Abstract:Diagnostic classification systems in psychiatry have continued to rely on clinical phenomenology, despite limitations inherent in that approach. In view of these limitations and recent progress in neuroscience, the National Institute of Mental Health (NIMH) has initiated the Research Domain Criteria (RDoC) project to develop a more neuroscientifically based system of characterizing and classifying psychiatric disorders. The RDoC initiative aims to transform psychiatry into an integrative science of psychopathology in which mental illnesses will be defined as involving putative dysfunctions in (...) neural nodes and networks. However, conceptual, methodological, neuroethical, and social issues inherent in and/or derived from the use of RDoC need to be addressed before any attempt is made to implement their use in clinical psychiatry. This article describes current progress in RDoC; defines key technical, neuroethical, and social issues generated by RDoC adoption and use; and posits key questions that must be addressed and resolved if RDoC are to be employed for psychiatric diagnoses and therapeutics. Specifically, we posit that objectivization of complex mental phenomena may raise ethical questions about autonomy, the value of subjective experience, what constitutes normality, what constitutes a disorder, and what represents a treatment, enablement, and/or enhancement. Ethical issues may also arise from the (mis)use of biomarkers and phenotypes in predicting and treating mental disorders, and what such definitions, predictions, and interventions portend for concepts and views of sickness, criminality, professional competency, and social functioning. Given these issues, we offer that a preparatory neuroethical framework is required to define and guide the ways in which RDoC-oriented research can—and arguably should—be utilized in clinical psychiatry, and perhaps more broadly, in the social sphere. Pediatric dentists rely on access to hospital operating rooms for safe, effective, and humane delivery of dental care. The children who benefit most from dental treatment in a hospital operating room are those who are very young, have dental anxieties or phobias, are precommunicative or noncommunicative, need extensive or invasive dental treatments, or have special healthcare needs. Diminishing access to hospital operating rooms for pediatric dental treatment has become an escalating problem in contemporary times. Financial barriers, hospital costs, reimbursement rates, (...) health insurance policies and deductibles, out-of-network hospitals, socioeconomic factors, and the COVID-19 pandemic are prominent contributing factors. This problem in access to care has resulted in long waiting times for hospital operating rooms, deferral of medically necessary dental care, and pain and infection among this vulnerable patient population. Pediatric dentists have responded to the problem by utilizing alternative methods of care delivery, such as in-office deep sedation or in-office general anesthesia, and by implementing aggressive medical management of dental caries. However, the youngest of pediatric patients and children with special healthcare needs still remain at a disadvantage in receiving definitive dental treatment. This article aims to highlight the ethical challenges faced by pediatric dentists in contemporary practice in the face of limitations in hospital operating room access through four case scenarios. The University of Michigan conference “Where Religion, Policy, and Bioethics Meet: An Interdisciplinary Conference on Islamic Bioethics and End-of-Life Care” in April 2011 addressed the issue of brain death as the prototype for a discourse that would reflect the emergence of Islamic bioethics as a formal field of study. In considering the issue of brain death, various Muslim legal experts have raised concerns over the lack of certainty in the scientific criteria as applied to the definition and diagnosis of brain (...) death by the medical community. In contrast, the medical community at large has not required absolute certainty in its process, but has sought to eliminate doubt through cumulative diagnostic modalities and supportive scientific evidence. This has recently become a principal model, with increased interest in data analysis and evidence-based medicine with the intent to analyze and ultimately improve outcomes. Islamic law has also long employed a systematic methodology with the goal of eliminating doubt from rulings regarding the question of certainty. While ample criticism of the scientific criteria of brain death (Harvard criteria) by traditional legal sources now exists, an analysis of the legal process in assessing brain death, geared toward informing the clinician’s perspective on the issue, is lacking. In this article, we explore the role of certainty in the diagnostic modalities used to establish diagnoses of brain death in current medical practice. We further examine the Islamic jurisprudential approach vis-à-vis the concept of certainty (yaqīn). Finally, we contrast the two at times divergent philosophies and consider what each perspective may contribute to the global discourse on brain death, understanding that the interdependence that exists between the theological, juridical, ethical, and medical/scientific fields necessitates an open discussion and active collaboration between all parties. We hope that this article serves to continue the discourse that was successfully begun by this initial interdisciplinary endeavor at the University of Michigan. (shrink)

Famously posed by seventeenth-century French philosopher René Descartes, the mind–body problem remains unresolved in western philosophy and science, with both disciplines unable to move convincingly beyond the dualistic model. The persistence of dualism calls for a reframing of the problem through interdisciplinary modes of inquiry that include non-western points of view. One such perspective is Islamic theology of the soul, which, while approaching the problem from a distinct point of view, also adopts a position commensurate with dualism. Using this point (...) of convergence as a conceptual starting point, we argue that bringing into dialogue contemporary neuroscientific, philosophy of mind, and Sunni Islamic theological discourses may provide a fruitful way of reframing the age-old mind–body problem. Background Professional misconduct undermines safe and quality care; however, little is known about its nature and influential factors. Aim This study aimed to explain the factors influencing professional misconduct in nurses. Research Design This qualitative study was conducted using the conventional content analysis method. Participants and Research Context Data were collected using semi-structured interviews with 19 nurses working in the hospital selected through a purposeful method and analyzed by Graneheim and Lundman approach. Ethical Considerations The ethics committee of Tehran University (...) of Medical Sciences approved this study with the ethics code IR.TUMS.FNM.REC.1400.187. Informed consent was obtained from all participants. Participants were assured of confidentiality. Findings Factors influencing professional misconduct by nurses were categorized into three main categories: human factors (nurses’ professional characteristics, personal characteristics of nurses and patient/companion, patient’s clinical condition), procedural factors (procedural conditions, possibility of proving misconduct), and organizational factors (recruitment process, conditions of resources, managing misconduct, bureaucracy, and ward characteristics). Conclusion This study assists in explaining the factors influencing professional misconduct by nurses. Therefore this study’s results can help managers and planners develop interventions to prevent and correct factors that contribute to misconduct and strengthen factors that prevent misconduct in order to ensure quality and safe patient care. There are several models for delivering bad news, the most important and widely used being the SPIKES protocol. Cultural differences in breaking bad news in different societies with different cultures call for special attention. Muslim societies are examples of communities with special cultural and religious requirements. Then, when collecting information about a person's perception of the illness or the incident, consider his or her view of spirituality and the effect of calamities on human transcendence so as to assess the type (...) and amount of information the patient or the family needs. When preparing a suitable setting for delivering bad news, pay attention to the spiritual needs of Muslims such as the possibility of prostration, recitation of Qur'an, and talking to God in private or even aloud. Be aware of, pay attention to, and respect particular religious views of the patient, and if necessary, correctly exploit such views to deliver bad news. Although some Muslim patients may have no demands for transcendence and human growth at the first stage, but over time and after accepting the condition, they will have demands. Therefore, health care providers must provide the necessary facilities for Muslim patients in terms of the required information, setting, and people present in the setting and provide information appropriate to the culture of these patients so as to give an excellent and comprehensive care. Despite the obvious link between spirituality, religiosity and ethical judgment, a definition for the nature of this relationship remains elusive due to conceptual and methodological limitations. To address these, we propose an integrative Spiritual-based model derived from categories presumed to be universal across religions and cultural contexts, to guide future business ethics research on religiosity. This article aims to empirically test in the context of Islam. It examines how different Muslims' views of God influence their ethical judgments in organizations, and (...) how this process is mediated by their religious practice and knowledge. Comprising focus group and vignette designs, the study was carried out with a random sample of 427 executives and management professionals from Saudi. After data collection, the study hypotheses were tested using structural equation modeling. The findings confirmed that a view of God based on hope might be more closely associated with unethical judgments than a view based on fear or one balancing hope and fear. Furthermore, religious practice and knowledge were found to mediate the relationship between Muslims' different views of God and their ethical judgments. These results provide unique theoretical insights into religiosity's influence on ethical judgment, with important implications for management. (shrink)

While the human agent must have the capacity for reflexivity, intentionality and consciousness, the same agent must also be affected by the social world in which she lives: herein lies the essence of the structure and agency dialectic. This paper argues that while some realists are in principle committed to a dialectical relationship between structure and agency, there is some dissonance between this commitment and the concepts of agency that they develop. I highlight the exclusion of the unconscious and habit (...) from realist notions of agency and argue that this oversight serves to unbalance the dialectic between structure and agency thereby leading to the over-empowerment of agency. The concepts of agency developed by Margaret Archer, Anthony Giddens and Pierre Bourdieu are discussed in this paper. Archer's concept of agency is argued to focus exclusively on reflexivity whilst neglecting to include the unconscious and habit. Giddens is shown to develop a much improved concept of agency, which includes the unconscious, however, his rejection of the independent causal powers of structure and agency problematises his commitment to the dialectic. A much improved approach to theorising agency, developed within a critical realist framework, is offered drawing on Bourdieu's concept of habitus. The paper concludes with a discussion of gender, and considers how the unconscious and habit can help to better understand the myriad ways in which gender functions in society. (shrink)

In this article, I offer a reassessment of the influence of two disparate bodies of thought – republicanism and existentialism – on Hannah Arendt. Arendt, I argue, is not involved in an ‘agonistic appropriation’ of Heidegger. Arendt identifies two opposed attitudes in Heidegger’s work. The first Promethean moment places Heidegger squarely in the tradition of Western political philosophy, and the second seemingly correcting for this recommends a quietism. Arendt rejects both these attitudes. Machiavelli rather than Heidegger, I argue, is the (...) key influence on Arendt's political thought. Through a reading of Arendt’s published works and her unpublished 1955 lecture notes on Machiavelli I show that the central categories of Arendt’s thought emerge from her meditation on Machiavelli’s texts. Arendt finds in Machiavelli a performative understanding of politics, an accurate understanding of the connection between morality and politics, and a primacy given to politics that she thought it deserves. On envisage trop fréquemment le conflit syrien à partir des éléments de son histoire très récente. Un retour sur les étapes qui ont précédé l’émergence de l’État-nation syrien permet de mesurer le poids de l’histoire et la complexité du processus de la construction nationale. L’Antiquité, le Moyen-âge, la période omeyyade, celle de l’Empire ottoman, puis la décolonisation et la construction nationale sont autant de strates de l’histoire syrienne qui permettent de comprendre comment s’articulent les identités, les sentiments d’appartenance et la (...) mobilisation des imaginaires constitutifs des différentes idéologies nationales à l’œuvre dans la période contemporaine et jusqu’à ce jour. Le nationalisme, l’islamisme, le panarabisme ont chacun idéalisé un pan du passé de la Syrie mais étaient voués à l’échec, chacun pour des raisons liées à leur propre structure. Ce retour sur quelques jalons de l’histoire syrienne contribue à expliquer l’inachèvement de la construction nationale jusqu’à la crise actuelle. Les fragmentations sociale, économique et politique plongent aujourd’hui la Syrie dans une situation de rupture violente et la mettent au défi de prendre acte de ce qui est mort et que puisse advenir ce qui n’est pas encore né. What is the conceptual status of modernity in the Muslim world? Scholars describe Muslim attempts at appropriating this European idea as being either derivative or incomplete, with a few calling for multiple modernities to allow modern Islam some autonomy. Such approaches are critical of the apologetic way in which Muslims have grappled with the idea of modernity, the purity and autonomy of the concept of which is apparently compromised by its derivative and incomplete appropriation. None have attended to the conceptual (...) status of this apologetic itself, though it is certainly the most important element in Muslim debates on the modern. This essay considers the adoption of modernity as an idea among Muslim intellectuals in nineteenth-century India, a place in which some of the earliest and most influential debates on Islam's modernity occurred. It argues that Muslim apologetics created a modernity whose rejection of purity and autonomy permitted it a distinctive conceptual form. (shrink)

Kidney transplantation is the most efficacious and cost-effective treatment for end-stage renal disease. However, the treatment’s accessibility is limited by a chronic shortage of transplantable kidneys, resulting in the death of numerous patients worldwide as they wait for a kidney to become available. Despite the implementation of various measures the disparity between supply and needs continues to grow. This paper begins with a look at the current treatment options, including various sources of transplantable kidneys, for end-stage renal disease. We propose, (...) in accordance with others, the introduction of compensated kidney donation as a means of addressing the current shortage. We briefly outline some of the advantages of this proposal, and then turn to examine several of the ethical arguments usually marshaled against it in a bid to demonstrate that this proposal indeed passes the ethics test. Using available data of public opinions on compensated donation, we illustrate that public support for such a program would be adequate enough that we can realistically eliminate the transplant waiting list if compensation is introduced. We urge a pragmatic approach going forward; altruism in living kidney donation is important, but altruism only is an unsuccessful doctrine. (shrink)

The purpose of this study was to examine how ethical approval and competing interests are addressed by medical journals in Iran. In a cross-sectional study, 151 journals accredited by the Publications Commission of the Ministry of Health and Medical Education were reviewed. Data collection was carried out by assessing journal guidelines and conducting structured phone interviews with journal managers, focusing on how ethical considerations and conflicts of interest (COI) are addressed. Overall, 135 of the 151 journals (89.4 percent) examined some (...) aspect of ethical considerations of submitted articles. Authors were required to disclose their financial sponsors by 98 journals (64.9 percent), while COI disclosure was required by 67 journals (44.4 percent). We conclude that the rate of addressing ethical considerations is not far from ideal, but the requirement for COI disclosure needs more attention. (shrink)

This paper attempts to reconsider the nature and function of the ‘spiritual’ dimension in literary texts and in literary study in the context of the present state of the discipline of literary studies. The present era is often defined as a ‘post-secular’ era, one in which themes of spirituality and mysticism are increasingly noticeable in literary works. The paper argues that to maintain its relevance to contemporary writers and readers, literary criticism has to address these themes in a concrete and (...) effective way. The paper recommends a comparative approach to the discussion of spirituality and mysticism in contemporary literature and literary criticism. In order to carry effective analytical potential, this approach, the paper emphasizes, has to be developed from specific spiritual traditions. The paper first discusses the disciplinary crisis literary studies have always been exposed to since their inception as a discipline of study in academic institutes. It then reviews the current state of the discipline and describes how the discipline came to be dominated by scientific and social approaches. Finally, it suggests the reinstitutionof the ‘spiritual’ element in literary study as a way out from the state of crisis in the discipline of literary studies. The paper thus attempts to strengthen the disciplinary identify of literary studies while exploring interdisciplinary aspects of the study literature. (shrink)

Numerous scholars have taken the popular stance against Roman emperor Nero’s actions. In the style of fictional narrative, this article argues for the defense of Nero against the widely held opinion. The article is written under the guise of Seneca, his tutor and advisor. It is written in the genre of biography and in the format of a letter addressed to Cassius Dio.

Purpose. Nietzschean proponents classify people into seemingly two distinct groups: those possessing 'Master' moralities and those with 'Slave' moralities. Each type of person is characterized to have certain qualities, traits, ideologies, and methods of dealing with everyday situations. This paper attributes these moralities to the personnel working in the corporate sector of Pakistan to observe their prospects of success. Originality. A specialized survey instrument was designed to gauge different Morality Types of the study subjects by calculating a Morality Quotient. The (...) respondents were then categorized into three categories possessing, Slave, Master and Neutral or Mixed traits. These traits are then contrasted to the position held by the subjects to see which morality type was more prevalent in higher management levels and whether the association was significant. Conclusions. Our results prove to be contradictory to common wisdom and shows that despite the leadership-like qualities of those with Master moralities, the proportion of those with Slave moralities was significantly higher at senior management levels of the corporate sector in Pakistan. Moreover, a trend of declining Master morality traits is observed with the rise in managerial level. In addition, an association between work experience and Morality type was also observed indicating adaptability of cross morality traits with increasing job experience. These findings are in line with the increasing focus on democratic and adaptable leadership instead of autocratic leadership styles in contemporary organizations. (shrink)

Internet of Things-based environments pose various challenges due to their anytime/anywhere computing, and the efficient cryptographic based key management is one of the major challenges in Internet of Things. The key management life cycle consists of initialization, key generation, key registration, key backup, key update, key recovery, and key revocation. Our contribution in this paper is to summarize the state-of-the-art key management schemes and techniques in different scenarios, such as mobile ad hoc networks, wireless sensor networks, and the Internet of (...) Things environments. Further different issues related specifically to the Internet of Things environment are discussed and the causes and effects pertaining to the security breach for Internet of Things are identified. Furthermore, in this research work, we develop a novel permutation of threshold and identity-based key management schemes for the Internet of Things environment and have proposed future directions to counteract the attacks on confidentiality, integrity, authentication, and availability of security services in the Internet of Things environment and identified the two key management schemes, that is, identity and threshold schemes for Internet of Things, to resolve Internet of Things key management issues and maximum possible security services effective implementation. This paper discusses how it is possible to find, in the Islamic tradition, a universal concept of “humanity”. It insists on the concept of “ummah” as a peculiar Muslim way to define a comprehensive humanity, and try to point out its internal limits. It shows the communal implication of this concept and the ways it can, or cannot, be considered as constitutive of a we-group concept that may eventually lead to conflict and violence. One of the consequences of Al-Qaeda’s terrorism has been its blurring of the distinction between national and international politics, both of which have lost a great deal of their former autonomy by serving as hosts for a set of new global concerns and practices. The Global War on Terror can be seen as an effort to externalize Al-Qaeda’s global threat by internationalizing it in a conventional war, and thus reinforcing both the autonomy of international politics and its separation from that (...) of a national variety. More than a conservative move to protect the international order, however, the War on Terror was also an ambitious attempt to remake global politics in the wake of the Cold War. Racism and ethnicity are usually considered as the same concepts while in fact ethnicity is a sub-class of racism. In every nation, there are some ethnic groups with the same origin and similar customs that may or may not be judged (...) equally by the power-handler ethnic groups. An example of such country is Afghanistan which is severely an ethnic country. This study explores the social, cultural, and scientific investigations done on the people's races and ethnical characteristics by using Afghan types as examples. Racism is not the result of scientific observation, but it is due to the human differences that happened between 16th and 19th century when people began differentiating among themselves. This aspect of racism is well expressed by the American sociologist "Feagin". In his view, the natural superiority of some people over others is rejected. The novel The Kite Runner depicts the two major Afghan ethnical populations, Pashtun and Hazara, and their social, cultural, and religious conflicts. Accordingly, this article will try to examine the root of ethnic prejudices and oppression among the Afghan people in the novel and the reasons for their ethnic conducts are explained and analyzed based on the Feagin’s denial of human difference. Moreover, by providing some evidence it is proved that the biological difference is just some excuses for the powerful section to gain their egotistic goals. (shrink)

L’espace de la ville, ses composants et (...) les pratiques qui s’y déroulent sont sollicités dans cette étude, comme point d’articulation de notre problématique qui consiste à savoir comment le pont en tant qu’élément architectural dans l’espace d’une ville, par son statut, les pratiques qui s’y passent et les valeurs qu’il porte, pourra fonctionner comme médiateur sémiotique. Nous tenterons de montrer à travers l’étude de notre corpus, le pont Khadjou à Ispahan, ville iranienne, comment ce rôle médiateur s’implante sur le niveau cognitif autant que passionnel des sujets qui le côtoient. Résumé Les études déjà menées depuis des années 1970 autour de la sémiotique de l’espace montrent que les fondements méthodologiques et épistémologiques de cette approche se situent bel et bien, dans la lignée de la sémiotique structurale. Pourtant, certains espaces et le sens qui y émerge en acte ne peuvent pas être réduits à un cadre théorique si restreint. Cet article se propose de montrer, à travers l’analyse d’une chambre décrite par Sohrab Sépehri, poète et peintre moderne iranien, comment l’explication (...) de la production du sens dans cet espace échappe aux fondements de l’appareil théorique classique. Notre hypothèse est que cette chambre en tant qu’espace qui ne se veut plus comme un objet-valeur, ni comme un espace opératoire ou manipulateur, prend sens dans un processus interactionnel comme un mouvement tourbillonnaire qui implique le sujet, l’entoure, l’engage et le fait ainsi vivre une expérience vécue. Cet espace éprouvé devient ainsi l’espace existentiel de la présence du monde du poète. Mais selon quelles caractéristiques déterminons-nous le régime d’espace auquel appartient la Chambre bleue de Sépehri? Comment et dans quelle mesure la Chambre qui appelle le poète sans arrêt, contribue à lui former un nouvel être au monde et à le faire avancer dans son itinéraire transcendant? BackgroundTo assess ethical concerns associated with participation in a financial incentive programme to help adolescents with type 1 diabetes improve diabetes self-management.MethodsFocus groups with 46 adolescents with type 1 diabetes ages 12–17 and 38 of their parents were conducted in the Seattle, Washington metropolitan area. Semistructured focus group guides addressed ethical concerns related to the use of FI to promote change in diabetes self-management. Qualitative data were analysed and emergent themes identified.ResultsWe identified three themes related to the ethical issues adolescents (...) and parents anticipated with FI programme participation. First, FI programmes may variably change pressure and conflict in different families in ways that are not necessarily problematic. Second, the pressure to share FIs in some families and how FI payments are structured may lead to unfairness in some cases. Third, some adolescents may be likely to fabricate information in any circumstances, not simply because of FIs, but this could compromise the integrity of FI programmes relying on measures that cannot be externally verified.ConclusionsMany adolescents with type 1 diabetes and their parents see positive potential of FIs to help adolescents improve their self-management. However, ethical concerns about unfairness, potentially harmful increases in conflict/pressure and dishonesty should be addressed in the design and evaluation of FI programmes. (shrink)

Shared decision-making has become a new focus of health policy. Though its core elements are largely agreed upon, there is little consensus regarding which outcomes to prioritize for policy-mandated shared decision-making.

The Bhagavad Gita's philosophical and political significance remains forever contemporary. Schistosomes are intravascular parasitic helminths (blood flukes) that infect more than 200 million people globally. Proteomic analysis of the tegument (skin) of these worms has revealed the surprising presence of glycolytic enzymes on the parasite's external surface. Immunolocalization data as well as enzyme activity displayed by live worms confirm that functional glycolytic enzymes are indeed expressed at the host–parasite interface. Since these enzymes are traditionally considered to function intracellularly to drive glycolysis, in an extracellular location they are hypothesized to engage (...) in novel “moonlighting” functions such as immune modulation and blood clot dissolution that promote parasite survival. For instance, several glycolytic enzymes can interact with plasminogen and promote its activation to the thrombolytic plasmin; some can inhibit complement function; some induce B cell proliferation or macrophage apoptosis. Several pathogenic bacteria and protists also express glycolytic enzymes externally, suggesting that moonlighting functions of extracellular glycolytic enzymes can contribute broadly to pathogen virulence.
