Three experiments explore the role of working memory in motor skill acquisition and performance. Traditional theories postulate that skill acquisition proceeds through stages of knowing, which are initially declarative but later procedural. The reported experiments challenge that view and support an independent, parallel processing model, which predicts that procedural and declarative knowledge can be acquired separately and that the former does not depend on the availability of working memory, whereas, the latter does. The behaviour of these two processes was manipulated (...) by providing or withholding visual (and auditory) appraisal of outcome feedback. Withholding feedback was predicted to inhibit the use of working memory to appraise success and, thus, prevent the formation of declarative knowledge without affecting the accumulation of procedural knowledge. While the first experiment failed to support these predictions, the second and third experiments demonstrated that procedural and declarative knowledge can be acquired independently. It is suggested that the availability of working memory is crucial to motor performance only when the learner has come to rely on its use. (shrink)

Participants struck 500 golf balls to a concealed target. Outcome feedback was presented at the subjective or objective threshold of awareness of each participant or at a supraliminal threshold. Participants who received fully perceptible feedback learned to strike the ball onto the target, as did participants who received feedback that was only marginally perceptible . Participants who received feedback that was not perceptible showed no learning. Upon transfer to a condition in which the target was unconcealed, performance increased in both (...) the subjective and the objective threshold condition, but decreased in the supraliminal condition. In all three conditions, participants reported minimal declarative knowledge of their movements, suggesting that deliberate hypothesis testing about how best to move in order to perform the motor task successfully was disrupted by the impoverished disposition of the visual outcome feedback. We also help everyone involved with the care of seriously ill patients focus on what is best based on the values of the most important person in the room; the one in the hospital bed. So, when we call for (...) a bioethics consultation from a clinical ethicist, it’s because something has gotten stuck or, perhaps, we could benefit from a reframe—or a new set of eyes to ground the scenario or the emotional miasma that tends to linger and drift when on the cusp between life and death.In the 1990s, at least in my medical school and residency programs, medical ethicists in the hospital were not often a part of direct patient care. They were like philosophers, available to curbside and advise on ethically murky cases but at a distance from the trenches. These wise, generally older men with white beards parceled out all potential ethical sides of the case, leaving us to decide the best way forward. The Socratic Method1 was often employed, so we were left to grapple with the ambiguity that exists in most issues about what’s right or wrong. It sometimes felt frustrating but now I realize the importance of honing the skill of tolerating the feeling of not immediately knowing the best way to proceed in the practice of medicine. As it turned out, by the time I started practicing palliative care in 2013 and began to regularly collaborate with what was then our clinical ethics partners in care, our ethics director Dr. F was also a wise older man with a white beard. But he and the rest of his (non-male, un-bearded) team didn’t have that untouchable ivory tower kind of protection around them. They were accessible. They dug into the hard stuff right alongside us when they were asked to get involved.One time when I consulted ethics on a case, I’d been navigating a switch from practicing internal medicine to palliative care. At the time I was dealing with personal ambivalence—feeling mired in the heaviness that infused my workdays and unsure if I could take it. It turned out that a clinical consultation, and a family meeting with Dr. F, helped in more ways than one.That day, we gathered in the medical intensive care unit conference room. It doubled as a storage closet, so the team, including our patient’s family, dragged in chairs and stools and crowded around the oblong melamine table next to the whiteboard with half-erased arterial blood gas calculations. We hunched together to see where things stood and make some essential decisions.As we do in family meetings, we reconfirmed our roles on the team: ICU fellow, resident, and medical student. Nurses fluttered in and out as their patient care allowed. The patient’s daughter with silver-streaked hair was in attendance, as well as her son, who was slumped in his seat. Engaging Eriugena, Eckhart and Cusanus contains two new essays and nine others published between 2005 and 2019. The essays explore Eriugena, Eckhart and Cusanus as bold thinkers deeply engaged with their times and culture. John Scottus Eriugena, Meister Eckhart and Nicholas of Cusa are key figures in the medieval Christian Neoplatonic tradition. This book focuses on their engagement with practical, experiential issues and controversies. Eriugena revises Genesis' Adam and Eve narrative and makes sexual difference and overcoming it central to his (...) Periphyseon. Eckhart's Annunciation sermons urge his hearers to give birth to God's son within their lives, and he develops a distinctive approach to pain and suffering. His radical preaching on the Eucharist and mystical union was judged heretical, but was later taken up by Nicholas of Cusa. Coins and banking became key symbols in Cusanus' exploration of humanity as created in God's image, and he used mechanical clocks in reflecting on time and eternity. 'Engagement' also describes these thinkers' reception of their predecessors and how later readers appropriated their works. Eriugena struggled with the legacy of Augustine and the Greek Fathers. Eckhart's theology of suffering provoked varied responses from his students Henry Suso and Johannes Tauler, and the twentieth-century therapist Ursula Fleming. Cusanus provides the volume's lynchpin as two articles analyse his reading of Eriugena and Eckhart, and a third discusses how he deftly countered Johannes Wenck's accusations of heresy. I have argued elsewhere that the traditional Augustinian account of the fall is conceptually defective. It offers causal explanations for the first instance of willing evil which violate affirmations of divine goodness and justice integral to Christian thought. Augustine is the most influential spokesperson for the conviction that the first human pair initiated fallenness on the earth by decisions and actions they took within time though indeed very near to time’s beginning. A supporting account, embellished imaginatively by the tradition, enlarges (...) the context to embrace a prior choice of evil by Satan early in his angelic career. The narrative structure of the Genesis tale of Adam and Eve, mistakenly read as a kind of history, converts easily into a doctrine of the fall as an event in time preceded and precipitated by other events in time regarded as its causes. The allure of this causal explanation arises from our natural wish to “understand” how the fall could occur. But by attributing the first sin to angelic or human pride, or collaterally to a supposed imperfection in the first humans as created, Augustine unwittingly shifts the ultimate responsibility for moral evil onto God. This makes creaturely freedom and responsibility into empty notions, thereby subverting the intentionality of the religious belief in fallenness. (shrink)

Karl Barth leaves room by his own principles for further, even different thinking about Jews and gender than he records in the Dogmatics. Now that Marquardt, Klappert, Sonderegger, Soulen, and others have offered sympathetic critiques from a generally Barthian point of view, and Eberhard Busch has exhaustively laid to rest any biographical questions of Barth’s relation to the Jewish people in his 1996 book, Unter dem Bogen des einen Bundes: Karl Barth und die Juden 1933–1945, the way lies open to (...) carry forward Barth’s theological critique of nineteenth century theology and of abstraction further into the areas of Jews and gender, and to propose constructive pneumatological and exegetical supplements to his thinking. Barth’s thinking on women and men, like his thinking on Jews, labors, for all its promise in its time, under a defect he calls abstraction. Barth faulted Calvin for abstracting both elect human beings and the electing God from Jesus Christ, whom Barth called the elect human being and the electing God in one. Yet on the election of the community Barth did not entirely escape the abstraction. In the manner of Augustine’s Retractions, Barth wrote to Maruardt toward the end of his life that he had been so busy with Israel that he had had no time for the Jews. Does Barth, as Marquardt and Sonderegger suggest, throw up despite himself a conceptual screen onto which he at once projects an abstraction and behind which he hides actual human beings , a screen resistant to the blowing of the Spirit? Do the concepts “ man” and “ woman” form a similar conceptual screen, dating from Schleiermacher’s dialogue Christmas Eve, in which men and women have essentially different responses to the incarnation of Jesus Christ? Das ewig M??nnliche and das ewig Weibliche, inherited from German romanticism, see to run afoul of Barth’s trinitarian particularism, as does “ Israel”. Yet further application of Barth’s own principles, including more attention to the Spirit, leaves him more options in his treatment of Jews and gender, and richer exegesis – including exegesis by anagogy. The author takes Eve Ensler as a companion in a journey which attempts to spit out phallocentric meaning and engage with wicked and wordy cunning linguists as they move towards more honest telling-theology from the body. The author examines the divine masquerade, which is encouraged by some Christian theology and calls for a stripping away of such pretence and an acknowledgement of what play we (...) are actually engaged in so that we may come closer to what it is we are attempting to express about the divine. While valuable work has been done addressing clinical ethics within established healthcare systems, we anticipate that the projected growth in acquisitions of community hospitals and facilities by large tertiary hospitals will impact the field of clinical ethics and the day-to-day responsibilities of clinical ethicists in ways that have yet to be explored. Toward the goal of providing clinical ethicists guidance on a range of issues that they may encounter in the systematization process, we discuss key considerations and potential challenges in (...) implementing system-wide ethics consultation services. Specifically, we identify four models for organizing, developing, and enhancing ethics consultation activities within a system created through acquisitions: train-the-trainer, local capacity-building, circuit-riding, and consolidated accountability. We note each model’s benefits and challenges. To our knowledge, this is the first paper to consider the broader landscape of issues affected by consolidation. We anticipate that clinical ethicists, volunteer consultants, and hospital administrators will benefit from our recommendations. (shrink)

This translation from Russian of The Philosophy of Hegel as a Doctrine of the Concreteness of God and Humanity marks the first appearance in English of any of the works of Russian philosopher Ivan Aleksandrovich Il’in. Originally published in 1918, on the eve of the Russian civil war, this two- volume commentary on Hegel marked both an apogee of Russian Silver Age philosophy and a significant manifestation of the resurgence of interest in Hegel that began in the early twentieth century. (...) Il’in’s colleague A. F. Losev accurately observed in the same year it appeared: “Neither the study of Hegel nor the study of contemporary Russian philosophical thought is any longer thinkable without this book of I. A. Il’in’s.” Some Hegel scholars may know this work through the abridged translation into German that Il’in produced himself in 1946. However, that edition omitted most of the original volume two. Noted Hegel scholar Philip T. Grier’s edition—with an introduction setting Il’in’s work in its proper historical, cultural, and philosophical contexts and annotation throughout—represents the first opportunity for non-Russian-speaking readers to acquaint themselves with the full scope of Il’in’s still provocative interpretation of Hegel. Checklists have been used to improve quality in many industries, including healthcare. The use of checklists, however, has not been extensively evaluated in clinical ethics consultation. This article seeks to fill this gap by exploring the efficacy of using a checklist in ethics consultation, as tested by an empirical investigation of the use of the checklist at a large academic medical system (Cleveland Clinic). The specific aims of this project are as follows: (1) to improve the quality of ethics consultations (...) by providing reminders to ethics consultants about process steps that are important for most patient-centered ethics consultations, (2) to create consistency in the ethics consultation process across the medical system, and (3) to establish an effective educational tool for trainers and trainees in clinical ethics consultation. The checklist was developed after a thorough literature review and an iterative process of revising and testing by a group of experienced ethics consultants. To pilot test the checklist, it was distributed to 46 ethics consultants. After a six-month pilot period in which ethics professionals used the checklist during their clinical activities, a survey was distributed to all of those who used the checklist. The 10-item survey examined consultants’ perceptions regarding the three aims listed above. Of the 25 survey respondents, 11 self-reported as experts in ethics consultation, nine perceived themselves to have mid-level expertise, and five self-reported as novices. The majority (68 percent) of all respondents, regardless of expertise, believed that the checklist could be a “helpful” or “very helpful” tool in the consultation process generally. Novices were more likely than experts to believe that the checklist would be useful in conducting consultations. The limitations of this study include: reduced generalizability given that this project was conducted at one medical system, utilized a small sample size, and used self-reported quality outcome measures. Despite these limitations, to the authors’ knowledge this is the first investigatation of the use of a checklist systematically to improve quality in ethics consultation. Importantly, our findings shed light on ways this checklist can be used to improve ethics consultation, including its use as an educational tool. The authors hope to test the checklist with consultants in other healthcare systems to explore its usefulness in different healthcare environments. (shrink)

When patients are not able to speak for themselves, surrogate decision makers are asked to guide treatment decisions and formulate a plan of care in accordance with what the patients would have wanted. This necessitates an exploration into the patients’ views about life and how it should be lived, how the patients constructed their identity or life story, and their attitudes towards sickness and suffering. When an individual appoints a surrogate, such as a healthcare power of attorney, a common presumption (...) is that this designation evinces merit. This obscures the possibility of multiple other considerations that influence individual choice. This article presents a clinical case in which the claim to know someone best created a controversy that brought treatment decisions to a standstill. Further, it illuminates how the question, “Given the current medical condition, what would this person want?” risks presuming that a singular, unambiguous preference exists and that one person can provide the answer. Clinical ethicists can play a vital role in situations when there is a dispute among a designated surrogate and family members over a patient’s preference. By embracing the complexity of the desire to synthesize seemingly irreconcilable perspectives about identity, uncovering the reasons that underlie disagreement, and guiding inquiry in such a way that allows stakeholders to move beyond the conflict, clinical ethicists can facilitate decision making that honors the patient and may mitigate moral distress. In this book, Christopher F. Zurn shows why a normative theory of deliberative democratic constitutionalism yields the best understanding of the legitimacy of constitutional review. He further argues that this function should be institutionalized in a complex, multi-location structure including not only independent constitutional courts but also legislative and executive self-review that would enable interbranch constitutional dialogue and constitutional amendment through deliberative civic constitutional forums. Drawing on sustained critical analyses of diverse pluralist and deliberative democratic arguments concerning the legitimacy of (...) judicial review, Zurn concludes that constitutional review is necessary to ensure the procedural requirements for legitimate democratic self-rule through deliberative cooperation. Claiming that pure normative theory is not sufficient to settle issues of institutional design, Zurn draws on empirical and comparative research to propose reformed institutions of constitutional review that encourage the development of fundamental law as an ongoing project of democratic deliberation and decision. The Moral Target: Aiming at Right Conduct in War and Other Conflicts comprises essays that discuss aspects of war and other conflicts in the light of nonconsequentialist ethical theory. Topics include the relation between conditions that justify starting war and those that justify stopping it, the treatment of combatants and noncombatants in war, collaboration, justice after war and other conflicts, terrorism, resistance to communal injustice, and nuclear deterrence.

Dr Taylor's book analyses the disease concept as it developed in medical history and seeks to clarify it with the help of concepts largely derived from logical class theories. A solution is proposed to the problem of how to distinguish between the class of 'patients' and the class of 'healthy persons' which corresponds to the actual diagnostic practices of doctors. The earliest theories of disease postulated concrete entities which exist independently of the body. The notion of disease entity has lost (...) its original ontological connotations and instead its important feature has become the possession of a unitary and self-contained character. Dr Taylor describes the modern theories as essentially 'reactive' in character, that is the symptoms of a disease are the bodily reactions to the 'noxae'. After seeing the subject in its historical content, Dr Taylor goes on to discuss in detail the notion of the classification of diseases, making extensive use of modern views on the logic of classes. This is the first comprehensive translation of the expanded French version of E.F.F. Chladni's Traité d'Acoustique, using Chladni's 1802 Die Akustik for reference and clarification. Chladni's experiments and observations with sound and vibrations profoundly influenced the development of the field of Acoustics. The famous Chladni diagrams along with other observations are contained in Die Akustik, published in German in 1802 and Traité d'Acoustique, a greatly expanded version, published in French in 1809. The present translation was undertaken by Robert T. Beyer, (...) PhD (1920-2008), noted acoustician, Professor of Physics at Brown University, and Gold Medal recipient of the Acoustical Society of America. Along with many other projects completed over the course of his career, Dr. Beyer translated Von Neumann's seminal work, Mathematical Foundations of Quantum Mechanics from the original German, spent 30 years translating Russian physics treatises and journals, served as editor of the English translation of the Soviet Journal of Experimental and Theoretical Physics, and also authored Sounds of our Times: Two Hundred Years of Acoustics. Effective documentation is considered a core competency for clinical ethics consultation. Ethics consultants within the Cleveland Clinic in Cleveland, Ohio, observed variation in the formatting of ethics chart notes across consultants and realized that this formatting was based on their own views of effectiveness. To minimize variation and optimize the readability and understandability of ethics chart notes for end users, a team undertook a quality improvement project to assess the formatting preferences of healthcare professionals who rely on ethics consultation notes. (...) The team developed three sample templates and conducted interviews with stakeholders to understand their preferences. A single standardized template was developed based on the preferences that emerged, which all consultants on the ethics consultation service then utilized. In the first five months of implementation, the percentage of end user respondents marking the highest Likert scale option on a post-consultation survey regarding whether the ethics consultation service provided helpful documentation increased from 60 percent to 72 percent compared to the same five-month period in the year prior.
