OBJECTIVE: How outside factors affect physician decision making remains an open question of vital importance. We sought to investigate the importance of various influences on physician decision making when clinical guidelines differ from patient preference. -/- METHODS: An online survey asking 469 primary care providers (PCPs) across four practice sites whether they would order magnetic resonance imaging for a patient with uncomplicated back pain. Participants were randomized to one of four scenarios: a patient's preference for imaging (control), a patient's preference (...) plus a colleague's opinion against imaging (colleague), a patient's preference plus a professional society's recommendation against imaging (profession), or a patient's preference plus an accountable care organization's quality metric that measures physician use of imaging (ACO). Demographic information and the reasoning behind participants' decisions also were obtained. -/- RESULTS: A total of 168 PCPs completed the survey, yielding a 36% completion rate. A majority chose not to pursue imaging: control 68%, colleague 85%, profession 87%, and ACO 78%. Multivariate logistic regression revealed that participants were more likely not to order advanced imaging only when reminded of a professional society recommendation (P = 0.017). Regression also suggested that practice site exerted an effect on the primary outcome. Evidence-based medicine and clinical judgment were the most cited reasons for the decision. -/- CONCLUSIONS: Our results reinforce the potential to leverage professional societies to advance evidence-based medicine and reduce unnecessary testing. At the same time, practice site appeared to exert influence, suggesting that these recommendations must be part of local institutional culture to be effective. (shrink)

Important features of space and time are taken to be missing in quantum gravity, allegedly requiring an explanation of the emergence of spacetime from non-spatio-temporal theories. In this paper, we argue that the explanatory gap between general relativity and non-spatio- temporal quantum gravity theories might significantly be reduced with two moves. First, we point out that spacetime is already partially missing in the context of general relativity when understood from a dynamical perspective. Second, we argue that most approaches to quantum (...) gravity already start with an in-built distinction between structures to which the asymmetry between space and time can be traced back. Soluble components of Notch signalling can be applied to manipulate a central pathway essential for the development of metazoans and often deregulated in illnesses such as stroke, cancer or cardiovascular diseases. Commonly, the Notch cascade is inhibited by small compound inhibitors, which either block the proteolysis of Notch receptors by γ‐secretases or interfere with the transcriptional activity of the Notch intracellular domain. Specific antibodies can also be used to inhibit ligand‐induced activation of Notch receptors. Alternatively, naturally occurring endogenous inhibitors of (...) Notch signalling might offer a specific way to block receptor activation. Examples are the soluble variants of the canonical Notch ligand Jagged1 and the non‐canonical Notch ligand Dlk1, both deprived of their transmembrane regions upon ectodomain shedding, or the bona fide secreted molecule EGFL7. We present frequently used methods to decrease Notch signalling, and we discuss how soluble Notch inhibitors may be used to treat diseases. (shrink)

Spacetime functionalism is the view that spacetime is a functional structure implemented by a more fundamental ontology. Lam and Wüthrich have recently argued that spacetime functionalism helps to solve the epistemological problem of empirical coherence in quantum gravity and suggested that it also (dis)solves the hard problem of spacetime, namely the problem of offering a picture consistent with the emergence of spacetime from a non-spatio-temporal structure. First, I will deny that spacetime functionalism solves the hard problem by showing that it (...) comes in various species, each entailing a different attitude towards, or answer to, the hard problem. Second, I will argue that the existence of an explanatory gap, which grounds the hard problem, has not been correctly taken into account in the literature. (shrink)

Dispositional monists believe that all properties are essentially causal. Recently, an overdetermination argument has been proposed by Trenton Merricks to support nihilism about ordinary objects. I argue that this argument can be extended to target both nihilism about ordinary objects and nihilism about physical particles when dispositional monism is assumed. It implies that a philosopher who both endorses dispositional monism and takes seriously the overdetermination argument should not believe in the existence of physical particles. I end up by discussing possible (...) objections. I suggest, then, that if we live in a world that is inhabited by causal properties but not by chairs and tables, then we also live in a world without electrons and quarks, a world of dispositional properties, that is, a world of causal fields. Les règles de la justice sont, d'après Friedrich Hayek, l'effet d'un ordre spontané et non de la volonté délibérée des hommes. Cette thèse renvoie à une conception de la règle abstraite et générale dont ce livre montre le lien avec les limites de la raison, l'abstraction constituant, selon Hayek, le moyen pour l'esprit de s'occuper d'une réalité que celui-ci ne peut entièrement comprendre. Une "primauté de l'abstrait" s'applique ainsi non seulement à l'ordre social - guidé par les règles abstraites de (...) la justice, mais également à l'ordre sensoriel, c'est-à-dire à notre perception du monde extérieur et aux dispositions à agir qui en résultent. Comment comprendre le lien entre cette primauté de l'abstrait et le libéralisme de Hayek, en particulier concernant sa conception de la "morale du marché" comme discipline des règles abstraites et son rejet de buts particuliers guidant l'action publique? Parce que ses applications concernent la plupart des dimensions de l'action humaine, que son organisation et sa dynamique ne sont plus dissociables de celles de la cité, la science rencontre naturellement les normes qui régissent le comportement humain, celles de la morale ou du droit. Nombre de controverses et de débats concernant les sciences et les technologies en témoignent : s'y invitent tour à tour des normes sociales, éthiques, scientifiques et techniques, et l'on évoque même des "normes du vivant". Ces (...) différentes normes renvoient-elles à des phénomènes comparables? Sont-elles réellement de même nature? La science bouscule les régimes de normes, mais elle est elle-même questionnée quant à sa responsabilité, son éthique ou sa déontologie. Comment science, éthique et droit interagissent-ils et selon quelle dynamique? Comment de nouvelles normes émergent-elles? Les normes juridiques et éthiques sont-elles toujours en retard par rapport aux sciences? Si les sciences et les technologies entretiennent une perpétuelle et stimulante incertitude, cette même incertitude rend parfois difficile l'entente sur les usages. Sciences et société n'évoluent pas nécessairement au même rythme, tandis que le champ des normes, qui ne cesse de s'accroître et de se complexifier, est, pour le droit, source d'indétermination. Penser le progrès comme la capacité à partager des normativités et des normes, telle est l'ambition de cet ouvrage, issu de la quatrième université d'été de l'IHEST qui a exploré sous cet angle les relations entre science, éthique et droit. Abstract Some fictions, it seems, represent the future as closed, in the sense that some future-tensed propositions are true in those fictions. Yet it is surprisingly difficult to accommodate this plausible thesis within an account of truth in fiction. A number of putative examples of closed fictional futures are discussed (Macbeth, Oedipus, Time and the Conways, The Time Machine) and the problems encountered in reconciling them with various accounts of truth in fiction (David Lewis', Gregory Currie's, Alex Byrne's) elaborated. Connections (...) are drawn between metaphysical views on time and theories of fiction, and an attempt is made to show how the tenseless theory of time (roughly, the denial that time flows) can illuminate accounts of fictional truth, and in particular the issue of how there can be true beliefs about what will happen in a fiction.
