Cardiovascular disease (CVD) and sustained high blood glucose as prediabetes are an established comorbidity. People's experience in reconciling these long‐term conditions requires deeper appreciation if nurses are to more effectively support person‐centred care for people who have them. Our analysis explores the initial experience of people admitted to hospital with CVD who then find they also have sustained high blood glucose. Our methodology is informed by the philosophy of Gadamer and applies interpretive description to develop an interpretation of participant experiences. (...) The major theme emerging from participant interviews was the ‘invisible disequilibrium’ characterised by three subthemes: ‘losing equilibrium’, ‘becoming embattled’ and ‘evolving illness’. This study examines CVD and prediabetes in conjunction with the Gadamerian notion of the ‘whole’, as being in a social and emotional world in which illness is also a component part. We explore how participants lived within an ‘invisible disequilibrium’, with prediabetes frequently remaining unnoticed, while CVD was manifest. To identify multiple conditions and support effective intervention to manage them as part of person‐centric care, nursing practice should explore the ‘whole’ of the person's experience, value people’s knowledge as potential indicators of complex illness, thereby reducing the risk of accelerating complex illness. In 1929, John Dewey said that “the problem of restoring integration and cooperation between man's beliefs about the world in which he lives and his beliefs about the values and purposes that should direct his conduct is the deepest problem of human life.” Using this as its theme, this article begins with an examination of Gilbert Harman's reasons for denying the existence of moral facts. It then presents an alternative account of the relationship between science and ethics, making use of (...) the writings of Dewey and Henry David Thoreau. For both Dewey and Thoreau, the dichotomy between a scientific approach to the world and an ethical approach to the world is a false one. The article explores the reasons for believing that the dichotomy is a false one, agreeing with Thoreau that there “is no exclusively moral law—there is no exclusively physical law.”. (shrink)

Conventional ethics of how humans should eat often ignore that human life is itself a form of organic activity. Using Henri Bergson’s notions of intellect and intuition, this chapter brings a wider perspective of the human organism to the ethical question of how humans appropriate life for nutriment. The intellect’s tendency to instrumentalize living things as though they were inert seems to subtend the moral failures evident in practices such as industrial animal agriculture. The customary assumption about dying is that one would rather not. The event of death itself should be postponed for as long as possible, and comfort may be gained from doctrines which promise a victory over it. We celebrate those who try to cheat it. The dying Henry James thought he was Napoleon, and there is something in that, over and above the pathos of a wandering mind, that exemplifies, however parodically, the mental set we expect to find and (...) what we relish in those who attempt to press their own strong case against the disintegrative flow of time and change. We assume that one should struggle against death, setting such a stamp on life that even if the body must die, something the mind has done may not. Attitudes that run counter to this stubbornness are thought defeatist or unwholesome. In his own decline, Charles Baudelaire, catching sight of himself in a mirror, bowed, thinking himself a stranger. That confusion is more chilling than Jams’ because it undermines the treasured integrity of the self: it shows that death is not an invader attacking suddenly from outside. We are in one sense always in its keeping. In this essay I shall argue that whatever revolt against death may catalyse the act of writing poetry, poems are intimately tied to death in ways that complicate and even undermine that revolt. Indeed, since the inception of Romanticism , a poem in order to be a poem has had to engage not only with the fact that in the midst of life we are in its negation, but also with death’s analogues: madness, trance, divisions and questionings of the self. The relationship between poetry and the disruption of the customary self may even be celebratory.But before investigating the relationship between poetry and death, we had better be sure that one can indeed die:At first glance, the preoccupation of the writer who writes in order to be able to die is an affront to common sense. It would seem we can be sure of at least one event: it will come without any approach on our part, without our bestirring ourselves at all; yes, it will come. That is true, but at the same time it is not true, and indeed quite possibly it lacks truth altogether. At least it does not have the kind of truth which we feel in the world, which is the measure of our action and of our presence in the world. What makes me disappear from the world cannot finds its guarantee there; and thus, in a way, having no guarantee, it is not certain. This explains why no one is linked to death by real certitude. No one is sure of dying.1No one can think to cheat death, but to contemplate death is to introduce into thought the epitome of doubt. The one thing I can never know in advance or know demonstrably, by my very nature and by its, is the actual instant of my own death. Conventionally, “I will go when my time comes”: the phrase gestures toward the privacy of each human death, and the Protestant tone of “my time”—part predestination, part ownership—barely hides the inaccessibility of death inside that privacy. With the election of Donald Trump to the presidency of the United States, the discourse of an authoritarianism and the echoes of a fascist past have moved from the margins to the center of American politics. A culture of war buttressed by the forces of white supremacy and militarization has been unleashed in a series of policies designed to return the United States to a history in which the public sphere was largely white and Christian, and the economy and the (...) state were governed by a ruling corporate elite. Militarization and a war culture have become normalized in the United States and this article explores the ways in which a neo-fascism has emerged that furthers war not only abroad but also at home, especially with regards to the ongoing assaults waged by the state against Muslims, immigrants, women’s reproductive rights, and poor minorities of class and color. Despite an extensive amount of research studying the influence of significant others on an individual's ethical behavior, researchers have not examined this variable in the context of organizational group boundaries. This study tests actual and perceptual sharing and variation in ethical reasoning and moral intent within and across functional groups in an organization. Integrating theory on ethical behavior, group dynamics, and culture, it is proposed that organizational structure affects cognitive structure. Departmental boundaries create stronger social ties within the group as (...) well as intergroup biases between the groups. Thus individuals will be more likely to share in ethical reasoning and moral intent with members of their own functional group (in-group) than with members of other functional groups (out-group). Additionally, they will perceive that they are more likely to share in ethical reasoning and moral intent with in-group members than with out-group members. Responding to two versions of two ethical scenarios, respondents contrasted their own ethical behavior to their expected ethical behavior of in-group and out-group members. Empirical results confirmed the hypotheses. Organizational group boundaries create actual as well as perceptual sharing and variation in ethical reasoning and moral intent. Furthermore, when comparing perceptual sharing to actual sharing, results show that individuals understate their sharing of ethical reasoning and moral intent with out-group members and overstate their sharing with in-group members. As organizational boundaries can create actual and perceived differences between groups that could lead to inter-group conflict, suggestions for management focus on removing or blurring inter-group boundaries.
