Listening and responding to patients’ stories for over 20 years as an emergency physician has strengthened my appreciation for the many ways that the skills and principles drawn from writing fiction double as necessary clinical skills. The best medicine doesn’t work on the wrong story, and the stories patients tell sometimes feel like first drafts—vital and fragile works-in-progress. Increasingly complex health challenges compounded by social, financial, and psychological burdens make for stories that are difficult to articulate and comprehend. In this (...) essay, I argue that healthcare providers need to think like creative writers and the skills and sensitivities necessary to story construction deserve a vital space in medical education. A thorough understanding of story anatomy and the imaginative flexibility to work stories into open spaces serve as antidotes to the reductive nature of clinical decision making and have implications as patient safety and risk management strategies. The examples that I have selected demonstrate how thinking like a creative writer functions at the bedside, providing tools for clinical excellence and empathy. This approach asks that we re-imagine the importance of story in clinical care: from a vehicle to a diagnosis to its place as a critical destination. (shrink)

“I'm not afraid of dying,” he says, despite his plea on arrival. “Listen up, douchebag. Are you calling my cousin or what?” The emergency department might be the only sphere of human exchange where one party—patients (and sometimes family)—are permitted to insult, threaten, and even spit at the very people on whom they depend for help, while the offended parties—physicians, nurses, and other health care providers—must not only tolerate the abuse, but treat their tormentors. Does the ED's collective duty to (...) greater numbers of patients demand a revised ethos of tough love for extreme cases of misbehavior? Can we ask these patients to leave without legal recourse after extending genuine, compassionate efforts to participate in their care—barring evidence of a medical explanation for their toxic comportment or a mental illness that puts them at risk of harming themselves or others? I’m avoiding Mr. G’s room. I shouldn’t have read the emergency room notes from the other hospital, where this middle‐aged man raised a stink about the wait. What type of person calls the triage nurse a bitch? From the timestamps on the electronic medical record notes, he stormed from that ER and drove his abdominal pain, vomiting, diarrhea, and discontent directly across town to us.I’m reminded of this oft‐quoted aphorism from Sir William Osler: “The good physician treats the disease; the (...) great physician treats the patient who has the disease.” It’s cited by medical educators as an invocation to never forget the humanistic heart at the center of clinical care. But what if the patient is a jerk? That I haven’t met Mr. G, only read briefly about him, doesn’t stop these ugly feelings from surfacing. (shrink)

I will, in this paper, set out the philosophical foundations and the basic structure of a new theory of justice. I will argue that both these foundations and the theory which is based upon them are intuitively attractive and theoretically sound. Finally, I will argue that both are supported by the fact that they lead to attractive implications such as the following: One can justify at least some governmental redistributive programs which presuppose that those receiving the wealth have a right (...) to it, without being committed to any form of egalitarianism. The justification for these redistributive programs respects property entitlements; it argues, in effect, that property entitlements and rights to some redistribution of wealth presuppose each other, rather than being in conflict with each other. The amount of redistribution to which each indigent person is entitled is directly proportional to the wealth of the society in question and inversely proportional to the number of indigents in the society in question. There are at this stage in the history of Western thought few entirely new ideas in philosophy. Too much has been written about the important issues to allow for entirely new approaches. My theory is no exception to these generalizations, for it draws upon the tradition of John Locke, of the agrarian reforms such as Paine, Spence, and Ogilvie, and of libertarian writers such as Robert Nozick. Nevertheless, I will argue, the resulting theory is a significant advance over the ideas of these earlier writers. (shrink)

It seems to me that those who place great value on the right to human freedom can be badly divided on the question of the use of force by states to defend the liberties of those who are not citizens of that particular state. Concerned about the liberties to be defended, they might be enthusiastic supporters of the use of such force by liberty-loving countries throughout the world. Concerned about the liberties that might be violated when the state marshals its (...) forces for use internationally, they might adopt a more isolationist approach to this issue. This paper is an attempt to help clarify this conflict by looking at some of the philosophical issues it raises. Because I wish to avoid factual debates about current conflicts, I will give no real-life examples. However, they are on my mind, and I hope the reader will keep them in mind as well. (shrink)

Kripke has argued that proper names, as rigid designators, cannot be equivalent in meaning to definite descriptions. in this paper, i argue that definite descriptions are sometimes used rigidly and that proper names are equivalent to definite descriptions used rigidly.

In this paper, I defend the view that the identity of indiscernibles could serve as an adequate basis for a general theory of identity. I then show how a theory of essentialism forces one to modify that general theory. In light of both the original and modified theory, I offer a new resolution of some of the classical and contemporary problems of personal identity.

In a recent article (Brody 2010), I analyzed the debates surrounding charges of biopiracy, that is, charges that developed countries use biotechnology patents to expropriate the biological/genetic heritage of less developed countries. Such charges often are accompanied by the additional charge that biotechnology patents are used to expropriate the traditional knowledge about the use of these resources possessed by indigenous communities in less developed countries. This study examines the influence of three components of corporate social responsibility on team performance. In the proposed model of this study, team performance is indirectly affected by three dimensions of perceived corporate citizenship (i.e., economic, legal, and ethical citizenship) via the mediation of team efficacy and team self-esteem. Surveying members of 172 teams confirms most of our hypothesized effects. Our results show that economic citizenship influences team performance via the mediation of both team efficacy and team self-esteem. However, legal (...) citizenship influences team performance via team efficacy alone, whereas ethical citizenship influences team performance only via team self-esteem. This article represents an analysis of the Jewish philosophy of the Modern and Contemporary as the holistic phenomenon. In contrast to antiquity and the Middle Ages, when philosophy was a rather marginal part of Jewish thought, in Modern Times Jewish philosophy is formed as a distinct part of the World philosophy. Despite the fact that representatives of Jewish philosophy wrote in different languages and actively participated in the different national schools of philosophy, their work has internal continuity and integrity. The (...) article formulates the following five criteria for belonging to Jewish philosophy: belonging to philosophy itself; reliance on Jewish sources; the addressee of Jewish philosophy is an educated European; intellectual continuity ; working with a set of specific topics, such as monism, ethics and ontology, the significance of behavior and practical life, politics, the problem of man, intelligence, language and hermeneutics of the text, Athens and Jerusalem, dialogism. The article provides a list of the main authors who satisfy these criteria. The central ones can be considered Baruch Spinoza, Moshe Mendelssohn, Shlomo Maimon, German Cohen, Franz Rosenzweig, Josef Dov Soloveichik, Leo Strauss, Abraham Yehoshua Heshel, Eliezer Berkovich, Emil Fackenheim, Mordechai Kaplan, Emmanuel Levinas. The main conclusion of the article is that by the end of the 20th century Jewish philosophy, continuing both the traditions of classical European philosophy and Judaism, has become an important integral part of Western thought. This dissertation investigates the nature of imaginatio in the works of Spinoza. The first three chapters are devoted to explicating the ways imaginatio figures in Spinoza's accounts of the attributes, extensio and cogitatio. I show how both attributes are aspects of the same force in which substance perseveres through its essence, and how imaginatio is the key to understanding the movement from corpus to mente. In chapters 4 and 5, my work explores the place of imaginatio in the nature of (...) each of these attributes, examining first the dynamics of extensio before turning to the distinctions essential to cogitatio. These two chapters also trace in great detail the ways in which Spinoza's account of imaginatio derives, on the one hand, from his critique of the principles of Descartes' physics and, on the other, from his critique of Maimonides' account of prophecy. Concluding chapters focus on the 'intensive-extensive' aspects of imaginatio insofar as Spinoza shows these to possess ethical/political implications for the constitution of the state and for human relations as the expression of historical, and a-historical, force. My brief contribution to this volume is not, strictly speaking, historical. No careful analysis of documents will be offered, no critical apparatus will be supplied, and some measure of descriptive inadequacy is likely to lurk behind it. Yet, it is historical in a broader sense. This article addresses the notion of “illusions of the multitude”, or the ideas created by the imagination in the analysis that Baruch Spinoza makes in his works. The text aims to explore the following questions: What characteristics reveal the ideas originating from the imagination? What role do these ideas play in the political and so- cial space? First, I emphasize Spinoza’s explanation of the imagination, as a first mode of knowledge. Chapters 17 and 18 of the TTP constitute a textual unit in which Spinoza submits the case of the ancient Hebrew state to close examination. This is not the work of a historian, at least not in any sense that we, twenty-first century readers, would recognize as such. Many of Spinoza’s claims in these chapters are highly speculative, and seem to be poorly backed by historical evidence. Other claims are broad-brush, ahistorical generalizations: for example, in a marginal note, Spinoza refers (...) to his Jewish contemporaries as if they were identical with the ancient Hebrews. Projections from Spinoza’s own experience of his Jewish and Dutch contemporaries are quite common, and the Erastian lesson that Spinoza attempts to draw from his “history” of the ancient Hebrew state is all too conspicuous. Even Spinoza’s philosophical arguments in these two chapters are not uniformly convincing, as I will attempt to show. Yet in spite of all these faults, the two chapters are a masterpiece of their own kind: a case study of the psychological foundations of politics and religion. The work that comes closest in my mind is Freud’s 1939 Der Mann Moses und die monotheistische Religion. The two works are similar not only in terms of their chronological subject matter – the Hebrews of Moses’s time – but also in their attempt to reconstruct the communal psyche of the Hebrews in order to demonstrate their respective social theories about the foundation of civilization. Needless to say, there are numerous differences between the two works, not the least of which are their distinct aims and the very different political contexts in which they were produced. We will return to this comparison with Freud’s Moses and Monotheism toward the end of the essay, but let me first stage the background for our discussion. Chapter 16 of the TTP begins a new section of the book which primarily deals with the relation between religion and the state. In this chapter Spinoza presents an outline of his political theory and his understanding of key notions such as right, power, the state of nature, the social contract, sovereignty, democracy, and justice. The title of chapter 17 announces its aim and focus: “showing that no one can transfer everything to the Supreme Power, and that this is not necessary; on the Hebrew Republic, as it was during the life of Moses, and after his death, before they elected Kings, and on its excellence; and finally, on the causes why the divine Republic [Respublica divina] could perish, and could hardly survive without rebellions” (III/201). The far less ambitious title of eighteenth chapter states that in it “certain Political doctrines are inferred from the Republic and history of the Hebrews” (III/221). Essentially, the two chapters present a surprising, ironic, and penetrating reading of the story of the divine Hebrew Republic, a reading which highlights both how much and how little was achieved by the use of the fantastic political device of attributing divine sanctification to the state and its sovereign. New pedagogical visions and technological developments have brought argumentation to the fore of educational practice. Whereas students previously 'learned to 'argue', they now 'argue to learn': collaborative argumentation-based learning has become a popular and valuable pedagogical technique, across a variety of tasks and disciplines. Researchers have explored the conditions under which arguing to learn is successful, have described some of its learning potentials and have developed Internet-based tools to support such learning. However, the further advancement of this field presently faces (...) several problems, which the present book addresses. Three dimensions of analysis - historical, theoretical and empirical - are integrated throughout the book. The urochordates, whose stem groups may have included the direct predecessors of the chordate line, serve as an excellent model group of organisms for a variety of scientific disciplines. One taxon, the botryllid ascidian, has emerged as the model system for studying allorecognition; this work has concentrated on the cosmopolitan species Botryllus schlosseri. Studies analyzing self–nonself recognition in this colonial marine organism point to three levels of allorecognition, each associated with different outcomes. The first level controls natural allogeneic rejections and (...) fusions, in which blood‐shared chimeras are formed. The second level leads to morphological resorption of partners within chimeras while the third allows the development of somatic and germ cell parasitic events. Recent studies on multi‐chimeric entities formed in allogeneic fusions reveal evolutionary links between allorecognition, stem cell biology and ecology.
