of violations remain consistent with those reported in the 1990s. Further, we develop a taxonomy based on prior accounting literature to determine whether the AICPA CPC is being enforced to defend the public interest and/or the private interests of the accounting profession. In contrast to prior studies, our results suggest that as the accounting profession emerges from a recession and period of intense public scrutiny, the AICPA CPC is largely being enforced to defend the public interest. This public interest focus was most pronounced during the recessionary years of 2008–2010, as evident from misconduct reported by parties such as the Internal Revenue Service, Securities and Exchange Commission, Public Company Accounting Oversight Board, and state boards of accountancy. Although key stakeholders have recently focused on reporting misconduct that threatens the public interest, we believe there are still areas in need of improvement, especially around the level of detail provided in the AICPA’s sanction records. Illness severity is a priority setting criterion in several countries. Age seems to matter when considering severity, but perhaps not small age differences. In the following article we consider Small Differences : small differences in age are not relevant when considering differential illness severity. We show that SD cannot be accommodated within utilitarian, prioritarian or egalitarian theories. Attempting to accommodate SD by postulating a threshold model becomes exceedingly complex and self-defeating. The only way to accommodate SD seems to be to (...) accept some form of relevance view, where some age differences are irrelevant. This view can accommodate SD, but at the expense of consistent priority orderings. Severity thus becomes unsuitable for systematic decision-making. We argue that SD should be dismissed and that we should accept a continuous relationship between severity of illness and age. (shrink)

The definition of acedia presents unique conceptual problems among the eight Evagrian logismoi. Its descriptions are so complex and varied as to render the concept seemingly incoherent. This article argues that the conceptual unity of acedia has been obscured by the translation of Evagrian logismoi into the ‘deadly sins’ tradition, resulting in a category error. Acedia is more properly understood, not as a psychological state or a sin, but rather as an array of demonic temptations with the unifying end-goal of (...) interrupting the anchoritic ideal of sustained contemplation. It has become increasingly common to conduct research on popular views on ethical questions. In this paper, we discuss when and to what extent popular views should be included in a reflective equilibrium process, thereby influencing normative theory. We argue that popular views are suitable for inclusion in a reflective equilibrium if they approximate considered judgments and examine some factors that plausibly contribute to the consideredness of popular views. We conclude that deliberation and familiarity contribute to the consideredness of popular (...) views, whereas prevalence does not. Furthermore, we raise the “fair trial” objection: that expert deliberation on popular views might fail to respect the consideredness of these views. Eliciting views approximating considered judgments directly from participants is therefore preferable to refining popular views or treating them as “bare” intuitions in the reflective process. These conclusions have important implications for which research methods will likely elicit popular views suitable for a reflective equilibrium process. Research focusing on the prevalence of views is unlikely to yield popular views suitable for a reflective equilibrium, whereas deliberative and in-depth work is more promising. (shrink)

ABSTRACTGraeme Wood’s The Way of the Strangers suggests that many scholars have denied or downplayed the Islamic State’s own account of its emphatically religious foundation. This tendency is heir to the Enlightenment strategy of defanging illiberal religion by claiming that only religions conforming to liberal principles are genuinely religious—raising anew questions that arose at the dawn of liberalism, in the wake of the Wars of Religion.

Intentional actions exhibit control in a way that mere lucky successes do not. A longstanding tradition in action theory characterizes actional control in terms of the _knowledge_ with which one acts when acting intentionally. Given that action theorists, no less than epistemologists, typically take for granted the orthodox thesis that knowledge is in the head (viz., realized exclusively by brainbound cognition), the idea that intentional action is controlled in virtue of knowledge is tantamount to the idea that the knowledge by (...) which intentional actions exhibit control supervenes intracranially. We raise some challenges for this idea, and in doing show, we show how epistemic theories of actional control are naturally aligned moreso than has been appreciated with cognitive extension in the theory of mind.
