The COVID-19 pandemic is putting the NHS under unprecedented pressure, requiring clinicians to make uncomfortable decisions they would not ordinarily face. These decisions revolve primarily around intensive care and whether a patient should undergo invasive ventilation. Certain vulnerable populations have featured in the media as falling victim to an increasingly utilitarian response to the pandemic—primarily those of advanced years or with serious existing health conditions. Another vulnerable population potentially at risk is those who lack the capacity to make their own (...) care decisions. Owing to the pandemic, there are increased practical and normative challenges to following the requirements of the Mental Capacity Act 2005. Both capacity assessments and best interests decisions may prove more difficult in the current situation. This may create a more paternalistic situation in decisions about the care of the cognitively impaired which is at risk of taking on a utilitarian focus. We look to these issues and consider whether there is a risk of patients who lack capacity to make their own care decisions being short-changed. (shrink)

Conflict is an important consideration in the intensive care unit. In this setting, conflict most commonly occurs over the ‘best interests’ of the incapacitated adult patient; for instance, when families seek aggressive life-sustaining treatments, which are thought by the medical team to be potentially inappropriate. Indeed, indecision on futility of treatment and the initiation of end-of-life discussions are recognised to be among the greatest challenges of working in the ICU, leading to emotional and psychological ‘burnout’in ICU teams. When these disagreements (...) occur, they may be within the clinical team or among those close to the patient, or between the clinical team and those close to the patient. It is, therefore, crucial to have a theoretical understanding of decision-making itself, as unpicking misalignments in the family’s and clinical team’s decision-making processes may offer strategies to resolve conflict. Here, we relate Kahneman and Tversky’s work on cognitive biases and behavioural economics to the ICU environment, arguing that these biases could partly explain disparities in the decision-making processes for the two conflicting parties. We suggest that through the establishment of common ground, challenging of cognitive biases and formulation of mutually agreeable solutions, mediation may offer a pragmatic and cost-effective solution to conflict resolution. The litigation process is intrinsically adversarial and strains the doctor–patient–relative relationship. Thus an alternative external party should be considered, however mediation is not frequently used and more research is needed into its effectiveness in resolving conflicts in the ICU. (shrink)

The shift towards "empirical bioethics" was largely triggered by a recognition that stakeholders' views and experiences are vital in ethical analysis where one hopes to produce practicable recommendations. Such perspectives can provide a rich resource in bioethics scholarship, perhaps challenging the researcher's perspective. However, overreliance on a picture painted by a group of research participants—or on pre‐existing literature in that field—can lead to a biased view of a given context, as the subjectivity of data generated in these ways cannot (and (...) should not wholly) be escaped. In response, we propose the implementation of a complementary approach of ethno‐immersion in bioethics research. By positioning oneself in the context being researched, the researcher can better understand the realities of that context. The researcher's understanding will, naturally, be subjective too. However, it will act as a better developed and more informed outsider view, when considering the picture painted by participants and previous studies, thus enabling the researcher to introduce more nuance when analysing data. We introduce this approach after examining what we call the context detachment problem, whereby some bioethics scholarship—empirical or otherwise—fails to reflect the reality of the healthcare setting it concerns. Our proposed ethno‐immersion (which differs from formal ethnography) is then explored as a response, highlighting its benefits, and answering the question of timing within a research project. Finally, we reflect on the applicability of our proposal to non‐empirical bioethics scholarship, concluding that it remains important but may require some adjustments. (shrink)

COVID-19 forced lockdown in India, leading to the loss of job, crisis of food, and other financial catastrophes that led to the exodus migration of internal migrant workers, operating in the private sector, back to their homes. Unavailability of transport facilities led to an inflicted need to walk back to homes barefooted without lack of any other crucial resources on the way. The woeful state of internal migrant workers walking back, with all their stuff on their back, holding their children, (...) was trending on social media worldwide. Their problem continued even after reaching home, including misbehavior of villagers, indifferent mannerism of family members toward them, inability to fulfill family responsibility, and financial crisis, which led to stress, fear, and trauma for these internal migrant workers. The present paper aimed to assess the idiosyncratic stressors of internal migrant workers throughout the pandemic era and their responses toward those problems, which helped them cope with it. In-depth semistructured telephonic interviews were conducted with 25 internal migrant workers who were working in different cities in India before lockdown. The analyses revealed that multiple stressors include financial crisis, unavailability of food, inability to continue education, inability to pay house rent, lack of support from neighbors and family, and other psychological stressors that affected them. However, they also tried multiple strategies to deal with the problems, including a cognitive appraisal of the problem and making oneself psychologically competent to deal with the situation. Social support of family and friends played a vital role in enhancing hardiness and increasing the level of happiness at scarce times. At the same time, some of them could not deal with stressors and opted for psychiatric help to manage the physical symptoms of stress. (shrink)

The design process in Kashmiri carpet weaving is distributed over a number of actors and artifacts and is mediated by a weaving notation called talim. The script encodes entire design in practice-specific symbols. This encoded script is decoded and interpreted via design-specific conventions by weavers to weave the design embedded in it. The cognitive properties of this notational system are described in the paper employing cognitive dimensions (CDs) framework of Green (People and computers, Cambridge University Press, Cambridge, 1989) and Blackwell (...) et al. (Cognitive technology: instruments of mind—CT 2001, LNAI 2117, Springer, Berlin, 2001). After introduction to the practice, the design process is described in ‘The design process’ section which includes coding and decoding of talim. Should nonprejudiced reproducers genetically select embryos for light skin under background conditions of racism and colourism, given that darker skin will be disadvantageous for their child? Many intuit that there are strong moral reasons not to select light skin in these contexts. I argue that existing procreative principles cannot adequately account for this judgement. Instead, I argue that a more compelling rationale for this intuition is that such selection completes an instance of race or colour injustice. Given this, I propose (...) a new, complementary principle—Procreative Justice—which holds that reproducers have strong pro tanto moral reasons to avoid completing race and colour injustices via their selection choices. While these reasons may be overridden by competing considerations, they nonetheless continue to exert normative force. (shrink)

Many scholars and activists have argued that the International Criminal Court holds potential for advancing the rights of women and girls, leading to extensive feminist engagement with and investment in the Court. As the ICC enters its second decade of existence, this article offers a reflection on both the possibilities and the challenges facing feminists. Can the international criminal law really offer a site for enhancing the rights of women? And if so, how? To explore these questions I focus on (...) the interaction between feminist activism and international criminal law institutions in relation to crimes of sexual and gender-based violence. I argue that some of the feminist strategies deployed to get sexual violence onto the international agenda have resulted in perverse outcomes. This should lead us to greater critical reflection regarding how international law conceives of sexual violence and direct our future engagements with international legal institutions. In particular feminist activists and scholars need to move away from focusing on the number of prosecutions towards challenging the international criminal law to characterise the nature of the harm in accordance with a recognition of sexual rights. In recent years, the network has become more complex, and the attacker’s ability to attack is gradually increasing. How to properly understand the network security situation and improve network security has become a very important issue. In order to study the method of extracting information about the security situation of the network based on cloud computing, we recommend the technology of knowledge of the network security situation based on the data extraction technology. It converts each received cyber security event into (...) a standard format that can be defined as multiple brochures, creating a general framework for the cyber security situation. According to the large nature of network security situation data, the Hadoop platform is used to extract aggregation rules, and perform model extraction, pattern analysis, and learning on a network security event dataset to complete network security situation rule mining, and establish a framework for assessing the state of network security. According to the results of the federal rule extraction, the level of network node security risk is obtained in combination with signal reliability, signal severity, resource impact, node protection level, and signal recovery factor. A simulation test is performed to obtain the intrusion index according to the source address of the network security alarm. Through the relevant experiments and analysis of the results, the attack characteristics obtained in this study were obtained after manually reducing the network security event in the 295 h window. The results show that after the security event is canceled, the corresponding window attack index decreases to 0, indicating that this method can effectively implement a network security situation awareness. The proposed technique allows you to accurately sense changes in network security conditions. (shrink)

The present paper explores and analyzes one intriguing form of death called self-willed death (Icchā maraṇa) as depicted in Mahābhārata in light of the contemporary debate between Bruce Sullivan and Phillip Ernest about the distinction between yogayukta and non-yogayukta forms of it. In the first part, the paper presents Sullivan’s distinction between yogayukta, one with disenchantment, world-weariness, and detachment with that of non-yogayukta, the one attached to glory, honor, and power which is contested by Ernest. While taking the debate further, (...) the paper argues that proper understanding of the concept of Icchā maraṇa requires a deeper analysis of the distinction between ‘will’ and ‘self-will’ and adequate study of the conception of life and death that the epic depicts. After taking up these two tasks in the second and third parts, the fourth section assesses the debate in light of the above two sections analysis. It argues that ‘self-will’ is an expression of freedom where the individual prescribes conditions, unlike the ordinary ‘will’ where choice is between available alternatives. This distinction leads us to the death of Bhīṣma, which is Icchā maraṇa in its presented form that moves beyond the distinction available in the debate between Sullivan and Ernest. (shrink)

The academic literature on unethical leadership is witnessing an upward trend, perhaps given the magnitude of unethical conduct in organisations, which is manifested in increasing corporate fraud and scandals in the contemporary business landscape. Despite a recent increase, scholarly interest in this area has, by and large, remained scant due to the proliferation of concepts that are often and mistakenly considered interchangeable. Nevertheless, scholarly investigation in this field of inquiry has picked up the pace, which warrants a critical appraisal of (...) the extant research on unethical leadership. To this end, the current study systematically reviews the existing body of work on unethical leadership and offers a robust and multi-level understanding of the academic developments in this field. We organised the studies according to various themes focused on antecedents, outcomes and boundary conditions. In addition, we advance a multi-level conceptualisation of unethical leadership, which incorporates macro, meso and micro perspectives and, thus, provide a nuanced understanding of this phenomenon. The study also explicates critical knowledge gaps in the literature that could broaden the horizon of unethical leadership research. On the basis of these knowledge gaps, we develop potential research models that are well grounded in theory and capture the genesis of unethical leadership under our multi-level framework. Scholars and practitioners will find this study useful in understanding the occurrence, consequences and potential strategies to circumvent the negative effects of unethical leadership. (shrink)

The páramos of Boyacá in Colombia are earmarked for delimitation to prevent the expansion of the agricultural frontier and protect endemic flora that contribute to water provision for cities. A varied conservation toolbox will be used, including the creation of protected areas for re-wilding and the ‘sustainable’ transitioning of livelihoods identified as environmentally destructive. Agriculture and cattle livestock farming has been identified for transitioning. Despite the negative discourse related to livestock holding, this paper argues that small-scale agropastoralism contributes to re-peasantisation (...) and provides the foundations for an agrobiodiverse conservation approach. Agropastoralism facilitates re-peasantisation through strong socio-economic networks, interconnected communities, the solidarity economy, and self-management of natural resources. Whilst, agropastoral mobility spatially binds social networks across large and disconnected spaces. Mobility is also fundamental to dynamic land access and pasture management, as it prevents over-grazing. This exemplifies how resilient socio-economic networks and mobile production strategies could be harnessed for agrobiodiversity, instead of land sparing and other sedentary ‘green’ economies. This paper makes conceptual contributions to ‘autonomy’ in re-peasantisation by empirically demonstrating the importance of mobile and flexible systems of production. It also makes a novel methodological contribution in applying a spatial lens that further unpacks how movement across the páramos facilitates autonomy and re-peasantisation. These themes are explored using interview data from 53 semi-structured interviews with key stakeholders and small-scale agropastoralists from across the páramos and field observation. The paper concludes by recommending a harnessing of agropastoral knowledge, to potentiate agrobiodiversity, for a more socio-ecologically just approach to farming and conservation in the páramos. After China, India has the most skewed sex ratio at birth. These two Asian countries account for about 90 to 95% of the estimated 1.2 to 1.5 million missing female births annually, worldwide, due to gender-biased sex selection. To understand this extreme discrimination against girls, this article examines the gendered biopolitics embedded in population policies, new sex selection technologies, and in the social reproduction of patriarchal society. The ethical consequences of advanced reproductive technologies, which remove the moral turpitude around gender-based (...) sex selection by reformulating it into a “modern”, “scientific” endeavour, facilitating the rise of “missing girls”, make this an issue of gender justice, as noted by the World Population Report 2020. This article argues that unpacking gendered biopolitics within the household is crucial to understanding the reproduction of son preference and daughter aversion since it is here that reproduction and parenthood are subjected to biopolitical governance. We discuss how “biosocial” strategies of the household aimed at producing the “desired” and “right” family of more sons at the cost of daughters are operationalized through women’s bodies with a view to family mobility. While women and girls continue to bear the burden and costs of social reproduction that lie at the heart of the patriarchal capitalist system of accumulation, a perusal of more recent studies suggests the beginning of an equalizing trend of parental investments, especially in the health and education of daughters who are “allowed” to be born. We suggest that familial enhancement of girls’ human capital can help as a means of developing girls’ capabilities and agency, enhancing their power in the biopolitics of the family and increasing their “bio-value” in parents’ eyes. For collection of data, a structured questionnaire and a structured teaching programme was used. t-test was used for analysis and interpretation of the data. Background As noise brings great error in the analysis of metallographic images, an adaptive weighted mean filtering method proposed to overcome the shortcomings of the standard mean filtering method. Methods The method used to detect the pulse noise points in the image, and then the modified mean method used to filter out the detected noise points. Patents on metallographic image processing have discussed for the development of the proposed methodology. Results It is shown that filter window can be filtered in (...) comparison with the conventional 3×3, 5×5 and 7×7 filt window to reduce noise detection and reduce the complexity of the weight calculation. Conclusion It can be concluded that this method can better protect the details of the image, has better filtering effect than the standard mean filtering, and its processing speed is faster than the median filtering of the large window, which has profound significance for the edge detection and processing of the metallographic image. This paper presents a method for the study of the influence of stability of a power transformer on the power system based on the vibration principle. Traditionally, the EMD and EEMD algorithms are employed to test the box vibration signal data of the power transformer under three working conditions. The proposed method utilizes a partial EMD screening along with MPEEMD method for the online monitoring of power transformer. A complete online monitoring system is designed by using the STM32 processor and (...) LabVIEW system. The proposed system is compared with EMD and EEMD algorithms in terms of the number of IMFs obtained by decomposition, maximum correlation coefficient, and mean square error. The inherent mode correlation, when compared with the mean square error of the reconstructed signal, shows that the reconstruction error of MPEEMD algorithm is 4.762×10−15 which is better than the traditional EMD algorithm. It is observed from the results that the proposed method outperforms both EMD and EEMD algorithms. (shrink)

This article examines the development of a multidimensional, transnational feminist research approach from and within Uganda in relation to a high-profile case of domestic violence and femicide of a middle-class, upper-caste Indian migrant woman in Kampala in 1998. It explores indigenous Ugandan public and Ugandan Asian/indian community interpretations and the dynamics of cross-racial feminist mobilisation and protest that emerged in response to the Joshi-Sharma domestic violence case. In doing so, it advocates for a transnational feminist research approach from and within (...) Uganda and the Global South that works against the grain of nationalist and nativist biases in existing feminist scholarly trends. This approach lays bare power inequalities and internal tensions within and across racialised African and Asian communities, and thus avoids the romanticisation of cross-racial feminist African-Asian solidarities. The welfare and protection of human subjects is critical to the integrity of clinical investigation and research. Institutional review boards were thus set up to be impartial reviewers of research protocols in clinical research. Their main role is to stand between the investigator and her human subjects in order to ensure that the welfare of human subjects are protected. While there is much literature on the conflicts of interest faced by investigators and researchers in clinical investigations, an area that is (...) less explored is CIs that may affect members of IRBs during the institutional ethics review of clinical investigations. This article examines the notion of CIs in clinical research and attempts to develop a framework for a clearer and more balanced approach to identifying CIs that may influence members of IRBs and impede their independence. It will also apply the proposed framework to demonstrate how IRBs possess, or at least may appear to possess, forms of financial CIs and non-financial CIs. The proper identification and management of these CIs is critical to preserving the integrity of clinical investigations and achieving the primary aim of human subjects protection. (shrink)

Cognition is predominantly seen as information processing in multidisciplinary landscape of cognition studies, despite having had a formidable opposition from embodied and embedded perspectives in the last few decades. This paper analyses cognitive processes involved in different task domains of Kashmiri carpet weaving practice from the theoretical frameworks of cognitivism and situated-distributed cognition. After introducing the practice and its task domains (Section −1), paradigmatic cognitive activities involved in them are discussed and how these are explained by the two theoretical paradigms (...) and why the latter framework explains these processes better (Section-2). This article describes various ways actors in Kashmiri carpet weaving practice deploy a range of artifacts, from symbolic, to material, to hybrid, in order to achieve diverse cognitive accomplishments in their particular task domains: information representation, inter and intra-domain communication, distribution of cognitive labor across people and time, coordination of team activities, and carrying of cultural heritage. In this repertoire, some artifacts position themselves as naïve tools in the actors’ environment to the point of being ignored; however, their usage-in-context unfolds (...) their cognitive involvement in the tasks. These usages-in-context are shown through artifact analysis of their routine, improvised, and opportunistic uses, where cognitive artifacts like talim—the central artifact of this practice—are shown to play not only multifunctional roles beyond representation, but are also complemented by trade-specific skills bearing strong cognitive implications in a task. Sports officials, players, and fans are concerned about overseas player rankings for the IPL auction. These rankings are becoming progressively essential to investors when premium leagues are commercialized. The decision-makers of the Indian Premier League choose cricketers based on their own experience in sports and based on performance statistics on several criteria. This paper presents a scientific way to rank the players. Our research examines and contrasts different multicriteria decision-making algorithms for ranking foreign players under various criteria to assess their (...) performance and efficiency. The paper uses three MCDM algorithms, TOPSIS, TODIM, and NR-TOPSIS, for foreign players ranking in the Indian Premier League. Our analysis is limited to the batsmen and bowlers only. We perform the analysis using Python language, a popular high-level programming language. Finally, we perform a sensitivity analysis to determine the stability of each method when the weights of the criterion or the value of a parameter was changed. Our analysis exhibits the superiority of TODIM over TOPSIS and NR-TOPSIS. (shrink)

Through the analysis of various examples, the paper endeavours to establish Vāda as a comprehensive way to attain correct knowledge. Examples from day-to-day life as well as the historic-literary world help to explicitly comprehend these tools that have been used for establishing the nearest truth from ancient times in India. The design process in Kashmiri carpet weaving is a distributed process encompassing a number of actors and artifacts. These include a designer called naqash who creates the design on graphs, and a coder called talim-guru who encodes that design in a specific notation called talim which is deciphered and interpreted by the weavers to weave the design. The technological interventions over the years have influenced these artifacts considerably and triggered major changes in the practice, from heralding profound cognitive accomplishments in (...) manually driven design process causing major alterations in the overall structure of the practice. The recent intervention is by the digital technology: on the one hand, it has brought precision and speedy processing in the design process, and on the other, it has eliminated some of the crucial actors from the practice. This paper, which forms part of a larger study on the situated and distributed cognitive process in Kashmiri carpet-weaving practice, describes the technological makeover of the design artifacts involved in this practice over the years and their resultant cognitive impact on the design process as well as on the practice. Healthcare facilities in receiving countries regularly encounter guest workers whose need for acute or subacute care triggers the prospect of termination of employment and repatriation. In these scenarios, country‐specific migration and employment policies and norms of medical professionalism and ethics offer some guidance, but also create tensions. It is not clear under what conditions such medical repatriation is ethically permissible.This paper analyses the application of a previously articulated criteria for the ethical medical repatriation of undocumented immigrants, to the situation of (...) documented guest workers, with focus on the context of Singapore. We examine how these standards could be adapted and applied to the provision of care for guest workers, and argue that healthcare institutions and medical professionals have a duty to intervene in employers’ decisions to repatriate guest workers for medico‐economic reasons when repatriation essentially amounts to ‘patient dumping’. Barriers and challenges in implementing the criteria, and their possible solutions will be discussed. (shrink)

Background Healthcare professionals face a myriad of ethical challenges during the coronavirus disease 2019 pandemic. However, there is limited literature examining the ethical challenges faced by HCPs in low- and medium-income countries. The research was designed to explore the ethical challenges experienced by HCPs in a Malaysian hospital setting during the pandemic. Methods Semistructured interviews were conducted via video calls with 10 Malaysian HCPs across different clinical disciplines involved in managing patients diagnosed with COVID-19 infections. The calls were audio-recorded, transcribed (...) verbatim and checked. Thematic analysis with constant comparison across transcripts was carried out to identify categories and themes. Results Three main themes emerged. Firstly, there was deprioritisation of care for non-COVID-19 patients resulting from resource limitations. HCPs raised concerns that there was curtailed access to various healthcare services by non-COVID-19 patients. There was also a trade-off between protecting individual patient safety and public health interests. Secondly, patients were disempowered from decision-making; the decision to segregate suspected COVID-19 patients to high-risk areas without seeking patients’ approval may result in an increased risk of infection. Lastly, HCPs expressed internal conflicts when balancing the professional duty of care against concerns about contracting COVID-19 and spreading it to their family members. Conclusion The study highlighted ethical issues faced by HCPs in Malaysia during the pandemic. It underscores the need for clinical ethics consultation services in hospitals to navigate the various ethical dilemmas. (shrink)

Night eating syndrome is currently classified as an Other Specified Feeding or Eating Disorder under the Diagnostic Statistical Manual−5. This systematic review aims to consolidate the studies that describe the sociodemographic, clinical and psychological features of NES in a population of patients with eating disorders, obesity, or those undergoing bariatric surgery, and were published after the publication of the DSM-5. A further aim was to compare, where possible, NES with BED on the aforementioned variables. Lastly, we aimed to appraise the (...) quality of the studies being included in the review. We conducted a systematic search on three databases which resulted in the selection of 22 studies for the review. We included the articles that studied patients with NES and their sociodemographic, clinical and psychological features in a clinical population, through a quantitative study design. Articles were excluded if the NES patients included in the study had a comorbid psychological disorder, and/or the sample was collected from a university/non-clinical population, and/or the study design was qualitative, and/or NES features were compared with any other disorder, except BED. Our study found that no conclusions about the link between any sociodemographic feature and an NES diagnosis could be made. Further, NES patients presented with elevated ED pathology and higher occurrence of depressive symptoms than controls. Contrary to the literature suggesting that NES and Binge Eating Disorder patients often report overlapping features, questioning the validity of NES as an ED diagnosis, we found that BED can be differentiated from NES by the higher occurrence of emotional eating, body related concerns and abnormal eating episodes. The review also suggested an overlap between NES and Sleep-Related Eating Disorder. We recommend that it is essential to study NES as an independent disorder to further develop its diagnostic criteria and treatment options, thereby, increasing the quality of life of the patients suffering from this syndrome. With the advent of modern technology, the way society handles and performs monetary transactions has changed tremendously. The world is moving swiftly towards the digital arena. The use of Automated Teller Machine cards has led to a “cash-less society” and has fostered digital payments and purchases. In addition to this, the trust and reliance of the society upon these small pieces of plastic, having numbers engraved upon them, has increased immensely over the last two decades. In the past few years, (...) the number of ATM fraud cases has increased exponentially. With the money of the people shifting towards the digital platform, ATM skimming has become a problem that has eventually led to a global outcry. The present review discusses the serious repercussions of ATM card cloning and the associated privacy, ethical and legal concerns. The preventive measures which need to be taken and adopted by the government authorities to mitigate the problem have also been discussed. (shrink)
