The use of quality of life (QOL) outcomes in clinical trials is increasing as a number of practical, ethical, methodological, and regulatory reasons for their use have become apparent. It is important, then, that QOL measurements and differences between QOL scores be readily interpretable. We study interpretation in two contexts: when determining QOL and when basing decisions on QOL differences. We consider both clinical situations involving individual patients and research contexts, e.g., randomized clinical trials, involving groups of patients. We note (...) the ethical importance of such understanding: proper interpretation and communication facilitate health care decision making. Communication that facilitates interpretation is of moral significance since better communication can attenuate ethical problems and inform choices. Much of what is communication worthy about QOL assessments is determined by the particular QOL instrument used in the assessment and how it is administered. In practice, these choices will be driven by the purpose of the assessment, but, it is argued, to maximize understanding, we should combine the information garnered from traditional standardized QOL instruments, from individualized QOL assessments, and from a recently proposed dialogic paradigm, where QOL is determined by shared conversation regarding the interpretation of texts. And, while some studies can surely succeed using abbreviated methods of administration (e.g., postal surveys may suffice for certain purposes), we will focus on methods of administration involving interviewer–respondent interaction. We suggest that during the QOL elicitation process, interviewer and respondent should engage in a two-way conversation in order to achieve a shared understanding of the “answers” to QOL “questions” and, finally, to reach a shared interpretation of the individual’s QOL. A medical student's ability to present a case history is a critical skill that is difficult to teach. Case histories presented without theatrical engagement may fail to catch the attention of their intended recipients. More engaging presentations incorporate ‘stage presence’, eye contact, vocal inflection, interesting detail and succinct, well organised performances. They convey stories effectively without wasting time. To address the didactic challenge for instructing future doctors in how to ‘act’, the Mayo Medical School and The Mayo Clinic Center for (...) Humanities in Medicine partnered with the Guthrie Theater to pilot the programme ‘Telling the Patient's Story’. Guthrie teaching artists taught storytelling skills to medical students through improvisation, writing, movement and acting exercises. Mayo Clinic doctors participated and provided students with feedback on presentations and stories from their own experiences in patient care. The course's primary objective was to build students' confidence and expertise in storytelling. These skills were then applied to presenting cases and communicating with patients in a fresher, more engaging way. This paper outlines the instructional activities as aligned with course objectives. Progress was tracked by comparing pre-course and post-course surveys from the seven participating students. All agreed that the theatrical techniques were effective teaching methods. Moreover, this project can serve as an innovative model for how arts and humanities professionals can be incorporated for teaching and professional development initiatives at all levels of medical education. A total of 1918 loci, detected by the hybridization of 938 expressed sequence tag unigenes from 26 Triticeae cDNA libraries, were mapped to wheat homoeologous group 4 chromosomes using a set of deletion, ditelosomic, and nulli-tetrasomic lines. The 1918 EST loci were not distributed uniformly among the three group 4 chromosomes; 41, 28, and 31% mapped to chromosomes 4A, 4B, and 4D, respectively. This pattern is in contrast to the cumulative results of EST mapping in all homoeologous groups, as reported (...) elsewhere, that found the highest proportion of loci mapped to the B genome. Sixty-five percent of these 1918 loci mapped to the long arms of homoeologous group 4 chromosomes, while 35% mapped to the short arms. The distal regions of chromosome arms showed higher numbers of loci than the proximal regions, with the exception of 4DL. This study confirmed the complex structure of chromosome 4A that contains two reciprocal translocations and two inversions, previously identified. An additional inversion in the centromeric region of 4A was revealed. A consensus map for homoeologous group 4 was developed from 119 ESTs unique to group 4. Forty-nine percent of these ESTs were found to be homoologous to sequences on rice chromosome 3, 12% had matches with sequences on other rice chromosomes, and 39% had no matches with rice sequences at all. Limited homology was found between wheat ESTs on homoeologous group 4 and the Arabidopsis genome. Forty-two percent of the homoeologous group 4 ESTs could be classified into functional categories on the basis of blastX searches against all protein databases. (shrink)

To be free is to escape all limitations and obstacles—or so we think at first. But if we probe further, we discover that freedom embraces its own necessities, a set of conditions without which it could not exist. _Freedom's Embrace_ explores these necessities of freedom. J. Melvin Woody surveys competing conceptions of freedom and traces debates about the nature and reality of freedom to confusions about knowledge, humanity, and nature that are rooted in some of the most fundamental assumptions of (...) modern Western thought. The preemption of freedom as an exclusively human privilege with all nature relegated to mechanical necessity is a fatal error that renders both humanity and nature equally unintelligible. What distinguishes human beings from other animals is not freedom but the use of symbols, which vastly extends the range of available options and enables us to envision freedom as an ideal by which customary institutions and norms may be judged and transformed. By carefully surveying its necessary conditions and limitations, Woody reconciles the salient competing conceptions of freedom and weaves them together into a richer and broader theory that resolves old controversies and opens the way toward an ethics of freedom that can meet the challenges of relativism and nihilism that arise from recognizing the historicity and malleability of culture. This series will include monographs and collections of studies devoted to the investigation and exploration of knowledge, information and data-processing systems of all kinds, no matter whether human, (other) animal or machine. Its scope is intended to span the full range of interests from classical problems in the philosophy of mind and phi losophical psychology through issues in cognitive psychology and socio biology (concerning the mental capabilities of other species) to ideas related to artificial intelligence and computer science. While primary (...) emphasis will be placed upon theoretical, conceptual and epistemologi cal aspects of these problems and domains, empirical, experimental and methodological studies will also appear from time to time. Among the most challenging and difficult projects within the scope of artificial intelligence is the development and implementation of com puter programs suitable for processing natural language. Our purpose in compiling the present volume has been to contribute to the foundations of this enterprise by bringing together classic papers devoted to crucial problems involved in understanding natural language, which range from issues of formal syntax and logical form to those of possible-worlds and situation semantics. The book begins with a comprehensive introduc tion composed by Jack Kulas, the senior editor of this work, which pro vides a systematic orientation to this complex field, and ends with a selected bibliography intended to promote further research. J. Arthur Baird is the author of several important books in New Testament studies, his best known perhaps being his Audience Criticism and the Historical Jesus. At his untimely death, he left a nearly complete manuscript, now published here. In this timely and relevant manuscript, Baird offers first a critical introduction to the historical paradigm, pointing out its limitations in terms of tracing the paradigm of New Testament formation. He then traces this development himself, beginning with the starting point of (...) the Holy Word and noting its development through the context, theological interpretation, and application, to its manifestation in the apostles, schools and fathers. He concludes with comments on its current implications. This book represents the last and lasting final thoughts of a scholar who was very much devoted to the importance of the Holy Word. (shrink)

On se demande quelle est la relation entre moi et ma parole, si la parole découle de l'essence même de mon être. Une analyse phénoménologique, qui partirait du dialogue comme d'un donné indubitable, ne suffit pas tout à fait, puisqu'il il s'agit d'un sens éventuel du dialogue pour le tout de mon être. Il faut donc aborder la question d'un point de vue (...) d'où le dialogue lui-même est mis en suspens. Commençons par l'indubitable absolu : l'être. Il y a quelque chose, quoi que ce soit, il y a au moins la question même de l'être. Mais cette question qui se met elle-même en question, se sait être. Et qu'est cela sinon moi-même, pas encore opposé, il est vrai, à toi, à lui, à nous ? Pourtant je ne suis pas l'être ; il y a en moi une dualité entre essence et être, et une dualité entre être et penser. Je sais que je suis, je cherche encore qui je suis. Eh bien, je suis « ici et maintenant » , c. à. d. je suis dans un ordre spatio-temporel, par mon corps, qui est donc ce qui m'exprime à moi-même, ma parole. Mais mon corps est originellement en commerce avec les êtres du monde, et j'ai le sentiment de cette situation par la perception et le comportement. En révélant le sens des êtres du monde, en leur donnant des noms de mon point de vue, je me nomme moi-même. En revenant sur la voie parcourue jusqu'ici, l'article démontre que ce n'est pas seulement mon monde que je nomme. Le « moi » de la certitude absolue est un « nous » pas encore détaillé. Et à l'intérieur de ce « nous » jaillit, par ta foi en moi, l'opposition communicative entre moi et toi, que j'expérimente surtout dans la corrélation de la voix et de l'ouïe. La parole fonde la communauté, puisqu'en exprimant mon être elle s'adresse à vous, et manifeste ainsi mon être comme « être-avec » . L'être même est infini, et donc communicabilité pure, et donc parole. Mais notre parole humaine n'est que participation déficiente de la communication immédiate qui serait l'identité de l'être, le connaître et l'aimer. Elle reste moyen, et donc succédané de l'immédiat, et donc ambigue. Notre dialogue doit se transcender dans l'inexprimable, dans le silence. La parole essentielle, la parole de l'amour, est pour les hommes la parole la plus dangereuse et la plus incertaine. Elle ne se prouve que par la fidélité. Ethical issues relating to the withdrawal of dialysis are discussed, comparing dialysis with other life-support systems, particularly artificial ventilation. It is argued that there is no ethical difference between discontinuing treatment in each case. One practical difference between the two is that patients with chronic renal failure are less likely to have reduced autonomy, and so can engage in discussions with their doctors regarding the situations in which their life-supporting treatment might be discontinued. It is argued that doctors caring for (...) patients on dialysis have an ethical duty to discuss with these patients the circumstances in which they may wish to discontinue dialysis. (shrink)

Open peer commentary on the article “Exploring the Depth of Dream Experience: The Enactive Framework and Methods for Neurophenomenological Research” by Elizaveta Solomonova & Xin Wei Sha. Upshot: This commentary aims to sharpen the conceptual distinction between the breadth and the depth of dream experience. I discuss several possible readings and argue that the best one construes breadth and depth as distinct but complimentary research strategies distinguished not just by the kinds of evidence they rely on, but also by the (...) degree to which different types of data are integrated and focused on the same experiential episode. The species of metaphysical speculation known as ontology is as old as philosophy itself. It is encountered in one form or another in all climes and times. In Western philosophy, it is already cultivated in a fairly sophisticated form in Parmenides: Being is positive and does not admit to the possibility of negation. Being is a plenum, etc. The problem which exercised the mind of Parmenides has lost something of its vitality in our time but it has recognizably survived in (...) such contemporary questions as “Are there negative facts?” At the present day, however, the liveliest issues of ontology revolve round a topic which has a different, if no less celebrated, ancestry. It is the problem of universals which, much discussed by Plato, was taken up and made a principal concern by the scholastics. It is being canvassed again in recent times under the heading of abstract entities. What I wish to investigate is the connection between the metaphysics of abstract entities and the discipline of logic as developed in the modern era. (shrink)

Gordon Kaufman's “constructive theology” can easily be taken out of context and misunderstood or misrepresented as a denial of God. It is too easily overlooked that in his approach everything is an imaginary construct given no immediate ontological status—the self, the world, and God are “products of the imagination.” This reflects an influence, not only of theories on linguistic and cultural relativism, but also of Kant's “ideas of pure reason.” Kaufman is explicit about this debt to Kant. But I argue (...) there are other aspects of Kant's legacy implicit in his method. These center around Kaufman's engagement with “observed patterns” in nature. With Paul Tillich's aid, I bring this neglected issue to the fore and argue that addressing it allows one to more readily capitalize upon the Kantian influence in Kaufman's method. This, in turn, encourages one to tap more deeply into the epistemic underpinnings of Kaufman's approach to the science–religion dialogue. (shrink)

The development of theories of global cortical dynamics, using linear wave theory, owes much to the pioneering work of Nunez. His work leads to clear predictions on relations of brain size, axonal conduction velocity, and the frequencies of the cerebral rhythms. These predictions do not appear to be fulfilled, but their falsification constrains the range of parameters applicable in further formulations.

Language, either oral or written, is meant both to convey and to preserve meaning. Semiotics is thediscipline which permits the extraction of a meaning from systems of linguistic signs. Written texts arestatic, while the world is about them is in flux. Meaning is thus intimately connected to this marriageof flux and stasis in texts.Here, three views on semiotics are examined:First, Plato’s treatment of signs and flux in the dialogue Kratylos is dissected. The conventional andmimetic aspects of signs are contrasted, and (...) a connection between inquiry and stasis is intimated.Next, the Augustinian theory of signs, as delineated in De Doctrina Christiana, is presented. Thediversity of signs and several principles of hermeneutics are considered. In addition, the existence ofthe some Post-Modern notions of polysemy, trace, and fore-structures of understanding in Augustine’sthought is indicated. Also, the concepts of ineffability and of res summa in De Doctrina Christiana areexamined as starting points for a deconstructive critique of Augustinian semiotics.Third, Derrida's treatment of signifiers in Of Grammatology is analyzed as a turn to a semiotics ofthe flux. Finally, the oral concept of memory reveals a connection between flux and stasis. Thememory plays a role in the system of signs in creating a "generative reconstruction". According toAugustine, sign-making is also "incarnational." Humans retain something of the signs in memory;memory preserves the basis of the sign. This mnemic fundament may be constantly adjusted to thepresent reality, i.e. the flux; the epistemological and semiotic data base in oral cultures is generative orfluid.Memory ameliorates the semiotic tension between the flux of the world and the stasis of information in the system of signs. We outline fresh findings that show that our macroscopic electrocorticographic (ECoG) simulations can account for synchronous multiunit pulse oscillations at separate, simultaneously activated cortical sites and the associated gamma-band ECoG activity. We clarify our views on the approximations of dynamic class applicable to neural events at macroscopic and microscopic scales, and the analogies drawn to classes of ANN behaviour. We accept the need to introduce memory processes and detailed anatomical and physiological information into any future developments of our simulations. On (...) the issue of intrinsic cortical stability and the role of extrinsic fibre systems in maintaining stability, we argue that this position is not in extreme contradiction to those of our commentators, and that the mechanisms implicit in our simulations' properties imply rich computational possibilities. We discuss some of the reasons for and against the existence of significant global resonances in the brain and explain why such behaviour appears absent in our simulations. Last, we discuss other phenomena, such as rhythmic driving of the cortex, which have not yet been introduced into our models, and indicate lines for future development of the simulations. (shrink)
