ObjectiveIn Belgium, people with an incurable psychiatric disorder can file a request for euthanasia claiming unbearable psychic suffering. For the request to be accepted, it has to meet stringent legal criteria. One of the requirements is that the patient possesses decision-making capacity. The patient’s decision-making capacity is assessed by physicians.The objective of our study is to provide insight in the assessment of decision-making capacity in the context of euthanasia for patients with psychic suffering caused by a psychiatric disorder.MethodTwenty-two semistructured interviews (...) with psychiatrists and neurologists were analysed with NVivo, a qualitative analysis software to code and organise transcribed data.ResultsDifferent views and approaches regarding decision-making capacity in the context of euthanasia emerged from the data.Most of the physicians have some knowledge of the cognitive ability approach on decision-making capacity. According to this approach, four abilities constitute decision-making capacity: communication, understanding, appreciation and reasoning. We observed differences in the way these abilities are valued in relation to competence. Some physicians take additional elements into consideration when assessing decision-making capacity.Physicians acquired their knowledge on the subject in many different ways. Most of the physicians reported that decision-making capacity was not part of their training.ConclusionWe conclude that physicians assess decision-making capacity in different ways and that personal values and beliefs influence their approach. As such, a common approach in assessing the decision-making capacity of a patient among the interviewed physicians is lacking. Less arbitrariness could be obtained by consistently implementing the cognitive ability approach. (shrink)

The particular situation of children in hospitals is discussed on the basis of a principle of equality which is founded on both childrens' rights and theology. It is just because of these equal rights of children that special medical care is required which does justice to the characteristics and the stage of development of children. Medical care like that does not at all have tobe a general obstacle to considerations of economic efficiency. The particular dependence of children on stable social (...) surroundings implies a priority of outpatient and home treatment as weil as the integration of parents and other important persans to whom the child relates closely when it comes to an inevitable admission in a hospital. A model of medical ethics based on »informed consent« needs particular aspects concerning the interests of children in hospitalssuch as child-orientated information and the graduated legal obligatory nature of the child 's will. Die Rede von Gott ist eine Weise, Wirklichkeit zu erschliessen. Diese Aussage erscheint in einer Zeit verwunderlich, in der die Naturwissenschaften zu der massgebenden Autoritat der Wirklichkeitsdeutung avanciert sind. In weit verbreiteten Reaktionen auf deren Erfolg wird der Aussagebereich einer Rede von Gott eher auf ein unspezifisches Jenseits oder auf das subjektive Empfinden und moralische Handeln von Individuen eingeschrankt. Die phanomenologische Analyse weist aber auf, dass die Wirklichkeit, mit der der leiblich existierende Mensch immer schon verbunden ist, weiter ist als (...) das, was objektivierende Darstellungen zeigen konnen. Narrative Formen und metaphorische Ausdrucke erschliessen die Wirklichkeit in einer gegenuber den Naturwissenschaften eigenstandigen Erscheinungsweise. Die Auslegung von Texten mittels hermeneutischer Methoden zeigt fragile und von Spannungen gekennzeichnete Ordnungen einer kontingenten und geschichtlichen Wirklichkeit. Dazu gehort auch die christliche Rede von Gott, die an die Erzahlungen des biblischen Kanons anknupft. Daruber hinaus gibt es eine Erscheinungsweise der Wirklichkeit mit nur indirekt beschreibbaren Phanomenen, die aber eine grosse Relevanz fur das Wirklichkeitsverstandnis hat. Die Rede von Gott erhoht mit Hilfe einer Vielzahl von Grenzausdrucken die Aufmerksamkeit auf die Phanomene dieser Erscheinungsweise. Einige Grenzausdrucke wie "Reich Gottes", "Gott der Vater", "In Christus sein" sollen mit phanomenologischen Methoden interpretiert werden. (shrink)

This article addresses the extent and ways in which gender inequality in the newspaper coverage of arts and culture has changed in France, Germany, the Netherlands, and the United States, 1955-2005. Through a quantitative content analysis, we mapped all articles that appeared in two elite newspapers in each country in four sample years 1955, 1975, 1995, and 2005. First, despite increasing women’s employment in arts and culture and a quantitative feminization of journalism, elite newspaper coverage of women in arts and (...) culture has hardly changed, making up about 20-25 percent consistently over the last 50 years. Second, our results show surprisingly few cross-national differences in the amount of the newspaper coverage devoted to women in arts and culture. Third, although women are underrepresented in the coverage of all artistic genres, there is some evidence of horizontal sex segregation—particularly in architecture and modern dance and fashion —as well as vertical sex segregation—in that attention to women has increased in “highbrow” genres that have declined in status. Finally, as the status of an actor type increases from laymen to artistic directors, the proportion of women decreases in newspaper attention to arts and culture. (shrink)

The task of transforming Durkheim's sociological perspective into an explanation of poverty in rural areas carries a triple handicap: Durkheim was not concerned with material well-being, he did not conceptualize structures of inequality, and his explanation of the division of labor was flawed. His late book on religion, however, contains an explanation of institutional innovation which offers a new starting point for understanding formal dimensions such as differentiation and pluralism, and these in turn as they are related to poverty. When (...) these dimensions are combined with technical organization, differences in welfare can be explained. When the summary formula for this combination of structure and technology is compared with those for Marxist and conventional economic explanations, the result is a dramatic contrast between the two materialist theories and a true sociological position.
