BackgroundMoral case deliberation as a form of clinical ethics support is usually implemented in health care institutions and educational programs. While there is no previous research on the use of clinical ethics support on the level of health care regulation, employees of regulatory bodies are regularly confronted with moral challenges. This pilot study describes and evaluates the use of MCD at the Dutch Health Care Inspectorate.The objective of this pilot study is to investigate: 1) the current way of dealing with (...) moral issues at the IGZ; 2) experience with and evaluation of MCD as clinical ethics support, and 3) future preferences and needs regarding clinical ethics support for dealing with moral questions at the IGZ.MethodsWe performed an explorative pilot study. The research questions were assessed by means of: 1) interviews with MCD participants during four focus groups; and 2) interviews with six key stakeholders at the IGZ. De qualitative data is illustrated by data from questionnaires on MCD outcomes, perspective taking and MCD evaluation.ResultsProfessionals do not always recognize moral issues. Employees report a need for regular and structured moral support in health care regulation. The MCD meetings are evaluated positively. The most important outcomes of MCD are feeling secure and learning from others. Additional support is needed to successfully implement MCD at the Inspectorate.ConclusionWe conclude that the respondents perceive moral case deliberation as a useful form of clinical ethics support for dealing with moral questions and issues in health care regulation. There is an increasing body of research on what kind of ethical challenges health care professionals experience regarding the quality of care. In the Netherlands the Dutch Health Care Inspectorate is responsible for monitoring and regulating the quality of health care. No research exists on what kind of ethical challenges inspectors experience during the regulation process itself. In a pilot study we used moral case deliberation as method in order to reflect upon inspectors’ ethical challenges. The objective of this paper (...) is to give an overview of the ethical challenges which health care inspectors encounter in their daily work. A thematic qualitative analysis was performed on cases that were collected from health care inspectors in a moral case deliberation pilot study. Eight themes were identified in health care regulation. These can be divided in two categories: work content and internal collaboration. The work of the health care inspectorate is morally loaded and our recommendation is that some form of ethics support is provided for health care inspectors. (shrink)

Ook al staan de beelden van Confucius, Lao Zi en Boeddha in sommige Chinese tempels weer broederlijk bijeen op het altaar, hun verschillende wegen tot zelfcultivering worden zelden in onderlinge samenhang beschreven. Dit boek benadert confucianisme, daoïsme en het Chinese boeddhisme in zijn vroege ontwikkeling vanuit hun visie op de praktijk van de zelfcultivering. Hoewel ze een volstrekt ander doel hebben dan de westerse zelfverwerkelijking, mogen de Chinese wegen zich in onze contreien toch in een grote populariteit verheugen. Omdat de (...) zelfcultivering in China primair gericht is op de emoties, vormt de emotiecultivering de rode draad in dit boek. Het begrip Qi, levenskracht, speelt een grote rol in alle cultiveringspraktijken en staat centraal in het klassieke Chinese denken. Het eerste hoofdstuk biedt een inleiding in dit begrip, waardoor de lezer een overzicht krijgt van een aantal Chinese kernconcepten en theorieën en daarmee ook van de context van de emotiecultivering. Naast de beschrijving daarvan in confucianisme, daoïsme en boeddhisme biedt het boek verder een beschouwing van de Chinese landschapsschilderkunst als "filosoferen met het penseel". In deze tijd van secularisering is het begrijpelijk dat men de filosofie van de religie vaak wil beperken tot een kritiek van de religie en tot de kritische bespreking van de bekende godsbewijzen. 0Dit handboek kiest voor een andere aanpak. De filosofie van de religie is geen monolithische discipline. Filosofie van de religie is soms een binnen-religieuze aangelegenheid waarin de filosofie wordt ingezet als een hulpwetenschap voor de theologie, soms gaat het om een zuiver rationele aangelegenheid, soms domineert een naturalistische (...) blik en soms poogt men vooral de betekenis van het religieuze te verhelderen. Al deze modellen van filosofie van de religie komen hieraan bod en worden in hun historische ontwikkeling besproken. Daarom vertrekt dit handboek niet van de 'gedachte God' van metafysici en wetenschappers ('des philosophes et des savants', Pascal), maar veeleer van de geleefde religie ('la religion vécue', 'lived religion') om na te gaan hoe het religieuze in de praktijk functioneert en wat de betekenis ervan is: "Wat betekent het religieus te zijn?" is een belangrijker vraag dan "Is religie waar?". NG van Kampen is a well-known theoretical physicist who has had a long and distinguished career. His research covers scattering theory, plasma physics, statistical mechanics, and various mathematical aspects of physics. In addition to his scientific work, he has written a number of papers about more general aspects of science. An indefatigable fighter for intellectual honesty and clarity, he has pointed out repeatedly that the fundamental ideas of physics have been needlessly obscured. As those papers appeared in various journals, partly (...) in Dutch, it was felt that it would be worthwhile to collect them (translating the Dutch material into English) and make them available to a larger audience. Het denken van de Nederlandse filosoof Leo Polak (1880-1941) heeft nog altijd niet aan actualiteit ingeboet. Hij bepleitte hartstochtelijk de autonomie van de menselijke rede, worstelde met de grondslag van onze ethiek en wierp zich in een tijd van politieke massabewegingen en toenemende verzuiling op als kampioen van het vrije denken. Zelfs tijdens de Duitse bezetting hield hij vast aan zijn zelfstandig oordeel – een houding die hij eind 1941 met de dood in een concentratiekamp moest bekopen. Al die jaren (...) hield hij een dagboek bij, voor hem een essentieel middel om greep te krijgen op zijn leven. Op basis hiervan worden in deze bundel nieuwe aspecten van Polaks leven en werk belicht: zijn denken over schuld en vergelding, zijn verhouding tot grote filosofen als Spinoza en Heymans en zijn houding tegenover het nationaalsocialisme, maar ook zijn familieleven, zijn boekenverzameling en zijn geestelijke nalatenschap. The thesis developed and defended in this paper is that is it false that all knowledge is founded on experience. Much of our knowledge (or alleged knowledge), it is argued, is based on testimony. Still, many philosophers have either not dealt with testimony at all, or treated it very unkindly. One of the reasons for this is that those philosophers (such as Descartes and Locke) work with a concept of knowledge according to which knowledge is certain, indubitable, and/or self-evident. And (...) if knowledge is what these philosophers say it is, then there is no such thing as knowledge based on testimony indeed. Thomas Reid is introduced as holding that we do have testimonial knowledge and that therefore Descartes' and Locke's concept of knowledge is untenable. Reid furthermore holds that human beings are endowed with a disposition to accept or believe what otherstell us („the principle of credulity”). The working of this principle is refined through all kinds of experience. What Reid says or shows is how this disposition in fact operates. Many epistemologists, however, have higher aspirations and look for reasons or arguments that can justify our factual acceptance of testimony. The inductive argument Hume offers, it is argued, is unconvincing. There is even reason to think that the principle of credulity can never be justified by adducing reasons. This does not imply, however, that acceptance of testimony is unjustified. Whether or not it is depends, among other things, on the concept of justification one uses. On an internalist concept of justification (as Locke's or Hume's) this disposition may never be justified. But on an externalist conception it may. This may be disappointing, given some widely held philosophical aspirations, but at the same time it is, as Alston has said, a lesson in intellectual humility. (shrink)
